1.Endoscopic examination and pathology of colonic polyps in children

Objective To retrospectively study the results of endoscopic manifestation pathology,and treatment of colonic polyps. Methods Clinical data of 121 patients diagnosed with colonic polyps and treated by endoscopy between November 2002 and September 2009 were studied. Complete colonic investigation was done by Olympus PCF240I electron endoscopy. Colonic polyp excision was done by high frequency electric coagulation. Results All patients had bloody stool with mucus for 1 week to 8 years. Ninty four(77.7%)patients had a single polyp,6(5.0%)had two polyps,21(17.3%)with multiple polyps,and 5 patients had familial polyposis coli. The size of the polyps ranged from 0.3 cm to 4 cm and the morphology of Yamada Ⅰ to Ⅳ. The pathological changes were juvenile polyps,inflammatory polyps,polypoid formation,and canalicular adenoma. Most common pathology types were Yamada Ⅳ and juvenile polyps,107 of the 121 patients(88.4%)had juvenile polyps,and one was found to have malignant transformation. 115 patients accepted high frequency electric coagulation excision under general anesthesia,without complication. Conclusions Colonic polyp is one of the common diseases in children. Children should be checked by total colonoscopy in time when they present with bloody stool,anemia,rectal polyps prolapse or intussusception. Polypectomy under endoscope is a most safe and reliable treatment method when the diagnosis is confirmed. For isolated juvenile polyp,regular recheck is not necessary after polypectomy except when bloody stool recurs. Regular follow up by colonoscope is necessary for juvenile polyp combined with adenoma and adenomatous polyps. Colonoscope is necessary at any time when bloody stool occurs. Familial polyposis coli is difficult to be treated by endoscopy,thus surgical procedure is suggested.
2.Non-linear rectification of sensor based on immune genetic algorithm.

A non-linear rectification based on immune genetic algorithm (IGA) is proposed in this paper, for the shortcoming of the non-linearity rectification. This algorithm introducing the biologic immune mechanism into the genetic algorithm can restrain the disadvantages that the poor precision, slow convergence speed and early maturity of the genetic algorithm. Computer simulations indicated that the algorithm not only keeps population diversity, but also increases the convergent speed, precision and the stability greatly. The results have shown the correctness and effectiveness of the method.

3.Preparation and antigenicity analysis of 38 ku protein of mycobacterium tuberculosis

Objective To Clone,express,purify 38 ku protein of mycobacterium tuberculosis,to study its immunological characteristics,and to evaluate its potenial value for serodiagnosis of tuberculosis.Methods Extract DNA from standard strain of H37Rv as the template,amplify gene of 38 ku protein by PCR,insert to PET-30a(+)and construct the recombinant plasmid,express 38 ku protein in E.coli BL21(DE3),purify by Nickel affinity chromatography,at last get target proteins of higher purity,analyze its immol/Lunological characteristics by Western blotting and ELISA technology from Feb.2003 to Mar.2004. Results The clone was analyzed at the nucleotide lever and showed the same DNA sequence coding for natural 38 ku protein. The recombinant protein expressed in inclusion body in E.coli BL21(DE3). The purity of terget protein was 92.7% by Nickel affinity chromatography,Western blotting assays showed that the recombinant protein had satisfactory antigenicity . 38 ku protein detected TB postive and negative refference serum based on the mechanism of indirect ELISA,results showed that the sensitivity was 80.5%(33/41)and the specificity reached to 96%(25/26).Conclusion The recombinant protein expressed in inclusion body in E.coli BL21(DE3)and had satisfactory antigenicity,and might be selected as one of serodiagnostic antigen of tuberculosis.
4.Screening techniques of two types of deletions of ?-thalassemia

