Objective To study the fouling mechanism during the filtration of traditional Chinese medicine decoction by inorganic ceramic membrane so as to make a valuable exploration both on membrane fouling prevention and the industrialization of membrane separation technology in the field of Chinese materia medica. Methods Micro-filtrating Qingluo Tongbi Decoction using alumina ceramic membrane, researching the applicability of the membrane with different pore sizes, observing the fouling on membrane surface by scanning electron microscopy, studying the resistance distribution of membrane in filtration process, analyzing the fouling existence shape, position, and forming rule. Results In this system, alumina membrane with the pore size of 0.2 ?m was most suitable, which had the biggest permeating flux and reservation rate of effective component with less fouling. Among the alumina membranes with three different pore sizes, auto resistance and pore block resistance made up inferior percentage in total resistance, fouling resistance distribution mainly consisted of the reversible resistance, such as concentration polarization and gel layer resistance. Conclusion Optimizing parameters of the techniques in membrane process, using suitable pretreatment method and cleaning method can reduce membrance fouling and make the memberane property recover.

BACKGROUND: Encopresis after operation for congenital.ectopic anus can cause psychological, physiological and social ability disorders.OBJECTIVE: To treat the children with encopresis with biofeedback training so as to improve the function of post-operative defecation.DESIGN: A self-controlled trial.SETTING: Department of Pediatric Surgery, Second Clinical College of China Medical University.PARTICIPANTS: Totally 20 cases of encopresis after treatment with operation for ectopic anus, were recruited from the Department of Pediatric Surgery, Second Clinical College of China Medical University, between January 1998 and October 2004. Among them, there were 4 cases of complete encopresis, 7 cases of loose encopresis and 9 cases of blotch. All the cases were followed up. There were 9 cases of ectopic anus in the middle and lower parts, and 11 cases of ectopic anus at the high part.METHODS: The objective measuring methods of biofeedback training such as anorectal pressure and anus sphincter electromyography were used to train the patients with postoperative encopresis. After one-month self-directed training in anus contraction and defecation habit, the children received proper biofeedback training. ① Biofeedback training to strengthen the muscles around the anus twice per day. The portable biofeedback-training machine was taken home after the children patients could automically contract the muscles around the anus 3 weeks later. ② Biofeedback training to improve rectal sensitivity and coordination of anus sphincter, and repeated training in expanding saccus. A normal defecation reflex was established. Anus sphincter presented reflex contraction to prevent encopresis once the rectum expanded. ③ Defecation training was performed for 30 minutes after meals every day. ④ Electrostimulation combined with biofeedback training was performed for 10 minutes twice a day for 3 or 4 consecutive weeks.MAIN OUTCOME MEASURES: ① The maximum contraction anal pressure, vector volume, and electromyographic amplitude of external sphincter of anus; ② positive rate of anorectal contraction reflex of the children patients before and after training.RESULTS: According to actual treatment analysis, all the 20 children patients entered the result analysis. ① In middle and lower parts groups, the maximum contraction anal pressure increased from (11.87±5.61) kPa before training to (24.88±16.58) kPa after training; in high part group, they increased from (5.76±3.84)kPa to (18.18±13.71) kPa (P< 0.05). ② In middle and lower parts groups, the vector volume increased from (139.17±130.02) cm (cm Hg)2 to 608.10±131.06 cm·(cmHg)2, whereas in high group it increased from (117.01±74.35) cm(cm Hg)2 to (452.17±69.43) cm(cm Hg)2 (P< 0.05). ③ In middle and lower parts groups, the electromyographic amplitude of external sphincter of anus increased from (152.20±37.42) μV to (324.12±67.78) μV; in high part group, it rose from (114.08±51.41)μV to (266.18±49.38)μV (P < 0.05). ④ The positive rate of anorectal contraction reflex improved from pretraining 55% (11/20) to post-training 90% (18/20).CONCLUSION: After biofeedback training, the maximum contraction anal pressure, anorectal sensitivity and the coordination of anus external sphincter, defecation habit, and contractibility of external sphincter of anus were all improved obviously, especially in those with ectopic anus in the middle or lower parts. Anus external sphincter function can be improved to the uttermost so as to cure encopresis.

Objective To improve the recognition and reduce the misdiagnosis of bipartite patella by analyzing and summarizing the imaging features. Methods X-ray,CT and MR images of 1 7 cases of bipartite patella were collected, imaging features were ana-lyzed and summarized. Results X-ray revealed the smooth interface of the main and accessory patella fragments with sclerotic mar-gins,the cleft was slightly wide but not sharp.CT showed the bone sclerosis of interface was mostly smooth, rough and vesicular interface could also be found in a few patients, low density soft tissue could be found in separation zone.MRI could detect the intact but thinned cartilage covering the main and accessory patella fragments, fibrous like tissues could be found in separation zone with MRI.Conclusion Imaging findings of bipartite patella are characteristic, these features are helpful in the diagnosis and differential di-agnosis of bipartite patella.

Object To investigate the effects of ultra-fine powder technique and granularity of pellets on dissolution rate in vitro. Methods The dissolution rate of ultra-fine ERMIAO PILL with different granule diameters in vitro was measured and compared with the index of berberine by UV spectro-photometry. Results The dissolution parameters T 50 and T d of four kinds of ultra-fine ERMIAO PILL are 61.60, 19.48, 17.84, 8.97 min and 102.3, 33.29, 26.98, 14.77 min, respectively. Those of general powder ERMIAO PILL with granule diameter of 2.4 mm are 89.61 and 155.68 min. Conclusion The dissolution rate of ultra-fine powder is quicker than that of general powder, and the rate increases with the granularity of PILL decreasing.

AIM To determine the relationship between the elevation of endogenous inhibitor of nitric oxide synthase (NOS)N G,N G-asymmetric dimethylarginine (ADMA) and metabolic control in diabetic rats. METHODS Diabetes was induced in Sprague-Dawley rats by a single intraperitoneal injection of streptozotocin. At 72 h after injection, half of diabetic rats received insulin treatment for 8 weeks (20 U?kg -1?d -1,ih, bid). Serum levels of ADMA were measured by high-performance liquid chromatography. Thoracic aortic rings from non-diabetic age-matched control, untreated diabetic, and insulin-treated diabetic rats were tethered in isolated organ baths, contracted with 1 ?mol?L -1 phenylephrine, and challenged with either the endothelium-dependent vasodilator acetylcholine or the endothelium-independent vasodilator sodium nitroprusside. Serum concentrations of glucose, glycosylated serum protein, and malondialdehyde, derived from lipid peroxidation were also examined to estimate metabolic control.RESULTS Serum levels of ADMA significantly elevated in untreated diabetic rats compared with control rats. This elevation of ADMA was accompanied by impairment of relaxation response to acetylcholine but not sodium nitroprusside in aortic rings. Chronic insulin treatment not only prevented the elevation of serum ADMA, but also improved the impaired endothelium-dependent relaxation in diabetic rats. Serum levels of glucose, glycosylated serum protein, and malondialdehyde were significantly increased in parallel with the elevation of ADMA in untreated diabetic rats compared with control rats. These parameters were normalized after diabetic rats received insulin treatment. CONCLUSION These results provide the first evidence that the elevation of endogenous inhibitor of NOS in streptozotocin-induced diabetic rats is close related to metabolic control of the disease.
