1.Transdifferentiation of mesenchymal stem cells isolated from the rabbit mobilized peripheral blood into cardiomyocytes in vitro

Objective To isolate mesenchymal stem cells(MSCs) from the mobilized peripheral blood and transdifferente them into cardiomyocytes for establishing an non-invasive cell source of stem cell transplantation for myocardial infarction therapy.Methods To granulocyte-colony stimulating factor((30 ?g/kg?day)) was injected into New Zealand White rabbits subcutaneously for 5 days,then MSCs were isolated from peripheral blood.After treatment with 5-azacytidine for 24 hours,the cultured cells were evaluated by immunohistochemical staining for actin,desmin and cardiac-specific troponin T(cTNT).Results After treatment with 5-azacytidine,the MSCs from the mobilized peripheral blood showed spindle-like and multinucleation,some cells aggregated and formed a ball-like appearance at 2 weeks.The ball-like cells and some spindle-like multinuclear cells were found with positively staining of actin,desmin and cTNT.Conclusion MSCs can be isolated from the mobilized peripheral blood and transdifferented into cardiomyocytes in vitro,suggesting that MSCs from the mobilized peripheral blood may be a new and non-invasive cell source of stem cell transplantation in myocardial infarction therapy.
2.Comparative analysis with three dimentional reconstruction coronary angiography and conventional quantitative coronary angiography

Objective To investigate the diagnostic accuracy of 3-dimensional reconstruction(3DR) coronary angiography in the evaluation of coronary artery disease compared with conventional quantitative coronary angiography with IVUS result as the reference standard.Methods Twenty vessel segments in nineteen patients who underwent coronary angiography and IVUS were studied.With the IVUS result as the reference standard,the 3D reconstruction images and the single plane(2D) images of each angiograph were retrospectively studied and compared.Results In all the 20 segments,the minimal vessel diameter/area rate of the stenosed vessels and the reference vessels,and the minimal cross-sectional stenosis rate had no significant differences among the conventional 2D angiograph the 3DR angiograph and the IVUS results.According to the IVUS results,all the 20 vessel/segments were further divided into two groups which were:the eccentric stenosis group(n=7) and the centripetal stenosis group(n=13).In the centripetal stenosis group,all the angiographic parameters detected by the three methods had no statistical difference.In the eccentric stenosis group,the 3DR angiograph showed more consistent results as detected by IVUS compared with the 2D angiograph in terms of cross-sectional stenosis rate.Conclusion Three dimentional reconstruction of coronary angiography showed higher accuracy in the quantitative analysis of coronary artery stenosis compared with conventional coronary angiography,especially in eccentric stenosis.
3.Angiographic follow-up study of the effect of the Cypher and Taxus stents in coronary antery disease

Objective To evaluate the efficacy of Cypher and Taxus drug eluting stent (DES)in the treatment of coronary artery diease. Methods A total 162 patients underwent coronary artery stenting with eigher the Cypher (n=93) or the Taxus (n=69) drug eluting stent. All patients received follow up angiography. Results The angiographic restenosis rate and TLR rate were 10.8% and 5.4% in the Cypher group whereas 11.6% and 5.8% in the Taxus group. There were no statistical differences between the two groups. In-Stent Late Loss and In-Lesion Late Loss were found less in the Cypher group than in the Taxus group. Conclusion Both the Cypher DES and the Taxus DES decreased the restenosis and TLR rates but the Cypher stent has a lower late loss rate than the Taxus stent.

5.Endovascular treatment for traumatic arterial lesions

Objective To discuss the feasibility of endovascular treatment for traumatic arterial lesions.MethodSix patients treated by endovascular stent grafts were retrospectively reviewed.Result Patients consisted of left common carotid artery pseudoaneurysm, right subclavian arteriovenous fistula,thoracic aortic pseudoaneurysm, righ iliac arteriovenous fistula,left iliac arterial pseudoaneursym and femoral arterial pseudoaneurysm. All of the patients abtained the blood flow reconstruction without any complications after endoluminal therapy under local anesthesia. No occlusion appeared during follow up.Conclusion The stent grafts appear to be associated with a low morbidity, high success rate, and less traumatic procedure. Endovascular management is an important tool for the treatment of vascular trauma.
6.Analysis on feasibility of extended hepatectomy for huge hepatic carcinoma

ObjectiveTo investigate the feasibility of extended hepatectomy for huge hepatic carcinoma.MethodsA retrospective analysis was made on the clinical data of 28 patients with huge hepatic carcinoma who underwent extended hepatectomy from December 2006 to June 2011.Twenty-two cases were Child-Pugh grade A,6 cases were Child-Pugh grade B,19 cases were indocyanine green retention at 15 minutes (ICGR15)≤10%,5 cases were 10% < ICGR15 ≤15%,4 cases were 15% < ICGR15 ≤20%.ResultsThe operative death rate was 7.1% (2/28) with Child-Pugh grade B,15% < ICGR15 ≤20% and liver resection volume 60%-70%.Postoperative complications were found on 15 cases,but all recovered after treatment.ConclusionExtended hepatectomy for huge hepatic carcinoma can be feasible in the patients with enough reserve of the liver function provided.
7.Transplantation of bone marrow derived mononuclear cells for the therapy of myocardial infarction in rabbits

