1.Clinical study of modifiedXiaochengqi decoction in the treatment of gastroparesis syndrome

Objective To evaluate the efficacy of modifiedXiaochengqi decoction in treatment of gastroparesis syndrome (GS).Methods A total of 81 patients with GS from 2010 July to 2013 July in HuiZhou Municipal Central Hospital were selected and randomly recruited into a control group and a treatment group. The control group was treated with domperidone, 15-30 min before the meal, while the treatment group was additionally treated with modifiedXiaochengqi decoction on the basis of the control group. Both groups were treated for 4 weeks. The clinical curative effect, and the symptoms in both groups were observed. The serum motilin and gastrin concentration were measured by radio-immunity, the preprandial and postprandial electrogastrography was performed.Results The total effective rate in the treatment group was significantly higher than that in the control group (72.5%vs. 31.7%;χ2=11.911,P=0.000). The serum levels of motilin (treatment group: 532.8±16.7 ng/Lvs. 355.2±14.3 ng/L,t=51.089,P=0.000; control group: 505.1 ± 21.3 ng/Lvs. 372.9 ± 18.4 ng/L,t=30.074,P=0.000) and gastrin (treatment group: 69.8 ± 15.3 ng/Lvs. 54.3 ± 13.8 ng/L, t=4.758,P=0.000; control group: 62.6 ± 14.2 ng/Lvs. 53.4 ± 12.3 ng/L,t=3.136,P=0.002) were significantly reduced, and their decrease in the treatment group were greater than those in the control group (t values were 6.503, 2.196, allP<0.05). After the treatment, the dominant power (preprandial period: 84.5 ± 14.8μVvs. 72.3 ± 15.0μV, t=3.684,P<0.01; postprandial period: 100.5 ± 17.3μVvs.89.3 ± 15.9μV,t=5.713,P=0.000) and dominant frequency (preprandial period: 4.3 ± 0.4 cpmvs. 3.9 ± 0.2 cpm,t=3.035,P=0.000; postprandial period: 4.7 ± 0.5 cpmvs. 4.4 ± 0.5 cpm,t=2.700,P=0.000) were significantly higher, while the percentage of bradygastria (preprandial period: 50.6% ± 16.6%vs. 60.1% ± 16.3%,t=2.599,P=0.000;postprandial period: 36.6% ± 14.8%vs. 57.4% ± 15.3%,t=6.217, P=0.000) was significantly lower in the treatment group compared with the control group at preprandial and postprandial periods.ConclusionXiaochengqi decoction can obviously promote the gastric motility, and reduce the serum levels of motilin and gastrin in patients with GS.
2.Analysis of therapeutic outcomes of polycystic ovary syndrome patients with hyperprolactinemia

Objective To study effect of drug treatment in polycystic ovary syndrome patients with hyperprolactinemia.Methods We retrospectively studied 50 women with polycystic ovary syndrome and hyperprolactinemia from the outpatient between January 2005 and April 2008.Acccording to the beginning time of bmmocriptine.all women were divided into two groups.Groups Ⅰ was composed of 38 cases who received bromocriptine before induction of ovulation cycies,and the dose of bromocriptine was modulated depending on the level of serum prolaotin.When serum prolactin was controlled at normal levels,we decreased the dosage of bromocriptine step by step(1.25mg once),and then continued the treatment at maintenance dosage for no less than 3 weeks.After a baseline ultrasonographic examination on day 3,patients were treated with clomiphene citrate at a dosage of 100mg(2 tablets/day)for 5 days of a normal cycle or progesterone-induced bleeding.On day 9.we monitored the growth conditions of follicles routinely with trails-vagihal ultrasound.If there was no dominant follicle,we added human menopausal hormone(HMG 75U/d)to the protocol.Human chorionic gonadotropin(HCG,5 000-10 000IU)was given intramuscularly when the mean diameter of a follicle reached at least 18mm.At the same time we iustmcted the patients to have sexual intercourses or carried out artificial inseminations before and after ovulation.Group Ⅱ were 12 cases in which induction of ovulations were commenced almost simultaneously with beginning of bromeoriptine.The same protucol was given to patients in group Ⅱ.The procedures of ovulation induction and the outcomes of treatment were analyzed and compared.Results Compared with groupⅡ.the days of using HMG in group Ⅰ was shorter by instructing the time of sexual intercourse.The difference was significant(P=0.004).And there were similar rosults in the artificial insemination cycles(P=0.009).The rate of pregnancy in group Ⅰ(42% 16/38)was higher than that in group Ⅱ(25%,3/12),but the difference was not obvious.Conclusion Bromocriptine administration before the stimulated ovulation therapy can decrease the total dosage and treatment course of ovulating drugs.Induction of ovulations simultaneously with start of bromocriptine therapy can shorten the treatment time of infertility.
3.Value of orbital division and imaging features in the diagnosis of orbital space-occupying lesions

