Objective To explore the mechanism of immune tolerance in chronic B hepatitis, the effects of HBV infection on the functions of macrophages and related mechanism of signal transduction by transfection in vitro were studied. Methods Peritoneal macrophages of mice were isolated regularly and cultured, transfected transiently with pcDNA3-HBV or pcDNA3 plasmid DNA and cultured under the stimulation of LPS. After 72 h, RT-PCR was performed to detect the expression of PreS1, TNF-? or IL-1? mRNA. To detect the expression of NF-?B RelA protein by FCM, and the level of nitric oxide in cultural supernatant was measured with Griess reaction. Results After being transfected with pcDNA3-HBV,peritoneal macrophages had the expression of PreS1 mRNA, but have lower level of TNF-?、IL-1? mRNA and transcriptional factor NF-?B, compared with pcDNA3-transfected control group; the level of nitric oxide in pcDNA3-HBV group was also decreased. Conclusions Transient transfection of pcDNA3-HBV could decrease the function of macrophages directly by inhibiting NF-?B activity and effector molecules production, which may be one of the mechanisms of immune tolerance in chronic B hepatitis.

Journal of China Medical University 2010;(9):710-712,716
Objective To study the effect of erythropoietin(EPO) on PI3-K/Akt signal pathway of myocardial ischemia-reperfusion injury(I/R) in immature rabbits. Methods Thirty immature rabbits were randomly divided into two groups (n = 15/greup ), The rabbits were intraperi- toneally injected with 2 ml of normal saline in control group, or 5 000 U/kg of EPO in treated group 24 h before I/R. The Langendorff isolat- ed perfused heart model was established, and the myocardial expression of PI3K, AKT and Caspase 9 were examined by immunohistochem- istry in 2 h after repeffusion. To monitor the myocardial apoptosis,we performed TUNEL in 2 h after reperfusion. Results The expression of myocardial PI3K and AKT in EPO group was significantly higher in EPO-treated group than those in control group. In EPO-treated group, the expression of myocardial caspase 9 was significantly lower than that in control group. The average optical density of myocardial PI3K, AKT and caspase 9 were significantly different between both groups (P 〈 0.01 ).The number of apoptotic cell was significantly fewer in EPO- treated group than that in control group. The myocardial apoptosis index (AI)was significantly different between two groups (P 〈 0.01 ). Conclusion EPO pretreatment can influence the PI3-K/Akt signal pathway and decrease the myocardial apoptosis, and significantly reduce the myocardial ischemia-reperfusion(I/R) injury in immature rabbits.

Abstract: Objective To analyze the risk factors for HIV/AIDS mortality after receiving antiviral treatment in Tongchuan City, and to provide a scientific basis for implementing precision treatment in Tongchuan City. Methods A retrospective cohort study was conducted to collect general demographic characteristics and survival-related information of HIV/AIDS individuals who received antiviral treatment in Tongchuan City from 2005 to 2023. Mortality table analysis was used to assess the cumulative survival rate, Cox regression model was used to analyze survival influencing factors, and a nomogram was constructed to predict the prognosis of antiviral treatment. Results A total of 410 HIV/AIDS patients underwent antiviral treatment, with the shortest observation period being 0 months and the longest 222 months, and the median survival time was 71 months (QR: 64-106); among the 362 cases followed up, there were 39 AIDS-related deaths. The cumulative survival rates after 1, 5, and 10 years of antiretroviral therapy were 96.2%, 91.3%, and 83.0%, respectively. Multivariate Cox analysis showed that patients with a higher level of education (college degree or above) (HR=0.349, 95%CI: 0.159-0.769) and without clinical symptoms (HR=0.357, 95%CI: 0.178-0.715) had a lower risk of death after antiretroviral treatment, whereas patients with a CD4 cell count <200 cells/μL group (HR=9.383, 95% CI: 2.879-30.578) had a higher risk of death. Based on the above three influencing factors, a nomogram model was constructed. Internal validation showed that the model's discriminative ability with a C-index of 0.859 (95% CI: 0.762-0.956), and the calibration curve indicated good consistency, demonstrating the model's good predictive ability. Conclusions Tongchuan City should expand health education on AIDS prevention and control, strengthen HIV monitoring, carry out education on compliance with antiviral treatment among HIV/AIDS individuals, conduct timely CD4 cell count detection, and use the constructed nomogram model to assist in precise diagnosis and treatment in clinical practice.

Double-blind randomized controlled trial design is the most recognized scheme of therapeutic experimental design. On the assessment of the efficacy of traditional medicine, randomized controlled clinical trial design also has the same value.Only when using the research method towards epidemiology to perform the rigorous design on the clinical trial of traditional medicine and make the objective evaluation of its effect, can it get the real understanding and approval from modern medicine. When making the therapeutic clinical trial under the traditional medicine, due to the characteristics of the subject, therapeutic measures usually need to be changed so they cannot be standardized, and the modification may result in the share of multiple intervention components, which leads to the inaccurateness of results under the effect of confounding bias.According to the characteristics of traditional medicine, a new method was proposed for therapeutic clinical trial.Firstly do the randomization with the inclusion criteria made by the same symptom arise in a certain stage of disease, secondly adopt the standardized intervention measures for processing.Eventually confirm whether there is significant statistical difference. The symptom directing standardized interventions have provided the experimental design of the traditional medical clinical therapy with a feasible scheme, which can solve several problems existing in this design.
