1.Protective Effect of Mouse Nerve Growth Factor on Amikacin -Induced Ototoxicity

Objective To investigate the effects of mouse nerve growth factor on amikacin (AK)-induced ototoxicity in guinea pigs .Methods 45 guinea pigs were randomly divided into three groups with 15 guinea pigs in each group .The control group received no drug treatment ,the poisoning group received an intraperitoneal injection of AK (400 mg · kg -1 · d-1 ) for 10 days ,and the treatment group received an intraperitoneal injection of AK (400 mg · kg -1 · d-1 ) for 10 days and an intramuscular injection of NGF (1 500 AU · kg -1 · d-1 ) for 14 consecutive days .Auditory brainstem responses were tested on the day before administration ,the 10th day and 20th day after administration .All the guinea pigs were sacrificed when the last ABR test was finished on the 20th day whose Corti organs were observed as well .Results The ABR thresholds of each group showed no significant difference before administration .The ABR threshold of the treatment group was lower than that of the poisoning group .With light microscopy ,the hair cells with normal morphology were observed in the control group .However ,there was an ex-tensive loss of outer hair cell in the poisoning group .For the treatment group ,the miss rate of hair cell was lower compared with the poisoning group .Conclusion Our data shows that NGF has played a protective role on amikacin-induced ototoxicity .

3.Optimization of Preparation for Venlafaxine Hydrochloride W/O Emulsion by Central Composite Design-Response Surface Methodology

Objective To optimize the formulations of venlafaxine hydrochloride ( VH ) emulsions by using central composite design-response surface methodology. Methods The effect of amounts of Arlacel P135, VH, PEG-400, and NaCl on the emulsion viscosity, centrifugation breakage, and mean diameter was systemically investigated, respectively. The desirable formulation that combining these three response variables was constructed. Linear equations and a second-order polynomial equation were fitted to the data, and the outcome equation was used to predict the responses in the optimal region. Results There was a quantitative relationship between 4 factors and 3 evaluation indexes and evaluation the “desirability” . The optimal formulation of the VH emulsion were as follows:taking 0. 48 g of Arlacel P135, 0. 40 g of VH, 0. 26 g of PEG-400, and 0. 025 g of NaCl. The experimental values of the response variables were highly closed to the predict values. Conclusion The model presents good prediction and can be used to optimize the preparation of VH emulsion, which obtaining stable W/O emulsion.
4.Inhibitory effect of small interfering RNA targeting peroxisome-proliferator-activated receptor-γ coactivator-1α on retinal neovascularization in the monse

Objective To evaluate the inhibitory effect of small interfering RNA (siRNA) targeting peroxisome-proliferator-activated receptor-γ coactivator-1α (PGC-1α) on retinal neovascularization in the mouse.Methods Eighty seven-day-old C57BL/6J mice were divided into normal group,model blank group,model control group and PGC-1α siRNA group,twenty mice in each group.Mice in the normal group were kept in normal room air.Mice in the model blank group,model control group and PGC-1α siRNA group were induced for retinal neovascularization by hypoxia.Liposome with PGC-1α siRNA (1 μl) and liposome with negative control siRNA (1 μl) were injected into the vitreous in the PGC-1α siRNA group and model control group respectively when mice were moved out to room air from the cabin (Postnatal 12).No injection were performed in the model blank group.At postnatal 17,fluorescein angiography was used to assess the vascular pattern.The proliferative neovascular response was quantified by counting the nuclei of new vessels extending from the retina into the vitreous in cross-sections.PGC-1α and vascular endothelial growth factor (VEGF) level in retina were measured by real-time polymerase chain reaction (real-time PCR) and Western blot.Inhibition efficiency of PGC-1α siRNA on PGC-1α and VEGF was calculated.Results Mice in the normal group showed reticular distribution of retinal blood vessels.Central nonperfused retina,neovascular tufts and fluorescein leakage were seen in the model blank group and model control group.Neovascular tuft and fluorescein leakage were decreased in the PGC-1α siRNA group compared to the model blank group and model control group.The neovascular nuclei were increased in the model blank group and model control group compared to the normal group (P<0.05).The neovascular nuclei were decreased in the PGC-1α siRNA group compared to the model blank group and model control group (P<0.05).The expression of PGC-1α mRNA and protein in retina was increased significantly in the model blank group and model control group as compared with normal group,while decreased 54% and 53% respectively in the PGC-1α siRNA group as compared with model blank group and model control group (P<0.05).The expression of VEGF mRNA and protein in retina was increased significantly in the model blank group and model control group as compared with normal group,while decreased significantly in the PGC-1α siRNA group (decreased 48 % and 40 % respectively) as compared with model blank group and model control group (P<0.05).Conclusions Intravitreal injection of PGC-1α siRNA mediated by liposome can inhibit retinal neovascularization in the mouse effectively.
5.Changes of plasma adrenomedullin and endothelin in essential hypertensive patients with renal impair

