1.Study of molecular mechanism of cardiac injury induced by adriamycin

AIM: To preliminarily study the molecular mechanism for the cardiac injury in rat by adriamycin and the mechanism for the acute repair in the body. METHODS: The male Sprague Dawley rats were randomly divided into four groups (n=10 in each): The first group was kept without treament and served as the control ;the second, the third and the fourth received ADR in different doses (10, 20, 40 mg?kg -1 , respectively) by injection of adriamycin. The content of malondialdehyde (MDA)in the serum was estimated with thiobarbituric acid. Cu Zn SOD was measured by its reaction with xanthine oxidase. GSH was measured by its reaction with 5, 5 nitrobenzoic acid. Using semi quantitative reverse transcription polymerase chain reaction (RT PCR),we analyzed the expression of the associated gene. BACKGROUND: Tissue-engineered skin, bone, cartilage, blood vessel, and muscle flap have achieved remarkable development in tissue repair and regeneration; meanwhile, the difficulty in tissue-engineered vascularization has attracted much attention.OBJECTIVE: To summarize studies concerning the mechanism of angiogenesis (vasculogenesis and revascularization),vascularization strategies (seed cells, scaffolds, growth factors) and models of vascularization in tissue engineering (in vivo and in vitro), and to provide theoretical foundation for basic research of vascularization in tissue engineering.METHODS: A computer-based research was performed for relevant literatures in PubMed, Springerlink, Web of Science,ScienceDirect, CNKI, CqVip and WanFang databases published between January 20002 and April 2016 using the keywords of issue engineering, vascularization, scaffolds, cell growth factor, vasculogenesis, angiogenesis in English and Chinese, respectively. Fifty-five eligible articles were included according to the inclusion and exclusion criteria.RESULTS AND CONCLUSION: In vitro microvascular construction is commonly used. The angioid scaffold is prepared,and then cells are seeded onto the scaffold, which will proliferate, migrate and form microvessels induced by growth factors. The cell-scaffold composites that have good vascularization in vitro have been introduced via transplantation in in vivo experiments. However, there are some problems, such as the inevitable inflammatory reaction that makes a direct effect on the composite function and survival, and vascularization is still a difficulty in tissue regeneration and repair in the tissue engineering research.
3.Application of superslim endoscope in treatment of colonic obstruction in left colorectal cancer

Objective To discuss the application of transanal placement of ileus tube in treatment of colonic obstruction in left colorectal cancer.Methods Thirty-two patients with left malignant colorectal obstruction were divided into ordinary colonoscope group (16 cases) and superslim endoscope group (16 cases) by random digits table.The catheter success rate,catheter operating time and exposure to X-ray time was compared between two groups.Results Fifteen cases were successful and 1 case was failed in ordinary colonoscope group.Thirteen cases were successful and 3 cases were failed in superslim endoscope group.There was no significant difference in the catheter success rate between two groups (x2 =1.143,P =0.285).The catheter operating time was (42 + 15),(20 +6) min in ordinary colonoscope group and superslim endoscope group,and there was significant difference between two groups (t =3.895,P =0.005).The exposure to X-ray time was (20 + 12),(5 + 2) min,and there was significant difference between two groups (t =-3.596,P =0.007).Conclusion Transanal placement of ileus tube is more successful and convenient by superslim endoscope combined with anesthesia than by ordinary colonoscope.
4.Features of Cervical Vertebra X-ray Examination and Transcranial Doppler in Children with Cervical Vertigo

Objective To analyze the features of cervical vertebra X-ray examination and transcranial Doppler (TCD) in children with cervical vertigo, and investigate the mechanism of the disease. Methods X-ray examination of cervical vertebra and TCD were performed on 80 children with cervical vertigo, and the features of the results were analyzed. Results The abnormal results of cervical X-ray examination mainly showed atlantoaxial joint or/and C2, C3 vertebral joint disorders, some of which combined with cervical instability, cervical curvaturestraightened and even reverse curvature, cervical scoliosis. The abnormal results of TCD showed the blood flow velocity in single or multiple branch of artery increased mainly, some of which decreased or became asymmetrial. Conclusion The cervical vertigo in children may result from various kinds of disorders of the atlas, axis or/and C2, C3 vertebrae.
5.Multi-center clinical studies on pazufloxacin sodium chloride injection in treatment for acute bacterial infections

