Objective To explore the feasibility of repairing the skin and soft tissue defect at finger palm side with the reversed free thenar mini-perforator flaps. Methods From October,2010 to October,2016,7 patients were treated with reversed free thenar mini-perforator flap to repair skin and soft tissue defects at finger palm side, including 5 males and 2 females, which the age were between of 17 and 56 (average,35 years old).Cause of injury:1 case of abrasive wear,1 case of machine twist injury, and 5 cases of heavy injury. All cases with varying degrees of phalanx and tendon exposed. The wound size was from 1.3 cm × 2.0 cm to 2.2 cm × 5.0 cm.According to the origin of the artery in the thenar area,3 kinds of reversed free thenar mini-perforator flaps were designed to repair the defective wound. In the middle of the large fish, the width less than 2.5 cm perforating flap was designed,so that the area can be directly sutured. Results All 7 flaps survived. The flap size was from 1.5 cm × 2.4 cm to 2.5 cm × 5.5 cm.All patients were followed-up from 3 months to 17 months (average,9 months). The appearance and skin texture of flaps was close to the normal finger palm side of the skin, with a clear fingerprint.Two-point discrimination was between 9 mm and 15 mm.The sensory recovery to S3 in 3 cases,S4 in 4 cases.Linear scars were left in the donor areas, with no discomfort. Conclusion This technique has the advantages of good appearance, such as good appearance and small damage to the donor area. However, the surgeon is required the super microscopic techniques.

In recent years, recombinant human urokinase (rhPro-UK) has been widely used in the treatment of a variety of thromboembolic diseases, with significant efficacy and no obvious adverse reactions. In addition, it has a wide range of applications in many new technology fields. This article focuses on the application of rhPro-UK in the treatment of acute myocardial infarction, cerebrovascular disease, lower extremity deep vein thrombosis, arterial thrombosis and other diseases. rhPro-UK has demonstrated good thrombolytic efficacy and safety in these diseases, especially in patients with acute myocardial infarction, and adjuvant PCI therapy can significantly increase myocardial reperfusion, improve cardiac function, and do not increase the risk of bleeding. For cerebrovascular disease, rhPro-UK can significantly improve the degree of neurological deficit and has a high safety profile. In the treatment of lower extremity deep vein thrombosis, rhPro-UK has shown superior thrombolytic efficacy and safety compared with urokinase. For arterial thrombosis and biological stents, the use of rhPro-UK has also achieved some efficacy, but more research is needed to confirm its efficacy and safety. In addition, ultrasound-mediated drug-loaded thrombolysis systems also have potential applications in rhPro-UK therapy. Future research on rhPro-UK will focus more on the development of new technologies.

Objective To explore the therapeutic effect of lymphatic venous anastomosis based on MRI lymphangiography classifi-cation on lower limb lymphedema, and to analyze the factors affecting the therapeutic effect of lymphatic venous anastomosis. Methods A total of 378 patients with secondary lymphedema of lower limbs were selected. All of them underwent MRI lymphangiography. The differ-ence of circumference diameter and complication rate of the lower limbs of different types of patients before and after lymphatic venous anastomosis were compared. After 6 months of follow-up,the therapeutic effect was evaluated according to the International Lymphology Society classification standards for lymphedema. The risk factors affecting the therapeutic effect of lymphatic venous anastomosis was ana-lyzed,and a prediction model was established to analyze the value of this model in evaluating the clinical efficacy of lymphatic venous anastomosis. Results There was no significant difference in treatment efficiency and postoperative complication rate among different types of patients(P>0.05).The difference of circumference diameter of the lower limbs of type 3,type 4,type 5 and type 6 before and after treatment was greater than that of type 1,type 2 and type 7(P<0.05).Age,concomitant infection and lymph node venous fistula were the factors affecting the clinical efficacy of lymphatic venous anastomosis(P<0.05).The risk prediction model for the clinical efficacy of lymphatic venous anastomo-sis was developed through logistic regression analysis:P=1/[1+e(0.531Xage+0.722Xconcomitant infection+0.807Xlymph node venous fistula-1.249)].The Hosmer-Leme-show test indicated that the fitted model matched the standard curve with x2=2.190,P=0.975.The area under the curve(AUC)of the prediction model for the clinical efficacy of lymphatic venous anastomosis was 0.985,with a standard error(S.E)value of 0.009 and a 95%confidence interval(CI)of 0.967 to 1.000.Conclusion Lymphatic venous anastomosis based on MRI lymphangiography classifi-cation has better clinical efficacy and fewer complications in the treatment of lower limb lymphedema, and the prediction model based on the risk factors analysis has a high predictive value for the clinical efficacy of lymphatic venous anastomosis.

Objective To establish new classification criteria for early rheumatoid arthritis (E-RA) based on large samples of early inflammatory arthritis patients and to evaluate the value of this criteria in China. Methods Patients who had arthritic complaints with disease duration less than one year were enrolled. They were divided into RA group and non-RA group according to the clinical diagnosis by experienced rheumatologists. The clinical and laboratory parameters were analyzed and those with high sensitivity or specificity were selected as the new classification criteria.Statistical analysis was carried out by using t test,x2 test and Logistic regression. Results ① A total of 803 patients with early inflammatory arthritis were included in this study.Five hundreds and fourteen patients were diagnosed as early RA and 251 were diagnosed as other rheumatic diseases, and the diagnosis of 38 patients remained unestablished by the end of follow-up.② New E-RA classification criteria were established based on the parameters with high sensitivity and/or specificity. The sensitivity of the new E-RA criteria was 84.4%,which was higher than 1987 ACR criteria (58.0%),while the corresponding specificities were similar, which were 87.4% and 93.6% respectively.③ Compared with the complex scoring system of 2010 ACR/EULAR criteria, the E-RA criteria was more simple and practical. The diagnostic sensitivity and specificity of E-RA criteria were higher than those of 2010 ACR/EULAR criteria reported in the literatures.④ New classification criteria based on scoring system using Logistic regression analysis was established. The sensitivity of this criteria was 86.4%,which was higher than 1987 ACR criteria (58.0%).Conclusion The diagnostic value of the E-RA criteria developed in this study for early RA is better than 1987 ACR criteria, and is more simple than 2010 ACR/EULAR criteria. It may be used as a new classification criteria for early RA diagnosis.