Objective To assess the change of plasma adrenomedullin(ADM)and endothelin(ET)in patients with essential hypertensin. Methods Concentrations of plasma ADM and ET were measured in 18 healthy control subjects and 52 patients with essential hypertension, 36 patients of which had no target organ damage, while 16 patients of which were complicated with renal impair. Results The concentrations of ADM and ET were significantly higher in patients with renal impair than in patients without organ damage. Plasma concentrations of ADM and ET were strongly positively correlated with blood urea nitrogen levels and serum creatinine levels. A strong inverse correlation between GFR and plasma concentrations of ADM and ET was found. Conclusion The increase of plasma ET and ADM may have an intimate relation to the renal function. ADM and ET may play some important role in the pathophysiologic makeup of hypertension and hypertension with renal dysfunction. ADM may act against excessive vasoconstrictor ET increased in essential hypertension and may be part of a compensatory mechanism to offset further development of renal impair.
6.Review of thyroid stimulating hormone receptor and thyroid carcinoma

Thyroid carcinoma is the most common malignant tumor in the endocrine diseases,with a rising morbidity.As more investigations were made,thyroid stimulating hormone receptor is showed up,and it is believed some contacts are existed between thyroid stimulating hormone receptor and thyroid carcinoma.We believe that making sure of these contacts can help patients in diagnosis,treatment,and prognosis.
7.The follow-up analysis of 219 cases of syphilis in pregnancy

Objective To investigate the status of pregnancy syphilis screening ,diagnosis,treatment,follow-up in Panyu district for improvement of management of pregnancy syphilis .Methods Using the method of a retro-spective survey,the follow-up data for 219 cases of patients were diagnosed with gestational syphilis from 2011.10.1 to 2013.9.30.Results 219 cases were diagnosed with gestational syphilis in 103 665 cases of pregnant women .The positive rate was 2.1%and all cases were latent syphilis .176 cases underwent a course of treatment during pregnancy , of which 33 patients received two courses of standard treatment .40 cases didn't receive treatment and 3 cases could not be followed up due to the phone empty ,shut down or refuse .14 cases were congenital syphilis ,the rate was 6.4%. Conclusion Pregnancy syphilis is a serious mother to child transmission of disease .After standard treatment ,close follow-up,strengthen publicity and education to improve patient compliance can reduce the incidence of congenital syphilis.
8.Evaluation on the virtual CT endoscopy of the simple congenital malformation and its clinical application.

OBJECTIVE:
To evaluate the preoperative virtual CT endoscopy about the simple congenital middle ear malformations and its clinical application.
METHOD:
Thirteen cases (16 ears) with simple congenital middle ear malformations were undergone high resolution CT scannig to collect the original image data, followed by 3-dimensional reconstruction to process the images to evaluate the possible lesions of the ossicular chain. Then, a comparative analysis was made between the finding of virtual CT endotoscopic and the finding that actually found during the operation of the ossicular chain lesions.
RESULT:
We found malleus and upper strcture of incus abnormity or embeded in upper inner wall of atticus in 5 ears; malleus, incus and stape all abnormity or absence in 3 ears, in the former two conditions we both selected auricular incision through attic pathway remaining intact canal wall; we found crus longum incudis hypoplasia orstructure suprastapedialis absence,stapes aberrance or absence in 8 ears, then we selected vestibular window fenestration and artificial stapes surgery. Postoperative external auditory canals and tympanic membranes were intact in 16 ears,and speech frequency hearing improved by average of 25-45 dB. The coincidence between virtual CT endoscopy and the ossicular chain abnormity under operating views were 100% in cases with congenital ear deformity.
CONCLUSION
Virtual CT endoscopy is a reliable reference for preoperative evaluation on congenital middle ear malformations.

9.Related variance analysis on auditory effects of tympanoplasty

OBJECTIVE To study the relative factors affecting auditory effects after tympanoplasty. METHODS 118 cases (126 ears) of tympanoplasty were reported,16 factors that might influence auditory effects were analysed. RESULTS There were 10 in 16 factors affecting auditory effects after tympanoplasty, including course, the degree of pathological changes, air-bone gap, function of eustanchian tube, remaining handle of malleus, the type of surgery, condition of tympanic mambrane after operation and so on. The results of single-variance analysis had statistical differences (P<0.05). CONCLUSION Auditory effects after tympanoplasty were influenced by a series of factors, the major factors were the degree of pathological change and type of surgery.
10.Evaluation of virtual endoscopy in tympanoplast

OBJECTIVE To study the clinical usability of virtual endoscopy(VE) in tympanoplasty.METHODS A total of 102 patients (204 ears) were observed by virtual endoscopy. Tympanoplasty was performed in 72 cases (75 ears) including 53 cases (55 ears) with chronic otitis media and 19 cases (20 ears) with congenital microtia and middle ear dysmorphia. Ossicular chain reconstruction was conducteded in 65 ears at the same time. RESULTS The ossicular chain was showed eroded in 19 ears of 23 patients with cholesteatoma otitis media pre-operatively by VE, but was found eroded in all of 23 ears during operation. The ossicular chain was showed eroded in 29 ears of 32 patients with osteitis otitis media pre-operatively by VE, but during operation it was found 23 ears with malleus and incus eroded, 11 ears without head of stapes or up-structure of stapes. VE showed 18 ears with congenital microtia and middle ear malformation and 2 ears with small tympanic cavity and no ossicular chain preoperatively, and operation proved 17 ears with severe ossicular malformation, 2 ears without stapes, 1 ear with vestibular window atresia. There were 2 ears with sudden hearing loss after tympanoplasty, the VE showed ossicular displacement. The coincidence rates between VE and operation view were 92 % in patients with otitis media and 100 % in patients with congenital microtia and middle ear malformation. CONCLUSION VE can supply reliability data for evaluation of the damaged ossicular chain and efficacy of tympanoplasty.
