1.Stratified therapeutic choices in elderly patients with diffuse large B-cell lymphoma

The prognosis of elderly (>60 years) patients with diffuse large B-cell lymphoma (DLBCL) is significantly worse compared with young patients, and there is currently no standard treatment. Elderly patients with DLBCL are highly heterogeneous, a stratification before treatment can help achieve precise medicine and improve outcome of them. R-CHOP (rituximab, cyclophosphamide, vincristine, doxorubicin and prednisolone) is still the recommended treatment for fit elderly DLBCL patients; and for unfit or very old patients, chemotherapy of reduced dose or palliative treatments should be considered. Choices for relapsed or refractory patients are limited, and novel compounds or therapies that are well tolerated may have a good application prospect.
2.Effects of comprehensive geriatric assessment intervention on depression and anxiety in patients with benign prostatic hyperplasia

ObjectiveTo investigate the effects of comprehensive geriatric assessment (CGA) intervention on depression and anxiety in patients with benign prostatic hyperplasia (BPH).Methods90 patients with BPH (aged 65 years and over) from June 2009 to June 2010 were randomly assigned to control(n= 45) and CGA (n = 45) group.Control group received conventional therapy,while CGA group was given CGA and consultation therapy as well as conventional therapy.The scores of international were assayed and compared between two groups before and 6 months after treatment.ResultsThe scores of international prostate symptom score (IPSS),quality of life scale (QOLS),self-rating depression scale (SDS) and self-rating anxiety scale (SAS) in control (t= 11.0,5.84,7.08and 9.68,respectively) and CGA (t= 14.0,10.4,9.16 and 6.1,respectively)group 6 months after treatment reduced as compared with those before treatmen (all P < 0.01).After 6 months of treatment,the scores of IPSS,QOLS,SDS and SAS in CGA group were lower than in control group (t= 4.25,5.55,3.45 and 2.88,respectively,all P<0.01).ConclusionsCGA intervention can improve the quality of life in BPH patients and alleviate their depression and anxiety.

Angiogenesis, i.e. neovascularization from preexisting vasculature, is involved in a variety of physiological and pathological processes of the body, including the initiation, maintenance and progression of tumors. The process of angiogenesis depends on the dynamic balance between the activities of proangiogenic and anti-angiogenic factors. Angiogenesis inhibitors specifically prevent endothelial cells from proliferation, migration and tube formation. One of the inhibitors is endostatin, which is of wide spectrum and possesses profound angiogenesis inhibition and therapeutic effects on tumors with rich vasculature. This paper aims to explain the mechanism of the action, structure and biological function of endostatin, and to discuss its application in anti-neoplastic therapies and the issues in the field of interventional radiology. The authors point out emphatically that the application scheme must be designed strictly and scientifically. Only when combination use of endostatin with other therapeutics which can effectively destroy tumor tissues is carried out to gain synergistic and intensified effect, can marked objective efficacy be obtained.
4.Effects of beraprost sodium on serum nitric oxide in elderly patients with lower extremity arteriosclerotic occlusive disease

Objective To investigate the therapeutic effects and safety of beraprost sodium on vascular endothelial dysfunction in elderly patients with lower extremity arteriosclerotic occlusive disease (LEAOD).Methods A total of 80 patients with LEAOD were randomly divided into 2 groups:the treatment group and the control group (n=40 each group).All patients were treated with conventional antihypertensive,antidiabetic,hypolipidemic,antiplatelet drug therapy and non-drug therapy such as smoking cessation,foot care.In addition,patients in the treatment group received 40 μg oral beraprost sodium at a dose of 120 μg/day,3 times/day for 30 days.The changes in symptoms,vital signs,the function of liver and kidney,hemorheology,and serum nitric oxide (NO)were observed before and after the treatment.Results No adverse reactions happened among all patients.After 30 days treatment,there was a significant difference in the total efficacy between the treatment and control groups (80% vs.35%,x2 =19.60,P<0.01).The levels of plasma viscosity,erythrocyte aggregation index and serum NO in the control group were (1.70±0.19) mpas.s,5.69±1.08,(43.86±5.20) μmol/L,respectively before the treatment and (1.36±0.14) mpas.s,3.23±0.67,(56.84 ± 7.05) μmol/L,respectively after the treatment.The levels of plasma viscosity,erythrocyte aggregation index and serum NO in the treatment group were (1.71 ±0.18) mpas.s,5.70±1.04,(44.24±5.40) μmol/L,respectively before the treatment and (1.10±0.12) mpas.s,2.80 ±0.52,(64.00±8.15) μmol/L,respectively after the treatment.Compared with pretreatment,the levels of plasma viscosity and erythrocyte aggregation index in the two groups were significantly decreased and serum NO level was increased after the treatment (t=9.07,12.17,17.85,15.76,9.38,12.78,respectively,all P<0.05).Compared with the control group,the levels of plasma viscosity and erythrocyte aggregation index in the treatment group were significantly decreased and serum NO level was increased (t =8.91,3.27,4.21,all P<0.05).Conclusions The oral beraprost sodium can effectively and safely improve the hemorheologic and endothelial function in elderly patients with LEAOD.
5.Effects of Jianpi Qinghua Recipe on angiotensin II/NADPH oxidase pathway in 5/6 nephrectomized rats