Objective To investigate the value of the imaging division and features of CT and MRI in the differential diagnosis of benign and malignant space-occupying lesions.Methods A retrospective analysis of the clinical and imaging data of 201 patients with orbital space-occupying lesions confirmed by pathology was conducted.Four sector method was used to analyze the distribution of the lesions.The shape,border and relationship with surrounding tissues,MRI signal,CT density,and calcification of the orbital wall were also analyzed.Results There was statistical difference in the distribution of lesions in each of the 4-sector between orbital benign lesions and malignant tumors(χ2总=48.93,P=0.000).Lesions located in separate orbital zone including extra-periosteum compartment (18 cases),optic nerve compartment (4 cases),intraconal compartment(39 cases) were benign.Benign lesions (64%, 66/103) were more than malignant lesions (36%,37/103) in 103 lesions located in extraconal compartment.Additionally,there was significance in the distribution in separate and mixed zones between benign and malignant lesions(χ2=25.559,P=0.000).Benign lesions (77%,127/164) were more than malicious lesions(23%,37/164) in 164 cases in separate zone.Malignant lesions (65%,24/37) were more than benignant lesions (35%,13/37) in the 37 lesions located in mixed zone.Signs of CT and MRI:the benign and malignant lesions were 140 and 61,respectively.The features of those lesions are as follows(ratio=benign lesions/ malignant lesions):regular morphology(ratio=93/10),clear boundary(ratio=108/24),eyeball-surrounded (ratio=24/39),bone destruction (ratio=4/11).Features of lesions including shape,border,eyeball-surrounded, bone destruction had significant difference between benign and malignant lesions,whereas no differences was observed in calcification,signal on T1WI or T2WI,displacement of the eyeball and the occurrence of single or bilateral lesions.Conclusion The characteristic distribution of orbital space-occupying lesions based on the 4-sector and the features of CT and MRI has a great value in the diagnosis.
4.Mutation detection in the interaction domains of NRF2 and KEAP1 in lung cancer cell lines

Objective To detect the interaction domains' gene mutations of KEAP1 and NRF2 in non-small-cell lung cancer cell lines,and to analyze the significance of these mutations on the study of lung cancer cell lines.Methods Six non-small-cell lung cancer cell lines were used as the research materials.The 2nd to 6th exons of KEAP1 and the coding sequences of DLG and ETGE motif were amplified by PCR,and the products were used for gene sequencing analysis.Obtained gene sequences were analyzed using NCBI databases to get the base locations of gene mutations,as well as the subsequent amino acid sequence changes.Meanwhile,the relative intracellular reactive oxygen species (ROS) concentrations in the tested cell lines were detected and used in the analysis of abnormal NRF2 transcriptional activity in lung cancer cell.Results Four mutations were detected in the 4th exon of KEAP1 gene from all the 6 cancer cell lines,several other missense mutations were also investigated in the 3rd exon of KEAP1 from some cancer cell lines.Multiple genetic variations were found in all the NRF2-ETGE motif-encoding sequences of the 6 cancer cell lines.All 6 cancer cell lines were found to have higher ROS concentration than the lung germ cell line.Conclusion Lung cancer cells generally contain high levels of ROS as well as gene mutations in KEAP1 and NRF2 genes which lead to abnormal transcriptional activity of NRF2 in lung cancer cell lines.

