1.Effect of Yishen Shugan Yangxue decoction combined with Climen on menstrual blood volume, number of antral follicles and pregnancy outcomes of patients with decreasing ovarian store

Objective To investigate the effect of Yishen Shugan Yangxue decoction combined with Climen on patients with DOS.Methods 140 patients with DOS who were admitted to our hospital between June 2013 to August 2014 were randomly divided into two groups,70 cases in each groups. The control group were treated with Climen hormone replacement therapy, and the observation group were treated with Yishen Shugan Yangxue decoction combined with Climen. The clinical efficacy, menstrual, the number of antral follicles and pregnancy outcomes were observed. Results The total effective rate of treatment in the observation group (84.3%) was significantly higher than the control group (P<0.05). The scores of menstrual blood volume etc.was significant lower than the control group, the number of antral follicles was significant more than the control group (P<0.05). The baby holding rate of the observation groupwas 62.9%, were significant higher than the control group(P<0.05). Conclusion Yishen Shugan Yangxue decoction combined with Climen can effectively improve menstruation, increase the number of antral follicles, and improve the baby holding rate.
2.Double inhibitors diffuse synergy test for two kinds of extended-spectrum ?-lactamases

Objectives To consider the reliability of using flucloxacillin (FCC) and clavulanic with extended spectrum cephalosporin synergy method (double inhibitors diffuse synergy test, DIDST) to detect AmpC ? lactamases (AmpC BLA) and extended spectrum ? lactamases (ESBLs). Method The FCC and clavulanic were regarded as the inhibitors of AmpC BLA and ESBLs respectively. The disks of ceftriaxone and cefepime were placed 20 mm away from the inhibitors, the disks of cefotaxime and the inhibitors were 25 mm apart. DIDST with a double disk synergy test, a cefoxitin three dimensional test, and a double disk enhancement test detected AmpC BLA and ESBLs in 56 strains of gram negative bacilli at the same time. Results 20 (35.71%) strains of ESBLs, 12 (21 43%) for AmpC BLA and 1 (1.78%) for ESBLs+AmpC BLA were found using DIDST, in 56 strains of G - B. During the double disk synergy test was used for ESBLs, 15 strains were found when the distance of the disks was within 25 mm, another 5 strains were found when the distance decreased to 20 mm. Twenty strains of ESBLs were found using the double disk enhancement test, and 12 strains of AmpC using the cefoxitin three dimensional test. The result of DIDST was similar to that of other three kinds of test. Conclusions DIDST is an accurate and reliable method using one plate to detect ESBLs and AmpC BLA of the same time. It is suitable for different levels of hospitals.
3.The effects of simulated family nursing on the cognitive function and activity of daily living in patients with Alzheimer's disease

Objective To evaluate the effects of simulated family nursing on the cognitive function and activity of daily living in patients with Alzheimer's disease.Methods Sixty-eight patients with Alzheimer's disease were assigned to routine nursing condition and simulated family nursing condition.The patients were assessed with Mini-Mental State Examination (MMSE) and Activity of Daily Living Scale (ADL) before intervention,and 3 months and 6 months after intervention.Results Compared with the control group,the patients' cognitive function (t=2.31,P=0.026) and activity of daily living (t=2.59,P=0.012) were improved significantly in the experimental group.Conclusion The simulated family nursing can improve the cognitive function and activity of daily living in patients with Alzheimer's disease.
4.Clinical observation of tirofiban for emergency interventional therapy in patients with acute ST segment elevation myocardial infarction

