1.Diagnosis and treatment of acute perforation of peptic ulcer in children

Objective To study the clinical characteristics and misdiagnostic causes of acute perforation of peptic ulcer (APPU) in children ,ao as to improve the diagnosis and treatment . Methods The clinical data of 26 cases of APPU in children were analysed retrospectively. Results The clinical manifestations include abdominalgia , abdominal distension, nausea and vomiting, fever; tension of abdominal muscle , epigastrium tenderness, rebounding pain,and gurgling sound attenuation or disappearance . The diagnosis was made preoperatively in 10 cases , and misdiagnosed in the other 16 cases . All patients underwent perforation repairing operation . There was no mortality in this series. All the patients were followed up for 1 to 2 years,and only 2 cases (7.7%) had peptic ulcer reccurrence. Conclusions Peptic ulcer in children often occurrs in schoolage and most of ulcers were duodenal ulcer. The clinical manifestation of APPU in children has no specific clinical presentation . The early diagnosis of APPU in children is difficult. Combined use of X ray with B ultrasonic examination can apparently improve the accurate rate of diagnosis. Once the diagnosis is made ,the surgical treatment should be performed as early as possible. The perforation repair should be chosen at first. Anti-Hp treatment should be given postoperatively.
2.Biomechanical study of atlas odontoid process replacement

[Objective] To investigate the stability and function of atlantoaxial segment after atlas odontoid process artificial joint replacement.[Method]Ten fresh adult human head and neck specimens were chosen for biomechanical models.The range of motion(ROM),neutral zone(NZ)and stiffness under intact state,post-decompression,post-replacement surgery and post-fatigue were measured respectively.[Result]After decompression,ROM,NZ and stiffness in flexion,extension,right and left lateral bending,and right and left axial rotation increased significantly(P0.05)than that in intact state group.[Conclusion]Artificial atlantoaxial joint in this study has been proved to have biomechanical significance in morphology and dynamic.It has advantages of stable replacement,simple operation,less injury and low wear characteristics.It provides a new choice for the atlantoaxial fusion of anterior or posterior approach.
3.Biomechanical comparison between artificial atlanto-odontoid joint replacament and Magerl transarticular screw fixation

[Objective]To determine the initial stability and function of a new artificial joint in a cadaveric cervical spine model by comparing it with a conventional method. [Methods]Twelve fresh human cadaveric cervical spines(C0~3) were randomly divided into 2 groups: group 1,resection of the odontoid with artificial atlanto-odontoid joint(AAOJ),and group 2,resection of the odontoid with Magerl atlas and axis by transarticular screw fixation(Magerl).For each specimen,the intact and resection of the odontoid underwent a flexibility test firstly,followed by the instrumented construct.Rotational angles of the C0~3 segment were measured to study the immediate stability and function of resection of the odontoid and AAOJ,compared with the intact and resection of the odontoid and Magerl.[Results]Compared to the intact state,resection of the odontoid and AAOJ resulted in a significant decrease in the range of motion(ROM) and neutral zone(NZ) during flexion,extension,and lateral bending(P0.05).Compared to the intact state,resection of the odontoid and Magerl resulted in a significant decrease in the range of motion(ROM) and neutral zone(NZ) during all 6 degrees of freedom(P0.05).[Conclusion]A new type of artificial atlanto-odontoid joint has been designed for correcting atlantoaxial instability resulted from C1、2 anterior decompression procedures.It can restore,to a great extent,the C1、2 axial rotation that is lost during current stabilization procedures.
4.An Electroencephalogram-driven Personalized Affective Music Player System: Algorithms and Preliminary Implementation.

In order to monitor the emotional state changes of audience on real-time and to adjust the music playlist, we proposed an algorithm framework of an electroencephalogram (EEG) driven personalized affective music recommendation system based on the portable dry electrode shown in this paper. We also further finished a preliminary implementation on the Android platform. We used a two-dimensional emotional model of arousal and valence as the reference, and mapped the EEG data and the corresponding seed songs to the emotional coordinate quadrant in order to establish the matching relationship. Then, Mel frequency cepstrum coefficients were applied to evaluate the similarity between the seed songs and the songs in music library. In the end, during the music playing state, we used the EEG data to identify the audience's emotional state, and played and adjusted the corresponding song playlist based on the established matching relationship.

5.Nutting for chronic skin ulcer patients who had accecpted local oxygen threrapy and traditional chinese medicine treatment

Objective In order to discuss the nursing points for chronic skin ulcer patients who had ac-cecpted local oxygen therapy and traditional chinese medicine treatment. Methods We gave totally nursing to 20 patients who accecpted local oxygen therapy and traditional chinese medicine treatment. Results Eighty percent patients had obtained clincal cure and got successful nursing effect. Conclusions Careful perioperation nursing and related monitoring can improve the cute rate of chronic skin ulcer, and then pre-vent complication.

8.Distribution of the hormone-sensitive lipase gene i6 polymorphism in Zunyi region

Objective To explore the polymorphism distribution of the hormone-sensitive lipase (HSL) gene i6 in Zunyi region. Methods HSL i6 in the Zunyi population was examined with ploymerase chain reaction-single strand conformation polymorphism (PCR-SSCP), and analyzed for its distribution. Results Six different alleles and 11 different genotypes were found in the HSL i6. The PIC is more than 0.7, in which allele 6 and allele 7 were most widely distributed. Conclusion The polymorphism of HSL i6 was identified and the result suggests in Zunyi HSL as a candidate gene of glucose and lipid metabolism.

10.Effects of ovarian cycle on medlan-effecttve target plasma concentration of propofol TCI caused loss consciousness

ObjectiveTo evaluate the effects of ovarian cycle on median-effective target plasma concentration (EC50) of propofol administered by target controlled infusion (TCI) caused loss consciousness.MethodsForty ASA I or II patients who had age 20 - 39 (28.5 ± 4.8) years and were scheduled for elective gynecologic laparoscopic surgery were divided into 2 groups (n =20 each group) : The follicular phase group (F group, serum progesterone concentration O.311.52 ng/ml) and luteal phase group (L group, serum progesterone concentration 5.16 ~ 18.56 ng/ral).No premedication was administered.Propofol was administered by TCI.TCI system was incorporated with Marsh pharmacokinetic model.EC50 was determined by up-and-down sequential trial.The target phsma concentration (Cp) was set at 3.5 μg/ml in the fh-st patient in each group.Each time, Cp was increased/decreased by 10% in the next patient depending on whether or not the loss consciousness occurred.ResultsThe ECho and 95% confidence interval of propofol TCI caused loss consciousness were 4.76 (4.52 -5.00) μg/ml in group F, 4.18(3.88 ~4.52)μg/ml in group L.EC50in group L was significantly lower than in group F (t =6.23, P <0.01).ConclusionsAs loss consciousness occurred, median-effective target plasma concentration of propofol in luteal phase was lower than that in follicular phase.
