2.Cytomegalovirus nosocomial infection following organ transplantation: detection and monitoring

BACKGROUND:Cytomegalovirus is one of the opportunistic infection viruses for organ transplant patients, and it can lead to functional loss of organ grafts and even endanger lives. Therefore, accurate diagnosis of cytomegalovirus infection at early phase is the key to clinical treatment. OBJECTIVE:To analyze the detection method, accuracy, detection time and clinical significance of cytomegalovirus infection after organ transplantation. METHODS: A computer-based search of VIP, PubMed and CNKI as wel as manual search of books were performed for literatures concerning organ transplantation and cytomegalovirus infection published from January 2007 to December 2014. The keywords were transplantation, cytomegalovirus in English and Chinese, respectively. Total 1 894 papers in English and 31 articles in Chinese were found. Among them, only 35 papers which were highly representative and published in related authoritative journals were chosen for further analysis. RESULTS AND CONCLUSION:The main non-invasive detection of cytomegalovirus infection is stil the test of CMV-pp65 antigen with good sensitivity and specificity which can accurately reflect the status of cytomegalovirus infection. This method is also the most important clinical diagnostic method. Molecular biological detection of cytomegalovirus can detect potentialy infected people, and this method can detect the infection earlier than CMV-pp65 antigen method, which is suitable for the early diagnosis of cytomegalovirus infection and early drug treatment. Quantitative nucleic acid test can be used to analyze cytomegalovirus-DNA copies so as to determine the concentrations of the virus in patients, which may provide a more sensitive monitoring way for inapparently infected patients. Cytomegalovirus-IgG and cytomegalovirus-IgM in serum can be used to determine whether organ transplant recipients have been infected by cytomegalovirus. Virus culture and histological examination have been used for several years, both of which are the gold standard for detecting cytomegalovirus. However, these methods cannot be used in early diagnosis and active infection. Immunological detection method provides a new ideal for the diagnosis of cytomegalovirus infection. This method can monitor cytomegalovirus infection from early phase accurately and assess the risk of cytomegalovirus, providing a new way for the development of the diagnosis of cytomegalovirus infection.
3.Clinical observation on catgut implantation at acupoint combined sodium valproate in treatment of cerebrovascular epilepsy

Objective To observe the effect of catgut implantation at acupoint on treating cerebrovascular epilepsy and discuss the therapeutic mechanism. Methods 58 patients were divided into an observation group and a control group. The control group was treated with sodium valproate, and the observation group was treated with catgut implantation at acupoint on the basis of the control group.Epilepsy scoring, sodium valproate adverse reactions, the change of blood concentrations of sodium valproate and curative effect evaluation and so on were observed before and after the treatment. Results The basic control and excellent rate in the observation group were 16.7%and 73.3%, better than the control group of 10.7%and 53.6% respectively. The epilepsy scoring and sodium valproate adverse reactions after treatment in the observation group were superior to the control group [observation group is(5.05±1.5),(5.09±1.53),control group is(6.40±1.74),(10.85±1.91),P<0.05]. The number of patients with blood concentrations of sodium valproate in range 50-100 mg/L after treatment was increased in the observation group(Z=2.07, P=0.038). Conclusion Catgut implantation at acupoint combined sodium valproate was more effective than sodium valproate in treating cerebrovascular epilepsy, which can obviously alleviate the adverse reaction of drugs and increase blood concentrations of sodium valproate.
4.Simulation design and research on forming principle of anthropometric ECG

Objective To vividly display the forming principle of anthropometric ECG by using mannequin.Methods A mannequin was made using conductive paper with"heart" designed in left chest and its ECG was measured.Results The forming principles of ECG from mannequin and real people were identical.Mannequin had the same physical characteristics as real human body.Conclusion The change of ECG caused by pure physical factors can be studied by using the mannequin,which helps doctors properly analyze ECG and make accurate diagnosis.
5.Study on Hepatitis B Virus Infection Status in Plancetas of Hepatitis B Surface Antigen Positive Pregnant Women During Middle and Late Period of Pregnancy

Objective To investigate Hepatitis B Virus (HBV) infection status in placentas of Hepatitis B Surface Antigen (HBsAg) positive pregnant women during middle and late period of pregnancy. Methods One hundred and sixty seven placentas from HBsAg positive pregnant women were collected, including 158 term placentas and 9 aborted second trimester placentas. HBsAg and hepatitis B core antibody (HBcAg) in placental cytotypes were detected by immunohistochemistry. Results Of the 158 term placentas, HBV infection rates in decidual cells (DC), trophoblastic cells (TC), villous mesenchymal cells (VMC) and villous capillary endothelial cells (VCEC) were 66 46% (105/158), 58 23% (92/158), 27 22% (43/158) and 12 66% (20/158), respectively. The HBV infection rates gradually decreased from maternal side to fetal side of placentas. Both HBsAg and HBcAg were mainly located in the cytoplasm of infected cells. The positive cells were mostly in focal distribution. One out of 9 aborted second trimester placentas was infected. Conclusions HBV infection was found in different cells of term placentas. The positive rates for HBsAg and HBcAg in placentas were gradually decreased from maternal side to fetal side. The positive rate was low in aborted second trimester placentas.

