1.Effects of Different Transdermal Enhancers on Percutaneous Absorption of Ephedrine in Compound Herba Ephedrae Adhesive Plaster

Objective To investigate the effect of different transdermal enhancers on percutaneous absorption of ephedrine in Compound Herba Ephedrae Adhesive Plaster.Methods Using modified Franz diffusion cell,the ephedrine content was determined to investigate the effect of different transdermal enhancers in promoting percutaneous absorption.Results Azone or azone used together with other transderml enhancers had a better effect in promoting percutaneous absorption,indicating that azone was the leading transderml enhancers for ephedrine.The mixed transdermal enhancer of azone and propylene glycol was the most effective combination and the rate constant of percutaneous absorption is 94.280 ?g?cm-2?h-1.Conclusion The mixture of azone and propylene glycol can enthance the percutaneous absorption of ephedrine and can be used as the transdermal enhancers for this compound plaster.
2.Classification and incidence of congenital coronary anomalies in adults

Objective To determine the pattern and incidence of congenital coronary anomalies (CCA) in adults undergoing coronary angiography. Methods The diagnostic angiographic data collected from October 2000 to October 2006 were reviewed, from which congenital coronary anomalies were found, and their angiographic features were studied. Results Among 1520 adults undergone coronary angiography, anomalous coronary arteries were found in 58 cases (3.82%). Depending on the anatomic characteristics of anomalous coronary arteries, these 58 cases were classified as following. Anomalies of origin and distribution were found in 26 cases (1.71%), among them separated origin of left anterior descending and left circumflex from left coronary sinus were found in 14 cases, left main trunk and left circumflex arising from right coronary sinus in 1 case for each, single left coronary artery in 2 cases, right coronary arteries arising from ascending aorta in 4 cases, from left coronary sinus in 3 cases, and from posterior coronary sinus in 1 case, respectively. Anomalies of coronary termination were observed in 11 cases (0.72%), and all of them were coronary artery fistulas. Anomalies of intrinsic coronary arterial anatomy in 21 cases (1.38%), among them myocardial bridge in 18 cases, hypoplasia of left circumflex artery in 2 cases, and sinus node artery arising from left posterior ventricular branch in 1 case. No anomalous collateral vessels were found. Conclusion Coronary angiography is one of the important methods of detecting anomalous coronary arteries. Classification based on the anatomic characteristics can help clarify the pattern and incidence of congenital anomalies of coronary arteries in adalts.

The Epstein-Barr virus (EBV) is a gamma herpes virus associated with several types of malignancies. The EBV encodes viral microRNAs (miRNAs) that can target genes within cells. The EBV participates in signal transduction as well as the proliferation and differentiation of cells. How the target genes and functions of EBV-encoded miRNAs contribute to the pathogenesis of EBV is an important research topic. Some target genes have been validated since EBV-encoded miRNAs were discovered and, in this article, we summarize them and their functions.

4.Study on Refining of Ephedra, Apricot kernel, Gypsum, and Licorice Decoction and Redujing Granules by Microfiltration Technique of Inorganic Ceramic Membrane

Objective: To evaluate effect of refining of the aqneous extract solution of compound Chinese traditional medicine by microfiltration technique of inorganic ceramic membrane. Methods: The aqneous extract solution of Ephedra, Apricot kernel, Gypsum, and Licorice Decoction and Redujing Granules were processed by microfiltration technique of inorganic ceramic membrane. The changes in the characters, whole solids and effective componds before and after process were studied. Results: The aqueous extract solution of compound Chinese traditional medicine all were turbid before microfiltering and became clear after microfiltrating. The whole solid of Ephedra, Apricot kernel, Gypsum, and Licorice Decoction were decreased by 16.12%, and the lost rates of ephedrine and amygdalin were 20.11% and 18.06% respectively. The whole solid of Redujing Granules were decreased by 27.58%, and the lost rates of chlorogenic acid and rhein were 18.28% and 22.86% respectively. Conclusion: The microfiltration technology of inorganic ceramic membrane has the better effects of clarification and removing impurity on the aqueous extract solution of compound Chinese traditional medicine.
5.Study on Refining of Decoctions of 7 Medicinal Materials of Radix and Rhizome by Microfiltration of Inorganic Ceramic Membrane

