1.Diagnosis and treatment of bacterial infection in children with febrile neutropenia

Infection rate was high in children with hematologic cancers and febrile neutropenia,especially the number of drug-resistant bacteria showed an increasing trend.Because of defects of the immune function,the typical symptoms,pathogenic bacteria and infection was not clear in these children,the fever may be the only sign of serious underlying infection,mortality related with infection was high,so it was important to evalute the clinical risk accuratly and select antibiotic properly.
2.Depression in Patients with Parkinson's Disease

Objective:To investigate the feature of depression in patients with Parkinson's disease(PD) and the related factors.Methods:Use Hamilton Depression Scale(HAMD) to measure the level of depression of 60 patients with PD.Functional state of the patients was evaluated by Motor Dysfunction Rating Scale for Parkinson's Disease(MDRSPD).Results:29 cases(48.3%) were identified to have depression,among whom 40% were of minor or moderate degree,8.3% were severe.The main manifestations of depression were anxiety/body,retardation,hopelessness and insomnia.Scores of hopelessness,retardation,and insomnia factor were significantly higher in the rigidity and tremor combining rigidity patients than those in tremor patients.The level of depression positively correlated with the duration of disease and degree of dysfunction due to PD,and it has no relation with age,sex,and education.Conclusions:The incidence of depression was higher in patients with PD.PD patients of different clinical types had different characteristics of depression.The rigidity type is more likely to get depression.The degrees are closely related to duration of disease and degree of dysfunetion.
3.Puerarin reduces serum levels of advanced glycation end products and monocyte chemoattractant protein-1 in diabetic rats

Objective To study the effects of puerarin on advanced glycation end products (AGEs) and expression of monocyte chemoattractant protein-1 (MCP-1) in diabetic rats. Methods The rats were randomly divided into normal control rats (CON rats), diabetic rats (DM rats), diabetic rats treated with aminoguanidine (AG rats), and diabetic rats treated with puerarin (PU rats). The diabetic rat model was induced by streptozotocin. The serum levels of AGEs and MCP-1 were quantified by fluorescence spectroscopy and ELISA respectively. Tissue sections with PAS staining and electronic microscopy were used for observation of the pathologic changes in renal tissues. Objective To study effect of piracetam combine with hyperbaric oxygen in treatment of acute carbon monoxide poisoning patients with electrocardiogram and its effects on Lactate clearance.Methods 60 patients of acute carbon monoxide poisoning who received therapy from February 2011 to February 2016 in our hospital were selected as research objects.All accord with the diagnostic criteria of acute carbon monoxide poisoning.According to draw method,those patients were divided into the experimental group(n=30)and the control group(n=30).The two groups were given a large number of sustained oxygen,intracranial pressure,protect brain cells,promote blood circulation and improve microcirculation and other basic symptomatic treatment.The control group on the basis,was treated with hyperbaric oxygen,one times a day,a total of ten times.while the experimental group was treated with piracetam combine with hyperbaric oxygen,hyperbaric oxygen method with the control group,intravenous drip of Piracetam and Sodium Chloride Injection,each 100ml,two times a day,a total of treatment for ten days.Then abnormal ECG,creatine kinase isoenzymes(CK-MB),troponin(cTnl),lactate clearance,incidence of delayed encephalopathy,mortality,therapeutic effect of two groups were compared.Results ECG abnormal rate there was no difference between the two groups before treatment,after treatment,the abnormal rate of the experimental group was significantly lower than the control group [6.66(2/30)vs.33.33%(10/30)](P<0.05); CK-MB、cTnl、6h and 24h after treatment,Lactate clearance rate was significantly higher than control group[(15.80±2.03)%vs.(10.26±2.01)%,(20.75±3.12)%vs.(13.07±2.56)%](P<0.05);DEACMP rate and mortality was significantly lower than the control group[6.66%(2/30)vs.33.33%(10/30),3.33%(1/30)vs.30.00%(9/30)](P<0.05); The total effective rate was significantly higher than the control group[95.56%(28/30)vs.75.56%(22/30)](P<0.05).Conclusion Piracetam combine with hyperbaric oxygen is well for acute carbon monoxide poisoning,which can improve the clearance rate of lactic acid,improve hypoxia and myocardial injury,and reduce the abnormal ecg.
6.Expression and the role of myeloid-derived suppressor cells in the peripheral blood in patients with chronic hepatitis B

Objective To investigate the correlation between the frequency of myeloid-derived suppressor cells (MDSC) and the frequency of regulatory T cells (Treg) in the peripheral blood in patients with chronic hepatitis B (CHB) and its clinical significance.Methods A total of 45 CHB patients including 23 mild-to-moderate CHB patients,22 severe CHB patients,and 15 healthy controls were enrolled.The frequencies of MDSC and Treg in the peripheral blood were studied using flow cytometry and its correlation with clinical data was analyzed by Sepearman correlation analysis.Results The median frequency of MDSC in CHB patients was 0.414%,which was significantly higher than that in healthy controls 0.226% (Z=-2.356,P=0.018 9).The frequency of MDSC in CHB patients was negatively correlated with the level of alanine transaminase (ALT) and aspartate transaminase (AST) (r=-0.480,-0.478; both P<0.01),but had no relations with hepatitis B virus (HBV) viral load (r=-0.049,P=0.75).An increase frequency of MDSC was observed in CHB patients with an ALT of 5 × upper limits of normal (ULN) or less or AST of 3 × ULN or less.The frequency of MDSC in CHB patients was positively correlated with that of Treg (r =0.345,P =0.02).Conclusions The activation and proliferation of MDSC may facilitate and maintain HBV persistent infection.The change of the frequency of MDSC is in line with that of Treg,indicating that immunosuppressive functions of MDSC may be related with the development of Treg in CHB.
7.Cost- benefit analysis of special nursing of stoma care

