1.Open acromioplasty for acromion impingement syndrome combined with rotator cuff injury

Objective To evaluate open acromioplasty for treating acromion impingement syndrome combined with rotator cuff injury. Methods From March 2004 to August 2007, 20 cases of acromion impingement syndrome combined with rotator cuff injury were treated in our department. During open acromioplasty, we repaired the rotator cuff, and reconstructed the insertion site with anchor screws for patients who had been diagnosed as complete rupture of superspinatus tendon. Postoperative rehabilitation program was conducted and all patients were followed up at our outpatient clinic. ASES (American Shoulder and Elbow Surgeons) scoring system was used to assess the outcome of the patients. Results Eighteen patients were followed up regularly for an average of 11 (6 to 13)months. The mean ASES score was 90. 5 postoperatively. A significant difference was found between the preoperative and postoperative ASES scores in this group (t= 13.865, P<0.01). Conclusion Open acromioplasty can achieve good outcome in treatment of acromion impingement syndrome combined with rotator cuff injury, and can be carried out by primary hospitals.
2.Multidrug resistance-associated proteins and their roles in multidrug resistance.

Multidrug resistance (MDR) is a major impediment to the success of cancer chemotherapy. Multidrug resistance-associated proteins (MRPs) play an important role in the process of MDR. As an ATP-binding cassette (ABC) transporter superfamily, MRPs are selective and specific drug efflux pumps. In this paper, physiological characteristics, structural characteristics and resistance profile of MRPs and the associated reversal studies are reviewed.
3.Analysis of clinical efficacy of neoadjuvant chemotherapy in patients with locally advanced cervical cancer

Objective To investigate the effects and clinical value of neoadjuvant chemotherapy in patients with locally advanced cervical cancer.Methods 102 cases with locally advanced cervical cancer treated were randomly divided into observation group and control group,the observation group received the surgery after the neoadjuvant chemotherapy,and the control group was given a direct surgical treatment.Operative time,blood loss and postoperative pathologic results were observed,and chemotherapy and toxicity evaluation were made for the observation group.Results The chemotherapy effective rate of the observation group was 74.51%,and the toxicity could be tolerated;the operation time and blood loss of the observation group were significantly lower than those of the control group(all P < 0.05) ;surgical trauma and postoperative complications' occurrence rate of the observation group were significantly lower than those of the control group (all P < 0.05) ; pelvic lymph node metastasis and vaginal cutting edge of the residues of the observation group were significantly lower than those of the control group (all P < 0.05).Conclusion For locally advanced cervical cancer,NACT has a positive significance in reducing the tumor volume,low-er tumor stage,and at the same time reducing the difficulty of operation,surgical trauma,postoperative complications,and preventing recurrence and metastasis,which can be used as a better adjuvant treatment program for locally advanced cervical cancer.
4.Regulation of osteoarthritis hypoxia-inducible factor-2alpharegulatory mechanism and application prospect

BACKGROUND:Hypoxia inducible factor-2 α(HIF-2α)is one of the key regulators in articular cartilage degeneration and endochondral osteogenesis. OBJECTIVE:To review the regulatory mechanism and application development of HIF-2α in articular cartilage degeneration. METHODS:A computer-based retrieval of PubMed and CNKI databases was performedforrelevant literatures published from January 1990 to October 2015 ,using the key words of “hypoxia inducible factor-2α; osteoarthritis; cartilage” in Chinese and English, respectively. RESULTS AND CONCLUSION:HIF-2α is a transcription factor encoded by the Epas1 gene, which is one of the key regulators in articular cartilage degeneration and endochondral osteogenesis by directly regulating the expressions of various catabolic genes to strengthen cartilage destruction in osteoarthritis. It has been reported that HIF-2α acts on the chondrocytes by regulating the expressions of Wnt/β-Catenin, nuclear factor-κ B, and interleukin-6.
5.Ultrasound-guided percutaneous ethanol injection in the treatment of hepatocellular carcinoma combined with portal vein tumor thrombosis

