Objective: Accurate evaluation of inflammation severity in ulcerative colitis (UC) can guide treatment strategy selection. The potential value of the pericolic fat attenuation index (FAI) on CT as an indicator of disease severity remains unknown.This study aimed to assess the diagnostic accuracy of pericolic FAI in predicting UC severity. Materials and Methods: This retrospective study enrolled 148 patients (mean age 48 years; 87 males). The fat attenuation on CT was measured in four different locations: the mesocolic vascular side (MS) and opposite side of MS (OMS) around the most severe bowel lesion, the retroperitoneal space (RS), and the subcutaneous area. The fat attenuation indices (FAI MS, FAI OMS, and FAI RS) were calculated as the fat attenuation measured in MS, OMS, and RS, respectively, minus that of the subcutaneous area, and were obtained in the non-enhanced, arterial, and delayed phases. Correlations between the FAI and UC Endoscopic Index of Severity (UCEIS) were assessed using Spearman’s correlation. Predictors of severe UC (UCEIS ≥7) were selected by univariable analysis. The performance of FAI in predicting severe UC was evaluated using the area under the receiver operating characteristic curve (AUC). Results: The FAIMS and FAI OMS scores were significantly higher than FAI RS in three phases (all P < 0.001). The FAIMS and FAI OMS scores moderately correlated with the UCEIS score (r = 0.474–0.649 among the three phases). Additionally, FAI MS and FAI OMS identified severe UC, with AUC varying from 0.77 to 0.85. Conclusion Increased CT attenuation of pericolic adipose tissue could serve as a noninvasive marker for evaluating UC severity. FAI MS and FAI OMS of three phases showed similar prediction accuracies for severe UC identification.

Objective:To evaluate the effect of lung recruitment maneuvers combined with individualized positive end-expiratory pressure(PEEP) on the degree of postoperative atelectasis in elderly patients undergoing laparoscopic surgery.Methods:One hundred and forty-three elderly patients, aged ≥65 yr, with body mass index of 18.5-30.0 kg/m 2, scheduled for elective laparoscopic surgery, were assigned to either individualized PEEP combined with recruitment maneuvers (group Ⅱ) or fixed PEEP (group Ⅰ) using a random number table method. PEEP was maintained at 6 cmH 2O starting from the beginning of procedure until the end of the procedure in group I. Individualized PEEP titration was performed after induction of anesthesia in group Ⅱ. The primary outcome measure was the 12-zone lung ultrasound score at 15 min after tracheal extubation. Other outcome measures were the occurrence of postoperative pulmonary complications within 7 days after surgery, Quality of Recovery-15 scale score on 3rd day after surgery, rate of unplanned admission to intensive care units, length of hospital stay, incidence of intraoperative hypoxemia, usage rate of intraoperative vasoactive drugs, and incidence of postoperative hypotension. Results:Compared with group Ⅰ, the lung ultrasound score, driving pressure and postoperative pulmonary complications were significantly decreased, the dynamic lung compliance was increased ( P<0.05 or 0.01), and no significant changes were found in the other parameters in group Ⅱ ( P>0.05). Conclusions:Individualized PEEP combined with recruitment maneuvers can reduce the degree of postoperative atelectasis in elderly patients undergoing laparoscopic surgery.

Objective:To evaluate the predictive value of preoperative frailty combined with nutritional status for prolonged postoperative ileus (PPOI) in the patients with gynecological malignancies.Methods:Patients undergoing elective surgery for gynecological malignancies in the Affiliated Hospital of Jiangnan University from April 2022 to February 2023 were selected. The Frail scale was used to evaluate the frailty within 24 h of admission, and the nutritional status was evaluated by the Controlling Nutritional Status score. The general characteristics of patients and occurrence of PPOI were recorded, and the risk factors for PPOI were analyzed by multivariate logistic regression. The ability of frailty, nutritional status and their combination to predict PPOI was assessed by the receiver operating characteristic curve.Results:Two hundred and fourteen patients were finally included, 52 cases developed of PPOI, and 98 cases were frail patients. Preoperative frailty combined with moderate to severe malnutrition was an independent risk factor for PPOI in the patients with gynecological malignancies ( P<0.05), and the area under the curve in predicting the occurrence of PPOI was 0.796 (95% confidence interval 0.736-0.857) in the patients with gynecological malignancies. Conclusions:Preoperative frailty combined with moderate to severe malnutrition has a higher accuracy in predicting PPOI in the patients with gynecological malignancies.