Objective To investigate the efficacy and safety of tirofiban for emergency interventional therapy in patients with acute ST segment elevation myocardial infarction.Methods The 96 acute ST segment elevation myocardial infarction patients having accepted emergency percutaneous coronary intervention (PCI) were divided into treatment group and control group according to the treatment method with 48 cases each.The treatment group was given conventional standard treatment combined with tirofiban treatment [tirofiban intracoronary injection 10 μ g/kg and then intravenous 0.15 μ g/(kg· min) for 24-48 h].The control group was given conventional standard treatment only.The postoperative 60 min electrocardiogram ST segment recovery,the coronary blood flow TIMI grade,the major adverse cardiac events (angina,myocardial infarction,heart failure,death) and postoperative bleeding in 4 weeks were compared between the two groups.Results The patients having postoperative 60 min electrocardiogram ST segment recovery in treatment group was 45 cases (93.8%,45/48),in control group was 35 cases (72.9%,35/48),and there was statistical difference (P < 0.05).The incidence rate of infarction vascular TIMI grade ≥ 3 grade flow in treatment group was 95.8% (46/48),in control group was 75.0% (36/48),and there was statistical difference (P < 0.05).The incidence rate of the major adverse cardiac events 4 weeks in treatment group was significantly lower than that in control group,and there was statistical difference (P < 0.05).There was no statistical difference in the incidence rate of bleeding between the two groups (P > 0.05).Conclusion In patients with acute ST segment elevation myocardial infarction,the emergency interventional therapy with tirofiban is efficacy and safety.
5.Construction of Prokaryotic Expression Vector for Trichomonas vaginalis Silent Information Regulator 2 and Its Expression

Total RNA was isolated from Trichomonas vaginalis and Tv-Sir2-like cDNA was amplified by RT-PCR and cloned into pGEM-T Easy plasmid. A fragment of Tv-Sir2-like cDNA was subcloned into the expression vector pET-41b and expressed in E.coli BL21 with induction of IPTG. The full-length of Tv-Sir2-like cDNA was cloned and sequenced. The prokaryotic expression system of pET-41b/Tv-Sir2-like was constructed. The fusion protein of Tv-Sir2-like was expressed in E.coli BL21, occupying 30% of the total bacterial protein after being induced by IPTG for 5 h. SDS-PAGE analysis showed that the fusion protein was about Mr 59 000. The recombinant protein of Tv-Sir2-like is efficiently expressed in E.coli BL21.
6.Expression of hepatitis C virus H strain envelope glycoprotein in insect cells

Aim To express hepatitis C virus glycoprotein E (gE) deleting carboxy-terminal 31 amino acids, and detect anti-E antibody in HCV patients using expressed gE. Methods E gene derived from HCV H strain was inserted into baculovirus transfer vector containing a polyhedrin promotor. The recombinant plasmid was cotransfected into insect cell sf9 with a viral expression vector. The expression of gE was analyzed with Western blot, and the cells were used for dectecting antibodies against E1 and E2 in 35 hepatitis C patients by indirect immunofluorescence. Results A series of proteins with different relative molecular masses(Mr) could be detected by Western blot. Results from indirect immunofluorescence staining showed and only 4 patients were anti-E antibody positive gE was located in cytoplasm. Conclusion HCV gE is expressed successfully in insect cells, the study lay the foundation for further developing HCV vaccine.

Objective primary hospitals at the county level in xiaogan City 17 years for the production of scientific papers in qualitative and quantitative statistical analysis,summary of research progress and existing problems in county hospitals,and provides references for research management in local hospitals.Methods Wanfang medicine network included in 1998-2014 year published Xiaogan city 7 county hospital as the object;All biomedical scientific papers published as the first author to statistical analysis.Results 7 County Hospital of xiaogan City 17-year total output articles 1909,General journals 1273articles(66.68% per cent),Core journal 636 papers (33.31% per cent).Hanchuan city people's Hospital total the largest number of papers;total 544 articles (28.50% total),yingcheng hospital quality highest core journal papers and the highest proportion;total 186 (proportion of the total core thesis 29.25 %),minimum quantity and quality of outputs anlu city people's Hospital,only 15 articles in periodicals of General The core journals and 2 papers (0.89% of the total number of accounts for the paper,core paper accounted for 0.31% of the total).On the whole 2006-2009 published papers began to gradually slow growth,The number 2010-2014 entered the stage of rapid growth,but the core papers for slow growth.Conclusions xiaogan city grass-roots scientific articles in recent years in county hospitals are generally presented a trend of rapid growth,but the quality of growth is relatively slow,output quantity and quality distribution imbalances between hospitals.

