1.Relationship Between Patient Factors And Specific Learning Needs Of Women On Breast Cancer

The type of specific learning needs of women with breast cancer using the Information Needs Questionnaire (INQ) had been identified prior to developing a breast cancer education package. It is also important to determine the types of patient factors such as demographic and clinical factors that influence women’s specific learning needs on breast cancer. This study thus reports the findings on what type of patient factors influence women’s specific learning needs related to the types of INQ. A total of 140 women with breast cancer who sought treatment at Universiti Kebangsaan Malaysia Medical Centre participated in this cross sectional study. Age showed a significant relationship with specific learning needs on sexual attractiveness (p=0.001) and self-care (p=0.048). Duration of illness was related to information on spread of breast cancer (p=0.040) and self-care (p=0.047). Education level showed a significant relationship with cure (p=0.001), sexual attractiveness (p=0.007) and spread of breast cancer (p=0.003). Occupation showed a significant relationship with specific learning needs on sexual attractiveness (p=0.005), chemotherapy (p=0.043), radiotherapy (p=0.039) and hormonal therapy (p=0.043). On treatment received, a significant relationship was noted with sexual attractiveness (p=0.009), radiotherapy (p=0.029), hormonal therapy (p=0.038) and targeted therapy (p=0.047). Ethnicity and Marital status showed no significant relationship with all the specific learning needs. Findings of this study concluded that certain patient factors had significant relationship with certain specific learning needs. All the patient factors studied and their influence on the specific learning needs were taken into consideration prior to developing the breast cancer education package.

4.Rubella outbreak amongst residential students in a military vocational school of Malaysia.

An outbreak of rubella occurred amongst 303 newly recruited residential Form IV students in a military vocational training school in Malaysia. Of the 303 Form IV students, 77 gave a history of acute illness. Rubella specific IgM was detected in the sera of 46.5% (141/303) whereas rubella specific IgG was detected in 100% of all Form IV students. Sixty five students with no clinical history of acute illness during the outbreak period had detectable rubella IgM in their sera and rubella specific IgM was detected in the sera of all symptomatic students except one. Maculopapular rash was the commonest presenting clinical feature among students with acute rubella infection in this outbreak (97.4%) followed by fever (88.2%). The duration of rash ranged from one to nine days with a mean of 4.6 days. Of the 65 students that had both fever and rash, 56 (85.2%) students had maculopapular skin eruption on the same day as the date of onset of fever, six (9.2%) developed the rash a day after the onset of fever and three (4.6%) had the rash after two days of fever. The duration of fever ranged from one to eight days with a mean of 3.5 days. The duration of conjunctivitis ranged from one to four days with a mean of 2.3 days, and all those who developed conjunctivitis had mild eye-discharge without photophobia. The duration of arthralgia ranged from one to three days with a mean of 2.1 days. The commonest type of joints affected was knee joints (66.7%, 12/18), followed by elbow and shoulder joints (27.8%, 5/18) and wrist joints (5.6%, 1/18). A good clinical history of the temporal relationship between the occurrence of rash and fever during the outbreak could easily differentiate rubella illness from that of measles.

5.Medical Students’ Perceptions on the Adequacy and Effectiveness of Pharmacology Teaching during Preclinical Years

Pharmacology teaching during preclinical years is important for medical students
to make rational choices in choosing suitable treatment for patients in future.
Therefore, the present study determined the adequacy and effectiveness of
pharmacology teaching in the undergraduate medical program at the Universiti
Kebangsaan Malaysia Medical Center (UKMMC). Suggestions for improvement of
the curriculum were also identified. An online questionnaire on the perceptions
of pharmacology teaching methodology was distributed to a total of 459 medical
students in 4th and 5th year at UKMMC. The questionnaire covered demographics,
perceptions about pharmacology teaching, the ideal teaching learning
methodology for learning pharmacology, pharmacology topics which are useful
for future clinical practice, the pharmacology topic which was most interesting and
recommendations for improvement. The response rate was 46.4% and majority of
the participants were females (65.7%). Most of the students agreed that interactive
learning was more helpful than didactic lectures (88.0%). Seventy percent of the
students reported that pharmacology lectures in the preclinical years were helpful
during the clinical years. Percentage of students who agreed that pharmacology
teaching in their preclinical was adequate for their clinical practice was 47.0%.
There was no association between demographic variables (gender, race, year of
study and medical family background) and interest in pharmacology (p>0.05). In
conclusion, the pharmacology teaching during preclinical years was perceived
to be effective and useful for students’ clinical practice. More pharmacology
teaching sessions in clinical years was suggested which may improve adequacy of
pharmacology teaching.
6.Correlation of HIPK2 expression with HPV infection and apoptosis in cervical cancer.

