This paper discussed the detection of tumor micrometastasis, and analysed the influence of chemotherapy and blood transfusion on the immunity of tumor patients. Chemotherapy and blood transfusion can increase the opportunity of tumor metastasis. By analysing the mechanism of the treatment of tumor with traditional Chinese medicine, the author put forward the new method about eliminating tumor micrometastasis with traditional Chinese medicine.
2.Relationship of the occurrence and development of rheumatism arthritis with T lymphocyte subsets and CD4+ TH1/TH2 cell functional subsets

BACKGROUND: Rheumatoid arthritis(RA) is an autoallergic disease,but its immunological pathogenesis has not been completely known. T lymphocytes, especially CD4+ TH1/TH2 cells, may have an important effect in the occurrence and development of RA.OBJECTIVE: To investigate the action of CD4+ TH1 and TH2 cells which separately mediate cellular immunity and humoral immunologic response (respectively) function in the occurrence and development of RA.DESIGN: Case-control, comparative observation.SETTING: Department of Immunology, Medical College of Chinese People's Armed Police Forces.PARTICIPANTS: Totally 15 patients with RA hospitalized in the Department of Internal Medicine of General Hospital of Tianjin Medical University between March 1999 and March 2000 were selected for a RA patient group, consisting of 2 males and 13 females. Of them, 12 patients whose serumal rheumatoid factor (RF)was positive and the three others' was negative. At the same time, 30 healthy individuals from persons receiving health examination in the hospital or from our department were selected for a healthy control group, consisting of 4 males and 26 females. Informed consents were obtained from the participants.peripheral blood mononuclear cells(PBMC) from the two groups of subjects were detected by enzyme-linked immunospot assay (ELISPOT). 200-500 lymphocytes were counted under a normal light microscope, and the perwhich secreted cytokines were detected by ELISPOT, and those which had red spots in their plasma after staining were positive cells. Among them,the cells secreting γ-interferon (IFN-γ) were Tm cells while those secreting interleukin-4 (IL-4) were TH2 cells. 200-500 lymphocytes were counted under a normal light microscope and the percentages of TH1 and TH2 cells square test were used for comparing statistical differences of data between the two groups.MAIN OUTCOME MEASURES: Quantitative analysis of T lymphocyte subsets (including CD3+ T cells, CD4+ T cells and CD8+ T cells) and CD4+ TH1/TH2 cells in peripheral blood from the two groups of subjects.RESULTS: Analysis of the results was from the study on all of subjects percentages of total T cells (CD3+ T cells), CD4+T cells and CD8+F cells in peripheral blood were not significantly different between the two significantly higher in RA patient group than that in healthy control group [(24.44±5.25)%, (14.93±3.82)%, P < 0.05],while in RA patient group the percentage of TH2 cells and ratio of TH1 cells to TH2 cells from peripheral blood were not significantly different as compared with those of control group(P > 0.05).CONCLUSION: Cellular immunologic function mediated by TH1 cells may be associated with the occurrence and development of RA.
3.Study on the relationship between nurses' organizational career management and job satisfaction in the third-grade class-A hospital

Objective To investigate nurses' organizational career management and job satisfaction in the third-grade class-A hospital,and analyze the relationship between them.Methods A total of 576nurses were recruited by two-stage stratified random sampling method from four third-grade class-A hospitals.They were investigated with Nurses' Organizational Career Management Questionnaire and Job Satisfaction Scale.Results The mean score of nurses' organizational career management and job satisfaction was(2.94±0.52)and(3.32±0.46).There were significant differences on the scores of job satisfaction in nurses with different ages,nursing age,shift,posts,professional tifles,departments,monthly income and type of hospitals.Nurses' organizational career management was positively correlated to job satisfaction.After controlling for the demographic variables,the overall state of organizational career management could explain 34.2 percent for the variances of job satisfaction respectively.Conclusions It is suggested to pay close attention to nurses' organizational career management and take effective career management measures to improve nurses' job satisfaction so as to stabilize the nursing team and retain the excellent nurses.
4.Surgical treatment in ischemic cerebrovascular disease: evaluation with MR imaging perfusion-weighted