Objective To evaluate efficacy and safety of locally-produced pazufloxacin mesilate sodium chloride injection in the treatment of bacterial infections of respiratory and urinary tract.Methods A multi-center double-blind randomized controlled clinical trial was carried out to evaluate efficacy and safety of pazufloxacin mesilate sodium chloride in treatment for acute bacterial infection, as compared to those of levofloxacin hydrochloride and glucose injection as control treatment.A total of 244 patients with acute bacterial infection of respiratory and urinary tract were enrolled in the studies.120 in trial group and 120 in control group, with four withdrawals.Pazufloxacin mesilate and levofloxacin were administered intravenously by drip at a dose of 300 mg and 200 mg, every 12 hours for 7 to 14 days for trial and control groups, respectively.Resuits Overall efficacy of pazufloxacin mesilate was 77.0 percent and 93.5 percent in treatment for acute bacterial infections of respiratory and urinary tract.respectively, and that of levofloxacin was 80.6 percent and 89.6 percent, respectively.Overall bacterial clearance rate WaS 91.5 percent for pazufloxacin mesilate, 89.6 percent for respiratory tract infection and 94.1 percent for urinary tract infection, respectively.and 93.4 percent for levofloxacin, 97.3 percent for respiratory tract infection and 89.7 percent for urinary tract infection, respectively.No significant difference in adverse drug reactions between the two groups(P>0.05)was found, with 4.88 percent and 7.44 percent for trial and control groups, respectively.Conclusions Pazufloxacin mesilate sodium chloride injection produced locally is a safe and effective antibiotic in treatment for acute infections of respiratory and urinary tract.
6.Efficacy comparison of different hysteroscopic surgical procedures in treatment of endometrial polyps

Objective To compare the efficacy of different hysteroscopic surgical procedures in the treatment of endometrial polyps. Methods Ninety-two patients with endometrial polyps from January 2016 to December 2017 were selected. Among them, 37 patients with fertility requirements underwent polypectomy with MyoSure as group A; 25 patients without fertility requirements underwent polypectomy with resectoscopic hysteroscopy as group B, and 30 patients underwent polypectomy with hysterosopy and curettage as group C. The duration of the procedure, blood loss and hospital stays, effective rate, recurrence rate and postoperative pregnancy rate of the3 groups were compared. Results There was no significant difference in intraoperative blood loss and hospital stays among the 3 groups (P> 0.05). The duration of the procedure of group A and B was longer than that of group C, and the difference was statistically significant (P < 0.05). The effective rate of treatment was high in all 3 groups, and inter-group comparison indicated no statistically significant difference, but the recurrence rate of group C was significantly higher than that of group A and B. For those with fertility requirements, the pregnancy rate in group A was slightly higher than that in group C within one year after surgery, but the difference was not statistically significant. Conclusion Hysteroscopic procedures of endometrial polypectomy has the advantages of accuracy, safety, effectiveness and rapid recovery. The treatment with MyoSure and resectoscopic hysteroscopy can reduce the recurrence rate of patients effectively. For women with fertility requirements, the treatment with MyoSure can protect the endometrium and improve the postoperative pregnancy rate.
7.Clinical study of lateral cervical approach surgery in the treatment of primary hyperparathyroidism

Objective To explore the feasibility and clinical outcome of lateral cervical incision via sternocleidomastoid intermuscular approach(SMIA)in the treatment of primary hyperparathyroidism.Methods The clinical data of 64 patients with primary hyperparathyroidism who underwent unilateral parathyroid surgery in the First Affiliated Hospital,School of Medicine of Zhejiang University from January 2019 to June 2022 were retrospectively analyzed.They were divided into lateral cervical incision via sternocleidomastoid intermuscular approach group(SMIA group)and linea alba cervicalis approach group(LACA group)based on the surgical incision and access route.The differences in clinical features,surgery-related outcomes and postoperative functions of the anterior cervical region were compared between the two groups.The EQ-5D-5L scale was used to assess the subjective feeling of postoperative neck discomfort,while the Hollander Wound Assessment Scale was used to assess the clinical outcome of incision healing.Results There were no statistical differences between the two groups of patients in terms of age,gender,intraoperative bleeding,parathyroid hormone or blood calcium levels before and after surgery(P>0.05).The duration of surgery was significantly shorter in the SMIA group than in the LACA group[(39.77±5.69)min vs.(54.41±4.66)min].There was a statistical difference between the two groups in functional protection of the anterior cervical region at 1 month and 12 months after surgery(1 month,84.67±3.74 vs.79.47±5.38,P<0.001;12 months,93.80±2.52 vs.89.94±2.39,P<0.001),and the SMIA group was better than the LACA group.The Hollander Incision Assessment Scale scores of the SMIA group were better than those of the LACA group at 6 months and 12 months after surgery,and the difference was statistically significant(6 months,1.93±0.58 vs.2.41±0.66,P=0.003;12 months,1.03±0.67 vs.1.74±0.62,P<0.001).Conclusion Parathyroidectomy via sternocleidomastoid intermuscular approach through lateral cervical incision is a simple,safe and effective surgical procedure,which makes it easier to search for parathyroid lesions and shortens the surgical time compared with the traditional incision,and has obvious advantages in the protection of anterior cervical region function.
8.Disease burden of prostate cancer from 2014 to 2019 in the United States: estimation from the Global Burden of Disease Study 2019 and Medical Expenditure Panel Survey

