1.The application of blood pressure self-measurements in the management of hypertension in elderly patients in the community

Objective To compare the effects of different methods of blood pressure management in elderly hypertensive patients in the community,so as to explore the effective model for the management of chronic diseases.Methods 302 elderly hypertensive patients in Puhuangyu community health service center were divided into two groups by random number method:(1) receiving conventional antihypertensive drugs (control group,n=152);(2) receiving antihypertensive drugs combined with blood pressure self-measurements at least twice-weekly and real-time adjusting antihypertensive management (blood pressure self-measurements group,n =150).Medication adherence,levels of blood pressure control and blood pressure-targeting rate were compared between groups one year after intervention.Results Medication adherence rate was higher in self-measurements group than in the control group [94.7% (142 cases) vs.75.7% (115 cases),P< 0.05].After intervention,systolic pressure was decreased from (168.5±37.7) mmHg to (155.3±25.2) mmHg(1 mmHg=0.133 kPa) in the control group and from (168.0±38.1)mmHg to (141.7 ±22.9) mmHg in self-measurements group (all P<0.05).Diastolic pressure level was similar in the two groups.There was a significant difference in the blood pressure-targeting rate between groups (P <0.05).Conclusions Blood pressure self-measurements are more effective in controlling blood pressure and can improve the self-management ability in elderly patients with chronic diseases in the community.
2.Validation of multiplex microsatellite DNA amplification system for use in monitoring umbilical cord blood cell transplantation

Objective To study the validation of fluorescent multiplex microsatellite amplification technique for use in the caseworks of umbilical blood cell transplantation monitor. Methods Six post-transplant recipients of umbilical cord blood cell transplantation were monitored by analyzing microsatellite DNA loci. DNA samples were amplified using a fluorescent labeling primers multiplex amplification system of 15 microsatellites markers, followed by typing on a DNA automated sequencer. Recipients' or donors' microsatellites DNA profiles were compared before and after transplantation. Results All recipients and donors exhibited different DNA profiles. Without reference samples of pre-transplant or donors, the changes of the 15 microsetellites genotypes of the post-transplant recipients still could be analyzed. The recipient type turned to donor type was observed over time. Conclusion Under the condition of using multiplex amplification of the 15 microsatellites to monitor the umbilical blood cell transplantation, reference sample of pre-transplant or donor did not need to be detected simultaneously.
3.Electrochemical sensor for acetaminophen based on layer-by-layer self assembly technique.

A novel type of carbon nanotube-coated Au nanoparticle and [bmim]BF4 composite modified glassy carbon electrode was fabricated by a layer-by-layer self-assembly technique. The electrochemical performance of acetaminophen (ACOP) on the modified electrode was investigated by cyclic voltammetry. The Nafion/GNPs/RTIL/MWNTs/GC electrode showed an excellent electrocatalytic activity for the oxidation of ACOP and accelerated electron transfer between the electrode and ACOP. For ACOP, the reversible electrochemical process was observed on the Nafion/GNPs/RTIL/MWNTs/GC electrode, while irreversible electrochemical process occurred on the GC electrode. For the Nafion/GNPs/RTIL/MWNTs/GC electrode, the anodic peak potential of ACOP was moved from 0.562 V to 0.413 V, with a potential drop of 149 mV. At the same time, the reduction peak potential was 0.384 V, and the potential difference was only 29 mV. It was shown that the modified electrode possessed higher electrocatalytic activity and more sensitive effect for the detection of ACOP than both MWNTs/GC electrode and GC electrode. The effects of the different experimental conditions on the electrochemical behaviors of ACOP were explored. Under the optimum conditions of preparation and experimental, the linear calibration curves of ACOP were obtained in a wide range of 2 x 10(-1) to 4.0 x 10(-4) mol x L(-1) with a correlation coefficient 0.999 2 and a detection limit of 2.6 x 10(-8) mol x L(-1) (the ratio of signal to noise, 3:1). The recovery rate was 97.9%-100.8%. This method can be used to determine ACOP in paracetamol tablets with satisfactory results.
4.Effects of Gs proteins α subunit T393C genetic polymorphism on carvedlol response in congestive heart failure patients

