2.Comparison between EUS-guided ethanol ablation and surgical treatment of benign insulinoma

Objective To evaluate the safety and efficacy of endoscopic ultrasound (EUS) guided ethanol ablation of benign insulinoma and compare its' advantages and disadvantages with surgical treatment. Methods From April 2011 to February 2016, clinical data of 38 patients with benign insulinoma treated by EUS-guided ethanol ablation or surgical treatment were retrospectively analyzed. Results 97.4% (37/38) patients had a typical clinical manifestation of Whipple's triad, and the I/G ratio of 82.9% patients (29/35) was more than 0.3 with their onset of hypoglycemia. The positive preoperative etiologic diagnosis rates of transabdominal ultrasonography, CT, MRI, PET/CT and EUS were 50.0%, 67.6%, 66.7%, 75.0%, 89.7% respectively. In the current study, 18 patients underwent EUS-guided ethanol ablation (EUS-FNI group) and 20 patients received surgicaltreatment (surgical group). Compared with the surgical group, the operation time, intraoperative hemorrhage volume, postoperative complications, length of stay and hospitalization costs were significantly reduced in the EUS-FNI group (P < 0.05). No treatment-related complications was observed in EUS-FNI group, while 40.0% (8/20) patients in surgical group had complications. During the follow-up period, all these patients maintained stable blood glucose without taking medication, and there's no recurrence of insulinoma in EUS-FNI group after the last treatment with alcohol injection; In surgical group, only 90.0% (18/20) patients had no recurrence, episode of hypoglycemia was less after the operation in 10.0% (2/20) patients. Conclusion EUS-guided ethanol ablation of benign insulinoma is safe and effective, compared with traditional surgical treatment, EUS-guided ethanol ablation is minimally invasive, costs less, recovers fast after treatment and has fewer complications.
3.The correlation of plasma homocysteine and intima-media thickness with incipient cerebral infarction

Objective To explore the relationship between the level of plasma homocysteine(HCY)and intima-media thickness(IMT)with incipient cerebral infarction.Methods According to the clinical diagnostic criteria,112 patients with incipient cerebral infarction were randomly selected as the cerebral infarction group,and 102 healthy people were randomly selected as the control group.All carotid arteries of both groups were checked by carotid artery ultrasound and IMT was tested.The levels of plasma HCY and plasma folate as well as vitamin B12 were measured in both groups by chemiluminescence immunoassay.Results The plasma HCY level in the cerebral infarction group [(24.89 ± 1.96) μmol/L] was significantly higher than (8.58 ± 2.34) μmol/L in the control group (P < 0.01).The levels of plasma folate [(4.79 ± 2.35) μg/L] and vitamin B12 [(228.67 ± 114.75) ng/L] in the cerebral infarction group were significantly lower than [(8.65 ± 2.64) μg/L and (320.53 ± 154.78) ng/L] of the control group (P < 0.01).The IMT of the cerebral infarction group [respectively the left common carotid artery fork (1.12 ± 0.25) mm,the right common carotid artery fork(1.09 ± 0.28) mm,the left internal carotid artery(1.18 ± 0.23) mm,the right internal carotid artery(1.03 ± 0.24)mm] were obviously higher than those of the control group[respectively the left common carotid artery fork (0.65 ± 0.18) mm,the right common carotid artery fork (0.61 ± 0.24) mm,the left internal carotid artery(0.58 ±0.22)mm,the right internal carotid artery(0.61 ±0.19)mm] (P <0.05).Significant positive correlation between IMT and HCY was found in patients with cerebral infarction(r =0.834,P < 0.01),but no corre lation was found in the control group(r =0.081,P > 0.05).Conclusion The increase of plasma HCY and the thickening of IMT are closely related with incipient cerebral infarction.Examination of plasma HCY and IMT has important clinical significance for the secondary prevention of incipient cerebral infarction.
4.Changes of cerebral mitochondrial respiratory function and ultrastructure after traumatic brain injury in response to hypothermia

