1.DEPRESSIVE AND/OR ANXIETY DISORDER IN PATIENTS WITH AWARENESS DURING GENERAL ANESTHESIA

Objective To reveal the prevalence of depressive and/or anxiety symptom and disorder in patients with awareness during general anesthesia.Methods 10 patients which had the awareness during general anesthesia were selected into AR group (awareness with recall during general anesthesia group).Face-to-face interviews were used in data collection together with the self-completed HAD scale for depressive and/or anxiety symptom screening.Subjects getting a HAD score of 9 and above were further assessed for depressive and/or anxiety disorders with HAMA scales and HAMD scales.Results The difference of common general state of patients, such as age, weigh and the time during operation were all insignificant between two groups (p>0.05).The rate of patients received general anesthesia only in AR group was higher than that in control group(p<0.05).The prevalence of depressive symptom, the prevalence of depressive or anxiety symptom and the prevalence of depressive and anxiety symptom in AR group were higher than that in control group(p<0.05).The prevalence of depressive disorder, the prevalence of depressive or anxiety disorder and the prevalence of depressive and anxiety disorder in AR group were higher than that in control group too(p<0.05).Conclusion There was high prevalence of depressive and/or anxiety symptom and disorder in patients with recall-awareness during general anesthesia.

Methods:494 elementary school students were measured with CSES (Children's Self-Esteem Scale) and BMSLSS (Brief Multidimensional Students' Life Satisfaction Scale). 3.Life Satisfaction between Urban and Rural Middle School Students

Objective: To compare the differences between urban and rural middle school students on life satisfaction. Methods: 1152 urban and rural middle school students were measured by Multidimensional Students'Life Satisfaction Scale ( MSLSS - CV) . Results: ( 1 ) The urban students had higher scores than their rural counterparts on all the satisfaction dimensions ( mean value differences = 0. 2 - 0. 4. For example, the family satisfaction of urban and rural students were 4. 6?1. 0, 4. 2?1.0 respectively, mean value difference =0. 4; the serf satisfaction of urban and rural students were 4. 6?0. 7 , 4. 4?0. 7 respectively, mean value difference = 0. 2 ) . ( 2 ) The mean values rank (from high to low) of the five dimensions on the satisfaction was family, self, friend, school and living environment in urban students. 4.Study on The Effects of Chronic Renal Failure on Hearing and Its Mechanism

[Objective]To observe the effects of chronic renal failure (CRF)on hearing and investigate its mechanism.[Methods] Twenty-nine male wistar rats were randomly divided into four groups:Ⅰthe normal rats as control for 12 days (n=6); Ⅱrats given adenine(ig) for 12 days (n=8); Ⅲthe normal rats as control for 24 days (n=6);Ⅳ rats given adenine(ig) for 24 days(n=9). The rat model of CRF was produced by being perfused of adenine [300mg/kg]for 12d and 24d. Brain-stem auditory evoked potential(BAEP) was utilized to evaluate the auditory function and renal tissue morphology was observed; Serum sampling was used to determine the content of Cr, BuN. The expression of NOSⅡin the cochlear was measured by immunohistochemical technique.[Results] After adenine given for 12d or 24d, the Cr and BuN were in significant higher level than that in control group. The BAEP thresholds were gradually higher than that in control group, the BAEP delitescence prolonged. Light microscopic observation of renal tissue showed that the degree of fibrosis was gradually higher. Data of immune histochemistry about the cochlear tissue showed that the expressions of NOSⅡ were feebly positive in the cochlear vassal texture, inner-outer hairy cell and helical-ganglion cell, and following functional damage of the kidney, their expressions reinforced gradually, amongst which strongest expression was vassal texture, the next helical-ganglion cell, the last inner-outer hairy cell.[Conclusion]CRF can result in deafness. It is likely concerned with toxic effects of the metabolizable substance, the metabolizable dysfunction of the electrolyte, shortage of trace elements, etc. One of the important mechanisms of the effects of CRF on hearing is the toxic damage of NO to cochlea.
7.The Role of Viral Mutation in the Pathogenesis of Chronic Viral Hepatitis