ObjectiveTo assess the clinical value of ultrasound-guided percutaneous ethanol injection (PEI) in the treatment of hepatocellular carcinoma (HCC) combined with portal vein tumor thrombosis (PVTT).MethodsNinety HCC patients with different type PVTT underwent ultrasound-guided PEI once or twice a week.The follow-up lasted 12 to 60 months,and the 1-,2-,3-,4- and 5-year survival rates were calculated.ResultsThe median survival time of HCC combined with PVTT patients treated with PEI was 12 months,and the 1-,2-,3-,4- and 5-year survival rates was 52.22%,23.33%,15.56%,12.22% and 10.00%,respectively.The survival rate correlated significantly to the type of PVTT.The lower the type of PVTT was,the longer the patients lived.ConclusionUltrasound-guided PEI is a safe and effective method for the treatment of HCC combined with PVTT,and can prolong the survival time of HCC patients with low type PVTT.
6.Review on long non-coding RNA DANCR enhances chondrogenic differentiation and proliferation of human synovium-derived mesenchymal stem cell

Long non-coding RNA( LncRNA) is an RNA molecule that is longer than 200 nucleotides and is not translated into a protein.LncRNA DANCR has been identified in hepatocellular carcinoma ( HCC) and markedly increased stemness features of HCC cells to promote tumorigenesis.Recent studies show that DANCR contributes to the differentiation and proliferation of synovium-derived mesenchymal stem cell ( SMSCs) and may be as a key point for this process.This article reviews the role of long non-coding RNA DANCR in enhancing chondrogenic differentiation and proliferation of human SMSCs.
7.Ultrasonic Characteristics of Complex Fibroadenoma of Breast and Its Differential Diagnosis from Invasive Ductal Carcinoma

Purpose To explore ultrasonic characteristics of complex fibroadenoma (CF) of breast and its differential diagnosis from invasive ductal carcinoma (IDC).Materials and Methods Sixty-one patients with CF and 81 patients with IDC from January 2010 to February 2016 in the Affiliated Union Hospital of Fujian Medical University were enrolled,and the CF and IDC were confirmed by pathology after operation.The preoperative sonographic findings of CF and IDC were retrospectively analyzed and compared.Results Fifty-nine cases with CF were detected by ultrasonography in 61 cases.Ultrasonic findings of 59 cases of CF showed as follows:79.7% (47/59) of the CF cases with an aspect ratio <0.7;79.7% (47/59) with blood flow signal of 0 to 1 level;72.9% (43/59) with liquid dark area,including 67.8% (40/59) with round or oval cystic fluid area showing local sievelike or diffused distribution;72.9% (43/59) with irregular shape;61.0% (36/59) with obscure boundary;61.0% (36/59) with the largest diameter of tumor <2 cm;59.3% (35/59) with calcifications,including 33.9% (20/59) with coarse calcification,25.4% (15/59) with microcalcification and 85.7% (30/35) with distribution of calcified lesions along the linear high echo zone;54.2% (32/59) with echo heterogenicity showing linear high echo and high echo in the middle and low echo.There were significant differences in the detection rates of internal regular fluid area,internal linear high echo or high echo area,and alcification distribution along the linear high echo between CF and IDC lesions (all P<0.01).Compared with IDC,there were higher rates in CF for detecting lesions of largest diameter of tumor <2 cm,more regular shape,more clear border,poorer blood flow signal (P<0.05),but there were lower rates in CF for detecting axillary lymph node enlargement,lesions with edge or spiculation and hyperechoic halo (P<0.01).Conclusion Color Doppler ultrasonography is of great value in the diagnosis of CF and its differential diagnosis from IDC.
8.Long non-coding RNA DANCR enhances chondrogenic differentiation and proliferation of human synovium-derived mesenchymal stem cells

