1.CALCIUM ANTAGONIST AND ENDOTHELIN IN THE ACUTE SPINAL INJURY

Objective Acute spinal injury(ASI) is a kind of disease commonly seen in the orthopedic department, with secondary pathological injury causing the delayed damage of tissue structure. This study is focused on finding the injury mechanism of endothelin and the relation with calcium in SCI, and developing an effective treatment of SCI through animal experiment for clinic application in the future. Methods ASI animal model with radioimmuolo gical techniques are applied to examine the level of endothelin, and to find the pathological changes under microscope and electron-microscope. Results The quantity of endothelin and calcium in the cell with 1, 4-dihydropyridine calci um channel inhibitor is decreased, as a result, depolarization was lightened. The mechanism delays the development of secondary injury significantly. Conclusion This type of treatment may be used in emergency for spinal cord injury in order to protect the function and to gain much precious opportunity for spinal recovery and other treatment.
2.The research progress in endoplasmic reticulum stress with the correlation of osteoporosis

ERS is a kind of universal existence in various cellular stress response.Recently, the research shows that ERS may affect bone metabolism balance from different ways.However, the specific mechanism is still not clear.The relationship between ERS and osteoporosis needs further research.This article will firstly introduce the pathways of ERS, then the relationship between the ERS and osteoporosis will be discussed from pancreatic beta cells, osteoblasts, osteoclasts and mesenchymal stem cells.Lastly, I will give perspectives on the future research on treatment of osteoporosis drug.
3.The use of omentoplasty after esophagectomy for esophageal cancer preventing complications: a meta-analysis

Objective To evaluate the length of hospital stay and the incidences of complications after omentoplasty with non-omentoplasty for the patients with esophageal cancer.Methods The databases including Pubmed,Embase,The Cochrane Library,Web of Science,CBM,CNKI,VIP and Wanfang data were searched for collecting randomized controlled trials on the omentoplasty.According to the inclusive and exclusive criteria,the datas were extracted.Two reviewers independently screened literatures and assessed the qualities of the included studies and extracted data.Meta-analysis was performed by using of RevMan 5.2 software.Results A total of 6 RCTs including 2 167 patients from 206 original articles were included in this analysis.In terms of the anastomotic leakage after esophagectomy and the hospital stays,the incidence of anastomotic leakage (OR =0.19,95% CI:0.09 ~ 0.39,Z =4.55,P < 0.000 01) and hospital stays (MD =-1.91,95% CI:-2.26--1.57,Z =10.87,P < 0.000 01) with omentoplasty was significantly lower than those of the non-omentoplasty,with significant differences.However,in terms of anastomotic stricture (OR =0.76,95% CI:0.29-2.01,Z=0.55,P=0.58) and mortality rate (OR =0.72,95% CI:0.24-2.21,Z=0.57,P=0.57),there wrer no significant differences.Conclusion Comparing with non-omentoplasty,the use of omentoplasty has beneficial effects for the postoperative complication such as anastomotic leakage and hospital stays,and does not increase the incidence of anastomotic stricture and mortality rate.
4.EFFECTS OF NERVE GROWTH FACTOR ON ENDOTHELIN AFTER SPINAL CORD INJURY IN RATS

Objective To investigate the protective mechanisms of nerve growth factor (NGF) on spinal cord injury.Methods The spinal cord injury (SCI) of Wistar rats was performed by a 10g×2.5cm impact on the posterior T12 spinal cord.The experimental animals received NGF liquid by subarachnoid space tube.The radioimmunological techniques were applied to examine the level of endothelin.Results The level of endothelin was significantly increased after the injury as compared with that in control group(P<0.01).The level of endothelin in NGF group as obviously lowered as compared with that in normal saline group 4 h after injury (P<0.01).Conclusion NGF can protect spinal cord against injury in vivo.One of the mechanisms is that NGF could inhibit endothelin-induced vicious circle.
5.Value of construction of bone morphogenetic protein-2 recombinant adenovirus on genetic treatment of bone defect

