4.The clinical significance of DJ-1 over expression in pancreatic neuroendocrine neoplasms

Background and purpose:The symptoms of pancreatic neuroendocrine neoplasms (PNENs) are complicated, that might lead to misdiagnosis. This study aimed to detect the expression level of DJ-1 in PNENs and explore the clinical significance of DJ-1 in PNENs.Methods:DJ-1 protien levels in the serum of 16 cases of PNENs patients and 25 cases of healthy persons were detected by ELISA analysis; The expressions of DJ-1 in 78 cases of PNENs tissues were detected by immunohistochemical staining.Results:The DJ-1 level in the serum of PNENs patients was signiifcantly higher than healthy persons (36.19±6.71vs 24.68±5.94 ng/mL;P<0.001);53.8% (42/78) cases of PNENs tissues showed DJ-1 positive staining; DJ-1 expression level in PNENs tissues had signiifcantly positive correlation with lymph node metastasis (P=0.033), distant metastasis (P=0.017), TNM stage (P=0.012), and pathology grade (P=0.049); As a dependent risk factor, DJ-1 expression was signiifcantly associated with shorter OS (P=0.003) and DFS (P=0.018) of PNENs patients.Conclusion:High expression of DJ-1 protein correlated with invasion and metastasis, disease progression, and poor prognosis in PNEN.
5.PLAGL1 gene expression and methylation level and correlation with SDHB mutations in benign and malignant pheochromocytoma

Background and purpose:PLAGL1 gene is a newly discovered gene, which could induce tumor cells’ apoptosis and cell cycle arrest. This study aimed to investigate the expression and the promoter methylation level ofPLAGL1 gene and its correlation withSDHB gene mutations in pheochromocytoma.Methods:The expressions of PLAGL1 mRNA was detected by real-time quantitative reverse transcription polymerase chain reaction (qRT-PCR) method in 13 cases of normal adrenal medulla, 32 cases of benign pheochromocytoma group and 54 cases of malignant pheochromocytoma. We examined the promoter methylation status by methylation-speciifc polymerase chain reaction (MSP) andSDHB gene mutation by qRT-PCR ampliifcation and direct sequencing.Results:The relative expression volume ofPLAGL1 gene in malignant tumor tissue was (0.527±0.201), which was signiifcant lower than those in benigntumor tissue and normal adrenal medulla[(1.517±0.662) and (1.734±0.756)]. There was no remarkable difference between benign tumor tissue and normal tissue forPLAGL1 gene expression and promoter methylation.PLAGL1 gene methylation rate in malignant tumor tissue, benign tumor tissue and normal adrenal medulla were 68.5%, 18.8% and 15.4% respectively.SDHB gene mutation was detected in 7 malignant tumor tissues, while no mutation was found in benign and normal group.Conclusion:Lower expression status ofPLAGL1 gene showed signiifcant correlation with promoter methylation which may be associated withSDHB gene mutation.
6.Primary hepatic amyloidosis:report of 4 cases and reviews of the literature in and abroad

Objective To comprehend the clinical characteristics and treatment of primary hepatic amyloidosis.Methods A total of 10 cases of primary hepatic amyloidosis included. 4 cases from Peking Union Medical College Hospital and 6 cases from other hospitals.reported in the literature in recent 20 years were reviewed,Results (1)The frequent clinical symptoms related to primary hepatic amyloidosis presented as hepatomegaly(90%),abdominal distension(80%),shifting dullness(60%),anorexia(50%),fatigue(40%),edema(40%),weight loss(40%) and abdominal pain(30%).(2)Laboratory tests revealed elevated serum alkaline phosphatase which was (558.3?517.2)U/L,and other liver function involvement were rare:six patients(85.7%)had either a serum or urine monoclonal protein.(3)Liver biopsy is golden standard.The amyloid deposition was mainly located at sinusoida(4 casese,57.1%),and only 2 cases was at vascular(28.6%).Complication of bleeding after liver biopsy was reported(1 case).(4)The classical therapy scheme was melphalan and prednisone,four cases died from liver function failure and infection.Conclusion The clues to the diagnosis of primary hepatic amyloidosis include elevated monoclonal protein,hepatomegaly and an unexplained elevated serum alkaline phosphatase level which is not parallel with other liver functions.Live biopsy is golden standard.The treatment scheme include melphalan and prednisone,but its prognosis is poor.
7.Change in vascular endothelium and blood coagulation and fibrinolysis system induced by severe decompression sickness in New Zealand rabbits