The interaction between genetic and environmental factors influences the onset,development and prognosis of cerebral ischemia.Recently,epigenetic regulation is increasingly becoming a research focus on cerebral ischemia,and there has been great progress in research on DNA methylation,histone modification and miRNA network.Epigenetic regulation provides a new idea for identifying potential therapeutic targets of cerebral ischemia.
6.Role of p75 neurotrophin receptor in neuronal apoptosis after cerebral ischemia

p75 neurotrophin receptor (p75 NTR) is a member of the tumor necrosis factor receptor superfamily, which interacts with tropomyosin receptor kinase (Trk) receptor or binds neurotrophic factors. It mediates a variety of complex signal transduction pathways, induces synaptic growth and affects cell survival. After acute cerebral ischemia, p75 NTR binds effector factors such as pro-nerve growth factor (proNGF) and sortilin, activating downstream apoptotic signal molecules and leading to neuronal death. Therefore, elucidating the pathways and molecular mechanisms of p75 NTR that mediate neuronal apoptosis in acute cerebral ischemia is of great significance for the development of new therapeutic drugs for acute cerebral ischemia.
7.The study of visual pathway of gloucoma patients with DTI

Objective To study the microstructural characteristics of white matter fiber tracts of visual pathways using diffusion tensor imaging (DTI)in the patients with primary glaucoma.Methods Twenty patients with primary bilateral glaucoma and 30 nor-mal adults were recruited and scanned with visual pathways magnetic resonance imaging (MRI)and DTI,obtaining T1 FLAIR ima-ges and directionally encoded color (DEC)images.On DEC images,ROIs were put in the anterior,middle and posterior part of bi-lateral optic nerve,optic tract and optic radiation,to calculate the average FA and MD value of each region.Results T1 FLAIR and DEC images could clearly show the optic nerve,optic tract and optic radiation.In patients with glaucoma,the FA values of optic nerve,optic tract and optic radiation were:0.372±0.040,0.340±0.036,0.31 5±0.026,while the MD values were:1.760±0.1 1, 1.831±0.09,1.927±0.10.The FA values of optic nerve,optic tract and optic radiation of normal adults were:0.538 ±0.073, 0.460±0.082,0.455±0.083,whilst the MD values were:0.995 ±0.27,1.312 ±0.40,1.347 ±0.37.Compared with the normal controls,FA values of all parts are deceased whereas the MD values of all parts are increased in patients.Conclusion Glaucoma is a central nervous system disease involving the whole visual pathways.Use DTI can evaluate the microstructural characteristics of white matter fiber tracts of visual pathways in the patients with glaucoma.
8.Preparation of an Ag85A-CD226 DNA vaccine and evaluation of its immunogenicity in a mouse mod-el

