1.Therapeutic effect of modified S-P approach without disconnecting the rectus femoris in patients with Pipkin typeⅠandⅡfractures

Objective To investigate the therapeutic effect of modified S-P approach without disconnecting the rectus femoris in patients with Pipkin typeⅠandⅡfractures. Methods The clinical data of 58 patients with Pipkin type Ⅰ and Ⅱfractures were retrospectively analyzed. Among the patients, 29 patients were treated with conventional S-P approach (conventional group), and 29 patients were treated with modified S-P approach (modified group). The incision length, operation time, bleeding, visual analogue score (VAS), postoperative complications and hip joint function at the final follow-up were compared between 2 groups. Results The patients of 2 groups had not incision infection, and showed primary. X-ray of 3 months after surgery showed fracture healing. There was no statistical difference in the time of follow-up between conventional group and modified group:(16.1 ± 5.2) months vs. (15.8 ± 5.6) months, P>0.05. The incision length, bleeding, VAS 3 d and 4 weeks after surgery in modified group were significantly lower than that in conventional group:(8.1 ± 0.8) cm vs. (12.3 ± 1.2) cm, (144.3 ± 16.5) ml vs. (172.4 ± 21.6) ml, (4.5 ± 1.1) scores vs. (5.9 ± 1.5) scores and (1.6 ± 0.7) scores vs. (2.4 ± 0.8) scores, and there were statistical differences (P<0.05). There were no statistical differences in operation time, VAS 3 months after surgery, incidence of postoperative complications and excellent and good rate of hip joint function at the final follow-up between 2 groups (P>0.05). Conclusions The modified S-P approach without disconnecting the rectus femoris is a better method in patients with Pipkin type ⅠandⅡ fractures, and it has the advantages of revealing more clearly, minimal invasion, lower bleeding and slight postoperative pain.
3. Application of ultrasmall superparamagnetic iron oxide in MRI diagnosis of central nervous system: Recent progress

Ultrasmall superparamagnetic iron oxide (USPIO), as the contrast agent of MRI, possesses two major properties: long half time in the plasma and specific binding with macrophages. Compared with gadolinium, widely-used in clinic presently, USPIO has its unique advantages in diagnosis of central nervous system diseases, though which still need further clinical verification. This article reviews the application of USPIO in MRI diagnosis of some central nervous system diseases.
4.Dopaminergic neuroprotection of Poacynum hendersonni in MPTP mouse

Aim To study the dopaminergic neuroprotection of quercetin-3-O-sophoroside and total flavonoids derived from Poacynum hendersonni in MPTP-induced mice model of Parkinson's disease.Methods To inject MPTP intraperitoneal to C57/bl6 mouse at 30 mg·kg~(-1)·d~(-1) 5 days for PD model preparation, qercetin-3-O-sophoroside(25 mg·kg~(-1))and total flavonoids(250 mg·kg~(-1))were treated through intragastric administration 7 days before.Watching the mice behavior change through autonomic activities and pole test after final treatment, dopamine content in striatum was determined with fluorescence photometer.The neurons pathological change in substantia nigra(SN)was observed by pafaffin slice and immunohistochemistry staining to analyze the dopaminergic neuroprotective effects of Poacynum hendersonni.Results Treatment of quercetin-3-O-sophoroside for 7 days in advance, can improve PD mice motivation and dopamine concentration in striatum, tyrosine hydroxylase immune-reactive neurons in SN increased as well.Conclusion Quercetin-3-O-sophoroside can attenuate the impairment of MPTP in mice model of PD.
5.Brief analysis on liver depression and herpes zoster

According to years of treatment experience on herpes zoster,combined with its current onset characteristics as well as modem medicine,the authors probed into the relation between liver depression and herpes zoster on the following two aspects:one was liver depression and healthy qi (immunity) ; the other was liver depression and the incidence of herpes zoster.We considered that long-term liver depression impairs healthy qi.Then herpes zoster appears because detriment of healthy qi leading to decreased immunity,reactivation and reproduction of virus.Liver depression is a significant factor for the onset of herpes zoster.
7.Changes of immunological functions in the aged patients with bacterial pneumonia

Objective In order to provide scientific evidence for revealing the pathophysiological changes, levels of the humoral and cellular immunity and cytokines in elderly patients with bacterial pneumonia were determined. Methods The changes of immunological markers including IgA, IgG, IgM,C3, C4,T cell subpopulation (CD3 +, CD4 +, CD8 +) and CD4 +/CD8 +, IL 2, IL 6, IL 8, TNF ? were examined for the elderly bacterial pneumonia group(BPG), the aged control group(ACG) and the young control group(YCG). Results The level of CD3 +〔(53 90?7 10)% vs (62 67?6 08)%〕, CD4 +〔(33 85?4 69)% vs (3 57?3 40)%〕, IL 2〔(4 09?0 69) ?g/L vs (6 72?0 52) ?g/L〕 and IL 8〔(0 28?0 07) ?g/L vs (0 39?0 09) ?g/L〕 in ACG were significantly lower than those in YCG. The levels of IgA, IgG, IgM, C3, CD8 +, IL 8〔(0 33?0 08) ?g/L vs (0 28?0 07) ?g/L〕, and TNF ?〔(1 25?0 23) ?g/L vs (1 10?0 20) ?g/L〕in BPG were significantly higher than those in ACG; and in BPG the levels of IgG, IgM, C3, IL 8 and TNF ? were significantly higher than those in YCG. However, the levels of CD3 +, CD4 + and the ratio CD4 +/CD8 +in BPG were markedly lower than those in YCG;the IL 2(2 730?0 612 ?g/L) was much lower than that of ACG〔(6 72?0 52) ?g/L〕 and YCG〔(4 09?0 69) ?g/L〕, respectively. Conclusions The occurrence and the progress of bacterial pneumonia in the elderly are related to the immunological functioning including the excessive humoral immunity, the super activation of complement system, the dysequilibrium of T cellular immunoloregulation network and the super excretion of inflammatory cytokines.
8.Application of Laparoscopy in Diagnosis and Treatment of Neonates and Infants under 6 Months Age:A report of 297 cases

