1.Role of procalcitonin in early diagnosis of hand,foot and mouth disease with bacterial infection

Objective To evaluate the role of procalcitonin (PCT)in the early diagnosis for hand,foot and mouth disease (HFMD)with bacterial infection.Methods Clinical data of 234 HFMD children who were hospitalized be-tween January and July 2012 were analyzed retrospectively,according to discharge diagnosis,data were divided into simple viral infection group (n=178)and viral associated with bacterial infection group (n=56),and data of 20 healthy children were selected as the control group.Serum PCT,C-reactive protein (CRP)and peripheral white blood cell (WBC)count were compared.Results There was significant difference in the level of PCT,CRP and WBC among three groups (F=381.94,24.18,and 26.46,respectively,all P<0.05).The positive rate of PCT,CRP and WBC among three groups was significantly different(χ2=178.25,38.98,and 71.21,all P<0.05),PCT,CRP and WBC in bacterial infection group(92.86%[52/56],85.71%[48/56],and 87.50%[49/56]respectively)were significantly higher than those of simple viral infection group (3.93%[7/178],62.36%[111/178],and 30.90%[55/178]respectively)and healthy control group (5.00%[1/20],10.00%[2/20],and 5.00%[1/20]respectively).The sensitivity rate of PCT,CRP and WBC was 92.86%,85.71%,and 87.50% respectively,specificity rate was 95.00%,90.00%,and 95.00% respectively.Conclusion The level of PCT has important value for the early diagno-sis of HFMD with bacterial infection,and its accuracy rate and sensitivity are better than CRP and WBC levels.
2.Study on correlation between homocysteine and diabetes mellitus

Objective To explore the correlation between homocysteine(Hcy),lipids amd type 2 diabetes mellitus (T2DM). Methods The laboratory test results of 533 cases of patients with T2DM(T2DM group)and 362 cases of healthy individuals (healthy control group)were retrospectively analyzed.Results The serum levels of Hcy and rates of abnormal serum cholesterol (TC),triacylglycerol(TG),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C)levels in the T2DM group were higher than those in the healthy control group,had statistically significant differences(P <0.05).Conclusion The high serum level of Hcy and abnormal lipid metabolism are correlated with T2DM,which might be risk factors of diabetes mel-litus and cardiovascular disease.
3.Effects of Danhongtongjing formula combined with levofloxacin on serum cytokines in patients with chronic prostatitis

Objective To investigate the effect of Danhongtongjing formula combined with levofloxacin on serum cytokines in patients with chronic prostatitis. Methods 74 cases of patients with chronic prostatitis from May 2014 to April 2016 in our hospital were selected,and randomly divided into two groups,37 cases in each groups.The control group treated with levofloxacin,the observation group were treated with Danhongtongjing formula The observation group on the based of control group.The clinical symptoms,TCM syndrome and the changes of SIgA,VCAM-1 and serum VCAM-1 in prostatic fluid of two groups were compared. Results The total effective rate was 94.59% in the observation group and 78.38%in the control group,the difference was statistically significant (P<0.05).After treatment,the NIH-CPSI index of the two groups was significantly decreased,and the observation group was lower than the control group,the difference was statistically significant (P<0.05);the TCM syndrome score of both groups decreased,and the observation group was lower than the control group,the difference was statistically significant (P<0.05).The total effective rate of TCM Syndrome in observation group was 91.89%,which was significantly higher than that of the control group 72.97%,the difference was statistically significant (P<0.05).After treatment,the levels of SIgA,VCAM-1 and serum VCAM-1 in prostatic fluid were decreased,and the observation group was lower than the control group,the difference was statistically significant (P<0.05). Conclusion Danhongtongjing formula combined with levofloxacin in the treatment of chronic prostatitis patients with significant effect,can effectively regulate serum cytokines, control inflammation, improve the treatment efficiency.
4.Study on a rat model of intestinal endotoxemia induced by thioacetamide