Objective To study the effect of hypothermia on cerebral mitochondrial respiratory function and ultrastructure after traumatic brain injury(TBI). Methods Sprague-Dawley rats were subjected to moderate brain injury by using lateral fluid-percussion(LFP)and randomly divided into sham operation group,normothermic TBI group(rectal temperature for 36-37℃)and hypothermic TBI group(rectal temperature for 31-32℃ lasting for two hours).The ipsilateral brains were dissected and homogenized brain tissues were extracted to obtain mitochondfia by density-centrifugation and speed-centrifugation at 2,24 hours and at days 3 and 7 after TBI.The mitochondrial uhrastructure was studied by electron microscope.The indices of respiratory control rate(RCR)and P/O ratio of mitochondrial respiratory function were measured after oxygen consumption was determined with a Clark-type electrode.Results The mitochondrial uhrastructure of normothermic TBI group was damaged severely while that of hypothermic TBI group kept relatively integrated.The RCR and P/O ratio were markedly decreased two hours after TBI and reached the lowest level at the 24th hour(P<0.01).At day 7,RCR kept at a lower level compared with sham operation group but P/O ratio recovered to normal.Change of RCR was similar in hypothermie TBI group and normothermic TBI group.However,RCR of the hypothermic TBI group was significantly higher than that of the normothermic TBI group within three days after TBI.In the meantime,P/O ratio recovered to normal three days after TBI. Conclusion Hypothermia can improve cerebral mitochondrial respiratory function and protect the mitochondrial structure after TBI.
5.Mechanism of proteinuria prevention by combination of telmisartan and pioglitazone in diabetic nephropathy rats

Objective To study the combination effects of telmisartan and pioglitazone on the expression of heparanases (HPA) and podocin in the kidney of diabetic nephropathy (DN) rats and its possible mechanism. Methods DN model rats were established by intraperitoneal injection with STZ for 12 weeks. All the DN rats were randomly divided into telmisartan group (T group), pioglitazone group (P group), combination of telmisartan and pioglitazone group (L group), and DN group (D group). Healthy rats were chosen as healthy control group(N group). After garage with drugs for 12 weeks, 24-h urinary protein and serum biochemical indicators were examined. RT-PCR and immunohistochemistry methods were applied to detect the mRNA and protein expression of HPA and podocin. Results Compared with T group and P group, 24-h urinary protein of L group was markedly decreased(P<0.05). Compared with the N group, the level of fasting blood glucose, relative renal weight, BUN and Scr in other 4 groups were markedly increased (P<0.05). Compared with T group and P group, the Scr level and the expression of HPA mRNA and protein in L group was markedly decreased (P<0.05), and the protein and mRNA expression of podoein in L group was markedly increased (P<0.05). Conclusion Combination of telmisartan and pioglitazone can down-regulate the mRNA and protein expression of HPA of glomerular basement membrane and up-regulate the protein expression of podocin of podocyte in DN rats, which may ameliorate the proteinuria.
6.Design of the porous orthopedic implants: research and application status

BACKGROUND: Porosity has been proven to improve the stability of orthopedic implants, and the architecture of pores is considered as a significant factor to improve the osseointegration of implants. OBJECTIVE: To introduce the research advance in porous architecture. METHODS: WOS database was searched for the articles addressing the porous structure of the implants published from January 2000 to April 2016 using the keywords of scaffold, pore size, porosity, osteogenesis. The literatures were screened according to the inclusion and exclusion criteria.RESULTS AND CONCLUSION: (1) The stability of traditional implants cannot meet the requirements in some specific circumstances, while the implants with porosity can improve the stability due to its osteogenesis ability. (2) Porous structure is a hotspot, and the osseointegration of porous implants in vivo is explored through series of in vitro and in vivo experiments. (3) It has been shown that higher porosity, proper pore size, microporous morphology and microstructure of the pore wall may contribute to the growth and differentiation of bone tissue under enough mechanical support. (4) However, most studies on the porous implants are still on the in vivo and animal experimental stage, and its promoting effects on the osteogenesis and bone in-growth are needed to be investigated in depth.
7.A case report of primary hepatocellular carcinoma with gallbladder cancer

