Circadian rhythms are endogenous 24 h variations found in virtually all physiological processes and behaviors, which are controlled by the transcriptional translational oscillator that consists of a series of core clock genes (bmal1, clock, cry and per) and clock controlled genes (rev-erbα, rorα, dbp, tef and hlf).Clock genes exist in immune organs, tissues and cells, leading to the immune cell function (migration and chemotaxis, phagocytosis, cytotoxicity and so on) and a variety of immune parameters (factor level of circulating immune cells and subsets of the relative and absolute number of cells) showing circadian rhythm changes, and playing an important role in maintaining immune homeostasis.In addition, some immune related diseases are closely correlated with circadian rhythms abnormalities.This paper will focus on the effect of circadian rhythms on immune functions and their roles in some immune related diseases.

Objective To explore the clinic effects in duplication blepharoplasty with the multiple tiny inci- sions.Methods The line of drooping blepharon was set in routine in 183 patients,the incisions with 3～4mm long would be made in middle and the both canthi sides on the line,cutting 1.5～2mm long flaps of the muscle orbicularis oculi off in the down rim of the incisions.Excessive fats also released form the orbital septum in patients with swelling blepharon.Then put up the muscle tendon of blepharon and sutured with the slcin on the up rim of tarsal plate.Results The reaction of tissue after operation was very light,swelling in the incision could disappear after 7～23 days and the incision scars weretiny and unclear,tbe double-fold eyelids had a good appearance in long time.Con- clusion It is an effective and usable procedure to the patients without skin loosening or serious swelling blepharon by means of multiple tiny incisions in blepharoplasty.

Objective To establish a clinical score system of giant cell tumors (GCT) according to its morphological features presented on three-dimensional (3D) computed tomographic imaging. Methods Sixteen patients with GCT around knee were included from January 2006 to March 2009. Of the tumors, 9 were located in the distal part of femur, and 7 were in proximal part of tibia. Each patient was exposed to spiral CT preoperatively. Then these primitive CT dates were inputted into digital Orthopedics clinical research platform. With 3D surfaces reconstruction and volume rendering, we reconstructed 3D morphology of GCT. The measurement index included pathological fracture, the degree of involvement of cortical bone, the volume of tumor, the distance between tumor and joint surface and the percentage of involvement areas of articular surface. On account of previous literature and above data, clinical score system of GCT was established. Its feasibility was testified by clinical data. Results A new clinical score system of GCT was established. It was named Hu-Chen Giant Cell Tumor Scale. Full score of the system was 12. In the 16 patients, 5patients whose points was more than 9 elevated by Hu-Chen Giant Cell Tumor Scale preoperatively were treated by wide excision and prosthetic replacement. The postoperative average MSTS score of 5 patients was 27, and there were no recurrence. The 5 patients whose points was 6-8 elevated by Hu-Chen Scale were treated by intralesional excision and structured allograft. The postoperative average MSTS score of the patients was 29, and 1 case underwent recurrence whose points was 8. The 6 patients whose points were less than 6 elevated by Hu-Chen Scale were treated by intralesional excision and morsellized allograft. The postoperative average MSTS score of the patients was 27, and there were no recurrence. Conclusion Hu-Chen Giant Cell Tumor Scale established based on digital techniques includes lots of factors which determined surgical strategy. The grating system is an effectively, reliable method in treatment of GCT.

Objective:To explore and compare the therapeutic effect of hysteroscopic cesarean scar pregnancy and transvaginal scar pregnancy.Methods: 63 cases of uterine scar pregnancy patients were divided into treatment group (32 cases) and control group (31 cases) depend on the different surgical methods. The difference of operation time, intraoperative blood loss, HCG descent, postoperative hospital stay and postoperative recovery situation were compared. Results: 32 cases of hysteroscopic surgery patients were successful in one-time operation, no serious complications. Every indicator of hysteroscopic surgery group, such as operation time, intraoperative blood loss, postoperative hospital stay and postoperative blood HCG decline situation, was better than the vaginal surgery group (t=6.743, t=4.329,t=2.482,t=2.181;P<0.05).Conclusion: Hysteroscopy surgery is an important method for minimally invasive treatment of cesarean scar pregnancy, and different individual treatment program should be adopted to treat different cases of CSP and objective in clinical practice.

Objective To analyze the telephone follow-up response rate and related factors in lung cancer patients thus to explore the countermeasures to increase the telephone response rate.Methods One thousand six hundred and eight post-surgery lung cancer patients were followed up by telephone interview,calculate response rates and analyze the impact factors on response rates of surgical patients. Results The lung cancer patients' overall response rate of one thousand six hundred and eight cases was 83.1%.Reservation phone type, phone number,family history of cancer are major factors affecting lung cancer patients' telephone followed up response rate(P<0.05). Conclusions Telephone follow-up of patients with lung cancer response rates is at a low level.To improve tumor response rates of follow-up and follow-up quality is a long and arduous task,requiring medical personnel continue to learn professional knowledge,summing up a reasonable follow-up methods,but also needing patients and their families to understand and cooperate.

Objective To explore the recent and distant effects of early active training after the operation for lumber intervertebral disc herniation.Method 79 patients after the operation for mono segmental lumber intervertebral disc herniation had been divided into early active training group and routine control group randomly, and accepted training, regular re examination, and follow up of 1～ 6 years respedtively.Results The early active training group had better recent and distant objective effect, and more patients (97.6％ ) were satisfied with the operational effects.Conclusions The early active training after the operation for lumber intervertebral disc herniation is positive significant for operational effects.

A design scheme of a tele-screening system for diabetic retinopathy (DR) has been proposed, especially the communication subsystem. The scheme uses serial communication module consisting of ARM 7 microcontroller and relays to connect remote computer and fundus camera, and also uses C++ programming language based on MFC to design the communication software consisting of therapy and diagnostic information module, video/audio surveillance module and fundus camera control module. The scheme possesses universal property in some remote medical treatment systems which are similar to the system.
