1.Treatment of corneal perforation with lamellar keratoplasty combined with autogenous corneal stromal padding

Objective:To study the clinical outcome of lamellar keratoplasty combined with autogenous corneal stromal padding for treatment of corneal perforation.Methods:An observational case series study was conducted.A total of 14 patients (14 eyes) with corneal perforation underwent lamellar keratoplasty in Henan Provincial People's Hospital from January 2017 to August 2018 were collected.During the operation, the corneal stromal autograft was taken to pad the corneal perforation.Visual acuity, intraocular pressure, corneal epithelial healing time, infection recurrence, and the status of corneal graft and anterior chamber were recorded on the 1st, 7th, 14th day, 1st, 3rd, 6th, 9th and 12th month after operation.OCT examination of anterior segment was performed at 1 month, 6 and 12 months postoperatively to observe the interlamellar structure of corneal implantation bed and stromal graft, anterior chamber and iris.The safety and effectiveness of the operation were evaluated according to the 1-year follow-up.The study protocol adhered to the Declaration of Helsinki and was approved by the Ethics Committee of Henan Eye Hospital (No.HNEECKY-2019[18]). Written informed consent was obtained from each patient prior to entering the study cohort.Results:The healing time of corneal epithelium was 2-4 days after operation, (3.01±0.59) days on average.At one day after surgery, all patients had normal anterior chamber depth without aqueous leak and no double chamber was observed.The corneal grafts attached to the implantation bed well and showed mild to moderate edema.There was no recurrence of corneal infection ulcer two weeks after operation, and the corneal ulcer was healed and anterior chamber was well formed in 14 patients.The corneal graft was transparent, and the anterior segment OCT image showed corneal implant bed and graft were well attached, and the anterior chamber depth was normal three months after operation.The best corrected visual acuity of 9 eyes was more than 0.3 one year after operation, and the vision acuity of the 14 patients improved obviously, and no refractory glaucoma occurred after operation.Conclusions:Lamellar keratoplasty combined with autogenous corneal stromal padding is a safe and effective surgical procedure for treating corneal perforation.
2.Continuous Detection Method for Ractopamine with Surface Plasmon Resonance Biosensor

The interaction between the anti-Ractopamine (Rac) monoclonal antibody and the Rac derivation immobilized on the sensor chip surface was studied with surface plasmon resonance (SPR) biosensor. A continuous detection method based on the linear response during the association phase was developed. The sensor chip surface was regenerated after several tests performed continuously, thus the detection step was simplified and the life span of the chip was prospected to be prolonged. The detection was performed as an inhibitive immunoassay. The mixture of anti-Rac monoclonal antibody and the sample flowed over the surface with Rac derivation was immobilized. The relative response was in inversely proportion to the concentration of Rac. The detection limit was less than 4 μg/L with a detection time of 15 min.
3.Micro-Electro-Mechanical Systems-based Gas Chromatography Columns with High Performance

The fabrication and experimental test results were presented for a micro-electro-mechanical systems (MEMS)-based gas chromatography column. Compared with conventional column, this micro-gas chromatography column is suitable for on-line analysis and monitoring because of the small size, rapid analysis and other characteristics. These 0.5, 1 and 3 m GC columns were fabricated using a deep reactive-ion etching (DRIE) and were coated the stationary phase via a dynamic coating procedure. These GC columns perfectly separated the mixture of benzene, toluene and o-xylene in less than 200 s, where, the 3.0 m of the GC column achieved) a high resolution of 14.3 between toluene and o-xylene and yielded approximately 6160 plates. Moreover, the effect of separation performance for different column length were examined and compared. These GC columns are suitable for the application of a variety of disciplines, including environmental analysis, methane gas probes and homeland security.
4.Effects of Active Fraction of Angelica Sinensis Radix on Immunological Function in Mice under High Altitude Hypoxia Condition

