1.Systematic Review of the Efficacy of Acarbose Treatment in Impaired Glucose Tolerance

OBJECTIVE:To evaluate the efficacy of acarbose treatment in impaired glucose tolerance (IGT)and provide evi-dence-based reference for the clinical treatment. METHODS:Medline,Cochrane library,PubMed,EMBase,VIP database,CJFD and Wanfang database were retrieved to collect the randomized controlled trials(RCT)of comparison treatment(test group)of acar-bose and placebo(control group)in IGT. After the information collection and quality evaluation,Rev Man 5.0 was conducted for Mata-analysis. RESULTS:26 RCT,involving 2 221 patients were include in total. Meta-analysis result showed,compared with place-bo,acarbose could reduce FPG[WMD=-0.76,95%CI(-0.99,-0.53),P<0.001],2 h PG[WMD=-1.58,95%CI(-1.82,-1.34), P<0.001],HbA1c[WMD=-0.46,95%CI(-0.82,-0.11),P=0.01],BMI[WMD=-1.10,95%CI(-1.77,-0.44),P=0.001], TC[WMD=-0.35,95%CI(-0.59,-0.12),P=0.003],TG[WMD=-0.30,95%CI(-0.44,-0.16),P<0.001] significantly;it had no significant difference on HDL-C[WMD=-0.05,95%CI(-0.11,0.01),P=0.11] and LDL-C[WMD=-0.13,95%CI(-0.31, 0.05),P=0.17]. CONCLUSIONS:Acarbose has better efficacy in the treatment of IGT. Due to the low quality of included studies, it remains to be further verified by RCT with large sample and high quality.
2.Influence of strong concept on postoperative personality and quality of life of esophageal cancer patients

Objective To explore the effect of strong concept on the postoperative personality and quality of life of patients with esophageal cancer. Methods One hundred esophageal cancer patients during June to December 2014 were set as controls, and another 100 esophageal cancer patients during January to June 2015 as the observation group. The latter were intervened by strong concept including strong cognitive adjustment, belief adjustment and behavioral modification. Eysenck Personality Questionnaire and esophageal cancer QOL were used for evaluation. Results After the intervention, the scores on neuroticism, psychoticism and lie scale in the observation group were lower than those in the control group as well as pre-intervention , so it was with the scores on intrinsic and extrinsic scales. The quality of life in the observation group was significantly higher than that of the control group and before intervention (P<0.05). Conclusion Strong concept used for intervention can improve the personality of postoperative personality and quality of life of esophageal cancer patients by helping them to be confident and optimistic to face life positively.
3.Perioperative anxiety and depression in ovarian cancer patients:a qualitative study

Objective To explore the causes of perioperative anxiety and depression among patients with ovarian cancer. Methods With phenomenological method of gicalitative research,10 patients with ovarian cancer were interviewed and the data were analyzed with content analysis.Results The causes of perioperative anxiety and depression among the young patients with ovarian cancer included psychological barrier,care and self-accusation about their family; changed marital relationship,worries about increased economic burden on families; reduced sense of self-worthiness.Conclusion Nurses should know and master the causes of their anxiety and depression,pertinently work out nursing measures to eliminate the anxiety and depression and promote patient’s active cooperation with treatment so as to improve curative treatment and quality of life.
4.Optimizing Fermentation Conditions for Expression of Esterase B1 Fusion Protein in Recombinant Bacteria

Objective In order to research the best fermentation conditions of recombinant bacteria pThioHisA-B1/DH5? which contains gene of esterase B1.Methods Recombinant bacteria pThioHisA-B1/DH5? was induced in different culture media, induction temperatures, different inductor concentrations, different induction time in order to obtain the best expression effect.The expression products of different conditions were detected by SDS-PAGE and thin-layer gel scanning analysis.Results Based on the experiments, the optimized fermentation condition were that the culture medium was TB, inducing temperature was 28 ℃ and inducing time was 6 h,the concentration of IPTG was 0.4 mmol/L.The expression quantity of target protein attained 40.3% in total bacterial protein, and the soluble fusion protein accounted for 66.2% in total target proteins at the optimum conditions.Conclusion The optimized fermentation conditions of expressing fusion protein in recombinant bacteria have been found and in the conditions, soluble esterase B1 fusion protein of high yield can be obtained.
5.Sterile repositionable hemostasis clipping device in combination with high frequency electric snare in resection of colorectal polyps with wide and long peduncle