Fructus Bruceae, the dry and ripe fruits of Brucea ja-vanica( L. ) Merr. ( Simaroubaceae) , has been used for dysenter-y, tumor, malaria and furunculosis treatment, and topical appli-cation for warts and corns. Quassinoids are the main chemical constituents of Fructus Bruceae, which have been proved to have anti-tumor, anti-inflammatory, insecticidal, anti-viral, anti-bac-terial and hypoglycemic activities, and so on. In view of the ex-tensive pharmacological activities of Fructus Bruceae, and better development and utilization of its medicinal value, the advances of quassinoids in Fructus Bruceae and their pharmacological ac-tivities are reviewed in this paper.
9.Study on bacteria contamination during the collection, processing and storage of umbilical cord blood

ObjectiveTo evaluate bacteria contamination during collection,processing and storage of cord blood to gain insight into contamination mechanism and direct prevention.MethodsFresh cord blood was separated by hydroxyethyl starch (HES) to harvest nucleated cells.The bacteria contamination was tested by culturing 10 ml plasma-red cells with BacT/ALERT 3D-480 automatic blood culture system.Total 87 positive samples were further identified for bacteria species.Ninety six cord blood nucleated cells concentrate with bacteria positive stored in liquid nitrogen(LN2) for 6-7 years were thawed at 37 C and re-cultured for bacteria analysis.ResultsWe collected 19 062 umbilical cord blood.Among them,336 was bacteria positive ( contamination rate 1.8 % ).Eighty-seven positive samples were further investigated with facultative bacteria 58 (66.7 % ),aerobic 38(43.7% ) and anaerobic 17( 19.5% ),Gram- negative accounted for 68% while positive 32%.The most frequent bacteria were Escherichia coli ( 25.3% ),Streptacoccus intermediate ( 14.9% ) and Chromobacteria violaceum(9.2% ).Ninety-six nucleated cells concentrate with bacteria positive were cryopreserved at liquid nitrogen for researching.Of them,83 samples( 86% ) showed positive of bacteria culture after deep-low temperature storage for 6-7 years.ConclusionsBacteria contamination rate of the cord blood collection,processing and storage in 2000 ~ 2007 was 1.8%.Stored in liquid nitrogen for 6-7 years,the viability of bacteria was 86%.The aseptic procedures of cord blood collection in delivery room should be intensified.The bacteria re-culture following thawing of cord blood cells is necessary before clinical transfusion.
10.Factors affecting blood lipid control among residents at a high risk of atherosclerotic cardiovascular disease

Objective:
To investigate the proportion of achieving the blood lipid control target and its influencing factors among residents at a high risk of atherosclerotic cardiovascular disease (ASCVD), so as to provide insights into management of blood lipid among residents at a high risk of ASCVD.
Methods:
Residents at a high risk of ASCVD and at ages of 35 to 70 years were sampled using a multi-stage cluster sampling method from 6 counties (districts) in Shaoxing City from May to July 2021. The residents' demographics, smoking, alcohol consumption and medical history of chronic diseases were collected using questionnaires, the height, weight, waist circumference (WC) and blood pressure were measured, and the total cholesterol (TC), triacylglycerol (TG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C) and fasting blood glucose were detected. The proportion of blood lipids achieving the control target was analyzed, and factors affecting the proportion of blood lipids achieving the control target were identified using a multivariable logistic regression model.
Results:
A total of 1 695 individuals at a high risk of ASCVD were enrolled, including 940 men (55.46%) and 755 women (44.54%), with a mean age of (62.56±6.08) years. There were 285 participants that achieved the target of blood lipid control (16.81%). Multivariable logistic regression analysis identified gender (male, OR=1.962, 95%CI: 1.396-2.758), age (OR=1.037, 95%CI: 1.013-1.061), WC (OR=0.979, 95%CI: 0.964-0.995), diastolic blood pressure (OR=0.981, 95%CI: 0.967-0.994), smoking (OR=1.485, 95%CI: 1.034-2.133), alcohol consumption (OR=0.684, 95%CI: 0.498-0.941), hypertension (OR=1.428, 95%CI: 1.006-2.207), administration of hypoglycemic drugs (OR=2.326, 95%CI: 1.720-3.144) as factors affecting the achievement of the target for blood lipid control among residents at a high risk of ASCVD.
Conclusions
Individuals at a high risk of ASCVD with higher WC, higher diastolic blood pressure and alcohol consumption are less likely to achieve the target for blood lipid control, while male individuals with older age, hypertension and administration of hypogcemic drugs are more likely to achieve the target for blood lipid control.