Objective To explore the intervention effect of active fraction of Angelica Sinensis Radix in mice under high altitude hypoxia condition. Methods Totally 72 healthy SPF mice were randomly divided into control group (K), model group (M), Rhodiola rosea group, and active fraction of Angelica Sinensis Radix groups (B, C, X). The mice were administerted corresponding treatment by gavage for 21 days. Control mice were given normal saline in same volume. From the 8th day, all mice excepted control mice were exposed to high altitude hypoxia cabin after 0.5 hour gavage treament. On the 22nd day, after got out of the cabin and their body weight were measured, mice were put to death through eyeball blood sampling to prepare splenic lymphocyte suspension. The proliferation and transformation capacities of lymphocyte cell and killing activity of NK cells were detected by MTT. The content of IL-2 in the serum of mice in each group were detected by ELISA. Results Compared with the control group, the body weight of mice, the proliferation and transformation capacities of lymphocyte cell, the killing activity of NK cells, and the content of IL-2 were all significantly decreased (P<0.05, P<0.01). Experiment tests showed that the proliferation and transformation abilities of lymphocyte cell and the killing activity of NK cells were all increased in the mice of group B, C, and X compared with those of the model group (P<0.05, P<0.01). The stimulate index of lymphocyte cell was raised after X intervention compared with those of the model group (P<0.05). The content of IL-2 in the serum was enhanced after intervention of active fraction C and X of Angelica Sinensis Radix compared with those of the model group (P<0.05, P<0.01). Conclusion Active fraction of Angelica Sinensis Radix shows increasing immunological function of mice exposed to hypoxia.
5.Correlation of serum albumin with short-term functional outcome of acute ischemic stroke

Objective No consensus has yet been achieved on the relationship of serum albumin with the functional out-come of acute ischemic stroke.The aim of our study was to determine whether the serum albumin level was associated with the short-term functional outcome of acute ischemic stroke in well-nourished patients. Methods Totally, 113 patients with first-ever acute ischemic stroke were recruited from Nanjing Stroke Registration Program between January and June 2015.Baseline data including de-mographic and body parameters, vascular risk factors, and laboratory results were collected.The NIH Stroke Scale ( NIHSS) was used to evaluate the severity of neurological deficits and the modified Rankin Scale ( mRS ) employed to assess the short-term functional outcome.According to the mRS at discharge, the patients were divided into a good outcome group ( mRS<3 ) and a poor out-come group ( mRS≥3 ) .The independent predictors of the short-term functional outcome were evaluated by multivariate logistic regression analysis. Results Of the 113 acute ischemic stroke patients included, 52 (46.0%) were in the good outcome group, and 61 (54.0%) in the poor outcome group.Those in the former group had a significantly higher BMI, lower serum LDL-C, lower WBC count, and lower NIHSS at admission than those in the latter .Multivariate logistic regression analysis showed that low serum albumin, NIHSS at admission, and arteriole occlusion were independent predictors of the poor short-term functional outcome ( OR=0.684, 95% CI:0.490-0.956, P=0.026). Conclusion Low serum albumin is an independent predictor of poor short-term functional outcome in acute ischemic stroke patients in well-nourished status.
7.USPIO-enhanced MRI for lymph-node metastases from abdominal and pelvic malignancies: Meta-analysis

Objective To investigate value of ultrasmall superparamagnetic iron oxide (USPIO)-enhanced MRI in diagnosis of lymph node metastasis from abdominal and pelvic malignancies with Meta-analysis.Methods A systematic search was conducted in PubMed,Embase,Cochrane Library,Wanfang,VIP and CNKI databases.The literature were screened according to inclusion and exclusion criteria,and four tabular data were extracted.With Meta Disc version 1.4 and STATA 11.0 software,statistical analysis was performed and heterogeneity of the included articles was tested.Based on the result of heterogeneity test,proper effect model was selected to calculate the pooled sensitivity and specificity.Summary receiver operating characteristics curve was obtained,and the area under curve (AUC) was calculated.Results Totally 20 English literature were enrolled,including 1 211 patients and 3 583 lymph nodes.The pooled sensitivity and specificity for USPIO-enhanced MRI in diagnosis of lymph node metastasis was 0.89 (95%CI [0.86,0.91]) and 0.96 (95 %CI [0.95,0.96]),and AUC was 0.98,respectively.Regression analysis revealed that the heterogeneity may result from the location of tumors,and subgroup analysis showed that pooled sensitivity in diagnosis of lymph node metastasis in abdominal malignancies was good.Conclusion USPIO-enhanced MRI has good diagnostic efficacy in diagnosis of lymph node metastasis from abdominal and pelvic malignancies.
8.Nursing status of patients with mechanical ventilation and evidence-based consideration

