Objective To analyze the clinical value of 24 h dynamic electrocardiogram (Holter) for cardiogenic syncope caused by arrhythmia and myocardial ischemia.Methods A total of 134 patients from February 2010 to October 2012 diagnosed syncope who received Holter and coronary arteriography (CAG) or electrophysiological examination were enrolled in this study.The daily record and the time of syncope were recorded exactly.Results Seventy-eight patients had final diagnosis by CAG and electrophysiological examination,and 59 patients were diagnosed by Holter.The true positive was 42 cases,the true negative was 39 cases,the false positive was 17 cases and the false negative was 36 cases.Thus,the sensitivity,specificity,positive predictive value and negative predictive value were 53.85％ (42/78),69.64％ (39/56),71.19％ (42/59) and 52.00％(39/75).Conclusions Holter is a very important diagnostic method for cardiogenic syncope caused by arrhythmia and myocardial ischemia.Repeated and multiple examination can improve the diagnostic yield.

BACKGROUND: Diabetic cardiomyopathy is closely related to the high incident rate of diabetic heart failure. It is extremely significant to set up the successful dietetic cardiomyopathy model for the study on improving cardiac function.OBJECTIVE: To understand the influence of high blood sugar on myocardium by setting up the model of type 2 diabetes and diabetic cardiomyopathy.DESIGN: Completely randomized design, randomized and controlled study,in which, the experimental animals were taken as the objects.SETTING: Department of endocrinology of a university.METHODS: In the experiment, 99 newly born Wistar male rats were employed, randomized into experiment group (75 rats) and the control group(24 rats). In the control group, abdominal injection was done with sodium citrate buffer and the single abdominal injection was done with strptozocin(STZ)90 mg/kg in the experiment group. Ten weeks later, the glucose tolerance was assayed to determine the set-up of diabetes model. The ultrastructure of myocardium of rats was tested under electronic microscope after being sacrificed.MAIN OUTCOME MEASURES: To observer whether the myofibrillae and mitochondria of myocardiocytes alter or not in diabetic rats.RESULTS: In the experiment group, the fasting blood sugar[(8.87± 0.76)mmol/L] was slightly higher than that[ (5.88 ± 0. 45)mmol/L] in the control group, the blood sugar 2 hours after meal was remarkably higher than that in the control group and the glucose tolerance was abnormal. Under the electronic microscope, the myofibrillae presented focal degeneration and disarranged order of myofilament; mitochondria of myocardiocytes presented swelling and focal proliferation.CONCLUSION: This model is the satisfactory carrier for the study on diabetes Ⅱ and its myocardiopathy, which lays an experimental foundation for the study on cardiac function of type 2 diabetes.

Objective To study the effects of erythropoietin(EPO) and angiotensin receptor blocker on the expression of eNOS and Bcl-2 after cerebral ischemia.Methods The model of focal cerebral ischemia was made by occluding middle cerebral artery(MCA) for 2h and reperfusing for 48h in healthy Sprague-Dawley rats.The rats received EPO 3 000u · kg-1 · d-1 or valsartan 40mg · kg-1 · d-1 by intraperitoneal injection just before the beginning of reperfusion and after reperfusion.The expression of eNOS and Bcl-2 was detected.Results Compared with ischemia group,Bcl-2 and eNOS expressions in EPO-treated group and EPO-ARB treated group were significantly increased (P ＜ 0.05).eNOS expression in ARB-treated group was significantly increased (P ＜ 0.05),Bcl-2 expression was up-regulated,but it had no significant difference.Conclusion EPO had protective effects on cerebral ischemia-reperfu-sion injury in rats,which partially mediated by up-regulating of Bcl-2 and eNOS expression.Angiotensin receptor blocker had protective effects on cerebral ischemia-reperfusion injury in rats,which partially mediated by up-regulating of eNOS expression.

