1.Observation of the sectional anatomic structure of the double inferior vena cava and visualization of its main structures in the Virtual Chinese Human Male Ⅲ

BACKGROUND:Double inferior vena cava (DIVC), usually found at routine autopsies, is rare in the clinic. It has been described in numerous reports over the years since Adachi first depicted it in 1940. But all these reports were based on two-dimensional (2D) observation, and no three-dimensional and animated images reported. OBJECTIVE: To observe and establish the digital visible models of the structure of DIVC of the Virtual Chinese Human Male (VCH-M) Ⅲ.DESIGN: Repetitive measurement design by taking VCH-M Ⅲ as the subjects.SETTING : Department of Traumatic Orthopedics and Anatomy, Nanfang Hospital Affiliated to Southern Medical University.MATERIALS: The experiment was conducted in Southern Medical University from February 2006 to May 2006. The cross-sectional images of fresh tissues from VCH-M Ⅲ dataset were reviewed and 1 060 serially-sectioned slices of VCH-M Ⅲ, from the 2 600th slice to the 3 660th were taken as the source for the 3D models in our study.METHODS: Cross-sectional images of fresh tissues from the VCH-M Ⅲ dataset were reviewed and the structures of the DIVC were confirmed on a section-by-section basis. 3D computerized reconstructions of DIVC and its adjacent structures were generated from these data using Arnira 3.1 (TGS) imaging software.MAIN OUTCOME MEASURES: Cross-sectional images and 3D reconstruction of VCH-M Ⅲ.RESULTS: The cross-sectional images of the VGH-M Ⅲ could fairly display the main structure of DIVC. The digitized model of brachial plexus offers unique insights into the abnormity anatomy of DIVC.CONCLUSION: The VCH-M Ⅲ dataset can provide complete and accurate data of DIVC.
2.THE INFLUENCE TO BONE FORMATION OF DIFFERENT SOURCES OF PERIOSTEAL GRAFT

Periosteal autograft from different sources have been used to repair 1.5cm bone defects of radius in 10 rabbits.On the left side,sharp-dissected grafts were implanted and the animals were sacrificed 4,8,14,30 and 60 days after operation.Bone formation was studied with X-ray and histologic technique.The results showed that the way of sharp-dissecting can preserve the periosteum completely,and good bone formation was found in this group.It suggested that the sharp-dissecting of the periosteum is the important key for periosteal graft.

5.Research progress on the management of no packing after septoplasty.

Packing the nose after septoplasty is common practice. The use of postoperative packing has been proposed to reduce the dead space between the subperichondrial flaps and minimize postoperative complications such as hemorrhage, septal hematoma, and formation of synechiae. Additionally, postoperative packing is thought to stabilize the remaining cartilaginous septum and minimize persistence or recurrence of septal deviation. Despite these theoretic advantages, evidence to support the use of postoperative packing is lacking. Additionally, nasal packing is not an innocuous procedure. The use of nasal packing actually cause these complications such as postop- erative pain, mucosal injury, bleeding, worsening of breathing due to sleep disorders, and postoperative infections. Routine use of anterior nasal packing after septoplasty should be challenged for not presenting proven benefit. As alternatives to traditional packing, septal suturing, septal stapler and fibrin glue have been used recently. The purpose of this article is to summarize the progress of traditional packing to no packing after septoplasty.

6.Biomechanical effects of osteoporosis vertebral augmentation on the adjacent intervertebral levels using a three-dimensional finite element analysis

Objective To investigate effects of volume,distribution and leakage of bone cement in ver- tebroplasty on the adjacent intervertebral bodies using an experimentally calibrated and anatomically accurate fi- nite-element model of elderly L4-L5 vertebral bodies.Methods Computed tomography(CT)scanning was done,at 1 mm intervals,on L4-L5 vertebral bodies of the lumbar spine of an old man cadaver that had no abnormal findings on rnentgenograms.The L4-L5 motion segment data were obtained from the CT scans to establish in computer a three-dimensional finite element model of L4-L5 functional spinal unit(FSU)with the software Ansys 7.0.Compressive load after virtual vertebroplasty on the damaged model,unipedicular and bipedieolar injections and leakage of cement into the intervertebral space,and the resulting endplate and disc stresses of the adjacent vertebral bodies were analyzed in various spatial distributions of the filling material and different loading conditions. An anatomically accurate finite-element model of elderly L4-L5 vertebral bodies was developed.Results The FSU study suggested that changes in stress and strain at adjacent levels were minimal.Furthermore,endplale and disc stresses of the adjacent vertebral bodies were not influenced by bone cement filling volumes or distributions except under bending,whereas asymmetric distribution and leakage of bone cement to the disk increased the stress of adjacent endplates.Conclusion Since asymmetric distribution and significant leakage of cement into the intervertebral space can increase the stresses of endplates of adjacent vertebral bodies and may lead to a fracture, symmetric placement of cement in operation should be achieved and leakage of cement avoided.
7.Levonorgestrel-releasing intrauterine system for treatment of dysmenorrhea associated with adenomyosis

