1.An immunohistochemical study of mixed hepatocarcinoma

Objective Pathologically, sometimes it is rather difficult to make differential diagnosis among hepatocellular, cholangiocellular and mixed type of hepatic tumors. The aim of this study was to analyze the immunohistochemical features of mixed type of hepatocarcinoma and to propose some useful diagnostic criteria. Methods Fifteen cases of the hepatic tumor were examined by routine histological and immunohistochemical methods with S P technique. Results The mixed type of hepatic tumor accounted for 3.39% of all cases of primary hepatocarcinoma in the same period. Histologically, all the mixed type of hepatic tumors revealed some characteristics of hepatocellular and cholangiocellular carcinomas, one of them was collision tumor. Immunohistochemically, 10 cases of the tumor were positive for ? fetal protein(AFP) in the hepatocellular carcinoma area, while all 15 tumors were negative for AFP in all cholangiocellular carcinoma areas. Cytokeratin(CK8) was positive in the hepatocellular carcinoma areas of all mixed tumor, whereas positive in cholangiocellular carcinoma areas of 13 cases; CK19 was positive in cholangiocellular carcinoma areas of all the tumors, while positive in hepatocellular carcinoma areas of 13 cases. Bcl 2 gene product was positive in 66.7% of this tumor, in which it was mainly expressed in cholangiocellular tumor area and in hepatocellular areas of five cases. AB/PAS stain for mucin was positive in cholangiocellular tumor regions of all the tumors. Conclusions Diagnosis of mixed hepatocarcinoma should be made mainly based on combined analysis of the data from pathological features, immunohistochemical phenotypes and histochemical stain for mucinous material.
2.The protective effect of LY367385 on impairment of cultured mouse cerebral cortical neurons induced by sodium glutamate or oxygen-glucose deprivation

ObjectiveTo investigate the protective effect of LY367385 on impairment of cultured mouse cerebral cortical neurons induced by sodium glutamate (Glu) or oxygen-glucose deprivation (OGD).MethodsNeuron damage induced by Glu or OGD, as well as the action of (S)-(+)-a-amino-4-carboxy-2-methylbenzeneacetic acid (LY367385) were measured by determining the leakage of lactate dehydrogenase (LDH) from neurons. Immunocytochemistry and immunofluorescent methods were used to detect the expression of anti-mGluR1α. Morphological observation of primary cortical neurons was performed by phase contrast microscope.ResultsFollowing the exposure to 0.1 mmol/L Glu for 1 h or OGD for 1 h, LDH leakage from neurons obviously increased (P< 0.01 ). 50 mmol/L LY367385, when co-incubated with Glu or OGD, markedly reduced the LDH leakage (P<0.01). The 24-h leakage of LDH was increased from cells exposed to 0.1 mmol/L Glu for 15 min. Pre-and post-treatment with LY367385 (50 mmol/L ) decreased the leakage of LDH. The cultured neurons expressed mGluR1α.ConclusionLY367385 has protective effect on neurons damaged by Glu or OGD. It may be related to antagonizing mGluR1α.

OBJECTIVE:To establish culture management concepts in hospital pharmacy professions and to standardize the behaviors among pharmacists,patients,colleagues and other health professionals.METHODS:Theories about the corpo?ration management development,definition of corporation culture,the3levels of corporation culture management and the operation of corporate culture construction were briefly introduced;the hospital pharmacy profession culture management principle was established with a reference of the corporate culture management concept at all times and in all over the world.RESULTS:Harmonious,friendly,kindly and placatory atmosphere can be established through the culture construction in material,system,behavior and spiritual levels;pharmacists should be urged to win the trust of patients so as to achieve the best therapeutic effects for the patients.CONCLUSION:The establishment of hospital pharmacy profession culture management principle can inspire pharmacists'passion and enthusiasm for work;thereby the development of pharmaceutical enterprises can be promoted.
4.Effect of Earle's solution and MEM on activities of neurons in culture

ObjectiveTo observe the activities of cultured neurons incubated with Earle's solution, new minimal essential medium (MEM) or former MEM.MethodsActivity changes of cultured neurons were measured by determining the leakage of lactate dehydrogenase (LDH) and metabolic rate of 3-(4,5-dimethylthiazol)2,5 diphenyltetrazolium bromide (MTT).ResultsActivities of neurons incubated with different culture medium for 12 h or 24 h were significantly different (P<0.01).ConclusionDifferent culture medium can influence neuronal activities.
5.Risk factors for infection following percutaneous nephrolithotomy