Objective: To evaluate effect of refining of decoctions of medicinal materials of radix and rhizome by microfiltration of inorganic ceramic membrane. Methods: The decoctions of 7 medicinal materials of radix and rhizome were processed by inorganic ceramic membrane, and the whole solids and effective ingredients were determined. Results: The decoctions of Chinese traditional medicine became clear after microfiltration. The whole solids were decreased by 15~38%, and the lost rate of effective ingredients was lower than that of whole solids. Conclusion: The microfiltrating technology of inorganic ceramic membrane can make decoctions of Chinese Traditional Medicine clear.
6.Decision-making under risk condition in benign epilepsy children with centrotemporal spikes

Objective To investigate the ability of decision-making under risk condition in benign epilepsy children with centrotemporal spikes (BECT).Method Seventy-eight BECT patients and seventyfive healthycontrols were studied by Iowa Gambling Task (IGT) with inexplicit probability and Game of Dice Test (GDT) with explicit probability.Results (1) There was a significant difference in IGT task scores between the two group (t=-2.56,P<0.05).The total number of advantageous choices in IGT were lower in BECT group (49.35±5.38) than that in healthy control group (52.24±6.23).BECT group (-1.41 ± 10.60) performed worse on task than healthy control group (4.43±11.88),and the difference was statistically significant (t=-3.21,P<0.05).The control group scored higher on Blockl,Block4 and Block5 than that of BECT group (P<0.05).(2) There was no significant difference between the BECT group (4.42±7.74) and the control group(5.49±6.81) in the GDT test scores (t=-0.91,P=0.37).(3) Pearson correlation analysis showed that there was no significant correlation between the age,age of onset,education,seizure frequency and onset duration and the score of IGT and GDT (r =-0.091-0.274,all P > 0.05).Conclusion There is no significant decision-making disability in children with BECT under the condition of certain risk probability,while there is significant impairment under the condition of uncertain risk probability.The decision-making disability may be associated with impaired frontal lobe function.
7.Study of white matter hyperintensities of Parkinson's disease by MRI

ObjectiveTo explore the correlation between white matter hyperintensities(WMH) and the clinical features of Parkinson's disease(PD) when examined by magnetic resonance imaging(MRI).MethodsThe correlation between WMH,periventricular hyperintensities(PVH),deep white matter hyperintense signals(DWMH) in MRI and age,disease duration,distribution in 64 patients with PD and 45 age-matched healthy controls was studied.ResultsThe frequency of WMH was higher in patients with PD,but no significant difference compared with controls(P>0.05).The patients with WMH were significantly older(P<0.01) and more severity(P<0.01) than those without WMH,but disease duration was no significant difference(P>0.05).The frequency of DWMH was higher in patients with PD,but no significant difference compared with controls(P>0.05).The patients with DWMH were significantly older(P<0.01) than those without DWMH,but disease duration and severity were no significant difference(P>0.05).The frequency of PVH was higher in patients with PD and having a significant difference compared with controls(P<0.05).The patients with PVH were significantly older(P<0.01) and more severity than those without PVH,but disease duration was no significant difference(P>0.05).The distribution of DWMH was not significantly different between patients with PD and controls (P>0.05),but distribution of PVH was significantly different between patients with PD and controls(P<(0.05)).ConclusionPVH may influence some clinical features of PD characterized by a greater disease severity.
8.The study on the immune suppression with severity of HBV-related acute-on-chronic liver failure patients