Objective To evaluate the effect of surgical treatment in ischemic cerebrovascular disease using perfusion-weighted magnetic resonance imaging (PWI) technique.Methods The clinical and imaging data of 24 patients with ischemic cerebrovascular disease were analyzed retrospectively. Cerebral perfusion was compared before and after operation by PWI.Results In our series, there were 9 cases of stenosis or occlusion of unilateral internal carotid artery (ICA), 8 cases of unilateral stenosis or occlusion of middle cerebral artery (MCA), 5 cases of multiple stenoses of intra- or extracranial artery system, 2 cases of moyamoya disease. PWI revealed 1 case of normal regional cerebral blood flow (rCBF) and mean transit time (MTT), 15cases of normal rCBF with increased MTT, and 8 cases of decreased rCBF with increased MTT. 23 patients received surgical treatment. There were 13 patients performed by stent angioplasty, 4 patients by endarterectomy, and 6 patients by STA-MCA bypass surgery. Cerebral perfusion improved in 19 cases (82.6%), which included 12 cases (92.3%) performed with stent angioplasty, 4 cases (100%) with endarterectomy and 3 cases (50%) with STA-MCA bypass surgery.Conclusions Being a new type of examination, PWI is helpful to evaluate the status of cerebral perfusion. Surgical methods are effective in treatment of ischemic cerebrovascular disease.
5.Differential gene analysis and expression of GST-? mRNA in esophageal cancerous and noncancerous tissues

Objective To investigate the role of GST-? mRNA in the development and pathogenesis of esophageal cancer. Methods Twenty two matched pairs of esophageal cancerous and noncancerous tissues were obtained from 22 patients with esophageal cancer in an endemic region. AP-PCR was used for determining the differential gene fragments and RT-PCR for detecting the expression of GST-? mRNA. Results Differential random amplified fragments were found in 6 cancerous tissues, and none of the noncancerous tissues. The 5T differential gene fragments were of 1.0 kb, and by cloning, sequencing, and sequence homology analysis, no homologous sequence was found in the gene library. The expression rates of GST-? mRNA in cancerous and noncancerous tissues were 54.5%(12/22) and 18.2%(4/22), respectively. Conclusions Whether the 5T differential gene fragment is a new gene candidate or is a marker of oncogene remains to be further studied. The expression of GST-? mRNA in esophageal cancerous tissue was markedly enhanced.

Objective:To prepare bifunction antibody of anti-human laryngocarcinoma/anti-CD 3 for the active immune therapy of laryngocarcinoma.Methods:To corss link monoclonal antibody of anti-human laryngocarcinoma and anti-CD 3 into bifunction antibody by chemical method,which can guide the killer function on carcinoma cells.Results:The killer rate of effector cells guided by bifunction antibody is higher than that of anti-CD 3 which can arise with the elevation of effect target ratio.Conclusion:Bifunction antibody of anti-human laryngocarcinoma/anti-CD 3 prepared by chemical cross linking method has the potential value for clinical application. 7.Effect of the training scheme of microvascular anastomosis in different time courses

Objective To investigate the effect of the training scheme of microvascular anastomosis in different time courses. Methods A total of 31 neurosurgeons were trained with different training schemes of microvascular anastomosis with ultrashort time course (n = 9;4 h),medium time course (n =12;12 h),and long time course (n = 10,300 h)were selected respectively,including 22 neurosurgeons from the top three hospitals and 9 from other levels of hospitals. Before training,the average median time of working in a department of neurosurgery was 6 (range,0 to 19)years. After training,the trainees accepted the assessments,such as performing the rat common carotid artery end to end anastomosis under a surgical microscope. Their completion time,anastomosis quality score,and proportion of vascular patency after anastomosis were compared. The measurement data of normal distribution used the single factor analysis of variance. The skewed distribution used rank sum test. The comparison of count data used Fisher exact test. Results The trainees who participated in the ultrashort time course training,the time of completion of carotid end - end anastomosis was 78 ± 37 min,the anastomosis quality score was 8. 1 ± 2. 8,and 2 vessels were patent;the trainees who participated in the medium time course training,the anastomosis time was 69 ± 20 min,the anastomosis quality score was 15. 8 ± 6. 8,and 10 vessels were patent;the trainees who participated in the long time course training,the anastomosis time was 34 ± 7 min,the anastomosis quality score was 23. 5 ± 1. 3,and 10 vessels were patent. There were significant differences in the completion of anastomosis time among the 3 groups of trainees (F = 9. 50,P = 0. 001). The completion time of the long time course group was shorter than that of the medium time course group and the ultrashort time course group. There were significant differences. There were significant differences in the anastomosis quality score among the 3 groups (F = 26. 870,P = 0. 000). As for the number of the patent vessels,there were significant differences between the medium and long time course groups and the ultrashort time course group (P < 0. 01 ). Conclusion If the vascular anastomosis skills of the trainees achieve relative proficiency and stability,they need to choose the long time course training.
8.The analysis of the clinical and pathologic features in patients with peripheral neuropathy as main manifestation of microscopic polyangiitis

