1.Correlation between the expression of gene protein of c-fos, bcl-2 and gastric cancer

Purpose:To investigate the correlation between the expression of gene protein of c-fos,bcl-2 and gastric cancer and to provide the evidence of molecular pathology for grouping and early diagnosis.Methods:The expressions of gene-protein of c-fos,bcl-2 of the resected specimen of 54 patients with gastric carcinoma were detected by immunohistochemical method of S-P.All of the data were input into computer by system of Excel and SAS for statistical analysis.Results:The expressions of gene protein of c-fos,bcl-2 in intestinal type of gastric carcinoma were significantly higher than those in diffuse type (P=0.012, 0.0001, respectively).The expressions of gene protein of c-fos ,bcl-2 in the group of lymphaden metastasis were higher than that in the group of no lymphaden metastasis (c-fos 63.64%,54.55%;bcl-2 50.00%,27.27%), but no statistical significance.The expressions of c-fos , bcl-2 in atypical hyperplasia tissue nearby carcinoma and high differentiated carcinoma were stronger.Conclusions:The expressions of gene protein of c-fos,bcl-2 in intestinal type of gastric carcinoma were significantly higher than those in diffuse type.It showed that the pathogenesis of gastric carcinoma have difference between the intestinal type and diffuse type and it maybe provide proofs for gastric carcinoma grouping.The expression of c-fos,bcl-2 can be used as the molecular pathological symbols for early diagnosis.

Objective To investigate the characteristic and diagnosis of left ventricular diverticulum. Meth-ods As one left ventricular diverticulum patient was diagnosed by echocardiography in our hospital,we reviewed and analyzed about published in China. Results Left ventricular diverticulum is a rare disease of congenital heart malfor-marion, Echocardiographic characteristics of the left ventricular diverticulum is the saccular evngination of the left ven-tricular wall with the neck of the diverticula being smaller than the body. Left ventricular diverticulum is classified into contractile muscular type and fibrous type ,The saccular wall of contractile muscular diverticulum remain a normal mo-tion and is thinner than the normal wall The saccular wall of the fibrous diverticulum is due to a loss of myocardial thickness, as thin as aneurysm with a dyskinetic motion. Conclusion Left ventricular diverticulum is a rare disease in clinic, echocardiography is very important for diagnosing classifying.

4.Analysis of the prevention effect of early individualized and comprehensive therapy on MODS in patients with severe acute pancreatitis

Objective To investigate the effect of individualized and comprehensive therapy in treatment of severe acute pancrea-titis in early stage for preventing MODS .Methods 85 patients with severe acute pancreatitis treated in this Chengde medical college affiliated hospital from January 2008 to January 2012 were divided into two groups .The control group included 42 patients ,all used conventional basic treatment ;the test group included 43 patients ,and all used individualized and comprehensive therapy in early stage .The APACHEⅡ ,MODS and Ranson scores ,and serum inflammatory cytokines indexes of patients in each group before and after admission were all recorded .Results The APACHEⅡ and MODS scores after admission of test group were significantly low-er than the control group(P< 0 .05) .The TNF α,CRP and IL 10 after admission of two groups had significant difference (P<0 .05) .The MODS incidence of control group was 64 .3% (27/42) ,pancreatic encephalopathy incidence was21 .4% (9/43) ,the inci-dence of pancreatic infection was 33 .3% (14/42) ,hospital mortality incidence was 19 .1% (8/42) and length of hospital stay was (30 .4 ± 5 .7) d .The MODS incidence of test group was 27 .9% (12/43) ,pancreatic encephalopathy incidence was11 .6% (5/43) , pancreatic infection rate was 14 .0% (6/43) ,hospital mortality incidence was 4 .7% (2/43) and length of hospital stay was (23 .5 ± 4 .3) d .The incidence of MODS ,hospital mortality and pancreatic infection rates of test group were significantly lower than that of the control group (P<0 .05) .The average length of stay of test group were significantly lower than the control group (P<0 .05) . Conclusion To use individualized and comprehensive therapy in treatment of severe acute pancreatitis in early stage can reduce MODS ,protect organ function ,and reduce mortality .
5.Clinical effect of hemoperfusion combined with hemodialysis in treatment of severe organophosphate pesticide poisoning.