Objective To find out the distribution of the subunit of Gs proteins (GNAS1) T393C polymorphism genetic in congestive heart failure patients and to investigate the effects of genetic polymorphisms on the therapeutic efficacy of carvedlol. Methods Genotype of 65 patients with congestive heart failure (heart function was ranked Ⅱ~Ⅳ level by NYHA) underwent PCR-RFLP for GNAS1 polymorphism genetic analysis, and then were treated with carvedlol. The initial dosage of carvedlol was 12.5 mg once daily in patients , and increased by 6.25 mg every 1 ~ 2 weeks for 12 months to a maximum dosage within 50 mg/d. Degree of LVEF changes and heart function improvement was adopted as measuring index , effect of Gs proteins α subunit T393C genetic polymorphism on carvedlol response in congestive heart failure patients was analyzed. Results After carvedlol treatment for 12 months , NYHA class distribution and LVEF in congestive heart failure patients improved significantly (P < 0.05). After carvedlol treatment for 6 months and 12 months mutation symptoms patients′ heart function improved significantly (P < 0.05) and LVEF increased significantly (P < 0.01). Conclusions GNAS1 T393C genetic polymorphism can influence the therapeutic efficacy of carvedlol in chronic congestive heart failure patients. 393 homozygous mutant and mutated symptoms patients′ heart improved more obviously and their LVEF improved more significantly.
5.Intervention and intervention of cisplatin combined with gemcitabine or vinorelbine in the treatment of patients with non-small cell lung cancer

Objective To investigate the effect of psychological intervention on patients with non-small cell lung cancer treated with cisplatin or gemcitabine or Changchun vinorelbine. Methods 64 cases of patients with non-small cell lung cancer after surgery, nursing for patients in the group given basic treatment, routine care control group, the observation group based on routine nursing and psychological nursing effects were compared between the two groups. Results No significant difference of anxiety and depression in the patients of the two groups before nursing, after grouping nursing, the observation group improved significantly; compared two groups of patients with quality of life score, visible two group before the intervention had no significant difference after intervention group was clearly observed after the patients in the observation group were higher than that of the control group, comparison between groups were indicates that the difference is obvious (P<0.05). Conclusion Chemotherapy in non-small cell lung cancer after surgery for patients with psychological intervention, compared to conventional nursing, can improve the psychological status of patients with better, improve the quality of life of patients, so it is worthy of reference in clinical use.
6.Effect of isoflurane preconditioning on inflammatory responses during spinal cord injury in rats

Objective To evaluate the effect of isoflurane preconditioning on inflammatory responses during spinal cord injury (SCI ) in rats .Methods Sixty adult male Sprague-Dawley rats ,weighing 250-300 g , were randomly divided into 3 groups ( n= 20 each ) using a random number table :sham operation group (S group) , SCI group , and isoflurane preconditioning group (I group ) . The animals were anesthetized with intraperitoneal pentobarbital sodium 40 mg/kg .SCI was produced by a weight-drop contusion at the T10 level .The rats inhaled 2% isoflurane for 2 h ,and the model was established at 24 h after the end of isoflurane inhalation in I group . Neurological function was assessed and scored by using the the Basso , Beattie , Bresnahan (BBB ) Locomotor Rating Scale on 7 days after SCI .Five rats in each group were then chosen and spinal cord specimens were obtained and cut into sections which were stained with haematoxylin and eosin for determination of the viable neuron count .Fifteen rats in each group were sacrificed and the spinal cord was removed for detection of nuclear factor kappaB (NF-κB ) and interleukin-1β (IL-1β) expression (by Western blot ) .Results Compared with S group ,BBB score and the number of viable neurons were significantly decreased ,and the expression of NF-κB and IL-1βprotein was up-regulated in SCI group ( P<0.05) .Compared with SCI group ,BBB score and the number of viable neurons were significantly increased ,and the expression of NF-κB and IL-1βprotein was down-regulated in group I ( P<0.05 ) .Conclusion The mechanism by which isoflurane preconditioning protects the spinal cord is related to inhibition of inflammatory responses in rats .
7.Gastric cancer stem cells in tumor invasion and metastasis and its influence on angiogenesis ability

