Objective To determine the potential deficiency due to lack of anaerobic culture and evaluate the effect to reduce adverse reaction associated to transfusion-translated bacterial infection.Methods The result of 9 758 units of apheresis platelet concentrates (PCs)detected with automated microbial detection system were reviewed and the medical records of the patients that received the contaminated PCs were followed.Results The confirmed positive rates by aerobic and anaerobic cultures were 0.06％ (6/9 758)and 0.16％ (16/9 758),respectively.In 10 of 16 yield cases,only the anaerobic culture was positive.The most of the bacterial detected by anaerobic culture only were Propionibacterium acnes.Their mean detection time from inoculation was 96.8±18.21 hours.Conclusion Addition of anaerobic culture would enhance the detection of bacterial contamination in PCs.However,since only slow-growing bacteria were detected,and because their clinical significance was debatable,blood service should select feasible and costeffective projects using only aerobic bottle for bacterial screening,like the majority of licensed blood centers in North America and Hong Kong,China.

Retrospective analyses of clinical data were performed for 518 elderly cataract patients (856 eyes) of Disabled Persons' Federation in Hangzhou,Zhejiang Province.For 69 eyes (8.06％),corneal edema was a major factor of low vision during early postoperative period; for 23 eyes (2.69％),delayed cataract was an important cause of low vision during long-term postoperative period.The best corrected visual acuity of 120 eyes caused by nonsurgical factors was diabetic retinopathy (48 eyes,48.00％),senile macular degeneration (25 eyes,20.80％),high myopia retinopathy (17 eyes,14.20％),optic never injury in glaucoma (16 eyes,13.30％),macular membrane & hole (5 eyes,4.20％) and amblyopia & corneal leucoma (9 eyes,7.50％).Retinal disease is the most important reason of low vision after cataract surgery.And surgery remains the most effective treatment for cataract.

Objective To investigate the effect of liquid temperature, pressure, concentration, pH parameters on the membrane process and rejection in the process of using reverse osmosis membrane to concentrate oil-bearing water bodies ultrafiltrate of Compound Chuanxiong Capsules, and optimize the process. Methods With oil-bearing water bodies ultrafiltrate of Compound Chuanxiong Capsules volatile oil as the research object, the changes in flux under different operating parameters was measured. Results For this system, suitable operation parameters was as follows: pressure was 1.6 MPa, temperature was 35～ 40 ℃, pH was 9～10. Conclusion Using reverse osmosis membrane to concentrate oil-bearing water bodies ultrafiltrate of Compound Chuanxiong Capsules volatile oil, better flux and rejection can be obtained at the appropriate operating conditions.

Equipping tumor-targeting carrier with cell-penetrating peptide with high transduction efficacy has become a trend. According to the fusion modes and tumor-targeting mechanisms, cell-penetrating peptides can be divided into five categories: first, self-targeting cell-penetrating peptides; second, fusion carriers made of targeting ligands and cell-penetrating peptides; third, cell-penetrating peptide-modified nano-carrier; fourth, tumor-microenvironment-targeting cell-penetrating peptides; fifth, other special cell-penetrating peptides. Fusion carriers, which can not be effectively released into the cytoplasm after transducted into target cells, can be further modified to increase their efficacy. In all, cell-penetrating peptide introduced into tumor-targeting carrier should provide a new era for anti-tumor pharmacy research and cancer therapy.

Objective To define a suitable and adequate sample volume for the detection of bacterial contamination in cord blood (CB) via BacT/ALERT system. Methods A total of 1 376 cord blood units were routinely screened for aerobic and anaerobic microorganisms by culturing the erythrocyte fraction(EF) or plasma fraction(PF) with 20 ml and 10 ml sample volume. The microbial contamination was detected by BacT/ALERT 3D system, and the results of this method were compared with the improved Martin/thiogly collate broth(22 ℃) and thioglycollate broth(35 ℃).Results The rates of positive BacT/ALERT results were 1.16%(10 ml) and 1.24%(20 ml) for PF, and 3.20%(20 ml) for EF. The positive rate of Martin/thiogly collate broth(22 ℃/35 ℃) was 1.00% for EF. Conclusion Using BacT/ALERT system for microbial detection in cord blood bank can enhance the security of umbilical cord blood transplantation.

Objective To investigate the protective role of two-step decapping technique for superior parathyroid glands in total thyroidectomy surgery.Methods The clinical data of 136 consecutive cases underwenttwo-step decapping technique in total thyroidectomy plus central compartment dissection from April 2013 to January 2016 were retrospectively analyzed.And the incidences of postoperative hypocalcemia were was compared with conventional consecutive decapping surgical methods containing 113 cases from January 2010 to March 2013.All operations were performed by the same treatment group.Results In the two-step decapping group,28 cases(28/136,20.6％) displayed postoperative temporary hypoparathyroidism and a higher incidence were was observed in 37 cases (37/113,32.7％) in the conventional method group,the difference was statistically significant(x2 =4.737,P =0.03).Whereas,2 cases(2/136,1.5％) suffered permanent hypoparathyroidism in the two-step decapping group and 7 cases (7/113,6.2％) of permanent hypoparathyroidism in the conventional method group,the difference was statistically significant (x2 =3.954,P =0.047).Conclusion Two-step decapping technique can more effectively protect the parathyroid gland function.It is worthy of popularization.