OBJECTIVETo investigate the clinical effect of hemoperfusion combined with hemodialysis in the treatment of severe organophosphate pesticide poisoning.
METHODSNinety-eight patients with severe organophosphate pesticide poisoning who were admitted to the emergency department of our hospital from March 2005 to September 2013 were equally divided into control group and observation group according to treatment methods. The control group was given conventional emergency treatment, while the observation group was given hemoperfusion combined with hemodialysis and the conventional emergency treatment. The clinical outcomes and complications of two groups were compared.
RESULTSIn the control group, 35 patients were cured and 14 patients died, so the cure rate was 71.4%. In the treatment group, 46 patients were cured and 3 patients died, so the cure rate was 93.9%. The treatment group had a significantly higher cure rate than the control group (χ² = 8.611, P < 0.05). And the treatment group had significantly shorter duration of coma (P < 0.01), mean length of hospital stay (P < 0.01), and time to recovery of cholinesterase activity (P < 0.01) and a significantly reduced dose of atropine than the control group (P < 0.01). The control group had significantly more cases of urinary retention than the treatment group (18 vs. 6, χ² = 4.991, P < 0.05). And the control group had more cases of intermediate syndrome, respiratory failure, delayed neurological damage, and rebound than the treatment group.
CONCLUSIONHemoperfusion combined hemodialysis has a good clinical effect and causes fewer complications in treating severe organophosphate pesticide poisoning, so it is worthy of clinical promotion.

6.Serological characteristic and molecular basis of A2 subgroup in Shanghai population.

OBJECTIVETo investigate the similarity and difference in blood group serology and molecular biology of A2 and A2B phenotypes between healthy blood donors and patients.
METHODSThe A and AB phenotypes were screened with anti-A1. Exons 1 to 7 and intron 6 of the ABO gene were analyzed with polymerase chain reaction-sequence-based typing (PCR-SBT) method. The blood type was determined by referring to the Blood Group Antigen Gene Mutation Database (BGMUT).
RESULTSAmong 7111 tested individuals, 75 were assigned as A2 or A2B phenotypes. However, only 28 individuals still belonged to the A2-related allele group based on genetic analysis. Among these, A205/B101 was the most common genotype. Among those non-A2-related alleles, A102/B101 was the most common genotype. Based on serologic testing, there was an imbalance between the A2 and A2B subgroups. In both donor group and patient group, the proportion of A2B was significantly higher than that of the A2. There were statistical differences between different groups (χ² = 64.613, 33.137, 34.963, P< 0.01). At the gene level, the imbalance still existed in both the overall population and the donor group, though there was a statistical difference between the two (χ² = 17.678, 14.157, P< 0.01). The same imbalance did not exist in the patient group (with continuous correction, χ² = 2.351, P= 0.125).
CONCLUSIONThe concordance rate for blood type determined by serology and genetic analysis has been low and deserves attention. For A2 and A2B phenotypes by serological screening, A102/B101 was the most common gene among non-A2-related alleles. Further study is needed to clarify this phenomenon.

7.Prognostic Value of a Change in Troponin-Ⅰ Levels in Patients with Sepsis-associated Myocardial Dysfunction

Objective To investigate the expression of cardiac troponin-Ⅰ (cTnⅠ) levels in patients diagnosed with sepsis-associated myocardial dysfunction and explore the relationship between cTn Ⅰ and cardiac systolic and diastolic function.Additionally,we evaluated the prognostic value of cTn Ⅰ as a valuable biomarker.Methods We admitted 65 patients with sepsis.Using echocardiography,the ratio of early diastolic mitral inflow velocity to early diastolic mitral annulus velocity (E/e') and the left ventricular ejection fraction (LVEF) were measured as an evaluation index of left ventricular diastolic and systolic function,respectively.Patients were divided into a cardiac dysfunction and a normal cardiac function group.The cTn Ⅰ level was measured and compared between the two groups,and we determined the correlation between cTn Ⅰ levels and cardiac diastolic and systolic function.Based on assessment of 28-day mortality,cases were divided into a survivor and a death group.A receiver operating characteristic curve was constructed to predict the prognostic value of cTn Ⅰ.Results The cTn Ⅰ level in the cardiac dysfunction group was significantly higher than that observed in the normal cardiac function group (P < 0.05) and showed a positive correlation with E/e'(r =0.421,P =0.008).However,there was no correlation noted between the cTn Ⅰ level and LVEF (P > 0.05).Compared to the survivor group,the level of cTn]Ⅰ was significantly higher in the death group (P < 0.05).The prognostic value of cTn Ⅰ area under the curve was 0.892,with a cut-off value of 0.82 ng/mL (sensitivity =88.0% and specificity =82.5%).Conclusion The cTn Ⅰ level is noted to be significantly elevated in patients with sepsis-associated cardiac dysfunction and shows a positive correlation with left ventricular diastolic function.A cTn Ⅰ level ≥ 0.82 ng/mL can be used as a valuable predictor of mortality in patients with sepsis.
8.The Enlightenment of Palliative Care for Advanced Cancer Patients in Russia