BACKGROUND: At present, genetic recombinant technique has been utilized to express recombinant human bone morphogenetic protein-2 (rhBMP-2)and to induce either orthotopic or ectopic regenerated bone successfully. But,because that osteoblastic activity induced by rhBMP-2 is lower than that by natural BMP-2, the perfect vectors have not been discovered yet.OBJECTIVE: BMP recombinant adenovirus (rAdV) was constructed so as to provide feasible vector for the basic treatment of bone defect.DESIGN: Single sample was designed in the experiment.SETTING: Experimental Center of Molecular Biology in First Hospital of Xi'an Jiaotong University.MATERIALS: PACCMV-PLPA plasmid, PJM17 plasmid, 293-cell line.METHODS: The experiment was performed in Experimental Center of Molecular Biology in First Hospital of Xi'an Jiaotong University from September 2001 to June 2002. Reverse Transcription-Polymerase Chain Reaction (RT-PCR) was used to clone the whole-length gene of BMP-2 and construct AdV vector. DNA-calcium phosphate coprecipitation was used to transfect accessory plasmid PJM17 into 293cell and homologous recombination was used to construct rAdV. The titer was assayed and CsC1 density gradient centrifugation and purification were applied.construction of rAdV.binant plasmid is named as pGEM-T/hBMP2, (3 015+1 213)bp in size.It was indicated in agar gel electrophoresis that the practical value was in BMP2 shuttle plasmid: The plasmid was about 10 Kbp in size. Since PACCMV-PLPA plasmid multiclone sites belonged to PUC plasmid series,EcoR Ⅰ enzyme digestion was used to linerarizate recombinant plasmid,10 Kbp in size. 8.8 Kb and 1.2 Kbp visible fragments were obtained durtion of rAdV: PCR technique was used to identify rAdV. The target fragment 1.2Kbp that was amplified by BMP2 specific primer of the whole length was in conformity completely with the theoretic value.CONCLUSION: Successful construction of BMP2 rAdV lays a foundation for the feasible genetic treatment with vector of bone defect.
6.The study on the role of salinomycin in the proliferation and epithelial-mesenchymal transition of breast cancer MCF-7 mammosphere cells

Objective To investigate the effects of salinomycin on the cell proliferation and epithelial-mesenchymal transition (EMT) of MCF-7 mammosphere (MCF-7 MS). Methods Breast cancer MCF-7 cells were cultured in suspension in serum-free medium to obtain MCF-7 MS. The cell viability of MCF-7 MS cells treated with serial concentrations of 0, 10, 30, 100, 300, 1 000, 3 000 and 10 000 nmol/L of salinomycin for 24 hours were detected by CCK-8 assay. The half maximal inhibitory concentration (IC50) was calculated. Western blot analysis was performed to detect the expression levels of E-cadherin and Snail in MCF-7 MS cells treated with 30 nmol/L and 60 nmol/L salinomycin. The same capacity of DMSO was added to MCF-7 MS as control group. The xenograft tumors from MCF-7 MS transplant mice were divided into control group (the same capacity of normal saline) and salinomycin group (5 mg/kg salinomycin), then the expressions of E-cadherin and Snail were dectected by immunohistochemical staining. Results With the increased concentration of salinomycin, the cell survival rate of MCF-7 MS cells decreased (P<0.05). The IC50 after 24 h-treatment was 989 nmol/L. Both 30 and 60 nmol/L of salinomycin increased the expression of E-cadherin and decreased the expression of Snail compared with control group. In addition, 60 nmol/L treatment group showed more significant effect (P<0.05). In xenograft tumors from MCF-7 MS transplant mice, the expression of Snail decreased, and E-cadherin increased in salinomycin treatment group compared with control group (P<0.01). Conclusion Salinomycin can inhibit the cell proliferation and EMT in MCF-7 MS cells, which is a potential drug to target cancer stem cells.
7.Systematic review of esophageal stents plus radiotherapy in the treatment of patients with inoperable esophageal cancer