Objective:To study the effect of Jianpi Qinghua Recipe ( JPQHR) on angiotensin II/NADPH oxidase pathway in 5/6 nephrectomized rat renal failure model and the underlying mechanisms.
Methods:The animals were divided into 4 groups:the sham-operated group, the renal failure group, the JPQHR-treated group and the losartan-treated group. After 60-days therapy, serum nitrogen and creatinine were measured. The expression of angiotensin II type 1 receptor (AT1) protein and the expression of p47phox mRNA in renal tissue was determined. SOD and MDA were also examined.
Results:Compared with the sham-operated group, the levels of SCr and serum BUN and the AT1 protein and p47phox mRNA expression in the renal failure group were significantly increased. hTe activities of SOD in renal tissue from the renal failure group was signiifcantly down-regulated while MDA was up-regulated (P<0.05). Compared with the renal failure group, the levels of SCr and serum BUN and the AT1 protein and p47phox mRNA expression in both JPQHR-treated group and losartan-treated group were signiifcantly decreased. hTe activities of SOD in renal tissue from JPQHR-treated group and losartan-treated group were signiifcantly up-regulated whereas the content of MDA were down-regulated (P<0.05). Compared with the losartan-treated group, the activities of SOD in renal tissue from the JPQHR-treated group was obviously increased (P<0.05), the decrease in AT1 protein and p47phox mRNA was more evident but not statistically different (P>0.05). The level of SCr and serum BUN and the content of MDA were also not statistically different (P>0.05).
Conclusion:hTrough decrease the expression of angiotensin II and NADPH oxidase, JPQHR can reduce the oxidative stress in chronic renal failure and delay the renal ifbrosis progression.
6.Targeted therapy progress of indolent B-cell lymphoma

Indolent B-cell lymphoma is still an incurable disease. However, with the further understanding of the pathogenesis of B-cell lymphoma in recent years, the number of treatment drugs and protocols for indolent B-cell lymphoma is increasing, including targeted drugs CD20 monoclonal antibody, BTK inhibitors, immunosuppressive agents, proteasome inhibitors, immune checkpoint inhibitors, which may have effects on the future treatment strategies. This paper summarizes the key clinical trials of targeted therapies for indolent B-cell lymphoma according to the 59th American Society of Hematology Annual Meeting.
7.Progress of EB virus related lymphomas

The occurrence and development of some lymphomas are closely related to EB virus, including B cell lymphoma, NK/T cell lymphoma, Hodgkin lymphoma and post-transplant lymphoproliferative disorder. Many studies have shown that some gene expression products that related to EB virus latent infection play a crucial role in the development of lymphoma. In this paper, the structure and biological characteristics of EB virus, its gene expression products, the mechanism of tumorigenesis and the related lymphomas are reviewed.
8.Mechanism of Shenci capsule combined with cisplatin in reversing DDP resistance of human lung adenocarcinoma via PI3K/AKT/mTOR signa-ling pathway

AIM:To investigate the effect of Shenci capsule combined with cisplatin ( DDP) in reversing DDP resistance by PI3K/AKT/mTOR signaling pathway in nude mice bearing A 549/DDP tumor.METHODS:The patient-de-rived lung adenocarcinoma A 549/DDP cell xenograft model was established .The tumor-bearing nude mice were randomly divided into control group , Shenci capsule group , DDP group and Shenci capsule combined with DDP group .The mice in control group was treated with normal saline , while the mice in other groups were treated with different drugs for 21 d.After treatment, the mice were killed and lung cancer tissues were collected .Flow cytometry was used to analyze the cell cycle and apoptosis.FQ-PCR was used to determined the mRNA levels of PTEN , P-glycoprotein, PI3K, AKT and mTOR in A549/DDP lung tumor .RESULTS:Compared with control group , the cell proliferation in all the drug treatment groups was inhibited .Compared with other drug treatment groups , Shenci capsule combined with DDP blocked the cell cycle of A 549/DDP cells at G2/M phase, promoted the apoptosis rate , increased the mRNA expression of PTEN and inhibited the mRNA expression of P-glycoprotein, PI3K, AKT and mTOR.CONCLUSION:Shenci capsule increases the sensitivity of A 549/DDP resistant cells in nude mice to DDP by blocking PI 3K/AKT/mTOR signaling pathway , increasing the expression of PTEN or inhibiting P-glycoprotein-mediated resistance pathway .
9.A propensity score matching analysis of hepatectomy and radiofrequency ablation for patients with solitary hepatocellular carcinoma less than or equal to 5 cm and portal hypertension

