1.Entubulization nerve reconstruction in the experimental rat resulting in return of erectile function

Objective To investigate the feasibility of repairing surgically ablated cavernous nerves with silastic nerve tube conduit filled with nerve growth enhancing media. Methods 54 male adult Sprague-Dawley rats were randomized into 3 groups,the sham operated controls,the bilateral cavernous nerve ablation and entubulization nerve reconstruction with insulin like growth factor(IGF).1,3 and 6 months after surgery,rats models were evaluated with apomorphine test,followed by nicotinamide adenine dinucleotide phosphate (NADPH) diaphorase staining to identify NOS in penile nerve fibers of the proxiamal portion of penile shaft and penile crus. Results There was no statistically significant difference between the ablated and the entubulization nerve reconstruction groups at 1 month,however,at 3 and 6 months,Apomorphine test studies resulted in tumescence from 42% and 50% of the entubulization nerve reconstruction groups versus 0% and 0% of the ablated group,respectively (P
2.Survey of lower urinary tract symptoms in 2811 men in Wuhan City

Objective To assess the prevalence and presentation of low urinary tract symptoms (LUTS) in Chinese men. Methods A random sample was obtained from men dwelling at communities,factories,institutions and colleges in a district in Wuhan City.By completion of the questionnaire and exclusion of individuals with operation and trauma history,2811 men (age range,20~82 years) were assigned into 6 groups according to 10-year age stratum.Frequency of symptoms and severity of discomfort were assessed using the International Prostate Symptom Score (IPSS). Results The 2811 men accounted for 71.7% of the surveyed population. The occurrence of LUTS was as follows.In 20~29 year group,only 5 (0.6%) of 798 men had mild LUTS (IPSS,1~7).In 30~39 year group,55 (12.1%) of 453 men had mild LUTS.In 40~49 year group,143 (26.6%) of 537 men had LUTS;of them 118 were of mild LUTS,20 of moderate (IPSS,8~19),5 of severe (IPSS,20~35).In 50~59 year group,244 (52.5%) of 465 men had LUTS;of them 109 were of mild,117 of moderate,18 of severe.In 60~69 year group,240 (65.6%) of 366 men had LUTS;of them 78 were of mild,138 of moderate,24 of severe.In≥70 year group,162 (84.4%) of 192 men had LUTS;of them 47 were of mild,68 of moderate,47 of severe.Differences between age groups in incidence rates of LUTS were significant (?2=998.385,P
3.A neuroanatomical study of the neurovascular bundle at the lateral border of the prostate in adult male

Objective To investigate the position and distribution of the neurovascular bundle at the lateral border of the prostate in adult male. Methods The neurovascular bundle at the lateral border of the prostate was observed on the remaining 25 sides of the medially dissected 18 male formalin-preserved cadavers with an operative microscope.The neurovascular bundle at the lateral border of the prostate of an adult male cadaver was explored with hematoxylin and eosin staining and nitric oxide synthase (nNOS) immunohistochemistry staining. Results The neurovascular bundle,which stemed from the pelvic plexus,together with the corresponding vessels were formed and divided into 2 branches to pass the lateral border of the prostate.One of them traveled between the posterolateral prostate and anterolateral rectum.The other traveled between the bladder neck and the prostate to reach the lateral border of the puboprostatic ligament.After 2 nerve branches of the neurovascular bundles pierced the urogenital diaphragm, they met together to form a nerve bundle again.Two bundles and the urogenital diaphragm formed a triangle area at the lateral border of the prostate.There was no nerve and vessel in this triangle area.The nerve branches at the lateral border of the prostate contained nerve cell bodies and fibers that stained positive for nitric oxide synthase. Conclusions There are 2 neurovascular bundles at the lateral border of the prostate.One of them travels between the posterolateral prostate and anterolateral rectum.The other travels between the bladder neck and the prostate to reach the lateral border of the puboprostatic ligament.They contain nerve ganglia that stain positive for nitric oxide synthase.
4.Anderson-Hynes pyeloplasty and double J tube internal drainage for the treatment of pediatric ureteropelvic junction obstruction

Objective To evaluate the operative treatment of pediatric ureteropelvic junction (UPJ)obstruction. Methods A total of 34 cases of UPJ obstruction in children had double J tubes placed intraoperatively during Anderson Hynes pyeloplasty.Urethral catheterization was maintained for 5~7 days,the double J tube being removed on cystoscopy 4~6 weeks after operation.The outcome was evaluated with ultrasonography or intravenous pelviureterography 3~6 months postoperatively. Results The obstruction was successfully relieved in all the patients and no complications such as urinary infection and leakage of urine were noted on follow up. Conclusions Anderson Hynes pyeloplasty is an effective procedure for the treatment of pediatric UPJ obstruction. The routine use of double J tubes placed intraoperatively can reduce the chance of UPJ restricture and shorten the hospital stay.It is safe and dependable.
5.Survey to the teaching of hospital management course