7.Immunogenicity, immune mechanisms and immune modulatory effects of mesenchymal stem cells derived from bone marrow of patients with multiple myeloma

Objective To study the immunogenicity,immune modulatory effects and mechanisms of mesenthymal stem ceils (MSCs) derived from the bone marrow of patients with multiple myeloma (MM).Methods The mononuclear cells from the bone marrow of MM patients were obtained and cultured.Immunophenotypes were investigated by using FACS.The levels of cytokines were determined by using enzyme linked immunosorbant assay (ELISA).The endocytosis of monocyte-derived dendritic cells (DCs) was investigated by using FACS.Moreover,the immunoregulatory ability of DCs on proliferation of T lymphocytes was detected by using mixed lymphocyte culture assay.Results MM-derived MSCs had no expression of HLA-DR and costirnulatory molecules (CD40,CD80,CD83 and CD86).MM-derived MSCs could significantly suppress proliferation of T lymphocytes in a dose-dependent manner,which could be reversed by antiTGF-β1 and anti-HGF antibodies.MM-derived MSCs inhibit the endocytosis of DCs.MM-derived MSCs could inhibit the secretion of IL-12 and significantly inhibit the function of DCs on proliferation of T lymphocytes.Conclusion MM-derived MSCs harbored low immunogenicity and immunoregulatory effect in vitro,and this effect was achieved through cytokines.
8.Cardiac protection effect of fructose-1, 6-diphosphate in patients with primary bone cancer treating with chemotherapy

Objective To investigate the cardiac protection effect of venous fructose-1,6-diphosphate in pa-tients with primary bone malignant tumors. Methods 178 patients received 9 times of chemotherapy were randomly divided into non-cardiac protective therapy group (A group)and cardiac protective therapy group(B group). The car-diac toxicity was reflected by abnormal electrocardiographic changes. The abnormal electrocardiographic changes were compared in two groups. Results Abnormal electrocardiographic changes were significantly decreased in B group eompared with A group (30 vs 81 ,P <.05). Conclusion The cardiac protective medicine fructose-1,6-diphosphate efficiently decreased the cardiac toxicity in patients with primary bone malignant tumors treating with chemotherapy.
9.Clinical Study on Acupuncture plus Medication and Kinesiotherapy for Cerebral Stroke

Objective To observe the clinical efficacy of acupuncture plus medication and kinesiotherapy in treating cerebral stroke.Method A total of 126 patients with cerebral stroke were randomized into a treatment group of 42 cases, control group 1 of 40 cases and control group 2 of 44 cases. The treatment group was intervened by acupuncture, Chinese medication plus kinesiotherapy, control group 1 by acupuncture alone and control group 2 by kinesiotherapy alone. Before the intervention and respectively after 2-week and 4-week treatment, the three groups were observed by adopting the modified Barthel Index (MBI) and Fugl-Meyer Assessment (FMA), as well as the Activities of Daily Living (ADL).Result After the treatment, the MBI score, FMA score and ADL grading were significantly changed in the three groups (P<0.01). After 4-week treatment, the MBI score in the treatment group was significantly different from that in control group 2 (P<0.05). After 4-week treatment, the FMA score and ADL grading in the treatment group were significantly different from those in both control group 1 and 2 (P<0.05).Conclusion Acupuncture plus medication and kinesiotherapy is an effective approach in treating cerebral stroke and it can improve the ADL.
10.A identification method for clinical burkholderia cepacia isolates

Objective To approach a simple identification method for clinical Burkholderia cepacia isolates.Methods Thirty eight clinical isolates and 5 referenc strains were identified by phenotypic and genotypic methods. A simple method presented here included TSI agar, oxidase test, pigmentation test, catalase test and antibiograms.Results All but one B. cepacia isolate identified by phenotypic and genotypic methods were identified correctly by our method. One non B. Cepacia isolate identified by the genotypic method was identified as Burkholderia spp. by phenotyic and our methods.Conculsion The method we presented here was simple, practical for identification of clinical B. cepacia isolates.