Objective To explore the risk factors for infection following percutaneous nephrolithotomy(PCNL), and provide the basis for prevention of postoperative infection.Methods 96 patients who were performed PCNL in a hospital between August 2010 and August 2013 were chosen,clinical data of patients were retrospectively analyzed, the occurrence of postoperative infection,risk factors and isolated pathogens were analyzed.Results 35 patients (36.46%[35/96])developed infection following PCNL.Univariate analysis revealed that risk factors for postopera-tive infection were the size and shape of stone,duration of operation,and intraoperative perfusion (all P <0.05). 96 patients before operation and 18 patients after operation were performed bacterial culture for urine,a total of 39 strains were isolated,the main isolated pathogens was Escherichia coli (n=12),followed by Pseudomonas aerugi-nosa (n=8),Streptococcus viridans (n =5 ),Klebsiella pneumoniae (n =4)and Enterobacter cloacae (n =3 ). Conclusion Infection rate following PCNL is high,duration of operation and intraoperative perfusion should be re-duced,antimicrobial agents should be used rationally.
6.Clinical characteristics and management of kidney transplantation in the older adults: monocentric retrospective analysis

Objective To explore the clinical characteristics and management of kidney transplantation in the older adults.Method Forty older kidney recipients (≥60 years old) and 777 younger (18~59 years old) recipients from June 2009 to December 2012 were retrospectively reviewed to evaluate the clinical characteristics and managements.Result Of 40 older recipients,the comorbidities of diabetes and coronary artery disease in older group were higher than in the younger (25% vs.4.9%,and 32.50% vs.11.38%).During the first 6 months,7 older patients died,among which 4 died from severe pneumonia,2 from heart failure and 1 from pulmonary embolism.There were 31 deaths in younger group,among which 21 died from severe pneumonia,4 from heart failure,5 from cerebrovascular accident and 2 from pulmonary embolism.The mortality in the older group was higher than in the younger group (17.5% vs.4.6%).Six-month and 3-year survival rate in the older recipients was lower than the younger recipients (81.56% vs.95.35%,and 81.56% vs.94.5%,respectively).Six-month graft survival rate and 3-year survival rate in the older group were also lower than in the younger group (78.75% vs.92.02%,and 68.82% vs.85.40%).At the 1st and 2nd year during follow-up,the serum creatinines in the older group were close to those in the younger group,while lower level was observed 3 years after transplantation in the older patients (89.38 ± 11.34 (mol/ L vs.116.57±48.68 (mol/L).Conclusion The patients with ESRD older than 60 years have more preoperative concomitant diseases and worse health status,which contribute to the complex clinical characteristics.More aggressive indication selection,sufficient pre-operative preparation,wellmatched histocompatibility and high quality of donor kidney are the key factors of successful transplantation.Optimized immunosuppressant therapy,delicate perioperative management,preventve and effective treatment of related complications are necessary to promote the survival of recipients and graft in long term.
7.The Use of Multiphasic Contrast-Enhanced CT Scan in Diagnosis of Focal Nodular Hyperplasia of Liver

Objective To study the value of multiphasic contrast-enhanced CT in diagnosing focal noudlar hyperplasia(FNH).Methods 12 patients with FNH underwent plain CT scan and multiphasic contrast-enhanced CT scan.Arterial phases was at 25~30 s,venous phases was at 55~60 s,and delayed scans were taken after 180 s.Results The lesions showed hypodensity and slight hypodensity on plain CT in 10 cases,among them,irregular center lower density was found in 5 cases and isodensity was found in 2 cases.In the arterial phases,the lesions appeared obvious in 9 cases,among which 4 cases were enhanced homogeneously and 5 were heterogeneously.No obvious enhancement in the center scar.The supply arteries were seen in 3 cases,speckled enhancement in peripheral area of lesions was seen in 2 cases,no obvious enhancement in 1 case.In the venous phases,the lesions showed slightly hyperdensity in 6 cases,isodensity in 4 cases,and mixture of hyperdensity and hypodensity in 2 cases.12 lesions were hypodense as compared with liver in the delayed phases.Conclusion Multiphasic contrast-enhanced CT scan is specific and accurate in diagnosing FNH and can be used to guide the clinical treatment.

9.Prediction value of IL-28B gene polymorphism on antiviral curative effect in patients with chronic hepatitis B

Objective To investigate the role of IL-28B gene on the antiviral curative effect in the patients with chronic hepatitis B.Methods 205 HBeAg-positive patients treated with the antiviral therapy of pegylation interferon(PEG-IFN)were detected the IL-28B rs12980275 and rs12979860 genotypes by TapManSNP genotyping assay and the relationship between the IL-28B genotypes and HbeAg seroconversion at end of treatment was analyzed.Results Among 205 cases,6 cases of genotype were uncertainty,the frequencies of IL-28B genotypes in other 199 cases were 77%,19%,5% for AA,AG,GG genotypes at rs12980275 and 76%,20%, 4%,for CC/CT/TT genotypes at rs12979860 respectively.The IL-28B polymorphisms were significantly correlated with the HbeAg seroconversion at the end of PEG-IFN treatment (P =0.001).In rs12979860 genotype,by the HBV genotyp,age,HBV lev-el,ALT and the combined therapy,the odds ratio (OR)of AA versus AG/GG was adjusted to 3.16 by HbeAg seroconversion.The similar result also obtained for the rs12980275 genotype.The IL-28B genotype was also associated with the HBsAg clearance.Con-clusion The IL-28B gene polymorphism is associated with the antiviral curative effect in the patients with HBeAg positive chronic hepatitis B.