BACKGROUND:Gastric cancer stem cels involved in the neoadjuvant chemotherapy and conventional treatment are closely associated with relapse of gastric cancer. However, this conclusion has not yet been confirmed.
OBJECTIVE:To investigate the effects of gastric cancer stem cels in tumor invasion and metastasis and its effect on angiogenesis ability.
METHODS:We prepared nude mouse models of gastric cancer to isolate and culture gastric cancer stem cels. Harvested gastric cancer stem cels were detected in cel scratch test, ring test, inhibition rate test, cel migration test and tumorigenicity test.
RESULTS AND CONCLUSION:After 7 days of culture, the cels exhibited adherent growth but a lack of regularity that most cels were in a tadpole shape. In the cel scratch test, the scratch width was significantly different at 0 and 24 hours (P < 0.05). Under an inverted microscope, the cels were found to form a ring in the ring test. The 50% inhibiting concentration of gastric cancer stem cels induced by oxaliplatin was significantly lower than that induced by 5-fluorouracil (P < 0.05). The number of cels passing through the basilar membrane was significantly increased after cel migration (P < 0.05). After implantation of gastric cancer stem cels, the gastric tissue quality was significantly higher than that in normal nude mice of gastric cancer (P < 0.05). These findings indicate that gastric cancer stem cels responsible for tumor invasion and migration have stronger angiogenesis ability.
8.Effects of propofol on cytotoxicity of bupivacaine: intracellular Ca2+ concentration and nitric oxide synthase activity in PC12 cells exposed to bupivacaine

Objective To investigate the effects of propofol on intracellular free Ca2+ concentration and nitric oxide synthase (NOS) activity in PC12 cells exposed to bupivacaine. Methods The PC12 cells were provided by Shanghai Cell Biology Research Institute, Chinese Academy of Sciences and cultured in DMEM liquid culture medium. The cultured PC12 cells were seeded in 36 well plates and randomly assigned to one of 4 groups (n=9 wells each): group Ⅰ control (C);group Ⅱ propofol (P);group Ⅲ bupivacaine (B) and group Ⅳ propofol + bupivacaine (PB). In control group D-Hank solution was added. The cells were exposed to propofol 2 mmol/L and bupivacaine 0.09 mmol/L in group P and B respectively. In group PB the cells were incubated with propofol 2 mmol/L and bupivacaine 0.09 mmol/L simultaneously. After being incubated for 6 and 24 h the apoptosis in BC12 cells was assessed by flow cytometry. Apoptotic rate was calculated. NOS activity and intracellular free Ca2+ coneentration in PC12 cells were determined. Results Bupivacaine significantly increased the apoptotic rate of PC12 cells, the intracellular free Ca2+ concentration and NOS activity in PC12 cells in group B as compared with control group. Propofol significantly decreased the toxic effects of bupivacaine on PC12 cells in group PB compared with group B. Conclusion Bupivacaine is toxic to PC12 cells by increasing apoptosis, intracellular Ca+ concentration and NOS activity in the cells. The toxic effects can be prevented to some extent by concomitant administration of propofol.
9.Vacuum Packaging Technique for Storing Sterilized Packs:An Applied Study

OBJECTIVE To explore the effect of vacuum packaging technique for storing sterilized packs in order to extend the period of valid storing.METHODS From 420 sterilized packages 210 were randomly taken as a test group,and the other 210 packages as the control group.According to aseptic techniques the packages of test group were vacuumized and the control group was packed as by outine protocol.Two groups were simultaneously stored in aseptic closet,and then regularly taken out 30 samples from two groups respectively for microbial(culturing).RESULTS Bacterial growth was seen in the control group at first month.But none of the specimens in test group showed any microbial growth until the eighth month.There was a statistically significant difference((P

OBJECTIVE To explore the effect of smearing catheter surface with chloramphenicol for preventing catheter -associated urinary tract infections. METHODS Totally 100 cases of preoperative patients needed for indwelling urethral catheters were randomly grouped, 50 of 100 cases as test group, and the others 50 cases as the control group. Catheters after smearing surface with chloramphenicol were inserted using aseptic technique in the test group, urinary catheters without using chloramphenicol were inserted in the control group according to routine aseptic technique protocol, and then regularly taken out urinary specimen from two groups respectively for microbial culture.