Objective To explore the incidence of G-6-PD deficiency in Dongguan District,Dongguan,Guangdong province. Methods Cord blood was obtained in 9676 cases of live births. G6PD/6PGD ratio was measured. Results Two hundred and sixty-five cases were diagnosed as G-6-PD deficiency giving an incidence of 2.74%. The incidence in male (4.07%,230/5646) was significantly higher than that in female (0.875,35/4030) ( P

Objective To explore the influence of monosialoglycans ganglioside(GM1) on dynamic changes of plasma BDNF and NSE in the treatment of neonatal hypoxic-ischemic encephalopathy(HIE).Methods According to the digital table,82 children with HIE were randomly divided into study group and control group, 41 cases in each group.The study group was given GM1 treatment,the control group was given routine treatment.The BDNF and NSE levels after birth 1,7,14 d of the two groups were detected.Results The BDNF levels in the study group at 7,14 days were higher than those in the control group[(1 823.3±286.2)pg/mL vs (1 368.3±229.2)pg/mL,t=3.07,P<0.01;(958.3±89.3)pg/mL vs (863.2±88.6)pg/mL,t=2.03,P<0.05].The NSE levels of moderate and severe children in the study group at 7 days were lower than those in the control group[(9.55±3.32)ng/L vs (14.31±2.52)ng/L,(20.22±3.63)ng/L vs (32.05±5.73)ng/L,t=5.32,4.69,all P<0.05].The incidence rate of complication in the study group was 4.88%,which was lower than 19.51% in the control group,the difference was statistically significant(x2=4.10,P<0.05).Conclusion GM1 in the treatment of HIE can achieve significant effect,and the plasma BDNF and NSE levels increase and decrease rapidly.

AIM: To establish HPLC fingerprint of Ramulus cinnamomi.This fingerprint was expected to be standards of quality control and identification of the Chinese crude drug. METHODS: The HPLC method was used,chromatography condition were Kromasil C_(18)(250 mm?4.6 mm,5?m) column with gradual mobile phase of acetonitrile-0.5% acetic acid,UV detection wavelength at 280 nm and column temperature at 25(?C) with the flow rate of 1 ml?min~(-1). RESULTS: The retention time of Cinnamaldehyder,Cinnamic acid,o-Methoxycinnamic acid and Coumarin in Ramulus cinnamomi were determined.The fingerprint showed high similitude in samples from different regions. CONCLUSION: The HPLC fingerprint of Ramulus cinnamomi.can be used as a standard of quality control and identification of the Chinese crude drug.

Objective To establish an HPLC method for determination of simvastatin and its related substances.Methods The HPLC system consisted Luna C18 column(250mm×4.6mm,5μm),the mobile phase was 0.025mol/L sodium phosphate monobasic(pH 4.5):acetonitrile(35:65).The flow rate wns 1.0ml/min,and with detection 238nm.The column temperature was room temperature;injeetion volume WaS 20μl.Results SimvaStatin and its related substances can be completely separated by the method with a linear range of 2～200μg/ml.Calibration culwe was linear over the range of Y=3.256C×103+4.510×102(r=0.9996);for potency assay the mean recovery rate was 99.32% with RSD as 0.31%(n=9).Conclusion The method has good reproducibility,high accuracy and strong specificity.It was suitable for determination of simvastatin and its related substances.

Objective To evaluatate the impact of different anastomotic location and preventive ileostomy on postoperative anastomotic fistula in rectal cancer patients.Methods The clinical data of 316 cases of rectal cancer patients after total mesorectal excision from January 2015 to January 2016 were analyzed retrospectively.Patients were divided into anterior resection (AR) group,low anterior resection (LAR) group,uhra-low anterior resection (ULAR) group according to the anastomotic location.There were 33 cases of anastomotic fistula with preventive ileostomy or without and anastomotic leakage varied from mild to severe as grade A,B,C.Results There was no statistically significant difference in the incidence of anastomotic fistula between the different anastomotic location (x2 =3.829,P =0.147).In anterior resection group and low anterior resection group,there was no statistically significant difference in the incidence of anastomotic fistula whether or not a preventive ileostomy was performed (x2 =0.606,1.096,P =0.436,0.326).While in uhralow anterior resection group,preventive ileostomy helped dicrease the incidence of anastomotic fistula (x2 =11.667,P =0.001),and once happened the severity of the anastomotic fistula tended to be less severe.Conclusion The incidence of anastomotic fistula does not vary with anastomotic level in our study.Preventive ileostomy reduces the incidence of anastomotic fistula in patients with ultralow anterior resection.