To carry out the teaching of hospital management in-depth and improve teaching quality, the group of teaching hospital management in 2010, through questionnaires, discussion and other forms, found out and analyzed the teaching situation, including the nature of this course, start time and hours, teacher team, teaching content, teaching methods, evaluation form, and so on. The result was that most students thought it not necessary to open so many courses. Then the article put forward some suggestions and countermeasures to further improve the management course.
6.Genitofemoral nerve graft repair the ablated cavernous nerves in rats

ObjectiveTo investigate the feasibility of repairing surgically ablated cavernous nerves by using an interposition genitofemoral nerve graft and with nerve growth enhancing media (insulin-like growth factor-I,IGF-I) injection.MethodsA total of 54 male adult Sprague-Dawley rats were randomly divided into 3 groups (each of 18 rats),including sham operation controls, bilateral cavernous nerve ablation and nerve graft with IGF-I injection groups.At 1,3 and 6 months after surgery, the rat models were evaluated with Apomorphine Test.Then nicotinamide adenine dinucleotide phosphate-diaphorase (NADPH-d) staining was used to identify nNOS in penile nerve fibers of the proximal portion of penile shaft.ResultsAt 1,3 and 6 months after surgery, the rats of sham operation group had normal erectile function (erection rate,100%),while the rats of nerve ablation group lost erectile function at all (erection rate,0%).There was no statistically significant difference of ED between the nerve ablation and nerve graft groups at 1 month;however,at 3 and 6 months,Apomorphine Test resulted in tumescence of 50% and 66.7% of the nerve graft groups vs 0% and 0% of the nerve ablation groups, respectively ( P

Objective To investigate the mechanism of contralateral testicle spermatogenesis impairment following unilateral testicular torsion in rats.Methods Healthy male Sprague-Dawley rats(n=24) were randomly divided into 3 groups:group 1(sham-operation),2 and 3,each comprising 8 rats.Under surgical conditions,the left testes of the rats in groups 2 and 3 were rotated through clockwise 720? torsion,then detorsion after 12 h or 24 h,respectively.The contralateral testes were harvested after 1 month and the relative proportions of germ cells were measured by the flow cytometry(FCM).Anti-rat immunoglobulin G(IgG)antibodies against spermatozoa antigens were detected in contralateral testicular tissue by immunohistochemical method.Results In groups 1,2 and 3,the weight of contralateral testis was(1555.73?(72.34)),(1184.20?101.02) and(783.60?117.93)mg,respectively;the number of apoptotic cells was 53.25?8.61,1622.00?129.31 and 3401.25?179.75,respectively;the positive rates of antisperm antibodies were 0,0.55?0.02 and 0.69?0.03,respectively.There were significant differences in above parameters between groups(P
8.Clinical application and long-term follow-up of the substitution of a segmental sinus-body of stomach for urinary bladder

Objective To evaluate the reconstruction of bladder using a segmental sinus-body of stomach on the basis of a clinical study.Methods We retrospectively reviewed the medical records,laboratory evaluations,imaging examinations,cystoscopy,urodynamic studies of 30 patients(17 men and 13 women;mean age,55 years;age range,21-69 years) who underwent the reconstruction of bladder using a segmental sinus-body of stomach.Of the 30 patients,24 had primary bladder cancer and 6 had tuberculous contracture of the bladder.Results After operation,the new gastric bladder worked well in keeping and emptying urine.All patients micturated through the urethra.The bladder capacity was 280-580 ml(mean,385 ml).The maximum urethral pressure was 20-60 cm H_2O(mean,49 cm H_2O).The filling bladder pressure was 5-15 cm H_2O(mean,12 cm H_2O).The maximum bladder pressure was 35-65 cm H_2O(mean,55 cm H_2O),and it was 28-60 cm H_2O(mean,46 cm H_2O) during urination.Qmax and post-void residual urine were 10-28 ml/s(mean,18 ml/s) and 5-85 ml(mean,20 ml),respectively.Follow-up ranged from 9 months to 24 years(mean,8.2 years).There were no disturbance of water and electrolyte metabolism,no vesicoureteral reflux,no uracratia,and no damage to renal function.Complications included perineal and vesical pain in 4 cases,enuresis in 5 cases,which gradually remitted 3-6 months after surgery,and bladder stone formation in 1 case,who underwent surgery again.At 3.5 years after surgery bladder tumor relapsed in 1 case,who then underwent transurethral resection of bladder tumor. Conclusions Our data show that the substitution of a segmental sinus-body of stomach for urinary bladder worth popularizing because of low complication rate and approximately normal urologic indexes.