OBJECTIVETo evaluate the correlation of the expression of homeodomain-interacting protein kinase 2 (HIPK2) with human papillomavirus (HPV) infection and apoptosis in cervical cancer.
METHODSFormalin-fixed, paraffin embedded tissue samples from 50 cervical cancers and 15 normal uterine cervix cases were obtained. Apoptosis was quantified by TdT-mediated dUTP nick end labeling (TUNEL) assay and the expression of HIPK2 as well as HPV by immunohistochemical staining.
RESULTSHIPK2 protein expression was detected in 88.0% (44/50) of cervical cancers and 6.7% (1/15) of normal cervical tissues. HPV was found in 78.0% (39/50) of cervical cancers and 20.0% (3/15) of normal cervical tissue samples. The expression of HIPK2 protein was significantly and positively correlated with HPV presence (r=0.467, P<0.01), but negatively with apoptotic index (r=-0.370, P<0.05).
CONCLUSIONHIPK2 protein expression is positively correlated with HPV infection, but negatively with apoptotic index in cervical cancers. Therefore, HIPK2 may be involved in the mechanism of apoptosis in cervical cancer and may play an important role in cervical carcinogenesis.

7.A comparative evaluation of dengue diagnostic tests based on single acute serum samples for laboratory confi rmation of acute dengue

A prospective study was carried out to evaluate the sensitivity of dengue NS1 antigen-capture
ELISA in comparison with dengue virus isolation, conventional RT-PCR and real-time RT-PCR for
laboratory confi rmation of acute dengue based on single-acute serum samples. Four primary healthcare
centres were involved to recruit patients with clinical diagnosis of dengue illness. Patient’s
demographic, epidemiological and clinical information were collected on a standardized data entry
form and 5 ml of venous blood was collected upon consent. In the laboratory, six types of laboratory
tests were performed on each of the collected acute serum sample.
Of the 558 acute serum samples collected from 558 patients with clinical diagnosis of dengue
from mid-August 2006 to March 2009, 174 serum samples were tested positive by the dengue NS1
antigen-capture ELISA, 77 by virus isolation, 92 by RT-PCR and 112 by real-time RT-PCR. A total
of 190 serum samples were tested positive by either one or a combination of the four methods
whereas, only 59 serum samples were tested positive by all four methods. Thus, based on singleacute
serum samples, 190 of the 558 patients (34.1%) were laboratory-confi rmed acute dengue. The
overall test sensitivity was 91.6%, 40.5%, 48.4% and 58.9% for dengue NS1 antigen-capture ELISA,
virus isolation, conventional RT-PCR and real-time RT-PCR respectively. Statistically, dengue NS1
antigen-capture ELISA was the most sensitive and virus isolation was the least sensitive test for the
laboratory confi rmation of acute dengue based on single-acute serum specimens. Real-time RT-PCR
was signifi cantly more sensitive than the conventional RT-PCR.
8.Timing of corticosteroids in non-severe non-hospitalized COVID-19 patients: open-label, two-center, randomized controlled study (TICS-COV19 study)

Background/Aims:
Corticosteroids (CSs) are frequently used in coronavirus disease 2019 (COVID-19); however, their utility remains controversial in mild to moderate cases. The timing of CSs initiation during the disease course remains unaddressed. The study aims to evaluate the impact of early CSs in non-severe COVID-19.
Methods:
A randomized controlled, open-label study was conducted on 754 COVID-19 patients randomized into a study group (n = 377) in which patients received CSs with COVID-19 protocol and a control group (n = 377) in which patients received COVID-19 protocol only.
Results:
Both groups were comparable regarding baseline characteristics, presenting symptoms, and inflammatory markers. The composite endpoint (need for O2, need for hospitalization or 28-day mortality) was significantly (p = 0.004) lower in the CS group 42 (11.14%) versus the control group 70 (18.67%) with odds ratio 0.55 (95% confidence interval [CI], 0.36 to 0.83), absolute risk reduction 7.53% (95% CI, 2.46% to 12.59%) and number needed to treat of 13.29 (95% CI, 7.94 to 40.61). Regarding severity at day 10, only (11.1%) of the study group patients were severe versus (18.7%) of the control group patients (p < 0.001). The median time-to-return to daily activity in the CS group was 8.0 days, while in the control group, it was 22.0 days (p < 0.001).
Conclusions
In non-severe COVID-19, CS may decrease hospitalization, severity, and mortality.
9.Effect of Adding Physiotherapy Program to the Conservative Medical Therapy on Quality of Life and Pain in Chronic Rhinosinusitis Patients