Objective To investigate the correlation between dose and effect of thioacetamide (TAA) on rat model of intestinal endotoxemia. Methods The models of intestinal endotoxemia were induced by three different doses of TAA by gavage administration of TAA 200, 400, 600mg/kg respectively once per day for two days.The doses were given at same time point every day. Each group included 10 rats. The rats in the control group were administrated with 2 mL 0.9% NaCl saline gavage. The death of the rats was observed at 24 hours and 48 hours after administration. The blood samples of the living rats were drawn from abdominal aorta for determining the plasma endotoxin levels, serum alanine aminotransferase(ALT)and aspartated transaminase (AST) levels. The histopathological changes of liver were examined. Single factor analysis of variance was performed and comparision between groups was done using t test. Results No rat in the control group died. Two rats of 200 mg/kg TAA group, five rats of 400 mg/kg TAA group and eight rats of 600 mg/kg TAA group died during the experiment. The mean serum ALT levels of TAA model groups [(305.09±116.78)U/L,(901.67±274.31)U/L,(1454.84±473.49)U/L] were all significantly higher than that of the control group(47.81±22.61)U/L(t=14.583, 25.896 and 20.596, respectively; all P<0.05). The mean serum AST levels of TAA model groups [(465.88±139.96)U/L, (884.37±250.90)U/L,(1889.23±159.67)U/L] were all significantly higher than that of the control group (69.33±22.04)U/L(t=12.988,18. 455 and 13.542, respectively; all P<0.05). The mean plasma endotoxin levels of TAA model groups [(0.436±0.110)EU/mL, (0.550±0.095) EU/mL, (0.620±0.057)EU/mL] were all significantly higher than that of the control group (0.103±0.056)EU/mL(t=7.335, 5.260 and 8.191, respectively; all P<0.05). The histological results of TAA model groups showed hepatic cell degeneration and necrosis in different degrees. Conclusions TAA with 200-600mg/kg is proper to establish the rat model of intestinal endotoxemia. The death rate of rats in the 200mg/kg TAA group is lower than those of other model groups, which suggests that 200mg/kg TAA may be the best dosage for establishing rat model for further studies.
5.Effects of functional training on movement performance and balance in elite fencing athletes with patellar tendinopathy

Objective To investigate the effect of functional training on knee pain,functional movement screen (FMS) score and balance in Chinese elite fencing athletes with patellar tendinopathy.Methods Twenty-four fencing athletes with a diagnosed patellar tendinopathy were randomized into a treatment group (TG) and a control group (CG),each of 12.Both groups were given routine physical therapy,while TG received motor function training in addition for eight weeks.Both groups completed the numerical rating scale (NRS),FMS and balance test before and after the intervention.Results After the intervention,the average PRS and FMS of TG were 2.08± 1.24 and 16.25±0.97 respectively,which significantly outperformed those of TG before the intervention and those of CG after the intervention (P<0.05).Moreover,TG indicated superior results in parameters of static postural balance including center of pressure,total length of swinging pathway,maximal length of swinging pathway,and area of swinging pathway when compared to TG before the intervention and CG after the intervention(P<0.05).Conclusion The motor functiontraining is effective in improving functional movement and balance in elite fencing athletes with patellar tendinopathy.
6.Dynamic changes of peripheral blood lymphocyte subsets in fever with thrombocytopenia syndrome patients

