2.Effects of Fastigial Nucleus Stimulation on Depression

Objective To evaluate the clinical effects and safety of fastigial nucleus stimulation (FNS) combined with drugs on depression.Methods 60 patients with depression were randomly assigned into the FNS combined with drug group (treatment) group (n=30) andthe drug group (n=30). Hamilton Depression Scale (HAMD) and Pittsburgh Sleep Quality Index (PSQI) were used to evaluate the effects,and Treatment Emergent Symptom Scale (TESS) was used to evaluate the safety before and after treatment. Results The total effective rateof the treatment group (83%) was higher than that of the drug group (60%) (P<0.05). The HAMD of the treatment group decreased in theend of the 2th and 4th week after treatment compared with the drug group (P<0.05). The PSQI was lower in the treatment group than in thedrug group (P<0.05). There was no significant difference between two groups with TESS (P<0.05). Conclusion FNS combined with drugscan improve depression and insomnia of depression patients.
3.Clinical significance of expression of toll-like receptor 4 on CD14 + monocytes in peripheral blood from patients with chronic obstructive pulmonary disease

Objective To detect the effects of glucocorticoids on the expression of toll-like receptor 4 (TLR4) on CD14+ monocytes in peripheral blood from patients with chronic obstructive pulmonary disease (COPD) and explore the relationship between inherent immunity deficiency and the pathogenesis of COPD.Methods A total of 30 hospitalized acute exacerbations of COPD (AECOPD) patients from 2012 January to March were divided them into 2 groups of mild-moderate (A,n =15) and severe-critical (B,n =15) according to pulmonary functions.The same basic treatment (anti-infection,oxygen inhalation,bronchial smooth muscle relaxing and cough relief) was administered.And glucocorticoids were added to Group B.Two weeks later,AECOPD achieved a clinical remission and it was designated as a stable COPD group.Another 30 healthy cases over the same period were selected as control group.Flow cytometry was used to measure the relative mean fluorescence intensity (rmfi) and relative positive cell percent (rpcp) of TLR4 onr CD14 positive peripheral blood monocytes.Conventional pulmonary function test (FEV1 percentage of predicted value,FEV1/FVC value) was performed.Results The value of rmfi for TLR4 of AECOPD group was 5.14 ±0.26 and rpcp (4.24 ±0.17)% and they were similar to those of COPD group [4.46 ±0.24 and (3.49 ± 0.24) % respectively,all P > 0.05].AECOPD and COPD groups were both significantly lower than normal control group [13.75 ±0.28 and (10.25 ±0.13)% respectively,all P <0.01].The rmfi of TLR4 of Group B was 3.12 ±0.15 and rpcp (3.17 ±0.10)%.And both were significantly lower than those of Group A [5.48 ±0.23 and (5.12 ±0.20)% respectively,all P <0.01].The value of rmfi for TLR4 of Group B before glucocorticoid treatment was 3.12 ±0.15 and rpcp (3.17 ±0.10)%.And both were significantly lower than those of later glucocorticoid treatment [5.24 ± 0.25 and (5.44 ± 0.19) % respectively,all P < 0.01].Both values were significantly lower than those of normal control group [13.75 ± 0.28 and (10.25 ± 0.13) % respectively,all P < 0.01].The expression of TLR4 on CD14 + monocytes in peripheral blood of AECOPD patients was positively correlated with FEV1 percentage of predicted value(r1 =0.824,P<0.001) and FEV1/FVC (r2 =0.756,P<0.001).Conclusions Innate immunodeficiency plays an important role in the pathogenesis of COPD.And the expression of TLR4 is an important indicator for disease severity.Glucocorticoids treat COPD through inhibiting airway inflammation response and enhancing defensive function of innate immune system.
4.Comparison of PCR and HRM for detecting bacterial drug resistance gene aac (6′)- Ib-cr

Objective To study the difference of detection rate between PCR and high-resolution melt (HRM)for detecting the bacterial drug resistance gene aac(6′)-Ib-cr.Methods The PCR method was adopted to detect the aac(6′)-Ib gene in 299 strains of common Gram-negative bacilli,and the PCR positive products of aac (6′)-Ib were digested with BtsCI.Meanwhile Aac (6′)-Ib and aac (6′)-Ib-cr genes in above bacterial strains were detected by adopting the HRM technique.Results A total of 29 isolates were i-dentified as aac (6′)-Ib gene,and 21 mutations were aac (6′)-Ib-cr,with a mutation rate of 72.4% (21/29)for PCR and restriction enzyme digestion.And the results of the HRM test were in good agreement with each other.Conclusion PCR and HRM are similar for the detection of the bacterial resistance gene aac (6′)-Ib-cr,and aac (6′)-Ib-cr gene could be detected by a simpler HRM tech-nique instead of PCR and enzyme digestion,especially for a large number of screening.