Objective To discuss the efficacy and safety of sterile repositionable hemostasis clipping device in combination with high frequency electric snare in polypectomy of colorectal polyps with wide and long peduncle. Methods From January 2014 to December 2015, 21 cases of colorectal polyps with wide and long peduncle (diameter greater than 2 cm) in endoscopic treatment under electronic colonoscopy. We used 1 ~ 2 hemostatic clips to clip colorectal polyps roots, then used electric resection with high frequency electric snare electric coagulation. Postoperative bleeding, perforation were observed follow-up. Results 24 polyps in 21 cases were removed one-time successfully. Stump errhysis in 1 case, hot biopsy forceps is given to deal with local wound followed by Olympus Clip HX-610-135L EZ titanium clip. There was no complication such as bleeding and perforation in 3 to 6 months after the operation. In the colonoscopy examination, recurrence of polyps were not found in the original polyp resection site. Conclusion Sterile repositionable hemostasis clipping device in combination with high frequency electric snare in polypectomy of colorectal polyps with wide and long peduncle is safe and effective, without bleeding or perforation.
6.Research on Construction Validity of Examinations for Medical Subjects Using Factor Analysis

We selected scores for histology and embryology,physiology,biochemistry,pathobiology,pharmacology,paediatrics,and medicine of the first-year(2004),second-year(2003),third-year(2002)students at a medical school as subjects to analyze construction validity using factor analysis.The common factors were selected if their cumulative value above 60%.The result showed that variance percentage explained by each factor was lower,and that the three level structure formed by recall,comprehend and application would not be explained ideally by these scores.The author considered that the examination items should be improved and it was limited for factor analysis method to apply to analyze medical examination items.
7.Necessity of Bone Wax Stanching Bleeding at Sternal Edge in Patients With Cardiac Surgery by Median Sternotomy

Objective: To explore the necessity for application of bone wax stanching bleeding at sternal edge in cardiac surgery by median sternotomy. Methods: A total of 445 patients who receive cardiac surgery by the same surgeon performing median sternotomy in our hospital from 2011-01 to 2014-12 were studied. According to application of bone wax stanching bleeding, the patients were divided into 2 groups: Bone wax group,n=210 and Non-bone wax group,n=235. Re-thoracotomy for hemostasis, 12 h, 24 h and total draining volume after the operation and time of closing incision were compared between 2 groups. Results: There were 2 patients and 1 patient received re-thoracotomy for hemostasis in Bone wax group and Non-bone wax group respectively, which was not related to bone marrow cavity bleeding. In Bone wax group and Non-bone wax group, the draining volume at 12 h post-operation were (451 ± 240) ml vs (483 ± 238) ml, at 24 h post-operation were (615 ± 304) ml vs (639 ± 285) ml and the total volume were (842 ± 467) ml vs (842 ± 364) ml, allP>0.05; the time of closing incision were (68.0 ± 23.0 ) min vs (66.0 ± 19.0 ) min,P>0.05; the total transfusion rates were 21% vs 19%,P>0.05. Conclusion: Application of bone wax at sternum edge could neither affecting the drainage nor the speed of closing incision in cardiac surgery and therefore, it might be given up.
8.Effect and mechanism of neuroepithelial cell transforming gene 1 on cellular radiosensitivity

Objective To study the effect of neuroepithelial cell transforming gene 1 (Net1) on the cellular radiosensitivity and underlying mechanism.Methods Real-time quantitative PCR was used to measure the variations in Net1 expression level upon irradiation.Radiosensitivity was analyzed by colonyforming assay after Net1-siRNAs.Net1-associated proteins were identified by co-immunoprecipitation.Results The Net1 mRNA level in the cells was increased significantly (t =-10.52,P < 0.05) after irradiation.Compared to the control group,siRNA-mediated silencing of Net1 enhanced cell radiosensitivity (t =15.31,11.65,P <0.05).Net1 was found to interact with Ku70,Ku80 and DNA-PKcs under either normal conditions or after irradiation.Conclusions Net1 could protect cells from irradiation by interaction with DNA repair proteins in non-homologous end joining pathway.
9.Analysis of clinical symptoms and influencing factors of anti-platelet aggregation drugs in patients with upper gastrointestinal bleeding