Objective To compare the clinical efficacy of hepatectomy and radiofrequency ablation (RFA) for patients with solitary hepatocellular carcinoma (HCC) less than or equal to 5 cm (≤5 cm) and portal hypertension (PHT).Methods The propensity score matching and retrospective case-control study was conducted.The clinicopathological data of 154 patients with solitary HCC ≤ 5 cm and PHT who were admitted to the Qingdao University between January 2011 and July 2016 were collected.Of 154 patients,61 and 93 underwent R0 hepatectomy and RFA,respectively.Observation indicators:(1) the propensity score matching conditions and comparison of general data between groups after the propensity score matching:patients' data were matched by the propensity score matching,patients undergoing hepatectomy and RFA were respectively allocated into the hepatectomy group and RFA group;(2) follow-up and survival.Follow-up using outpatient examination and telephone interview was performed to detect postoperative tumor-free and overall survival up to July 31,2018.Patients received enhanced scans of computed tomography (CT) or magnetic resonance imaging (MRI) in 1 month postoperatively,and then reexaminations of liver function,level of alpha-fetoprotein (AFP),B ultrasound and pulmonary CT within 3 months postoperatively,once every 3 months from 3 months postoperatively to 2 years and once every 6 months after 2 years postoperatively.Measurement data with skewed distribution were described as M (range).Comparisons of count data were analyzed using chi-square test.The Logistic regression model was used for the propensity score matching.The survival curve and rate were respectively drawn and calculated by the Kaplan-Meier method,and Log-rank test was used for survival analysis.Results (1) The propensity score matching conditions and comparison of general data between groups after the propensity score matching:118 of 154 patients had successful matching,including 59 in each group.There were statistically significant differences in the tumor diameter and level of preoperative albumin (Alb) before the propensity score matching between groups (x2 =11.050,6.687,P<0.05),and no statistically significant difference after the propensity score matching between groups (x2=2.366,2.484,P>0.05).(2) Follow-up and survival:all the 154 patients were followed up for 4.5-91.4 months before the propensity score matching,with a median time of 44.4 months.The postoperative 1-,2-,3-and 5-year tumor-free survival rates were respectively 88.1%,79.5%,64.1%,40.3% in patients of hepatectomy group and 84.9%,68.6%,52.8%,43.4% in patients of RFA group,with no statistically significant difference in the tumor-free survival between groups (x2 =0.997,P>0.05).The postoperative 1-,2-,3-and 5-year overall survival rates were respectively 95.1%,88.5%,85.0%,70.1% in patients of hepateetomy group and 100.0%,95.7%,85.6%,73.7% in patients of RFA group,with no statistically significant difference in the overall survival between groups (x2 =0.053,P>0.05).One hundred and eighteen patients were followed up for 4.5-91.4 months after the propensity score matching,with a median time of 49.4 months.The postoperative 1-,2-,3-and 5-year tumor-free survival rates were respectively 89.4%,82.3%,66.4%,41.7% in patients of hepatectomy group and 83.1%,64.2%,47.4%,38.5% in patients of RFA group,with no statistically significant difference in the tumor-free survival between groups (x2=2.612,P>0.05).The postoperative 1-,2-,3-and 5-year overall survival rates were respectively 94.9%,89.8%,86.1%,70.8% in patients of hepatectomy group and 100.0%,91.5%,79.4%,67.6% in patients of RFA group,with no statistically significant difference in the overall survival between groups (x2 =0.383,P>0.05).Conclusion The both hepatectomy and RFA are reliable radical treatments for solitary HCC ≤ 5 cm and PHT.
10.Shenci capsule's effect on the immunologic function and hematological toxicity in the treatment of advanced non-small cell lung cancer adopting GP regime

Objective To observe Shenci capsule's effect on immunologic function and hematological tox-icity of patients with advanced non-small cell lung cancer.Methods 60 cases of patients with advanced non-small cell lung cancer were chosen and divided into the therapy group and the control group randomly with 30 cases in each group respectively. While patients in the control group were administered simple GP chemotherapy regime, patients in the therapy group were administered Shenci capsule treatment orally in addition to the therapy in the control group.Comparisons were made of the variations in the subgroup of T lymphocyte(including the absolute val-ue of CD3+cell,the absolute value of CD4+cell,the absolute value of CD8+cell and CD4+/CD8+ratio)of hematology before and after chemotherapy between the two groups of patients. Results No significant discrepancy was found in the comparison of the absolute value of CD3+cell,the absolute value of CD4+cell,the absolute value of CD8+cell and CD4+/CD8+ratio before the chemotherapy between the two groups(P > 0.05)and the absolute value of CD3+cell,and the absolute value of CD4+cell of both groups decreased significantly after the chemotherapy(P<0.05), especially in the control group(P < 0.05). The hematological toxicity of the therapy group was lower than that in the control group(P < 0.05)after the chemotherapy. Conclusion Shenci capsule was capable of enhancing the body immunity in advanced non-small cell lung cancer and alleviating hematological toxicity.
