1.Application of prospective nursing in preventing constipation of patients with acute myocardial infarction

Objective To explore the effect of prospective nursing on preventing constipation for patients with acute myocardial infarction(AMI). Methods 43 AMI patients during July 2008 and June 2010 in our hospital were classified as the observation group, besides the nursing routine of the acute myocardial infarction plus routine therapy to prevent constipation, prospective nursing was implemented.Through the retrospective analysis of medical record, 41 of AMI in our hospital during March 2006 and June 2008 were classified as the control group, they were given the nursing routine of the acute myocardial infarction plus routine therapy to prevent constipation and the nursing intervention after occurrence of constipation. The defecation condition was assessed for the two groups of patients on admission and the defecation and mortality rate was compared on the eighth day of hospitalization. Results There was significant difference in constipation and mortality rate between the two groups of patients during hospitalization. Conclusions The prospective nursing for AMI patients to prevent constipation can obviously reduce the incidence of constipation, thus to avoid serious complications caused by constipation and reduce mortality.
2.Changes and meaning of function and structure of diabetic rats' peripheral nerve

Objective The aim of this study is to undemtand the development of peripheral nerve injury on diabetes mellitus.Methods Diabetes was induced by STZ.We determined nerve conduction velocity,thermal pain threshold,morphoh)gy changes of sciatic nerve and quantitative analysis of sural nerve of diabetic model group and normal control group after 2 weeks,4 weeks,6 weeks,8 weeks,12 weeks respectively,and observed dynamically functional and morphologic changes of peripheral nerve of diabetic rats.Results After 2 weeks,diabetic rats showed that motor and sensory nerve conduction velocities slowed down and thermal pain threshold decreased.After 4 weeks.changes of pathology of sciatic nerve were observed.With the duration extending,the damages of nerve were aggravated gradually.Conclusions Neumpathy appeared in earlier stage on experimental diabetic rats and functional changes were preceded to morphologic changes.

4.Protective effects of lisinopril on peripheral neuropathy in diabetic rats

Objective To investigate the effects of angiotensin-converting enzyme inhibitor (ACEI) lisinopril on experimental diabetic peripheral neuropathy (DPN) in the earlier stage and its mechanism. Methods Diabetes was induced by streptozotosin (STZ) in rats. After 8 weeks of treatment, the effects of lisinopril on function and structure of peripheral nerves were observed; superoxide dismutase, malondialdehyde, Na+K+-ATPase activity in sciatic nerve and NO, NO synthase levels in plasma were determined, and capillary density of sciatic nerve were also assessed. Results With lisinopril, nerve structure and function was improved, oxidative stress state was ameliorated, and NO level, Na+K+-ATPase activity and capillary density of endoneurium were increased (all P<0.05). Conclusion ACEIs are effective in DPN. The possible mechanisms may be related to the amelioration of ischemic state and related metabolic disorders of the nerves.
5.Effects of angiotensin-converting enzyme inhibitor on microcirculation of peripheral nerve in diabetic rats

Objective To investigate prevention and therapy effects of ACEI on peripheral nerve in the earlier stage of diabetic rats and the influence on microcirculation of experimental DPN.Methods Diabetes was induced by STZ.After 8 weeks of prevention or therapy treatment.the effects of lisinopril on function and structure of diabetic peripheral nerve were observed,the NO,NOS level in plasma and capillary density of sciatic nerve were determined.Results Lisinopril prevention and therapy treatment improved nerve conduction velocity,ameliorated morphology abnormalities in nerve fiber and capillary at different degrees.The NO level and capillary density of endoneurium were increased. Conclusions ACE inhibitors may exert neuroprotective effect through improving abnormality of microcirculation.
6.Laparoscopic radical prostatectomy: A report of 10 cases

Objective To study the feasibility of transperitoneal laparoscopic radical prostatectomy. Methods Laparoscopic radical prostatectomy was performed according to Montsouris technique. Results All the operations were successfully completed. The operating time was 330~540 min (mean, 433 min) and the estimated blood loss was 100~550 ml (mean, 274 ml). No patients required a blood transfusion. Bladder injury was found in 1 patient and was sutured promptly during the operation. The postoperative hospital stay was 15~23 days (mean, 17 days). The catheterization time was 14~23 days (mean, 16 days). In 2 patients presenting with urinary leakage, the catheter was removed at 20 and 23 days after surgery respectively, while in the remaining 8 patients, at 2 weeks after operation. Mild urinary incontinence occurred in 3 patients and disappeared after 4 weeks of supportive treatment. Pathological examinations revealed stage pT_ 1c) in 3 patients, stage pT_2 in 6, and stage pT_3 in 1. A follow-up for 3~21 months (mean, 7.5 months) was carried out. The postoperative prostate-specific antigen (PSA) level was 0~2.70 ?g/L(mean,0.05 ?g/L). Two patients returned a normal sexual function. Conclusions Strict following surgical indications, adequate preoperative preparation, improvement of surgical techniques, and proper application of surgical instruments and devices make transperitoneal laparoscopic radical prostatectomy feasible.
7.Application Analysis of IgM Antibody and DNA Test Results in Children Mycoplasma Pneumonia Infection