Objective To explore the surgical treatment effect of hyperthyroidism.Methods The clinical datas of 508 hyperthyroidism patient operation cure were reviewed.Results 2 cases had hypothyroidsim,no signs indicated injuries of superior laryngeal nerve and recurrent laryngeal,no recurrence of hyperthyroidism,no one died.Conclusion The thyroidectomy is the best treatment for the hyperthyroidism.Sufficient prepare before operation can reduce complications.

Objective To investigate the protection of superior parathyroid glands by improving dissection procedure of thyroid superior pole in total thyroidectomy surgery.Methods 136 consecutive cases undergoing two-step dissociating procedure of thyroid superior pole(the improved group) in total thyroidectomy plus central compartment dissections from Sep.2012 to Oct.2015 were retrospectively analyzed.Intraoperative observation of superior parathyroid and incidences of postoperative hypoparathyroidism were compared with conventional surgical methods (the conventional group) containing 113 cases from Jun.2009 to Sep.2012.All operations were performed by the same treatment group.Results Intraoperatively,249 (249/272,91.5％) superior parathyroid glands were identified in the improved group and 215 (215/226,95.1％) superior parathyroid glands were identified in the conventional group.The difference had no statistical significance (P＝0.114).Whereas,among identified superior parathyroid glands,242 (242/249,97.1％)of them were conserved in situ in the improved group and 198 (198/215,92.0％) were conserved in situ in the conventional group.The difference had no statistical significance (P＝0.013).Among superior parathyroid glands conserved in situ,225 (225/242,93.0％) had good blood supply in the improved group and 170(170/198,85.9％) in the conventional group.The difference had statistically significance (P＝0.001).28 (28/136,20.5％) patients displayed temporary hypoparathyroidism and 2 (2/136,1.5％) cases suffered permanent hypoparathyroidism in the improved group after surgery.Whereas,in the conventional method group,a higher incidence was observed in 37 (37/113,32.7％) cases of temporary hypoparathyroidism and 7 (7/113,6.2％) cases of permanent hypoparathyroidism respectively.The difference had statisticall significance (P＝0.003).Conclusions Two-step dissociating procedure technique offers protective effect on superior parathyroid glands function.It is worthy of popularization.

BACKGROUND: Surgery is the common therapy for pterygium, and there are several surgical management techniques.OBJECTIVE: To clinically assess the effect of pterygium retro-resection followed by the transposition of conjunctival autograft rich in stem cells.DESIGN: Follow-up of the cases.SETTING: Department of Ophthalmology, the Fourth Affiliated Hospital of China Medical University.PARTICIPANTS: Fifty patients (60 eyes) with pterygium, who were treated in the Fourth Affiliated Hospital of China Medical University from May 2003 to May 2006, were selected. All patients agreed to receive the treatment and participate in the follow-up. The trial was permitted by the Hospital Ethics Committee.METHODS: The head of pterygium was separated from cornea, including the conjunctiva and the underlying proliferating tissue towards lacrimal caruncle until plica semilunaris. The pterygium was totally removed. The adjacent healthy conjunctiva harboring stem cells was transposed to cover the naked sclera. The patients were evaluated following the operation.MAIN OUTCOME MEASURES: ①Epithelization of cornea and conjunctiva; ②reoccurrence of pterygium.RESULTS: ①The epithelium of cornea and conjunctiva in all cases (60 eyes) healed within 1-2 days after the operation. ②The patients were followed for 8-16 months after the sutures were removed. Out of the total of 60 eyes, 26 were followed for 8-12 months and 34 for 13-16 months. The average length of observation was 12 months. Fifty-eight eyes healed completely, and reoccurrence took place in 2 cases.CONCLUSION: Pterygium resection followed by the transposition of adjacent conjunctival autograft harboring stem cells is easy to perform and effective to reduce the recurrence of the lesion.

Objective To explore the effects of CD4+CD25+Treg cell on the allograft after infusion of dendritic cells (DCs) with low expression of CD40 from donor in mouse heart transplantation.Methods In vitro,mouse bone marrow-derived DCs were infected by CD40-RNAi lentiviral vector,and tolerogenic DCs (Tol-DCs) with low expression of CD40 were prepared.A heterotopic abdominal heart transplantation model was established in mice,and the other three groups that were control group,noninfected DC group and lentivirus infected DC group were designed correspondingly.Cardiac allograft survival time was recorded and pathological grading for acute rejection was assessed on the 7 d after heterotopic abdominal heart transplantation.Concentrations of CD4+CD25+Treg cells in peripheral blood were analyzed before and after transplantation by flow cytometry.Results After 48 h infection of DCs by CD40-RNAi lentiviral vector in vitro,the expression of CD40 mRNA was down-regulated significantly,whose inhibition rate was 80.9％.The expression of CD40 was decreased from 74.37％ ±4.08％ to 40.07％ ± 4.03％ (P＜0.05) after 48 h infection.Compared with the control group and the noninfected DC group,the cardiac allograft survival time was significantly prolonged in the CD40-RNAi lentivirus infected DC group,which was (14 ± 4) d(P＜0.01) ; concentrations of CD4+CD25+Treg cells in peripheral blood were increased both on the 3 d and the 7 d after transplantation (P＜0.05) ; the pathological grading for acute rejection was decreased on the 7 d after transplantation (P＜0.05).Conclusion The CD4+CD25+Treg cell in peripheral blood was protective to cardiac allograft in prolonging its survival time in mouse heart transplantation.