Objective The aim of this study is to dynamically investigate peripheral blood lymphocyte subsets in fever with thrombocytopenia syndrome (SFTS) patients at different stages,to evaluate the influence of these changes in the infection process.Methods Case-control study was used in the research.Twelveconfirmedthrombocytopeniasyndromevirus ( SFTSV ) infectedpatientswere enrolled.According to SFTS prevention guide issued by Chinese Ministry of Health,these patients were divided into two groups,recovery group and death group.For each group,dynamic profiles of the CD3 + T cells,CD4 + helper T cells,CD8 + cytotoxic T cell and CD3 - CD16 + CD56 + natural killer cells were tested by flow cytometry.Meanwhile, the relationshipsbetween these dynamicchanges and liver function,leukocytes,and platelets were analyzed respectively.Two independent-samples t test was used to compare the difference of the peripheral blood lymphocyte subsets count between the SFTS patients and healthy control.Small sample was analyzed by Mann-Whitney U test.Results In the early stage of infection,Th cells in peripheral blood of recovery group were significantly reduced and Th/Tc ratio was reversed.On day 5,7,9 of post infection,Th cell counts in peripheral blood were (740.9 ± 6.4),(836.2 ± 272.3 ) and ( 1083.6 ± 319.7 ) cells/μl respectively,which were significantly lower than health control ( 1351.4 ± 295.1 ) cells/μl ( t value was -2.883,-4.235,-2.145 respectively,all P <0.05).Tc cell counts were significantly more than healthy controls (690.1 ± 194.8) cells/μl through the course,which were ( 1006.3 ±356.5),(1166.4±242.4),(1102.4±245.9),(991.3±205.1) and (886.5±154.5) cells/μl on day 7,9,11,13,15 of the course (t value was 3.312,5.661,4.574,3.874,2.382,all P<0.05).NK cells were decreased significantly from the ninth day of the course.Associated with abnormal changes of cell subsets,WBC and PLT decreased significantly,and serum ALT,AST,LDH and CK etc.were higher than normal level.With the disease recovery,the abnormality above was gradually improved.In contrast,death cases showed significant decrease in T and Th cells compared with health control (P < 0.05).On day 7,8,9 of the course,the counts of total T cell were (735.9 ± 359.9),(724.9 ± 125.9),(845.3 ± 389.3) cells/μl and the counts of Th cell were ( 533.2 ± 246.9 ),( 532.1 ± 105.7 ),( 551.7 ± 86.9 ) cells/μl,significantly lower than healthy control ( 1727.9 ± 230.2 ) cells/μl and ( 1351.4 ± 295.1 ) cells/μl,with statistically differences (z value was - 2.828, - 2.342,- 2.342 and - 2.828, - 2.342, - 2.342,all P < 0.05 ).On day 7,8,9 of the course,the numbers of NK cell in death group were ( 1141.8 ± 415.5),( 1047.2 ±68.4),( 1276.3 ±545.3) cells/μl,which were significantly more than health group (470.7 ± 242.2) cells/μl,with statistically differences (z value was - 2.180,- 2.335,- 2.258,all P <0.05).Conclusions SFTSV infection can induce cell immunity damage.The changes of lymphocyte subsets are associated with clinical classification and prognosis.Significant reduction of T cell and CD4 +cell in peripheral blood are accompanied with significant increase of NK cell,which may be a pivotal indicator of poor prognosis and play an important role in making appropriate strategy in clinical treatment.( Chin J Lab Med,2012,35:826-831 )
7.Evaluation of functional training specifically on physical and cognitive functions intervention among children aged 4-5

Objective:
To investigate effects of functional training on physical and cognitive function in 4-5 years old children, so as to provide a reference for the research on development of children s physical and cognitive functions.
Methods:
The 173 participants aged 4-5 were enrolled from 6 kindergartens in Xicheng District, Beijing and were divided into experimental (n=94) and control groups (n=79) by randomized digital tables. The experimental group were asked to receive a 18 week special designed functional movement training, which were not performed to the control group. The two groups were tested with physical and cognitive assessments before and after the intervention, and the results were compared pre/post in individual group and between groups with t tests.
Results:
Compared with control group, the score of standing long jump among children in the experimental group was improved by 5.72%, and that of feet jump was improved by 23.79%, that of 10-meter-shuttle run was improved by 13.95%, that of simple reaction was by 20.34%, and that of attention was by 18.96%, and all the improvement was of statistical significance(t=-2.75, 6.68, 10.79, 5.07, 4.32, P<0.01).
Conclusion
Both physical and cognitive functions were enhanced by the functional physical training in 4-5 year-old children.
8.Pharmaceutical Care for Oral Medication of Tumor Patients with Dysphagia by Clinical Pharmacists