OBJECTIVES:
The aim of this study was to evaluate the disease burden of prostate cancer (PC) and assess key influencing factors associated with the disease expenditures of PC in the United States.
METHODS:
The total deaths, incidence, prevalence, and disability-adjusted life-years of PC were obtained from the Global Burden of Disease Study 2019. The Medical Expenditure Panel Survey was used to estimate healthcare expenditures and productivity loss and to investigate patterns of payment and use of healthcare resources in the United States. A multivariable logistic regression model was conducted to identify key factors influencing expenditures.
RESULTS:
For patients aged 50 and older, the burden for all age groups showed a modest increase over the 6-year period. Annual medical expenditures were estimated to range from US$24.8 billion to US$39.2 billion from 2014 to 2019. The annual loss in productivity for patients was approximately US$1,200. The top 3 major components of medical costs were hospital inpatient stays, prescription medicines, and office-based visits. Medicare was the largest source of payments for survivors. In terms of drug consumption, genitourinary tract agents (57.0%) and antineoplastics (18.6%) were the main therapeutic drugs. High medical expenditures were positively associated with age (p=0.005), having private health insurance (p=0.016), more comorbidities, not currently smoking (p=0.001), and patient self-perception of fair/poor health status (p<0.001).
CONCLUSIONS
From 2014 to 2019, the national real-world data of PC revealed that the disease burden in the United States continued to increase, which was partly related to patient characteristics.
9.Xiaoai Jiedu Prescription-containing Serum Enhances Lethal Effect of NK Cells on Colon Cancer Cells

ObjectiveTo explore the influence of Xiaoai Jiedu prescription (XJP)-containing serum on natural killer (NK) cells′ lethal effect on colon cancer cells and the molecular mechanism. MethodXJP-containing serum (0.1%, 0.5%, 1%, 5%, 10%) was used to treat HCT-116 cells and NK-92MI cells respectively for 24 h, and methyl thiazolyl tetrazolium (MTT) assay was employed to detect cell proliferation. Then, low-concentration (0.1%, 0.5%, 1%) XJP-containing serum was selected to treat co-cultured HCT-116 cells and NK-92MI cells for 24 h and calcein acetoxymethyl ester/propidium iodide (Calcein-AM/PI) was applied to detect the killing effect of NK cells on colon cancer cells. Flow cytometry was used to detect apoptosis of colon cancer cells, Western blot the expression of apoptosis-related proteins and signal transducer and activator of transcription 4 (STAT4) pathway-related proteins, and enzyme-linked immunosorbent assay (ELISA) the secretion of interferon (IFN)-γ. ResultHigh-concentration (5%, 10%) XJP-containing serum inhibited the proliferation of HCT-116 and NK-92MI cells (P<0.01), while low-concentration (0.1%, 0.5%, 1%) XJP-containing serum had no obvious influence on cell proliferation compared with the blank group. As compared with the blank group, low-concentration XJP-containing serum enhanced the killing activity of NK cells against colon cancer cells in a concentration-dependent manner (P<0.01), and induced apoptosis of colon cancer cells (P<0.01). Moreover, XJP-containing serum (0.1%, 0.5%, 1%) down-regulated the expression of B-cell lymphoma 2 (Bcl-2) and B-cell lymphoma-extra large (Bcl-xl), and up-regulated the expression of Bcl-2-associated X (Bax) compared with the blank group (P<0.05, P<0.01). Compared with the co-culture group, XJP-containing serum (0.1%, 0.5%, 1%) increased the expression of p-STAT4 and IFN-γ (P<0.05). ELISA result showed that XJP-containing serum (0.1%, 0.5%, 1%) raised IFN-γ secretion (P<0.01). ConclusionXJP-containing serum can enhance the activity of NK cells to kill colon cancer cells. The mechanism is the likelihood that it activates STAT4 pathway, increases IFN-γ secretion by NK cells, down-regulates the expression of Bcl-xl and Bcl-2, and up-regulates the expression of Bax, thereby promoting the apoptosis of colon cancer cells.