Objective To develop techniques based on polymerase chain reaction(PCR) which can detect 2 of most common deletional ? thalassemia ? 3.7 deletion and ? 4.2 deletion in China accurately and speedily. Methods Two groups of primers were designed and synthesized. PCR conditions were optimized. The PCR pro ducts were analysed by 1.0% agarose gel electrophoresis. The gel was stained by EB and photographed using an UVP gel documentation. Results PCR product of a 1 700 bp DNA fragment with primers A′, B′, C3 indicates the ? 3.7 deletion while a 1 900 bp fragment indicates a normal or wild type of ? globin gene. Occurrence of the 1 700 bp and 1 900 bp simultaneously indicates a heterozygous of the ? 3.7 deletion. Neither of the 2 bands was presented, indicating a homozygous of South East Asia type of deletion (-? SEA ). According to patterns of 1 580 bp and 1 180 bp hand amplified by a PCR with primer G′, E, F′, we detected the ? 4.2 deletion and distinquished its heterozygous and homozygous. Conclusions The 2 PCR based techniques developed in our laboratory are accurate, simple, well reproducible and easy to use for screening of the 2 deletion types of ? thalassemia determinants.

7.Quality and efficiency of fetal heart screening by spatio-temporal image correlation with tomographic ultrasound imaging compared with two-dimensional ultrasonography

Objective To explore how to elevate the efficiency of fetal heart screening.Methods Volume data of 83 normal fetuses ranging between 20 to 24 gestational weeks were acquired by spatiotemporal image correlation (STIC) with the apical four-chamber view(4CV).Tomographic ultrasound imaging (TUI) was then used to present the different views of fetal hearts,and were compared with those aquired by 2-dimensional (2D) ultrasound.Results All the 83 volume datasets were obtained by STIC.(1) Qualification ratio of short axis view of the aorta (SAV) by TUI was higher than that of 2D (94.0% vs 84.3%,x2 =5.57,P=0.042).While,in the other 8 planes,the qualification ration by STIC were higher than that of 2D,however no significant difference was found (P>0.05).(2) In 4CV,5-chamber view (5CV),left ventricular outflow tract view (LVOT),right ventricular outflow tract view (RVOT) and SAV,STIC showed excellent consistency with 2D (Kappa =0.79,0.90,0.92,0.93 and 0.77,respectively).And in long axis view of the aortic arch (LAV-AoA),long axis view of the duct (LAV-DA),superior and inferior vena cava (SVC+IVC) and short axis view of the ventricle (SVV),STIC showed good agreement with 2D (Kappa=0.72,0.67,0.74 and 0.70,respectively).(3) Image collecting time of T2D,TSTIC,TTUI and TSTIC+TUI were (5.80±1.58) min,(0.85±0.18) min,(2.57±1.32) min and (3.29±1.13) min,respectively.There were significant difference between TSTIC and T2D(t=3.500,P=0.000) and between TSTIC+TUI and TTUI (t=2.877,P=0.001).Conclusion The technique of STIC-TUI used in this study can display the standard planes of fetal heart conveniently,clearly and in shorter time.It is helpful for improving the quality and efficiency of fetal heart screening.
8.Analysis of relationship between intraepithelial neoplasia or cervical squamous cell carcinoma and infection of Human papilloma virus

Objective To explore the relationship of human papilloma virus (HPV) genotype and loading dose with development of cervical intraepithelial neoplasia (CIN) or squamous cell carcinoma of the cervix ( SCC),and explore the relationship of HPV genotype and CIN or SCC.Methods One hundred and twenty four patients in Sun Yat-sen Memoral Hospital of Sun Yat-sen University with CIN or SCC from September 2005 to December 2010 were selected in this study.HPV DNA was detected by Hybrid capture Ⅱand flow-through hybridization and gene chip.The relationship between the infection of HPV and CIN or SCC was analyzed.The influencing factors of CIN or SCC were analyzed by logistic regression.Results The total detection rate of HPV was 75.8%,and it was 44.4%,70.0%,95.7% and 76.2% in CIN Ⅰ,CIN Ⅱ,CIN Ⅲ and SCC group,respectively.The detection rate of HPV in high-grade lesion group ( 84.5% ) was higher than low-grade lesion group(44.4% ).The median of HPV load decreased in order as CIN Ⅲ,SCC,CIN Ⅱ and CIN Ⅰgroup.Infection of multi-genotypes or single genotype of high-risk HPV accounted for 97.9%.In logistic regression,HPV loading dose had significant influence on degree of cervical lesion.Conclusions Infection of HPV is a main etiological factor for SCC.There is some kind of correlation between HPV loading dose and development of SCC.
9.Clinical significance of counting follicles in diagnosis of polycystic ovary syndrome by the three-dimensional ultrasound imaging with sonography based automated volume calculation method