Objective To explore the association of immune suppression with the severity of HBV-related acute-on-chronic liver failure(ACLF) patients.Methods From August 2009 to April 2010 in Shanghai Public Health Clinical Center, the peripheral blood samples and clinical data of 27 HBV-related ACLF patients (ACLF group), 28 patients wit h chronic active hepatitis B (CHB group)and 8 healthy individuals (Control group) were collected.APACHE Ⅲ score and the grade of hepatic encephalopathy were as quantitative index to evaluate the severity degree of the disease.The absolute counts of the subsets of T lymphocytes and human leukocyte antigen (HLA)-DR expression on the surface of monocyte in patients' peripheral blood were examined by flow cytometry, the proinflammatory cytokines and anti-inflammatory cytokines(IFN-γ, TNF-α, IL-2, IL-4, IL-10) in patients' plasma were detected by cytometric bead array (CBA) kit.The data was analyzed with SPSS 16.0 software.Results Compared with CHB group and control group, the level of anti-inflammatory eytokire IL-10 markedly increased ir HBV-related ACLF patients (Z= -4.279 ,U= 124, P<0.01;Z= - 3.871, U= 9.5, P= 0.0001 ), however the level of pro-inflammatory cytokines IFN-γ、 TFN-α、IL-2 、 IL-4 in plasma were at low limit of detectable value.Meanwhile the expression quantity of HLA-DR on the peripheral blood monocytes significantly down-regulated (Z= -4.714, U= 98, P<0.01;7= - 4.086, U = 4, P< 0.01), and there was negative correlation between HLA-DR expression quantity and APACHE Ⅲ score (R2 =0.2667, P=0.0167).In addition, the absolute counts of CD4+T lymphocytes in adaptive immune cells significantly decreased (Z= -4.411, U= 116, P<0.01; Z=-3.575, U= 17, P= 0.0004).Conclusions The immune system of HBV-related ACLF patients displays immune dysfunction like monocyte function inhibition; CD4+ T lymphocytes depletion and high level of anti-inflammatory eytokines, the persistent down-regulation of the HLA-DR expression on monocyte is an indicator for the severity of disease.
9.A single center experience of combined liver and kidney transplantation

Objective To summarize the experience of treating the end stage of liver disease complicated with renal failure using combined liver-kidney transplantation.Methods The clinical data of 28 cases receiving combined liver-kidney transplantation were retrospectively analyzed, including the inclusion criteria of surgical indications, modus operandi, protocol of immunosuppression and the prognosis post-operation.Results Among these 28 cases in our study, 22 cases suffered from liver and renal failure, accounting for 78.6%; 4 cases were diagnosed as having hepatorenal syndrome, accounting for 14.3%; and 1 case had hyperoxaluria and polycystic liver with polycystic kidney. As for the modus operandi we used, piggy-back procedure was adopted for 4 patients and classic procedure without bypass was used for the rest. Donor kidneys were all put in the right iliac fossa. During the follow-up period of 5 months to 7 years, one-and 3-year survival rate of the recipients was 92.9% and 78.3% respectively. Among these 28 recipients, 4 cases had the graft renal dysfunction early post-operation: One died and 3 recovered through consecutive therapy. One case received re-transplantation of the liver 3 months after the first due to the relevant complications and then recovered. During this period, no impact on the renal function occurred. Eleven cases had pulmonary infection post-operation, and 1 died. No acute rejection occurred.Conclusion Combined liver-kidney transplantation is the effective treatment to the patients with end stage liver disease complicated with renal dysfunction. Suitable case selection and perfect operation timing were the key points to the success of combined liver-kidney transplantation.
10.Response of concurrent radiation combined with interventional artery infusion chemotherapy in advanced cervical carcinoma

Objective To investigate the clinical curative effects and side effects of concurrent radiotherapy combined with interventional artery infusion chemotherapy in advanced cervical carcinoma.Methods 48 patients with advanced cervical carcinoma were randomly divided into two groups including concurrent radiotherapy combied with interventional artery.infusion ehemotherapy group (25 patients) and radiotherapy group (23 patients).Each patient in the concurrent radiotherapy combined with interventional artery infusion chemotherapy group was given THP,DDP and 5-Fu,repeatedly every 28 days,total 3-4 cycles.Radiotherapy was given at the same time.Results The effective rates of the concurrent radiotherapy combined with interventional artery infusion chemotherapy group and radiotherapy group were 92.0 % (23/25)and 69.6 % (16/23),respectively.The overall 3 year survival rates for the patients in concurrent radiotherapy eombined with interventional artery infusion chemotherapy group and radiotherapy group were 80.0 % (20/25)and 52.2 % (12/23),respectively,and there was statistically significant between the 2 groups (all P < 0.05).No more side effects were found in the concurrent radiotherapy combined interventional artery infusion chemotherapy group (P > 0.05).Conclusion Concurrent radiotherapy combined with interventional artery infusion chemotherapy in advanced cervical carcinoma can increase short term therapeutic effect and improve the survival rate of the patients without more side effects.