BACKGROUND: A number of studies have shown that long non-coding RNA DANCR can play an important role in various pathophysiological processes through Wnt/β-catenin signaling pathway.OBJECTIVE: To explore the effect of long non-coding RNA DANCR on the proliferation and chondrogenesis of synovium-derived mesenchymal stem cells.METHODS: Passage 3 synovium-derived mesenchymal stem cells were obtained and transfected with pcDNA3.1-GP (control) and pcDNA3.1-GP(DANCR Homo) (experimental). Cell viability was estimated at 1, 2, 3, 4, 5, 6 and 7 days after DANCR transfection. The passage 3 cells were cultured in the chondriogenic medium for 14 days. And the chondrogenesis potential of cells was examined by toluidine blue staining. The chondrogenic-specific marker genes Aggrecan, Type II collagen (Col2) and Sox9 were determined by Real-time PCR.RESULTS AND CONCLUSION: The synovium-derived mesenchymal stem cells exhibited S-shaped curves in the two groups, with cell arrest at 1-2 days and rapid proliferation beginning at 3 days. Cell counting kit-8 assay and toluidine blue staining showed overexpressing DANCR significantly promoted proliferation in synovium-derived mesenchymal stem cells. The aggregates from synovium-derived mesenchymal stem cells in the experimental group had a greater amount of toluidine blue staining than the control group. In addition, we detected the higher expression of chondrogenic specific marker genes, such as Col2, Sox9 and Aggrecan, in the experimental group than the control group at 14 days after chondrogenic induction (P < 0.05). These results demonstrate that long non-coding RNA DANCR could enhance chondrogenic differentiation and proliferation of human synovium-derived mesenchymal stem cells and increase the expression of chondrogenic specific marker genes.
9.Clinical and pathological study on retroperitoneal angiomyolipoma

Objective To analyze the clinical and pathological features,diagnosis and differential diagnosis,treatment and prognosis of retroperitoneal angiomyolipoma (RAML).Methods The clinical data of 3 cases diagnosed with RAML,during 2012 to 2014 were studied.The expression of AE1/AE3,Vimentin,HMB45,Melan-A,S-100,SMA,CD10,CD34 and Ki-67 were detected by full automatic immunohistochemistry instrument.Four cases were followed up,the relevant published articles were reviewed as well.Results Four patients contained 3 female and 1 male.Three patients of them were found because of abdominal pain and discomfort symptom,1 case was found on examination.All of them more than 10 cm,and the boundary was not clear.Tumor resection + nephrectomy were used for treatment.Macroscopically,the tumor were consisted of fat,muscle cells and thick walled blood vessels.But they had different proportion.Immunohistochemically:AE1/AE3 negative; Vimentin,HMB45,Melan-A,SMA were positive; S-100 was positive in 3 cases and negative in 1 case; Ki-67 proliferation index were <5%.Conclusions The RAML is a rare benign retroperitoneal stromal tumor,which has a complex and diverse pathological organizations.The diagnosis can be made by special immune phenotype in combination with microscopic appearance.
10.A novel nanometer non-viral gene transfer system for cartilage defects

Objective To develop a clinically applicable approach to enhance repair of cartilage defects by constructing an in vivo non-viral gene transfer system targeting chondrocytes. Methods High molecular weight chitosan (HMWC) was degraded to produce low molecular weight chitosan (LM-WC) that was combined with transforming growth factor-β1 (TGF-β1) plasmid to form stable nano-sizc complexes. After being tested in vitro firstly, these nano-size complexes were injected into the knee joint of New Zealand white rabbit models with full-thickness cartilage defects to detect their feasibility of delive-ring the growth factor gent in vivo. Results The results showed that LMWC/DNA nano-sizc comple-xes could deliver the gone into the cultured chondroeytes and cartilage tissue efficiently in vitro. When used in vivo, LMWC/TGF-β1 gene nano-size complexes could enhance the transfection efficiency and prolong the expression of TGF-β1 gone. In the animal models of articular osteechondral defect of rabbits, better healing and gentler degeneration could be observed in comparison with the control. Conclusion In vivo transfection of LMWC/TGF-β1 nano-size complexe is a safe and effective method to early promote the repair of osteochondral defects.