Objective To construct a eukaryotic expression plasmid of pcDNA 3.1-Ag85A-CD226, and to use it as DNA vaccine then further evaluate its immunogenicity through oral administration in a mouse model.Methods The CD226-PCR2.1-ToPo plasmid was used as the template to clone CD 226 gene by PCR.The CD226 gene was then inserted into pcDNA 3.1-Ag85A plasmid to construct the recombinant plas-mid of pcDNA3.1-Ag85A-CD226.After identified by restriction enzyme analysis and sequencing , the re-combinant plasmid was transfected into HEK 293 cells by using lipofection .The expression of Ag85A-CD226 gene in HEK293 cells was detected by RT-PCR, Western blot and indirect immunofluorescence assay .The purified recombinant plasmid was used to prepare the Ag 85A-CD226 DNA vaccine by liposomal encapsula-tion.The vaccine was administered intragastrically to mice .The activities of NK cells , the cytokine levels in the supernatants of spleen cell cultures and the mRNA level of cytokines in the intestines were evaluated to analyze the immunogenicity of Ag85A-CD226 DNA vaccine.Results The Ag85A-CD226 DNA vaccine was prepared successfully .The expression of Ag85A-CD226 fusion protein was detected in HEK293 cells.The activities of NK cells from mice vaccinated with Ag 85A-CD226 DNA vaccine were higher than those from other control groups (P<0.01).The level of TNF-α, IFN-γand IL-2 in the supernatants of spleen cell cul-tures and in the intestines were significantly up-regulated in comparison with other control groups ( P <0.01).The level of IL-4 in the supernatants of spleen cell cultures was down-regulated in the experimental group (P<0.01), but the level of IL-4 in intestines showed no significant difference among the five groups (P>0.05).Conclusion The Ag85A-CD226 DNA vaccine could significantly enhance Th1 type immune responses systemically and in the intestine as in comparison with those vaccinated with single dose of Ag 85A DNA vaccine or CD226 DNA vaccine.
9.18F-FDG PET/CT features of pulmonary sclerosing pneumocytoma

Objective To investigate the characteristics of 18F-FDG PET/CT imaging in pulmonary sclerosis pneumocytoma (PSP).Methods The clinical and PET/CT data of 16 patients with pathologically proved PSP were retrospectively analyzed.The location,shape,size,internal and external edge of the lesion,as well as the metabolism of the lesions were observed.The mean retention index (RI) was calculated in 6 patients with 18F-FDG PET/CT dual phase imaging.The difference of SUVmax between early and delayed phase were compared.And the correlation between the diameter of lesions and SUVmax were analyzed.Results There were 16 lesions in all 16 patients,including 7 cases located at right lung and 9 located at left lung.The lesions were round with the diameter of (1.97-4-0.61)cm.The uniform density were observed with the CT value of (29.87±4.71)HU.And there was no cystic degeneration and necrosis.Calcification was found in 5 lesions.The edge of 14 lesions was smooth,and the edge of another 2 lesions showed short spicular sign.Two lesions showed visible edges of ground glass opacity.There were 12 lesions with vascular welt sign and 3 lesions with air crescent sign.The SUVmax value of PSP was 2.71 ± 2.13.There was no significant difference between the early SUVmax (2.44±1.57) and delayed SUVmax (2.74±1.83) in patients with dual phase imaging (t=2.09,P>0.05).RI was (7.23±10.29)%.There was no correlation between PSH diameter and SUVmax(r=0.188,P>0.05).Conclusion Most of PSP showed solitary pulmonary nodules in PET/CT imaging.The radioactive distribution was mild and moderate increase.The vascular welt sign,air crescent sign and the surrounding ground glass opacity are the references findings of PSP.
10.Genistein inhibits the time coursed expression changes of ceruloplasmin in rats of focal cerebral ischemia

Aim To define the effect of genistein (GST) on ceruloplasmin (CP) expression in cerebral cortex of focal cerebral ischemia male rats.Methods The effect of GST on CP expression were investigated by means of immunohistochemistry and Western blot in cerebral cortex of focal cerebral ischemia male rats.Results The expression of CP was time coursed during brain ischemia. According to the sham group,CP expression decreased in 6 h-,12 h-and 24 h-ischemia groups,and then increased gradually in 48 h-and 72 h-ischemia rats. GST weakened the ischemia-induced CP change.Pretreatment with an antiestrogen tamoxifen (TAM) could completely abolish the effect of GST.Conclusion The present results indicate that GST may inhibit the time coursed expression changes of CP and then provide neuroprotective effects by inhibiting iron related oxidative stress during brain ischemia via estrogen receptor.