Objective To observe the changes in the relative indexes of vascular endothelium, blood coagulation and fibrinolysis system of rabbits with experimental severe decompression sickness (DCS), and to compare the above indexes of surviving rabbits with those of dead in order to analyze the mechanism of causes of death. Methods 14 rabbits were put into a decompression chamber. They stayed for 35 min under an atmospheric pressure of 0.55 MPa, followed by a pressure of 0.7MPa for 35min. Then they were subjected to a pressure of 0.1MPa for 4 minutes. Blood samples were drawn before the compression, under high pressure, and after decompression to determine endothelin-1 (ET-1), von Willebrand factor (vWF), fibrinogen (FIB), D-Dimer, blood coagulation factor Ⅷ(FⅧ), plasminogen (PLG), plasmin inhibitor (PL-IN), prothrombin time (PT), and activated partial thromboplastin time (APTT). The changes of above parameters of surviving rabbits were compared with those of the dead. Results After rapid decompression, 8 rabbits died within 30 minutes, while the other 6 rabbits survived and all symptoms of decompression sickness disappeared 24 hours after decompression. The plasma level of ET-1 increased from 1.33?0.33pg/ml to 2.74?0.87pg/ml after a 30min stay under 0.55MPa, while the activity of vWF increased from 2.62?0.69% to 3.64?1.48%. Compared with the surviving rabbits, those dead rabbits showed significant reduction in FIB (0.92?0.12g/L) and D-Dimer (55.63?12.12ng/ml), after rapid decompression. Conclusions There was a release of vasoactive substance in the blood of rabbits during the period when they stayed under high pressure. After rapid decompression, the most important difference between the survivors and the dead was that there were stronger coagulation activation, more consumption of FIB and weaker secondary fabrinolysis in the latter compared with the survivors.

Objective To explore the relationship of traditional Chinese medical syndrome patterns with renal pathological changes and clinical laboratory parameters in patients with lupus nephritis.Methods Syndrome patterns were differentiated according the clinical manifestations.The data of laboratory examination and renal pathology were collected and analyzed in 40 patients with lupus nephritis.Results(1) Of the fundamental syndrome patterns,pattern of spleen and kidney Qi deficiency had higher activated partial thromboplastin time(APTT),erythrocyte sedimentation rate(ESR) and 24-hour urine protein amount than the pattern without spleen and kidney Qi deficiency.(2) Of the incidental syndrome patterns,water-damp pattern had higher APTT than damp-turbidity pattern;hemoglobin(HGB) level in water-damp pattern and damp-turbidity pattern was lower than the normal,and HGB level in water-damp pattern differed from that in blood-staiss pattern and damp-heat pattern(P
9.Change of marker and morphology of vascular endothelium induced by simulating diving condition

Objective: To investigate the change of functional marker and morphology of vascular endothelium induced by simulating diving condition. Methods:14 rabbit were put in the chamber. They stayed for 35 min under 0.55 MPa, and another 35 min under 0.7 MPa, then decompressed to 0.1 Mpa following a stage decompression schedule. Blood sample were draw at 30 min before compression and after decompression for testing endothelin-1(ET-1) and von Willebrand factor (vWF). These parameters were compared to see the change from pre-compression to post-decompression. The morphological change of vascular endothelium was observed under electrical microscope after decompression. Results: After decompression, plasma levels of ET-1 was increased from (1.33?0.23)ng/L to (2.99? 0.35)ng/L and activity of vWF was reduced from (2.35?0.47)% to (1.89?0.34)%. Swelling and defluvium of vascular endothelium was found under electrical microscope. Conclusion: Compression-decompression can cause the damage of vascular endothelium.
10.Detection of HBV DNA with full automatic system and HBV genotyping in blood donor screening

Objective To establish fully automated sample pooling, nucleic acid extraction, amplification and detection method for HBV DNA testing, and investigate the seroconversion and genotype in HBV DNA positive donors. Methods Individual donor plasma samples serologically negative for HBV were pooled by STAR2000 sampling processor with a size of 24. Nucleic acid were automatically extracted by MPLC simultaneously, amplified and detected by Roche COBAS AMPLICOR system. The sensitivity of detection was determined by international standard. HBV DNA positive donors were genotyped and followed up by serological tests. Results The 95% detection limit for this automated HBV DNA testing system was 38.9IU/ml,with 95% confidence interval (21323),eight out of 16512 specimens were PCR positive for HBV DNA,with a positive rate of 0.049%. Three of the 8 DNA positive donors were genotype C,2 genotype B, 1 genotype D,and the other 2 uncertain。Six of the eight HBV DNA positive donors were followed up, and three of them seroconverted。 Conclusion Fully automated HBV DNA detection method can be applied in blood screening,and will further increase the safety of blood supply.