Objective To analyze the results of distribution of mycoplasma pneumoniae IgM antibody and DNA test,and ex-plore its application in diagnosis of Mycoplasma pneumoniae infection in children.Methods Retrospectively analysed myco-plasma pneumoniae antibody IgM (MP-IgM)and mycoplasma pneumoniae DNA (MP-DNA)results,114 374 passengers and 8 014 passengers,in 0 ~ 16 year-old children of Shengjing Hospital Affiliated to China Medical University from year 2013 to 2014.Analyzed of the relationship between gender,age,season and the positive rate of the MP-IgM and MP-DNA. Collected 5 106 patients with MP-IgM and MP-DNA results simultaneously and analyze the consistency between two posi-tive indexes in different ages.Results ①MP-IgM and MP-DNA positive rate varied in sex,age and season,the positive rate in female was higher than male.The positive rates of MP-IgM and MP-DNA at the age of 1~3 and 7~ 12 years old were higher than other age groups.The positive rate of MP-IgM in spring and autumn was higher,while MP-DNA was higher in summer and autumn and all differences were statistically significant (P < 0.001).②The positive rate of MP-DNA differed in specimens,the bronchoalveolar lavage fluid had the highest MP-DNA positive rate,49.95%.③The positive consistency of MP-IgM and MP-DNA was associated with age,with 4~6 and 7~12 year-olds the highest,respectively 35.2% and 45.8%. Conclusion ①Incidence of children pneumonia mycoplasma infection was mainly in preschool and school-aged children,more female than male,and autumn highest.② The detection of MP-DNA in bronchoalveolar lavage fluid was the best sample type.③For <1 and 1~3 year-olds,combined detection of MP-DNA and MP-IgM can improve diagnostic rates.
8.Left renal vein entrapment syndrome (report of 10 cases)

Objective To evaluate the diagnosis and treatment of left renal vein entrapment syndrome. Methods The clinical data of 10 cases of left renal vein entrapment syndrome were retrospectively analyzed.Their mean age was 24 years (range,14-37 years).Of them 7 cases presented with non-glomerular origin hematuria;2,with orthostatic proteinuria;1 with hematuria accompanied by proteinuria.Color Doppler sonography showed that the dilated segment of the left vein was three-fold than the strictured segment in diameter,and magnetic resonance angiography (MRA) showed that the left renal vein was compressed in 9 cases. Results Stent placement across the left renal vein was performed in 7 cases.Superior mesenteric arteriectomy and replantation were performed in 3 cases.The operations were all successful.Follow-up of 1-5 years showed that the clinical symptoms were improved and urine tests were normal. Conclusions Non-glomerular hematuria or orthostatic proteinuria is frequently found in left renal vein entrapment syndrome.Left renal vein entrapment syndrome can be diagnosed by Doppler sonography,MRA and cystoscopy in the presence of hematuria or proteinuria.Generally,conservative treatment and careful follow-up are conducted in these patients.Surgical and radiologic interventional procedures are effective alternatives for those with glomerular dysfunction and complications.

10.The Analysis of High Sensitive C-reactive Protein and Microvascular Complications and Its Relative Factors in Patients with Type2 Diabetes Mellitus

Objective To investigate the levels of serum high sensitive C-reactive protein(hs-CRP) in different stages of diabetic nephropathy and the relationship between hs-CRP and relative metabolic indexes.Methods According to their urinary albumin excretion rate(UAER),164 patients with type2 diabetes mellitus were divided into three groups: 51 patients with normal UAER,59 patients with microalbuminuria and 54 patients with clinical proteinuria.The concentration of hs-CRP,relative clinical biochemical profiles and the body height,weigh,systolic & diastolic blood pressure were measured and analyzed.Correlation analysis was also done between hs-CRP and other relative indexes.Result The levels of hs-CRP were increased with increment of UAER.Pearson correlation analysis and stepwise linear regression indicated that hs-CRP was positive correlated with duration,triglyceride(TG),BMI,MAP,HOMA-IR,fibrinogen(FIB),UAER,serum creatinine(Cr),HbA1C,but negative correlated with high-density lipoprotein(HDL-C) and HOMA-B.The levels of hs-CRP in female were higher than those in male.Conclusion High level of serum hs-CRP concentration maybe a risk factor for the onset of type2 diabetes mellitus and diabetic nephropathy.