Objective To investigate the actual cost and benefit of stoma nursing at present as a reference for the improvement of stoma care cost management program. Methods A cross sectional study was carried out in gastrointestinal surgery of a tertiary- level hospital in southwestern China from April 2014 to December 2014. The consumption of intestinal stoma care work, the average monthly income/average monthly working hours ratio and other indicators of stoma care nurse specialist were measured by the step cost allocation method, to investigate the actually cost of a single stoma care service which colostomy patients had received and analyze the gap between actually costs and charge standard of Sichuan Province. Results The directly human cost of stoma care expenses were (22.33 ± 3.66) yuan; working fee expenses were (4.18 ± 0.68) yuan;administrative expenses were (1.87 ± 1.32 ) yuan; research and education expenses were (1.87 ± 1.32 ) yuan; the total cost of investment was (29.14 ± 4.76) yuan, which was much higher than the current charge standard of Chengdu Bureau of Commodity Prices (t=119.50, P <0.05). Conclusions There is a difference between the actual cost and charge standard of stoma care. The situation of low pay to labor is not in line with economic laws, and not conducive to the future development of stoma care speciality.
8.Effect of low frequency electrical stimulation on hormone levels in the organs of Chinese tree shrew cadavers

Objective To study the effect of low frequency electrical stimulation on hormone levels in organs of Chinese tree shrews after death.Methods Giving Chinese tree shrews low frequency electrical stimulation.At 0 h, 3 h, 6 h, 12 h, 18 h, 24 h, 36 h and 72 h after death, the thyroid, liver, spleen were taken,and the levels of endothelin (ET), atrial natriuretic factor( ANF) , thromboxane ( TX) were determined by RIA method.At 0 h after death, midbrain ventral tegmental area ( VTA) of Chinese tree shrews was taken to detect the c-fos expression.Results After electrical stimula-tion, ET, ANF, TX levels in the cadaver organs and VTA c-fos expression of Chinese tree shrews were significantly in-creased than in the control group.The contents were decreasing with the time after death.Conclusions Low frequency e-lectrical stimulation can induce the synthesis and release of hormones in organs and c-fos expression in brain tissue of Chi-nese tree shrews.
9.Effect of sevoflurane pretreatment on renal ischemia-reperfusion-induced apoptosis in kidney in rats

Objective To investigate the effects of sevoflurane pretreatment on renal ischemia-reperfusion (I/R)-induced apoptosis in kidney in rats. Methods Thirty pathogen-free male SD rats weighing 220-260 g were randomized into 3 groups (n=10 each):group control (group C);group I/R and group sevoflurane(group S). Renal I/R was induced by clamping the left renal pedicle for 45 min in I/R and S groups. In group S inhalation of 2.2% sevoflurane in O2 was started at 30 min before operation and maintained throughout the experiment.Venous blood samples were taken at 3 h of reperfusion for determination of serum BUN and Cr concentrations. The animals were then sacrificed and the left kidneys were removed for microscopic examination, detection of apoptosis(by TUNEL)and determination of heme oxygenase-1(HO-1) mRNA and protein expression (by RT-PCR and Western blot).Results Renal I/R significantly increased serum BUN and Cr concentrations, apoptotic index(percentage of apoptotic cells) and the severity of necrosis of renal proximal convoluted tubules (0=normal,4=necrosis of whole segment of proximal convoluted tubules).Sevoflurane inhalation attenuated the I/R-induced changes mentioned above.HO-1 mRNA and protein expression was up-regulated by I/R and HO-1 mRNA expression was further up-regulated by sevoflurane inhalation.Conclusion Sevoflurane pretreatment can protect kidney against I/R injury by attenuating cell apoptosis.Up-regulation of HO-1 mRNA expression may be involved in the mechanism.
10.Analysis of 1 817 cases of papilloma virus infection in women

Objective To investigate the infection status and genotype distribution of human papillomavirus (HPV) infection in Changzhou district ,and to provide a theoretical basis for the prevention ,development and clinical diagnosis and treatment of HPV . Methods From October 2015 to July 2016 ,1 718 cases of female cervical epithelial cells were collected ,and 28 kinds of gene typing were detected by PCR‐reverse dot blot hybridization .Results The infection rate:1 718 cases of women were collected ,the positive HPV infections were 34 .23% .The infection types :single infection rate was 23 .57% (405/1 718) .The high‐risk HPV subtype in‐fections accounted for 17 .17% (295/1 718) and the low‐risk HPV subtype infections accounted for 5 .18% (89/1 718) ,suspected high‐risk infection was 1 .22% (21/1 718) .Multiple infection rate was 10 .94% (188/1 718) .HPV52 was the most common infec‐tion among high‐risk HPV infection ,the positive rate was 16 .16% (95/588) .HPV61 was the most common infection among low‐risk HPV infection ,the positive rate was 4 .08% (24/588) .There was no significant difference between age and HPV positive rate . The 61-70 age group had the highest HPV multiple infection rate in all age groups .Conclusion The high HPV infection is ob‐served in Changzhou district ,among which single HPV52 infection and the high‐risk HPV infection are the most common infec‐tions .There is difference in HPV infection among different age groups .