Objective Angiotensin(AngⅡ)is an important proinflammatory mediator in the cardiovascular system.The present study is aimed at investigating the correlation between ceramide and p53 and apoptosis of human umbilical endothelial cells induced by AngⅡ.Methods Human umbilical endothelial cells were cultured in vitro and treated with AngⅡalone or in combination with Losartan(an in- hibitor of AT1),PD123319(an inhibitor of AT2),FB1(an inhibitor of ceramidase),PFT-?(an inhibitor of p53),respectively.The ap- optosis of cells was detected with TUNEL,and p53 expression at protein and mRNA was assessed with Western blotting and reverse tran- scription-polymerase chain reaction(RT-PCR).Results Compared to AngⅡgroup,PD123319 and FB1 could inhibit endothelial cells ap- optosis induced by AngⅡat 24h;hut Losartan and PFT-?could not.Compared to the control group,no statistical difference existed be- tween PD123319 and FB1,but it did exist between Losartan and PFT-?.FB1 and PFT-?down-regulated the p53 expression at the protein and mRNA levels.Conclusion In the process of apoptosis induced by AngⅡ,AngⅡcan induce the activation of ceramide by binding with AT2.AT2 and Ceramide play a key role in the endothelial cells apoptosis induced by AngⅡ.Ceramide may be situated in the upper stream of p53 and induce the apoptosis by p53 pathway.
7.Determination of content of astragaloside A in Astragalus membranaceus powder for

To develop a method for the quality control of Astrgalus membranaceus (Fisch.) Bge. powder for injection (AMPI). Methods TLC-spectrophotometry was used to determine the content of astragaloside A in AMPI. The mobile phase was chloroform-methanol-water (65∶30∶10) and detected at the wave length of 418 nm. Results The regression equation within the spotted range of 80~240 μg was A=1.538 C+ 0.189, with good linearity, r=0.999 8. The average recovery rate was 98.68% and RSD=2.52%(n=5). Conclusion The method used simple and easily available instruments and the rusults obtained were reliable and specific, suitable for the quality control of AMPI.
8.A study on function of knee and quality of life in patients with knee osteoarthritis

Objective: To investigate the effect of function of knee on quality of life (QOL) in patients with knee osteoarthritis (OA). Method: Forty patients with knee OA were recruited. Their function, pain and range of motion (ROM) of knee were assessed with Lequesne index, Lysholm knee score (LKSS), visual analogue scale (VAS) and protractor measure respectively. QOL was evaluated with arthritis impact measurement scale (AIMS). Result: The scores of AIMS, Lequesne and VAS in female were significantly higher than that in male. And the LKSS in female was significantly lower than that in male (P<0.05). There was a positive correlation between AIMS score and Lequesne index, VAS score and limit of ROM, but a negative correlation between AIMS and LKSS (P<0.05). Conclusion: The function of knee and QOL in female patients with OA were worse than that in male patients. Pain, limited ROM and weaken function in knee could effect QOL in OA patients to some extent. Health education and clinical treatment should be provided to improve the function of knee and QOL in OA patients.
9.The effect of transcranial electrical stimulation on functional recovery and the expression of microtubule-associated protein-2 and growth-associated protein-43 after cerebral focal ischemia

Objective To assess the influence of transcranial electric stimulation (TES) on the recovery of motor function after cerebral focal ischemia and reperfusion and to explore the mechanisms in terms of neural plasticity.Methods An acute focal ischemia-reperfusion model was established by transient occlusion of the right middle cerebral artery (MCAO).Seventy-two male Sprague-Dawley rats were randomly divided into a TES group,a model group,a sham-operation group and a normal group.The TES group was given TES 24 h after MCAO;the model group received the operation without any treatment.Forelimb placing (FPT) and beam walking (BWT) were mea-sured at the 3rd,7th,14th and 28th day after reperfusion.Microtubule-associated protein-2 (MAP-2) and growth-associated protein-43 (GAP-43) and grey levels of reaction products in the peri-infarct region were examined by immunohistochemical techniques.Results The TES group rats had markedly better FPT and BWT performance at the 7th,14th and 28th day after MCAO,compared with the model group.Expression of MAP-2 had increased significantly more at the 14th and 28th day in the peri-infarct region in the TES group compared with the model group.Expression of GAP-43 was significantly elevated in the peri-infarct region in the TES group compared with the model group at all time points.Conclusions TES can improve motor function and neural plasticity following cerebral ischemia and reperfusion damage.The functional enhancement may be partly due to up-regulation of the expression of GAP-43 and MAP-2 in the peri-infarct region.
10.Eclampsia:Cerebral CT Findings and Clinical and Imaging Correlation

Objective To assess the cerebral CT findings and clinical correlation in eclampsia.Methods Cranial CT scan was performed in 18 patients with eclampsia,3 cases underwent contrast-enhanced CT.Eight of them followed-up by CT scan.Results 14 cases had extensive low-density lesions represented cerebral edema involving the subcortical white matter in cerebrum with predominance in the occipital lobes,the frontal and parietal lobes also involved.4 cases appeared as small low-density foci in the brain,2 cases accompanied with multiple small intracranial hemorrhage.Conclusion CT scan is a good method in showing the cerebral lesions in patients with eclampsia,which can early find the lesions and can be used for follow-up.