Objective To analysis of clinical symptoms and influencing factors of anti-platelet aggregation drugs in patients with upper gastrointestinal bleeding.Methods Using the research methods of retrospective analysis, collected 110 cases of gastrointestinal bleeding patients from June 2012 to June 2015 into the first affiliated hospital of xiamen university,and divided into two groups,the control group,79 patients were treated without antiplatelet aggregation drugs,the experimental group,31 patients were treated with single antiplatelet aggregation drug.observed the degree of gastrointestinal bleeding,the mode of bleeding, and the characteristics of endoscopic findings between two groups,according to the age,INR, and the type of drug taken, the clinical symptoms and influencing factors of the patients were analyzed further.Results The rate of gastrointestinal bleeding was significantly higher in the experimental group than in the control group(P<0.05).The older the patient,the higher rate of gastrointestinal bleeding,the main manifestations were gastritis or ulcers in endoscopy,patients with INR>3 had a significantly higher rate of gastrointestinal bleeding than patients with INR<3.Conclusion Anti-platelet aggregation drugs can significantly increase the chance of serious bleeding in the digestive tract,the patient's age, INR value and the type of medication can affect the incidence of gastrointestinal bleeding;long-term use of these drugs lead the most serious impact on the duodenum, can lead to the site intestinal mucosa damage,with the increasing age of patients,adverse reactions of the digestive tract increased.
10.Factors affecting the severity of disease symptoms in patients with acute leukemia

Objective To explore the factors related to the severity of disease symptoms in patients with acute leukemia (AL). Methods 198 AL patients with symptoms of disease (anemia, bleeding, infection, fever, etc.) from September 2013 to July 2016, were evaluated by the questionnaires of self-rating anxiety scale (SAS), self-rating depression scale (SDS), eysenck personality questionnaire (EPQA), Toronto alexithymia scale (TAS), family environment scale (FES), self-rating scale of illness conception and health seeking behavior (ICHSB). The results were compared with 198 healthy volunteers. Results The scores of SDS, SAS, factorⅠin FES, factor Ⅲ in FES, factor Ⅳ in FES, nervous and the total scores of EPQA, factor Ⅱ in TAS, factor Ⅱ in ICHSB were significantly higher in AL patients than those in healthy subjects [(41.09 ±6.85) scores vs. (36.74±6.99) scores, t=2.150, P=0.031; (38.64±7.51) scores vs. (31.79±7.57) scores, t= 3.327,P=0.001;(2.38±1.54) scores vs. (5.18±1.33) scores, t=3.319, P=0.001;(3.31±1.82) scores vs. (2.23±1.99) scores, t= 3.325, P= 0.001; (2.41±1.62) scores vs. (5.75±1.51) scores, t= 3.332, P= 0.001; (14.14±5.37) scores vs. (11.01±5.51) scores, t= 5.179, P= 0.000; (42.97±7.10) scores vs. (40.41±6.51) scores, t= 2.930, P=0.004;(22.97±4.57) scores vs. (21.54±4.13) scores, t=2.926, P=0.004; (16.37±3.89) scores vs. (15.92± 3.93) scores, t= 2.104, P= 0.034]. The difference was statistically significant (P< 0.05). The risk factors of AL were SAS (95 %CI= 1.064-1.210, P= 0.001), factor Ⅲ in FES (95 %CI= 1.104-1.694, P= 0.004), age (95 %CI= 1.027-1.094, P= 0.001), factor Ⅱ in TAS (95 %CI= 1.046-1.352, P= 0.005), besides, education level (95 %CI= 0.708-0.866, P= 0.001) acted as the protective role. Conclusions AL patients tend to the psychological problems such as depression, anxiety. Those will show much severer symptoms as a consequence of lacking of family warmth and concern, low expression of emotion, lacking of organization, low economic capacity, tension, and lacking of the ability to distinguish between emotion and physical feelings.