Objective To evaluate the efficacy and safety of a levonorgestrel-releasing intrauterine system(LNG-IUS)for the treatment of dysmenorrhea associated with adenomyosis.Methods We recruited 48 women with moderate or severe dysmenorrhea associated with adenomyosis.All women were inserted of LNG-IUS into their uterine cavity from days 5-7 of their periods and maintained for 12 months.We compared the visual analogue scale(VAS)scores and verbal rating scale(VRS)scores of their dysmenorrhea and dyspareunia at baseline and 12 monthes follow-up.Results Forty-four women completed the study. There were significant differences between mean VAS and VRS scores changes of dysmenorrhea and dyspareunia at baseline and 12 monthes follow-up,those of dysmenorrhea dropping from 75?13 to 11?11 and 2.3?0.4 to 0.4?0.3,those of dyspareunia dropping from 54?19 to 4?4 and from 1.6?0.8 to 0.2?0.2 respectively.Overall 29 women(66%)were very satisfied or satisfied with the one-year treatment. Conclusion Insertion of LNG-IUS alleviates moderate or severe dysmenorrhea associated with adenomyosis remarkably.
8.Isolated epidural hematoma in children: a report of 83 cases

Objective To investigate the diagnosis,treatment and prognosis of isolated epidural hematoma (EDH) in children.Methods Clinical data of 83 children,45 males and 38 females (at age of 0.7-14 years),with EDH treated between January 2012 and October 2014 were retrospectively reviewed.Slipping and falling were the most commonly causes of injury.While motor-vehicle accident was noted with increased age.In 16 patients lesion of hematoma was frontal,in 28 patients temporal,in 13 patients parietal,in 9 patients occipital and in 17 patients posterior fossa.Results Of the 28 patients treated surgically,the outcome evaluated using the Glasgow Outcome Score (GOS) was good in 22 patients,mild disability in 2,moderate and severe disability in 1 and death in 3 (one died of venous sinus rupture and one large area infarction).Of the 55 patients treated conservatively,the outcome was good in 52 patients,mild disability in 1,and death in 2 (both presented with bilateral cerebral hernia on admission and one of them was unable to breath spontaneously).Conclusions Overall prognosis of the patients with isolated EDH in children is excellent.Complications of venous sinus rupture,foramen magnum hernia and large area cerebral infarction may contribute to poor prognosis.
9.The Handbook of Nosocomial Infections Management:Evaluation of the Use and Effect

OBJECTIVE To promote the management of nosocomial infections continuous improvement in the clinical and medical technology department.METHODS According to the regulation of nosocomial infections management,a handbook of nosocomial infections management for clinical and medical technology department was designed,and the monitors of nosocomial infection could perform real time inspection and record according require of the handbook.The department of nosocomial infections management examined the monitoring work of clinical and medical technology department every month and summarized every year,and the results were internalized to the valuation of medical quality management.RESULTS After 3 years of the usage of the handbook,the capability of the monitor groups of nosocomial infection and the quality of the all monitoring items were significantly improved;the qualified rates were all above 96.30%.CONCLUSIONS The handbook of nosocomial infections management is useful to improve the quality of nosocomial infections management in the clinical and medical technology department.
10.Temperature gradient and elasticity gradient over the microwave-heated region in porcine liver in vitro

Objective To explore the characteristics of changes in temperature and stiffness within the microwave-induced ablation region.Methods A type of Thy-ablation microwave antenna was used upon a design of various combination of ablation duration and power to induce heated region in samples of fresh porcine liver.Three special sites were set to assess the corresponding temperature and stiffness in each heated region.The temperature was continuously measured by using electronic thermometer with microwave on going,while stiffness was determined 5min after the ending of ablation by using real-time ultrasonic elastography,strain ratio,a quantitative indicator.Results The SR values and temperature determinations decreased progressively from the central to the peripheral area of the heated region.There was a linear regression between the temperatures (X) and SRs (Y),with Y =0.666 X-37.17.A significant correlation exists between X and Y,with the correlation coefficient being 0.956(P <0.001).The stiffness and temperature at the central sites are variable with the changes of both ablation time and power,but little changed at the peripheral sites of heated-region,which we name a phenomenon due to the ending effect of microwave propagation.Conclusions Temperature and stiffness increase considerably following microwave radiation but they are heterogeneously distributed within the ablated region in porcine liver.The characteristic gradient changes are confirmed of both temperature and the elasticity.It is presumable that good recognition and understanding and full utilization of these characteristics are conducive to reasonable control of tumor microwave ablation therapy.