This paper introduced the present situation and the existing problems of palliative care and hospice care system in Russia.The coverage of palliative is narrow,regional development is not balanced,the medical professionals in palliative care is lack,the professional skills should be improved urgently,the training system of palliative care is not perfect and the quality of training is not high,the medical professionals have high work pressure,the brain drain is serious,the social cognition is not enough,the volunteer service system is underdeveloped,and the acquisition rate of mental and narcotic drugs is low.On the basis of the above,this paper introduced the development strategy of Russian palliative care,and discussed the enlightenment to our country.For example,government departments should attach importance to the introduction of legal norms of palliative care,perfect the service system of palliative care,establish the supporting facilities,reasonably plan palliative care system,implement the integration of service management and scientific research,and bring the guarantee palliative care fund into the national budget.
9.The distributive features of three kinds of metabolic genes polymorphisms in population of Han nationality in south area of China.

OBJECTIVETo find out the distributive features of some metabolic genes polymorphisms in Han population of south area of China.
METHODSStudy population was obtained from the controls of a community based case-control study, which included 290 blood relatives (inner control) and 404 non-blood relatives (outer control).
RESULTSFrequencies of CYP1A1, GSTM1 and GSTT1 polymorphisms had no significant difference among confounding factors, such as sex, living areas, stomach cancer family history and history of tobacco smoking etc. Some controls showed significant difference in age group and alcohol drinking which would be adjusted in analysis of the relationship between polymorphisms and cancers. CYP1A1 Ile/Val and Val/Val genotypes were 33.43% and 5.62% respectively, which were similar to other results from Chinese and Japanese, but higher than those from Caucasians in American, Europe and African-Americans. GSTM1 null allele frequency was 53.48% in our population, which showed difference even among Chinese in different areas. GSTT1 null allele frequency was 45.78%, which was significantly higher than that in Caucasians and African-American.
CONCLUSIONThe frequencies of CYP1A1 Ile/Val, Val/Val and GSTT1 null in Han population in south area of China are significantly higher than those in other races, while the ethnic difference of frequency of GSTM1 null is less.

10.Study on the Relationship between susceptibility of stomach neoplasm cancer and polymorphism of inducible nitric oxide synthase gene.

OBJECTIVETo study the relationship between polymorphism of inducible Nitric Oxide Synthase (iNOS) gene and the susceptibility of intestinal type stomach cancer and stomach cardia cancer in Chinese people.
METHODSA community-based case-control study was designed. Ninety-three intestinal type of stomach cancer and 50 stomach cardia cancer patients with endoscopy and pathology diagnosis were identified as cases. Two hundred and forty-six controls served as controls.
RESULTSC-->T polymorphism was found in exon 16 of iNOS gene, which changed the coding amino acid from serine to leucine, and formed a recognition site identified by Tsp 509 I restriction enzyme (we called it C-->T polymorphism). The T allele gene frequency in the control group was 13.21%. No statistically significant difference was found between C-->T polymorphism alone and the increased susceptibility to intestinal stomach cancer or stomach cardia cancer. A significant type 2 multiplicative interaction was found in increasing both the risk of intestinal stomach cancer and stomach cardia cancer when both C-->T polymorphism and tobacco smoking exposure existed. An additive interaction model, which showed statistically significant difference, was found to increase only the risk of stomach cardia cancer when CagA antibody shared negative but C-->T polymorphism occurred.
CONCLUSIONC-->T polymorphism of iNOS gene was considered as one of the possible susceptible genes, which specifically increased the risk of tobacco-related but CagA negative types of intestinal stomach cancer and stomach cardia cancer.