Objective:
To assess the effectiveness of combining physiotherapy techniques with conservative medical treatment in chronic rhinosinusitis (CRS) patients.
Methods:
Sixty-eight volunteers with CRS were randomly assigned. Group A received only traditional medical treatment, whereas group B received a physiotherapy program that included pulsed ultrasound therapy, sinus manual drainage techniques, and self-sinus massage technique in addition to traditional medical treatment. Interventions were applied 3 sessions a week for 4 weeks. The rhinosinusitis disability index (RSDI) served as the main outcome indicator for assessing the quality of life, and the secondary outcome measure was the pressure pain threshold (PPT) using a pressure algometer.
Results:
Wilcoxon signed rank test revealed a significant reduction (p<0.001) in total RSDI values from 71.08±1.13 pretest to 47.14±1.15 posttest for group A, while it decreased from 70.64±1.20 pretreatment to 31.76±1.04 posttreatment for group B; furthermore, Mann–Whitney U-test revealed a significant difference (p<0.001) in total RSDI values between both groups when comparing the change of the pre-post data values, it was 23.94±0.95 for group A and 38.88±0.67 for group B. The independent t-test revealed a highly statistically significant increase (p<0.001) in the PPT values in the experimental group compared to the control group.
Conclusion
The physiotherapy program which included pulsed ultrasound therapy, sinus manual drainage technique, and self-sinus massage technique in conjunction with conventional medical treatment was more beneficial for enhancing the quality of life and PPT than traditional medical treatment alone in CRS patients.
10.Body Mass Index (BMI) of Adults: Findings of the Malaysian Adult Nutrition Survey (MANS)

The Malaysian Adults Nutrition Survey (MANS) was carried out between October 2002 and July 2003, involving 6,775 men and 3,441 women aged 18 – 59 years.
Anthropometric assessment showed that the overall mean body weight and BMI were 62.65 kg (CI: 62.20, 63.09) and 24.37 kg/m2 (CI: 24.21, 24.53) respectively.
Based on the WHO (1998) classification of BMI, 12.15% (CI: 11.26, 13.10) were obese (BMI > 30 kg/m2), and 26.71% (CI: 25.50, 27.96) overweight (BMI > 25 - 29.9
kg/m2). Significantly, more women were obese [14.66% (CI: 13.37, 16.04)] while
significantly more men were overweight [28.55% (CI: 26.77, 30.40)]. Ethnicitywise,
prevalence of obesity was highest among the Malays [15.28% (CI: 13.91, 16.77)] while overweight was highest for the Indians [31.01% (CI: 26.64, 35.76)].
Both obesity and overweight were highest among those aged 40 - 49 years.
Obesity was highest for those whose household income was between RM1,500 - 3,500 while overweight was more prevalent for those whose household income
exceeded RM3,500. The prevalence of overweight was highest for those with primary education [31.90% (CI: 29.21, 34.72)]. There was no significant urbanrural
differential in both obesity and overweight. The study found 9.02% (CI: 8.82, 10.61) with chronic energy deficiency (CED) (BMI < 18.5 kg/m2). The
prevalence of CED was relatively higher in the indigenous population (Orang Asli) [14.53% (CI: 5.14, 34.77)], subjects aged 18-19 years [26.24% (CI: 21.12, 32.09)],
and with monthly household income of < RM1,500 [10.85% (CI: 9.63, 12.20)]. The prevalence of CED was not significantly different among the geographical zones
and educational levels, and between urban/rural areas and sexes. The results call for priority action to address the serious problem of overweight and obesity
among Malaysian adults as it poses a grave burden to the country’s resources and development.