ABSTRACT:Objective To a rare case of double primary cancer (hepatocellular carcinoma with gallbladder)to guide clinical application.Methods and Results We reported one case of primary hepatocellular carcinoma and gallbladder.A male patient,54 years old,had main complaints of intermittent right upper quadrant pain for 4 days. The abdominal CT of the local county hospital showed gallstones,gallbladder with liver infiltration.And then he went to the First Affiliated Hospital of Xi’an Jiaotong University for further treatment.Laboratory examination revealed:HBsAg(+),HBcAb(+),alpha-fetoprotein (AFP)> 60 500 ng/mL,carcinoembryonic antigen (CEA) 5.25 ng/mL.Abdominal CT showed hilar slightly stronger light echo groups:liver cancer or gallbladder cancer? Hepatic artery and portal vein CT imaging (CTA+CTV)examination showed the malignant tumor shadow in the inside of the left hepatic lobe huge,uneven thickening of the gallbladder wall,suspected liver disease with gallbladder infringement or gallbladder disease with liver infringement. With the preoperative preliminary consideration of primary liver cancer with infiltration of the gallbladder,we chose the operation as the resection of segment Ⅳ b and Ⅴ of the liver,cholecystectomy and T tube drainage.Pathological examination postoperative showed the bulky liver carcinoma grade Ⅲ and the poorly differentiated adenocarcinoma of gallbladder.A month later,abdominal CT showed the tumor spread intrahepatic,prompting the poor prognosis.Conclusion The two which are not continuous,which is the standard of double primary cancer,are not suitable for all double primary cancers.This case provides useful experience for future similar diagnosis and treatment of disease,and also helps us with timely and accurate identification of “metastatic”or “primary”,which is the key point for clinicians to give patients an effective treatment.
8.Analysis of the Causes and Countermeasures of Bile Buct Injury in Laparoscopic Cholecystectomy

Objective To reduce or prevent the incidence of bile duct injury in the laparoscopic cholecystectomy,and reduce the postoperative complications of Bile duct injury in the laparoscopic cholecystectomy by time and exact treatment during the surgical procedure.Methods We retrospectively analyzed the clinical data of 8700 cases of LC, including 12 cases of bile duct injury, from September 1991 to today. The corresponding treatment was used in LC to avoid bile duct injury, when bile duct injury occurred, the injury was recognized and immediately repaired by surgery. Result The mean follow-up period after surgery was 5-10 years,all 12 patients with bile duct injury were cured and discharged. Conclusions We should correctly handle the relationship of various duct and avoid burn or cut duct for reducing incidence of the extrahepatic bile duct injury. Correct treatment can reduce or avoid the incidence of postoperative complications.
9.Research advance in the osteointegration of surface bioactive coating on titanium alloys

BACKGROUND:Titanium alloy, a commonly used bone biomaterial, holds good biocompatibility and proper mechanical properties, but without osteointegration ability. Surface bioactive coating of titanium alloy can overcome such problems. OBJECTIVE:To review the application progress of surface bioactive coating on titanium alloys, and to explore the mechanism underlying its osteogenic induction. METHODS:PubMed and CNKI databases were searched for literatures concerning surface bioactive coating on titanium alloys published from January 2000 to March 2016, using the keywords oftitanium, titanium alloys, coating, surface modification, bonein English and Chinese, respectively. Through preliminary analysis, 42 eligible articles were selected. RESULTS AND CONCLUSION:The surface modification method has undergone the transition from surface roughening, oxidation, acidification treatment to the most widely used biomimetic coating technology. The surface biomimetic coatings of titanium alloys mainly include hydroxyapatite, bioactive glass, zeolite, which have been developed from a single coating to the composite coatings and gradient coatings. What's more, the bioactive factor or/and metal ions in the coating can improve the osteointegration of surface coating. Surface modification for titanium alloys is a complex process, during which, both the osteogenesis ability and the bonding strength between the coating and its substrate cannot be ignored. Improving the fabrication method of existing coatings and exploring new materials of biomimetic coatings are critical to achieve a high-quality surface coating modification.
