1.Evidence-based nursing of postoperative complications after percutaneous coronary intervention

Objective To study the best nusing care for postoperative complications of percutaneous coronary intervention. Methods 203 patients with coronary heart disease were selected, evidence- based nursing theory was used to find domestic related literature, looking for the best nursing programs for related complications after percutaneous coronary intervention, and applied to postoperative complications of percutaneous coronary intervention in nursing practice. Results After evidence- based nursing, the prognosis of complications was improved, the hospitalization time was shortened and satisfaction degree of patients was increased. Conclusions Reasonable evidence-based nursing plays an important role in reducing postoperative complications of percutaneous coronary intervention and improving prognosis. Evidence-based nursing care provides a reliable scientific basis for clinical decision- making and meets higher requirements of the patients for nursing care quality and professional compe-tence of nursing staff.
2.Effect assessment of oral rehydration salts treating orthostatic hypotension in children

Objective To explore the curative effect of oral rehydration salts treating orthostatic hypotension in children. Methods 80 children with orthostatic hypotension collected in our hospital from January 2011 to December 2013 were given oral rehydration salts, the heart rate, blood pressure, and record dizziness were observed after two weeks treatment. Results 36 children in 80 were markedly improved after treatment, accounted for 45.0%, 37 children were valid, accounted for 46.2%, 7 children were invalid, accounted for 8.8%. The total effective rate was 91.2%. The number of dizziness, dizziness, nausea vomiting, blurred vision, palpitate discomfort after treatment were less than before treatment. Tilt table test began in 3 minutes, 7 minutes,heart rates, systolic blood pressures were decreased signiifcantly before and after treatment(P<0.05). Tilt table test began in 3 minutes, 7 minutes,diastolic blood pressure were decreased signiifcantly before treatment(P<0.05), not after treatment. Conclusion Oral rehydration salts can obviously increase diastolic blood pressure and improve the upright intolerance symptoms in orthostatic hypotension children.

4.Function of MMP/TIMP on airway remodeling of bronchial asthma and treating effects of Zhichuan capsule

OBJECTIVE: To explore the mechanism of the bronchial asthma and to study the treating effects of Zhichuan Capsule on the airway remodeling of asthmatic model rats. METHODS: The rat model was established by being sensitized and activated with different density of ovalbumin through prolonged and repeated exposure for 8 weeks. The rats were randomly divided into model group, Zhichuan Capsule treated group, dexameson treated group, and Zhichuan Capsule and dexameson treated group. Another group of normal rats were taken as control. General histological changes were observed by hematoxylin and eosin stained sections. Being standardized by internal perimeter (Pi), the wall thickness (d), internal area (Ai), outer area (Ao) and wall area (WA) of the airway were quantified by computer-assisted image analysis system. The express of MMP-9, TIMP-1, Col I, Col III and ColV in the airway were examined by immunocytochemical methods. During the course of airway remodeling, the dynamic changes of model rats were observed at different time points (2, 4, 6 and 8 weeks after the activating). Statistical comparison was performed by ANOVA followed by Fisher LSD test. RESULTS: (1) Histologic examination showed eosinophil infiltration within the airway walls, epithelial damage, excessive mucus in the lumen and edema in the submucosa of the airways in model rats, and that the collagen deposition increased accompanied by increasing of TIMP-1. In the model rats, MMP-9 increased at the time point of 2 weeks, but it decreased in the late stage (8 weeks after activating) of airway remodeling. And the level of TIMP-1 was far higher than MMP-9 at the time point of 8 weeks. (2) Zhichuan Capsule could down-regulate the level of TIMP-1 in the airway wall, as well as the thickness of airway wall and the collagen deposition. And there were progressing effects when it was used together with dexameson. CONCLUSION: (1) The early increase of MMP-9 is a key point to start remodeling; and the increase of TIMP-1 in the late stage, which inhibits collagenase activity, may play an important role in developing airway fibrosis. Imbalance between MMP-9 and TIMP-1 is a marker of airway remodeling. (2) Zhichuan Capsule can decrease the deposition of collagen and suppress the airway remodeling by inhibiting the TIMP-1 expression.
5.Anatomical and clinical study of reconstruction of the pateller ligament with vascularized medial tenden of gastrocnemius muscle

Objective To provide a clinical method in the treatment of patellar ligament defect. Methods Based on morphologic observation and blood supply of the medial tenden of gastrocnemius muscle on 30 adult cadaverous lower limp specimens. Then transferring of sural arteries medial tendon of gastrocnemins muscle was designed and used in 4 patients clinically. Results The period of follow-up in all cases was 2-8 months. The knee joint recovered well with satisfactory function. Conclusion The operation is a simple and ideal procedure. The vascularizod adductor magnus tendon is a good donor in the reconstruction of patellar ligament defect. Microsurgical repair of patellar ligament defect is a effective method.
6.Clinical study on treating of the knee osteoarthritis by using glucosamine capsule and injecting sodium hyaluronate combined with appropriate functional exercise