Objective To evaluate the efficiency and safety of esophageal stents combined with radiotherapy compared with esophageal stents alone in the treatment of advanced esophageal cancer.Methods CBM,VIP,CNKI,Cochrane Library,Pubmed and Embase etc were searched by computer begining from the establishment of these datebases to December 2012.The related references as well as communicated with other researchers were also traced to obtain certain informations.Randomized and quasi-randomized controlled trials compared esophageal stents plus radiotherapy with esophageal stents alone in the treatment of advanced esophageal cancer were included.The statistical software RevMan 5.0 was used.Results Seven published articals were included (443 patients),and all trails methodological quality were grade C.The results of Metaanalysis showed that compared with esophageal stents along,esophageal stents combined with radiotherapy improve 1-year survival rates and reduce the local recurrence rates.Gastrointestinal bleeding rates,chest pain rates,gastro-esophageal reflux rates remained similarily.Conclusion Compared with esophageal stents along,esophageal stents combined with radiotherapy can improve 1-year survival rates and reduce the local recurrence rates.
8.The express and clinical significance of Trefoil factor 3 in patients with gastrointestinal failure of postoperation after esophageal cancer

Objective To evaluate the relationship between the Trefoil factor 3 (TFF3) serum,concentration and gastrointestinal failure(GIF) and discuss eaely diagnosis,treatment and prognosis in patients with GIF after esophageal cancer surgery.Methods To test the TFF3 levels of the serum during the postoperation of esophageal cancer by ELISA.Results Serum TFF3 concentrations measured prior to the occurrence of GIF were significantly higher than in control group (P < 0.01).serum TFF3 concentration was significantly related to gastrointestinal tract function score(r =-0.712).Cox proportional hazards model analysis showed that the serum TFF3 concentrations at the time of occurrence of gastrointestinal failure,and 48 hours later,could be used as prognostic factors in critically ill pediatric patients with GIF(r =1.443 and 1.872,respectively).Conclusion TFF3 may play an important role in predicting GIF in pediatric critical illness and has a protective function in the mucosal repair process.
9.The feasibility of postoperative nutrition of cervical gastric fistula after radical resection of thoracic esophageal carcinoma

Objective To explore the feasibility of postoperative nutrition of cervical gastric fistula after radical resection of thoracic esophageal carcinoma.Methods 288 patients with esophageal carcinoma from January 2015 to December 2017 of thoracic surgery in the first hospital of Lanzhou University were analyzed,in which 133 patients underwent cervical gastric fistula,88 patients retained the nasal and enteral nutrition tube,and 67 patients underwent jejunum fistula.The postoperative complications were observed and analyzed.Results The cervical gastric fistula was significantly superior to the enteral nutrition catheter and jejunum fistula in the patients with pulmonary infection,stoma infection,oropharyngeal comfort and so on after thoracic and laparoscopic resection of esophageal carcinoma.Conclusion The high cervical gastric fistula is a safe and feasible nutritional support method for the patients with thoracic and laparoscopic resection of the posterior sternum and the radical resection of esophageal carcinoma.
10.Clinical efficacy of nested anastomosis assisted by tubular stapler in McKeown resection of esophageal cancer

Objective:To evaluate the clinical efficacy of traditional tubular mechanical anastomosis versus nested anastomosis assisted by tubular stapler in McKeown resection of esophageal cancer.Methods:A total of 182 patients who underwent McKeown resection for esophageal cancer between January 2016 and August 2017 were recruited, including 85 patients in the traditional tubular mechanical anastomosis group and 97 patients in the nested anastomosis assisted by tubular stapler group. The incidence of anastomotic fistula and anastomotic stenosis was observed in 6 months after the operation.Results:Compared with the traditional mechanical anastomosis, the incidence of anastomotic fistula in nested anastomosis assisted by tubular stape[0(0/97)vs. 4.7%(4/85)], lung infection[3.1%(3/97) vs. 11.8%(10/85)], gastroesophageal reflux[7.2%(7/97)vs. 17.6%(15/85)], anastomotic stenosis[3.1%(3/97) vs. 10.6%(9/85)], the neck incision infection rate[1.0%(1/97)vs. 7.1%(6/85), the operative anastomosis time[(14.69±2.65) min vs.(20.25±4.31)min], the difference was statistically significant( P<0.05). There was no significant difference in arrhythmia, recurrent laryngeal nerve injury, chylothorax and anastomosis between the two groups. Conclusion:The incidence of complications such as anastomotic fistula, anastomotic stricture, gastroesophageal reflux, and pulmonary infection can be significantly reduced by nested anastomosis assisted by tubular stapler, which is an ideal choice for cervical anastomosis in McKeown resection of esophageal cancer.