Objective To investigate the clinical and pathologic features in patients of microscopic polyangiitis (MPA) with peripheral nerve abnormality.Methods We collected clinical data of 6 patients with MPA that was confirmed by immunological,pathological and clinical findings.Electroneurophysiologic examinations and sural nerve biopsies were performed in these patients.Two normal controls were included in these studies.Results All of 6 patients developed asymmetric polyneuropathies.Electrodiagnostic studies showed reduced amplitudes of the sensory nerve action potential and compound motor action potential with mild impaired conductive velocities.The patients presented vasculitis changes with cell infiltration by monocytes and lymphocytes.Sural nerve biopsies found loss of myelinated fibers in all of the patients indicating axonal degeneration.Some of them associated with regeneration clusters of myelinated fibers.Conclusions Asymmetric neuropathy with sensory nerve injuries is the mainfeature in the MPA associated polyneuropathy.Electrodiagnostic examination indicated axonal lesions in mononeuritis multiplex.Sural nerve biopsies confirmed the active axonal lesions and vasculitis.
9.Growth inhibitory effect of triptolide combined with hyperthermia on Hep-2 cells

Objective To explore the effect of triptolide(TL) combined with hyperthermia on proliferation and apoptosis of Hep-2 cells and its affect on cell cycle phases,and provide theoretical foundation for treatment of laryngocarcinoma.Methods Hep-2 cells at logarithmic growth phase were randomly divided into control,hyperthermia,TL,TL combined with hyperthermia groups.MTT method was adopted to investigate the proliferation inhibition rate of the Hep-2 cells.Cell cycle and apoptotic rate of Hep-2 cells were analyzed by flow cytometry,and then the changes of subcellular structures were observed by electron microscope.Results The proliferation inhibition rates in hyperthermia,TL,TL combined with hyperthermia groups were 21.2%,28.5% and 54.5%,respectively;the proliferation inhibition rate in TL combined with hyperthermia group was higher than those in hyperthermia and TL groups(P
10.Simultaneous surgery in patients suffering from tumor combined with coronary artery disease

Objective:To investigate the possibility and feasibility of the simultaneous coronary artery bypass grafting (CABG) with tumor excision in patients suffering from tumor combined with coronary artery disease(CAD). Methods:From August 2000 to July 2006, ten patients who were suspected of tumor (four patients suffered from urinary system tumor, two digestive system tumor, and four pulmonary tumor) with coronary artery disease were successfully treated by simultaneous surgeries. Surgical incisions were chose by the different tumor position. All carried on the CABG before tumor excision except one case. The two cases had cardiopulmonary bypass CABG, the rest eight had off-pump CABG. Follow-up was conducted after operation. Results:The combined operations were satisfied. There was no in-hospital mortality, no postoperative hemorrhage, no myocardial ischemia or infarction, with few complications. Postoperative in-hospital stay was 10-34 days, average 22.4 days. All discharged in cure. One patient lost the follow-up. Nine patients were followed up from six months to seven years three months. Two patients died from metastasis, and the others survived with satisfied follow-up without cardiovascular events. Conclusion:The simultaneous coronary artery bypass grafting with tumor excision for patients who suffer from either CAD with benign or malignant tumor is safe and feasible with satisfied short-and long-term survival.