Objective To investigate the clinical effect of three capsule combined with Function injection combined with functional exercise in the treatment of knee osteoarthritis.Methods120 patients with knee osteoarthritis were randomly divided into two groups, the study group took orally hydrochloride capsule combined with joints injection combined with functional exercise therapy, and the control group only oral glucose capsules.two groups of patients after a period of treatment.ResultsIn the VAS score, lequesne index and serum inflammatory molecular level of the two groups were significantly better than the control group (P<0.05).ConclusionWhich the clinical study on treating of the knee osteoarthritis by using glucosamine capsule and injecting sodium hyaluronate combined with appropriate functional exercise has a significant effect on knee osteoarthritis, and more favorable in improving the quality of life of patients.
7.Diagnosis and treatment of adenocarcinoma of urinary bladder (reports of 41 cases)

Objective To study the diagnosis accuracy and the treatment sequelae of adenocarcinoma of urinary bladder. Methods 41 cases of urinary bladder adenocarcinoma have been encountered from 1980 to 2000,all of them had undergone operation and the diagnosis being assessed on histopathology studies.There are 18 cases of primary bladder adenocarcinoma,among which 8 had undergone radical cystectomy and 10 partial cystectomy.Among 12 cases of urachal adenocarcinoma extending to the bladder,11 received extended resection.All the 11 patitents with metastatic focus in the bladder underwent local ablation. Results Urinary bladder adenocarcinoma comprised 3.2% of the bladder cancer.Thirty seven patients have been followed up for five months to ten years.The 1 year survival rate was 51.3%(19 cases),the 2 year survival rate 43.2%(16 cases) and the 5 year survival rate 21.6%(8 cases). Conclusions Radical total cystectomy is the treatment of choice for primary adenocarcinoma of bladder and extended partial cystectomy seems to be the best for urachal adenocarcinoma. Comprehension therapy should be undertaken for patients with metastatic adenocarcinoma or tumor recurrence so as to improve the survival rate.
8.Long-term results of the modified Stamey bladder neck suspension for female stress urinary incontinence

Objective To evaluate the modified Stamey bladder neck suspension for female stress urinary incontinence. Methods 52 cases with an average age of 46 and an average stress incontinence history of 7.5 years underwent the modified Stamey procedure,18 cases being degree Ⅰ,21 degree Ⅱ and the other 13 degree Ⅲ. Results 32 cases were treated before 1994 and the outcome was evaluate at 3.6 years and 9.6 years postoperatively.There were 3 failures because of operative complication.The cure rates and failure rates at 3.6 and 9.6 years were 87.5%,68.8% and 6.2%, 21.9%,respectively.20 cases were treated after 1994 with a follow up of 4.3 years.The cure rate has been 90.0%. Conclusions The Stamey procedure yields high initial cure rates yet the latter declines with time.
9.Diagnosis and treatment of extra-adrenal pheochromocytoma

Objective To highlight the diagnostic and t herapeutic procedures for extra-adrenal pheochromocytoma. Methods Thirty-eight cases of extra-adrenal pheochromocytoma proven patholo gically from 1998 to 2002 were reviewed and studied.Hypertension was observed in 34 cases and intermittent macrohematuria in 4 patients.Another 2 cases were adm itted to hospital with abdomen masses through routinely physical examination.Uri nary VMA,serum and urinary catecholamine were measured in 33 cases.38 cases had B-model ultrasound examination and 34 cases took CT scan.The level of urine VMA elevated in 30 cases(91%) and serum or urinary catecholamine were elevated in 2 8 (85%).The positive rates of localization of B-ultrasound and CT were 92% and 100%,respectively.29 cases took ?-receptor blocker for at least 2 weeks preope ratively.There were 6 cases operated under epidural anesthesia and 32 general an esthesia.Six cases were operated transvesical.10 cases with lumber incision and 19 with abdominal incision.The other 3 cases operated laparoscopically. Results Tumor location,6 cases in bladder wall,7 in renal hilum, 9 in upper pole of kidney,4 in low pole,10 in anterio-lateral to the adrenal gl and,and 2 cases of multifoci.The specimens were from 2 to 9 cm in size.Among 34 cases with hypertension,the blood pressure of 26 patients return to normal and 2 cases were still hypertensive.10 cases were assayed as malignant pheochromocyto ma.There were 9 cases of recurrence or metastases and 5 patients died during fol lowed-up from 13 months to 10 years. Conclusions Urine VMA level is a useful marker to detect extra-adrenal pheochromocytoma and CT sc an is the most reliable in localizing the lesions. 131I-MIBG scintigra phy is of great value for the localization and quantitative for extra-renal phe ochromocytoma with high sensitivity and accuracy.It may be also used as a therap eutic method.Surgical manipulation of the tumor is the best management and metic ulous perioperative management is extremely important.Measuring the level of uri nary catecholamine can monitor the recurrence or metastasis of the tumor.
10.A Study on Preventing the Recurrence of Hepatocellular Carcinoma By Hepatectomy with HACE and PVCE