Objective To explore the safety and feasibility of laparoscopy in diagnosis and treatment of neonates and infants diseases.Methods Laparoscopic operations were performed in 297 neonates and infants aging from 2 hours to 6 months,from January 2001 to May 2007.The diseases of 297 cases included 80 cases of congenital hypertrophic pyloric stenosis,67 cases of obstructive jaundice,35 cases of indirect inguinal hernia,31 cases of biliary atresia,21 cases of middle or high anal atresia,21 cases of biliary hyperviscosity,7 cases of congenital diaphragmatic hernia,7 cases of jejunal atresia,6 cases of choledochal cyst,5 cases of Hirschsprung's disease,5 cases of duodenal web,3 cases of uronephrosis,2 cases of esophageal hernia,2 cases of esophageal atresia,2 cases of intestinal malrotation,1 case of intestinal obstruction,1 case of intussuception,,and 1 case of congenital muscular troticollis.Results The laparoscopic operations were completed in 294 cases,of which 67 received diagnostic cholangiography,80 received pyloromyctomy,35 received hernia sac high ligation,21 received biliary irrigation,5 received operations for membranous stenosis of duodenum,2 received Ladd operations for malrotation of intestine,1 received reduction of intussusception,1 received enterolysis,and 1 received incision of lower end of sternocleidomastoid muscle;and some complicated procedures including 31 cases of hepatic portal duct-jejunostomy,21 anoplasty,7 repair of diaphragmatic hernia,7 enterectomy and entero-anastomosis of intestinal atresia,6 hepatic duct-jejunostomy,5 radical surgery for Hirschsprung's disease(megadolicho-colon),3 pyeloplasty,2 operations for esophageal atresia,and 2 fundoplication for hiatus hernia.No massive hemorrhage,intra-or post-operative blood infusion,accidental damage or peri-operative death was reported.Three cases of biliary atresia were converted to open surgery due to oozing of blood in hepatic portal.All case were followed for 2 months-6 years,and no long-term or short-term complication was found.Conclusions Laparoscopic technique is safe and reliable in the diagnosis and treatment of diseases in neonates and infants,which is characterized by minimal invasion,quick recovery and satisfactory cosmetic effects.
9.Atrial fibrillation originated from superior vena cava and its ablation

Objective To investigate the clinical and electrophysiological characters of atrial fibrillation(AF) originated from superior vena cava(SVC).Methods Patients with SVC-originated AF admitted into our centre during the past 2 years were retrospectively investigated,and their clinical and electrophysiological characters were systematically analyzed.Results The AF in 16 patients(M/F=12/4 cases,mean age of 53.4?10.6 years old,paroxysmal/persistent AF=11/5 cases) were confirmed to be originated from SVC,who contributed to 2.9%(16/545) of total cases underwent AF ablation during the same period.Among the 16 cases,2 patients were diagnosed as SVC-originated AF by surface ECG,and isolation of SVC terminated AF in these 2 patients.In the remaining 14 patients with non-classic surface ECG,AF or organized atrial tachycardia(Ata) still sustained after initial left atrial(LA) ablation.The sustainable AF and Ata were found to be driven by fast activations within SVC,and were terminated by isolation of SVC.The average applications and procedure time for isolating SVC were 6?2 times and 10?3 minutes respectively.The average muscle connections between SVC and LA were 3?1.After a mean period of 6-month-follow-up,only one patient recurred with organized Ata.Except femoral hematoma in one patient,no other complications were found.Conclusion In addition to pulmonary vein and LA,SVC could be the orgin of AF.Therefore,for patient with sustainable AF or organized Ata after initial LA ablation,SVC origin should be put under consideration.
10.In vitro effects of propranolol and isoproterenol on the expression of beta-2 adrenergic receptor on infantile hemangioma endothelial cells

Objective To explore the role of beta-2 adrenergic receptor in the pathogenesis of infantile hemangioma.Methods In vitro cultured infantile hemangioma endothelial cells were divided into propranolol and isoproterenol groups.The propranolol groups were further classified into 5 groups to be treated with propranolol solutions at concentrations of 10,15,20 μg/ml,EGM-2 medium (blank control group 1),and EGM-2 medium containing 0.16% DMSO (DMSO group) respectively,while the isoproterenol groups were classified into 4 groups to be treated with isoproterenol solutions at concentrations of 5,10,20 μg/ml and EGM-2 medium (blank control group 2) respectively.After 24-and 48-hour treatment,enzyme-linked immunosorbent assay (ELISA) was performed to measure the expression of beta-2 adrenergic receptor in cell culture supernatants in the above groups.Results After 24-hour treatment,15-μg/ml and 20-μg/ml propranolol groups showed significantly decreased expression of beta-2 adrenergic receptor compared with blank control group 1 and DMSO group (all P < 0.05).After 48-hour treatment,all the propranolol groups showed significantly decreased expression of beta-2 adrenergic receptor compared with the blank control group 1 (all P < 0.05).However,the expression of beta-2 adrenergic receptor was significantly higher in the 10-and 20-μg/ml isoproterenol groups than in the blank control group 2 after 24-hour treatment (all P < 0.05),and higher in the 20-μg/ml isoproterenol group than in the blank control group 2 after 48-hour treatment (P < 0.05).Conclusion Propranolol can down-regulate the expression of beta-2 adrenergic receptor on the surface of vascular endothelial cells,while isoproterenol can up-regulate its expression.