Objective: To investigate the effects of bone marrow-derived mononuclear cells (BMC) transplantation for the therapy of myocardial infarction (MI) in rabbits. Methods: 20 rabbits were randomly divided into 2 groups. MI was induced by ligation of the left anterior descending artery.In transplantation group(T,n=10), BMC transplantation was performed on 5-7 days after MI . Bone marrow (3-5 ml) was obtained from iliac crest and labeled with bromodeoxyuridine (Brdu) for 24-48 hours, BMC were transplanted into infracted myocardium through intramyocardial injection. Control animals (C,n=10) didn′t receive any treatment after MI. Echocardiography was performed for evaluating the cardiac function in 1 week and 5 weeks after MI. Hemodynamic and histological studies were performed in the 5 th weeks after MI. Results: LV ejection fraction of group T had no change, but group C decreased in the 1st week and 5th weeks after MI. The results of Group T having higher LV max +dP/dt and max-dp/dt, lower LV end-diastolic pressure showed comparing with that of group C in the 5th weeks after MI. Histological studies revealed that there were Brdu positive cells in the infarcted area in group T, and the vascular density of group T in the infarcted area was significantly greater in comparision with group C. No regeneration of smooth muscle cell and cardiomyocyte were found in the infarcted area. Conclusion: Transplantation of BMC may avoid the deterioration of cardiac function through vasculogenesis in the infarcted area,but the efficacy in amelioration of cardiac function is limited.
8.Comparison between mobilization and transplantation of bone marrow stem cells for the therapy of myocardial infarction in rabbits

AIM: To compare bone marrow stem cell mobilization with bone marrow-derived mononuclear cells (BMCs) transplantation for the therapy of myocardial infarction (MI) in rabbits, and to explore more effective and practical stem cell therapeutic strategy for MI. METHODS: In mobilization group (M, n=10), granulocyte-colony stimulating factor (G-CSF) (30 ?g?kg~ -1 ?d~ -1 ) was injected subcutaneously 3 hours after MI and every 24 hours for 5 days. On the 5th day, the BMCs from 10 mL peripheral blood were labeled with bromodeoxyuridine (BrdU) for 24-48 hours, then reinjected intravenously. In transplantation group (T, n=10), BMCs transplantation was performed 5-7 days after MI. After being obtained from bone marrow (3- 5 mL ) of iliac crest and labeled with BrdU for 24-48 hours, BMCs were transplanted into infracted myocardium through intramyocardial injection. Control animals (C, n=10) did not receive any treatment after MI. Echocardiography were performed for the evaluation of cardiac function 1 week and 5 weeks after MI. Hemodynamic studies and histological study were performed 5 weeks after MI. RESULTS: LV ejection fraction increased significantly in group M, had no change in group T, and decreased 1 week and 5 weeks after MI in group C. Group M and group T had higher LV max +dp/dt and max -dp/dt, lower LV end-diastolic pressure compared with group C 5 weeks after MI. CONCLUSION: Bone marrow stem cell mobilization with G-CSF and transplantation of BMCs both significantly improve the cardiac function for the therapy of MI through vascular genesis in the infarcted area. Bone marrow stem cell mobilization may offer a new and non-invasive therapeutic strategy for MI.
9.MAPKs activity and c-fos mRNA in vascular tissues increased after coronary artery balloon injury in swine

AIM: To observe the changes in mitogen-activated protein kinases (MAPKs) activity and gene expression of c-fos after coronary artery balloon injury in swine. METHODS: Six of the seventeen Chinese swine were as control group, and the others underwent coronary angioplasty to LAD or CLx. The animals were sacrificed at three and thirty days following the procedure. The cross-sections were stained hematoxylin-eosin, strichrome, and Verhoef-van Giesen after the target segments were dissected free from the hearts, and the morphologic characteristics were investigated by computer-assistant analysis system. The target segments were also processed to examine the gene expression of c-fos by reverse transcription-polymerase chain reaction (RT-PCR) and to measure the activity of MAPKs by biochemistry. 10.Correlation of Videodensitometric Scale and Single Photon Emission Computerized Tomography in Assessment of Myocardial Microperfusion

ObjectiveTo evaluate the feasibility and the value of videodensitometric scale (VDS) in assessing myocardial microperfusion. Methods11 Beagles were embolized small coronary artery with polystyrene microbubble (100 μm). Coronary angiography was performed after the embolization and the VDS was calculated. Single photon emission computerized tomography (SPECT) was also performed after embolization and SPECT scores were also calculated. ResultsVDS after embolization was (15.2±3.8); and SPECT score was (17.6±6.4). VDS correlated negatively with SPECT scores (r=-0.85, P<0.05).ConclusionVDS keeps a closely correlation with SPECT. It can be used to assess myocardial microcirculation in clinical.