Objective To investigate clinical significance of counting follicles classification by three-dimensional imaging with sonography based automated volume calculation(SonoAVC)in the diagnosis of polycystic ovary syndrome(PCOS).Methods Eighty cases with PCOS were counted classified follicles and determined ovarian volume by three-dimensional(3D)imaging with SonoAVC method matched with 60 infertile women with fallopian tube or male factors as control.Main clinical.biological and other ultrasonographic markers were assessed during the early follicular phase,and the relationship between the follicle number range per ovary or the volume per ovary and the major hormonal features of PCOS was studied.Results Three-dimensional ultrasound imaging with SonoAVC methed provides a new path for objective quantitative assessment of follicle count.ovarian volume,total follicle numbers.The volume of (11 ±8)ml,total numbers of 27 ±14 follicle and number of22 ±19 follicle with diameter of≥2-<6 mm in PCOS patients were significantly higher than(6 ±4)ml in ovarian volume.6 ±4 in total follicles and 2 ±3 in follicle with diameter of≥2-<6 mm in controls(P<0.05).while follicles were similar for the≥6-≤9mm range(P>0.05).Total follicle numbers and follicles≥2-<6 mm had significantly positive relationships with ovarian volume ( r= 0. 600, 0. 618, P<0. 01 ) and level of testosterones ( r= 0. 364,0. 291, P<0.05), follicles ≥2-<6 mm also had significantly positive relationships with total follicle number (r=0. 916,P<0. 01 ). The follicles within the ≥6 - ≤9 mm range was significantly and negatively related to ovarian volume and total follicle numbers ( r = - 0. 618, - 0. 263, all P = 0. 001 ), but no significantly related to the major hormonal features of PCOS. The ovarian volume was significantly positively related with luteinizing hormone ( LH)/follicle stimulating hormone (FSH) ratio ( r= 0. 282, P = 0. 010)but negatively related to FSH level (r = - 0. 226, P = 0. 042). Conclusions Ovarian volume, total follicle numbers and follicles ≥2 - <6 mm in PCOS patients were significantly higher than those in controls. The larger ovarian volume might produce more total follicle and follicles ≥ 2 - <6 mm. The higher level of testosterone might produce more total follicle probably, which mainly result in more follicles ≥2 -<6 mm.These morphologically ultrasonographic characteristics could reflect pathophysiological changes in PCOS. Obviously, it has important clinical significance to count follicles in patients with PCOS by the threedimensional ultrasound imaging with SonoAVC method.
10.Clinial implication of tThe expression of Aurora B in normal endometrium and endometrial carcinoma.

The expression of Aurora B in normal endometria and endometrial carcinomas and its relation with clinicopathologic parameters of endometrial carcinomas were investigated. Streptavidin-biotin peroxidase (SP) immunohistochemical technique was used to detect the expression of Aurora B in 10 cases of normal proliferative phase endometria, 10 cases of normal secretory phase endometria and 72 cases of endometrial carcinomas respectively. According to the 1988 International Federation of Gynecology and Obstetrics (FIGO) grade, there were 37 patients in grade 1, 23 in grade 2 and 12 in grade 3 respectively. According to the FIGO stage, there were 59 patients in stage I-II and 13 patients in stage III-IV. Aurora B was expressed in both normal proliferative phase endometria, secretory phase endometria and endometrial carcinomas, but its positive labeling index (PLI) in proliferative phase endometria was significantly higher than that in secretory phase endometria (P<0.01) and endometrial carcinomas (P<0.01). The PLI of Aurora B was lower in tumors with well differentiation (G(1)), low surgical staging (I-II), and =1/2 myometrial invasion than that in tumors with moderate and low differentiation (G(2)-G(3)), higher surgical staging (III-IV), and >1/2 myometrial invasion (all P<0.01). Aurora B exerts its functions in the replication of normal endometrial glandular cells; Expression of Aurora B is significantly correlated with biologic behavior of endometrial carcinoma, indicating that Aurora B may be a promising prognostic factor in endometrial carcinoma.