Objective To compare the value of standard coronal MRI and multi-planar reconstruction(MPR)images for evaluation of anterolateral ligament(ALL).Methods Data of 130 patients who underwent knee joint MR examination were retrospectively analyzed,including standard coronal MRI and MPR images.ALL were identified on standard coronal MRI and MPR images and classified as fully visible,partially visible or invisible.The visibility of bilateral ALL on both standard coronal MRI and MPR images were compared,while Kappa test was used to evaluate the consistency on both kinds of images.Results Among 130 cases,on standard coronal MRI and MPR images,the left ALL was fully visible in 83 and 93 cases,partially visible in 21 and 12 cases but invisible cases in 26 and 25 cases,respectively,while the right side ALL was fully visible in 66 and 80 cases,partially visible cases in 29 and 15 cases but invisible cases each in 35 cases,respectively.Significant difference of visibility of bilateral ALL were found between standard coronal MRI and MPR images(both P＜0.05),both with excellent consistency(both Kappa＞0.80).Conclusion MPR could display bilateral ALL better than standard coronal MRI.If the scanning conditions for MPR could not be met,standard coronal MRI might be used to evaluate ALL rather accurately.

Purpose This study aims to observe the hemodynamic state of the carotid artery in healthy volunteers and diabetes patients via ultrasound vector flow imaging(V-Flow),and to quantitatively analyze the characterization of wall shear stress(WSS)in different degrees of atherosclerosis carotid artery.Materials and Methods This was a cross-sectional study.Forty four diabetic patients diagnosed in Sichuan Provincial People's Hospital from April 2019 to June 2019 were retrospectively analyzed.Routine carotid ultrasound examination and V-Flow imaging were performed on 44 diabetic patients(88 vessels)and 15 controls(30 vessels).According to the results of carotid ultrasound examination,diabetic patients were divided into normal intima-media thickness(IMT)group,IMT thickening group and plaque group.The differences of WSS in different parts of carotid artery between diabetic patients and controls were observed,and the changes of WSS in different groups of diabetic patients were further compared and analyzed.Results In this study,the WSS(including WSSmax and WSSmean)of the bifurcation and middle segment of the common carotid artery in diabetic patients were significantly lower than those in the normal control group(P=0.001,P=0.003).WSS at the bifurcation of common carotid artery was significantly smaller than that at the middle segment of common carotid artery in both normal control group and diabetic group(P=0.001).WSS at the bifurcation of common carotid artery in diabetic patients was changed with the severity of atherosclerosis(P=0.018 and P=0.043),and was highest in the plaque group[WSSmax(2.58±0.69)Pa;WSSmean(0.86±0.25)Pa],followed by IMT thickening group[(2.37±0.69)Pa;(0.77±0.35)Pa].The WSS was lowest in the normal IMT group[(2.13±0.52)Pa;(0.69±0.20)Pa].There was no significant difference in WSS of the middle segment of common carotid artery among the three groups(P>0.05).Conclusion The WSS in the bifurcation and middle section of common carotid artery in diabetic patients is lower than that in controls,and it changed with the increase of atherosclerosis degree.Therefore,the V-Flow technique of ultrasonic vector flow imaging has a certain value in evaluating the progression of carotid atherosclerosis in diabetic patients.

Abstract: As a Th17 cell-specific transcription factor, retinoic acid receptor-related orphan receptor γt (RORγt), can induce differentiation of Th17 cells and production of inflammatory factor IL-17, playing an important role in inflammation and autoimmune diseases. RORγt inverse agonists have become a research hotspot in both academia and pharmaceutical companies around the world in recent years, with great development potential. A variety of skeletal structure types have been reported, including orthosteric and allosteric inverse agonists. In this paper, the structure and functions of RORγt are introduced, and RORγt inverse agonists in clinical and preclinical studies are reviewed in order to provide reference for further research and development of RORγt inverse agonists.