Objective To study the nursing status of the patients with mechanical ventilation in China. With the reference literature and guidelines, seeking and providing the best evidence and the basis for best practice nursing about mechanical ventilation. Methods We use the convenience sampling method that under the organization and call of the hospital infection nursing professional committee from China nursing association from July to September 2015. 87 hospitals of 13 provinces were selected, and the self-designed questionnaires were used to survey the status of 948 patients with mechanical ventilation. Results For the position management, 26.9% of patients were supine position;69.6% of patients used semireclining position;26.9% of patients had orders about position management. For the oral nursing, 4.0% of patients were 1 times/day;49.7%of patients were 2 times/day;43. 7% of patients were 3 to 4 times/day; 37. 5% of patients used chlorhexidine solution to cleaning the oral cavity;39.3% of patients used normal saline; 84.8% of patients had orders about oral care. For the airbags secretion clean, 78.6% of patients used methods to attract the airbags secretion. For the replace the outside line, 77.8% of patients were 1time/week;5.4% of patients were 1 time/2 weeks;1.1%of patients were 1time/4weeks;3.8% of patients were no indications and no replacement. For the prevention of peptic ulcer, 56.5% of patients used medicine to prevent peptic ulcer. Conclusions Clinical nurses hadn′t achieve unified norms and standards in nursing for patients with mechanical ventilation. There was also existing great gap between guideline recommendations and clinical practice. Currently, the biggest challenge of clinical workers is how to introduce the best evidence to clinic effectively.
9.Drug use evaluation of bemiparin sodium injection based on weighted TOPSIS method

Objective To establish the drug use evaluation(DUE)criteria for bemiparin sodium injection,and evaluate the use of bemiparin sodium injection by weight technique for order prefer by similarity to ideal solution(TOPSIS),and to provide reference for improving the rational use of bemiparin sodium injection.Methods Based on the instructions of bemiparin sodium injection,and referring to relevant guidelines and literatures,the DUE criteria of bemiparin sodium injection was established by expert consultation.The weighted TOPSIS method was used to evaluate the rationality medication in the cases of inpatients who use bemiparin sodium injection from June to July 2021 in the Tongji Hospital of Tongji University.Results In the established DUE criteria of bemiparin sodium injection,the top two relative weight coefficients of ten secondary indicators were contraindications and adverse reaction disposal,and the bottom two were administration methods and indications.A total of 100 medical records were including.There was not a case close to the optimal regimen(Ci≥0.8)(reasonable);Ci was between 0.6 and 0.8(basically reasonable)in 83 cases(83.00%);and Ci<0.6(unreasonable)in 17 cases(17.00%).The unreasonable uses of bemiparin sodium injection mainly appeared in off-lable uses of indications and the dosing method,a potential drug-drug interaction,inappropriate dosage,and violation of drug contraindications.Conclusion The drug use evaluation method of bemiparin sodium injection based on weighted TOPSIS method can synthesize multiple evaluation indexes,and the evaluation results are objective and reliable.The results showed that most clinical application of bemiparin sodium injection in this hospital was basically reasonable,but to provide basis for rational clinical drug use and to ensure patients'medication safety,it is necessary to accelerate the off-label evidence-based evaluation process and strengthen the management of rational drug use.
10.Application of multidisciplinary integrated fracture liaison service in the prevention of postoperative refracture after osteoporotic vertebral compression fractures

Objective:To explore the application effect of multidisciplinary integrated fracture liaison services in preventing postoperative refracture after osteoporotic vertebral compression fractures (OVCF) .Methods:Using the convenient sampling method, a total of 230 OVCF patients who were admitted to Orthopedics Department of Beijing Friendship Hospital Affiliated to Capital Medical University from March 2022 to August 2023 were selected and they were randomly divided into the control group and the observation group by the random number table method, with 115 patients in each group. The control group received routine follow-up management after surgery, while the observation group received multidisciplinary integrated fracture liaison services on the basis of the control group, with an intervention period of 1 year for both groups. Before and after intervention, Osteoporosis Knowledge Tests (OKT), Oswestry disability index (ODI), and lumbar quantitative computed tomography (QCT) were used to evaluate the osteoporosis knowledge level, lumbar spine function and bone density of the two groups. After a one-year follow-up, the incidence of recurrent fractures was compared between the two groups.Results:A total of 7 cases fell off during the intervention period, and 110 cases were collected in the control group and 113 cases in the observation group. After intervention, the total score of OKT and the scores of all dimensions in the observation group were higher than those in the control group, and the differences were statistically significant ( P<0.05). After intervention, the ODI score of the observation group was lower than that of the control group, and the bone density value was higher than that of the control group, and the differences were statistically significant ( P<0.05). The incidence of recurrent fractures in the control group and observation group was respectively 20.91% (23/110) and 7.96% (9/113), and the difference was statistically significant ( P<0.05) . Conclusions:Multidisciplinary integrated fracture liaison services can effectively improve the knowledge level and bone density of postoperative patients with OVCF, improve lower back function and reduce the occurrence of recurrent fractures.