OBJECTIVE:To explore the mode of pharmaceutical care for oral medication of tumor patients with dysphagia by clinical pharmacists ,so as to promote clinical rational and safe drug use. METHODS :Based on typical cases ,the problems existing in the oral medication of tumor patients with dysphagia were analyzed ,and the medication guidance and pharmaceutical care given by clinical pharmacists were expounded. RESULTS :Clinical pharmacists found that tumor patients with dysphagia had some problems during oral medication ,such as improper administration position ,tube feeding of slow-controlled release preparations after opening the capsule ,limited administration route ,biting cytotoxic drugs ,unknown solubility of targeted drugs ,interaction of multiple drugs. In view of the above problems ,clinical pharmacists assisted physicians and nursing staff to provide pharmaceutical care to patients in combination with the patient ’s disease characteristics ,pharmacy,pharmacokinetics,pharmacodynamics,drug solubility,drug interaction and other factors ,such as skillfully using appropriate drug delivery location and mode ,using local drug delivery mode ,selecting appropriate dosage form based on pharmaceutical knowledge (the characteristics of new dosage form ), guiding patients to use drugs correctly by being familiar with the characteristics of cytotoxic drugs and targeted drugs ,paying attention to potential drug interactions and guiding nurses to use drugs correctly. The situation of improper medication was improved. CONCLUSIONS :Clinical pharmacists should participate in clinical therapy actively ,and make appropriate interventions with professional knowledge of pharmacy so as to promote the correct use of oral medication for tumor patients with dysphagia.
9.Preparation of Rehmanniae Radix Juice for Baihe Dihuangtang Based on UPLC-Q-TOF-MS

ObjectiveTo compare the four preparation methods of Rehmanniae Radix juice described in ancient literature and find the method that is most suitable for the preparation of Rehmanniae Radix juice used in Baihe Dihuangtang. MethodThe ancient medical books record four methods for preparing Rehmanniae Radix juice: crushing fresh Rehmanniae Radix for juice, steaming fresh Rehmanniae Radix for juice, boiling fresh Rehmanniae Radix for juice, and boiling dry Rehmanniae Radix for juice. Ultra performance liquid chromatography-quadrupole time-of-flight mass spectrometry (UPLC-Q-TOF-MS) was employed to detect the compounds in the four juice samples, followed by principal component analysis (PCA). Result① Totally 27 compounds were identified in the juice samples, including 10 iridoid glycosides, 14 phenylethanoid glycosides, 2 phenolic acids, and 1 irisone. Among them, 15 common compounds were shared by the four juice samples, including 7 iridoid glycosides, 7 phenylethanoid glycosides, and 1 phenolic acid. ② Five common compounds in the four juice samples can be matched with the reference standards, which were catalpol, aucubin, rehmannioside D, ajugol, and purpureaside C. ③ Verbascoside and isoacteoside were not detected in the juice prepared by crushing fresh Rehmanniae Radix, while it was detected in the other three juice samples, which indicated that the two components were produced after heating rather than being the original components in fresh Rehmanniae Radix. ④ The comparison of the ion fragments demonstrated that verbascoside was produced from purpureaside C after the cleavage of the glycosidic bond and removal of a molecule of mannose. ⑤ Isoacteoside could be isomerized from verbascoside, and its relative content increased with the extension of heating time. However, the relative content of verbascoside and purpureaside C did not decrease significantly. Therefore, it was hypothesized that purpureaside C was produced from its upstream component. ConclusionThe juice prepared by crushing fresh Rehmanniae Radix has the chemical composition significantly different from the juice samples prepared with the other 3 methods, while the latter 3 juice samples had similar chemical composition. Although all the four methods can be used, it is more suitable to prepare Rehmanniae Radix juice by steaming fresh Rehmanniae Radix, boiling fresh Rehmanniae Radix, and boiling dry Rehmanniae Radix.
10.A survey of loss function of medical image segmentation algorithms.

Medical image segmentation based on deep learning has become a powerful tool in the field of medical image processing. Due to the special nature of medical images, image segmentation algorithms based on deep learning face problems such as sample imbalance, edge blur, false positive, false negative, etc. In view of these problems, researchers mostly improve the network structure, but rarely improve from the unstructured aspect. The loss function is an important part of the segmentation method based on deep learning. The improvement of the loss function can improve the segmentation effect of the network from the root, and the loss function is independent of the network structure, which can be used in various network models and segmentation tasks in plug and play. Starting from the difficulties in medical image segmentation, this paper first introduces the loss function and improvement strategies to solve the problems of sample imbalance, edge blur, false positive and false negative. Then the difficulties encountered in the improvement of the current loss function are analyzed. Finally, the future research directions are prospected. This paper provides a reference for the reasonable selection, improvement or innovation of loss function, and guides the direction for the follow-up research of loss function.