The quasispecies nature of hepatitis B and C virus (HBV, HCV) plays an important role in the pathogenesis, immune escape and drug resistance during chronic infection. Although there is still a lack of effective treatment for hepatitis C, a series of nucleoside analogs (NA) have been developed for the treatment of hepatitis B. NA resistant HBV mutants can accumulate during prolonged therapy and lead to the failure of anti-HBV therapy. Switching to other sensitive NAs can inhibit the emerged resistant mutants. Therefore, understanding the evolution of viral quasispecies under drug pressure is crucial for the establishment of antiviral strategy and the monitoring of antiviral process. Immune response and escape are complicated process, during which both host and virus factors may play their roles. Further understanding of the interaction and interrelationship between host and these viruses may lead to optimized prevention, diagnosis and treatment for chronic hepatitis.
8.Analysis of factors related to long bone fracture and hip-knee joint replacement accompanied with fat embolism syndrome

BACKGROUND: Fat embolism syndrome is a commonly seen severe complication in the field of opthopaedics in clinic. It frequently occurs after long bone fracture and hip-knee joint replacement. However, its etiological factors and pathogenesis are not identified.OBJECTIVE: To sum up the onset influencing factors, pathogenesis and therapeutic methods of long bone fracture and hip-knee joint replacement accompanied with fat embolism syndrome.RETRIEVE STRATEGY: Using the terms "fat embolism syndrome, pathogenesis, treatment, prevention, diagnosis", we retrieved PubMed database to identify studies published in the English language. Fifty-five literatures were searched.Meanwhile, we searched the medical information network of Shanghai Jiao Tong University with the same terms in the Chinese language. Inclusive criteria: studies, which can reflect the diagnosis and treatment as well as pathogenesis of fat embolism syndrome. Exclusive criteria: repetitive studies.LITERATURE EVALUATION: The involved 38 literatures are all about the diagnosis and treatment as well as pathogenesis of fat embolism syndrome, among which, 6 were review and the others were clinical or basic studies.DATA SYNTHESIS: ① The influencing factors of fat embolism syndrome included trauma factor, operation factor and other factors. ② The pathogenesis of fat embolism syndrome involved mechanical obstruction theory, biochemical theory,condensation theory and inflammatory reaction theory. ③ The treatments of fat embolism syndrome include respiration supporting, glucocorticoid application, protecting brain function, drug treatment, heat shock treatment and so on.CONCLUSION: Study on bone fracture and joint replacement accompanied with fat embolism syndrome can provide evidence for the diagnosis and treatment of this syndrome.
9.Extremely low-frequency magnetic fields and cancers

OBJECTIVE: All the people in modern world are exposed to the industrial frequency magnetism (extremeiy low-frequency magnetic fields, ELFMF). There are important academic and practical significance to understand the influence of ELFMF on human diseases.DATA SOURCES: A computer-based online search was conducted in HighWire database for English literatures about the relation between ELFMF and human cancers from January 1991 to March 2006 by using the key words of "extremely low frequency magnetic fields, cancer".STUDY SELECTION: All the collected literatures were screened. Inclusion criteria: ① To browse the abstracts of the literatures to examine primitively, and select the articles experimented with the cytological, molecular biological and epidemiological methods. ② Choosing the separate articles by studying with different animals but having got the same results.③ Comparing and analyzing the original and reviewed articles about the relations between ELFMF and cancers.DATA EXTRACTION: A total 90 original and 4 reviewed literatures about ELFMF and cancers were collected and 29 and 4 papers were accordance with the criteria, respectively. In the 47 eliminated articles, 32 of them were unrelated or repetitive to our topic, 5 of them were published earlier or the conclusions were ambiguous, and other 10 of them used only the epidemiological methods.DATA SYNTHESIS : ① For a long time, the question whether 50/60 Hz ELFMF has the carcinogenic effects has been disputed in epidemiology and cytology. ② Majority statistical results in epidemiology showed that the crowds exposed to ELFMF for long or frequent time had an higher ratios suffering cancers than the normal, thereinto, the dangerousness such as leucocythemia, brain cancer and breast cancer is especially high. ③ However, the observation in cytology indicated that the normal cultural cells exposed to ELFMF had not increased the cell divisions or appeared cancer signals, and even in some reports, ELFMF may accelerate apoptosis, and reduced the possibility of cancerization. ④ Although ELFMF may not produce cancerization to normal cells, it may act as an promoter or co-promoter of cancer-prone cells, to accelerate the cancerization and exacerbation course of those cells. ⑤ The effect is related to influencing various links of signal transduction, changing translation of gene, changing activities of some enzymes and increasing production of free radicals.CONCLUSION: Although ELFMF may not produce cancerization to normal cells, it may act as an promoter or co-promoter of cancer-prone cells, to accelerate the cancerization and exacerbation course of those cells.
