Objective To summarize the experience of early management in elderly patients with serious burning in order to raise cure rate. Methods Elderly patients (63 cases) with serious burning (burned area exceeding 30% or Ⅲ? area exceeding 10% ) admitted from 1990 to 2002 were retrospectively studied. Patients were divided into two groups according to the admission date, before and after January 1, 1996 since new management was extensively applied after 1996. These measures consisted of rapid and adequate fluid resuscitation, early enteral feeding and autograft after eschar excision. 2.Effect of Radiofrequency Thermocoagulation Guided by Spiral CT on Severe Trigeminal Neuralgia in the Oldest Old Patients: 21 Cases Report

Objective To investigate the efficacy and safety of radiofrequency thermocoagulation guided by spiral CT on severe trigeminal neuralgia in the oldest old patients. Methods 21 patients above 80 with severe idiopathic trigeminal neuralgia were treated with radiofrequency thermocoagulation guided by spiral CT. They were assessed with numeric rating scales (NRS) and the complication were observed. Results The scores of NRS decreased significantly after treatment (P<0.01). No serious complication was observed after treatment. One patient recurred 23 months after operation, and the pain released after radiofrequency thermocoagulation once again. Conclusion Radiofrequency thermocoagulation guided by spiral CT is effective and safe for severe trigeminal neuralgia in the oldest old patients.
3.Three-dimensional EH composite artificial template for repairing cranial defect in 53 cases

OBJECTIVE: To observe effects of three-dimensional EH composite template on repairing cranial bone defect. METHODS: A total of 53 inpatients were analyzed retrospectively at the Department of Neurosurgery, Taixing People's Hospital from July 2004 to May 2007, comprising 31 males and 22 females, aged 19-68 years. They were in accordance with cranial repairing indication. Reasons for skull defect: traumatic brain injury (41 cases), hypertension (7 cases), intracranial cancer (3 cases), intracranial aneurysm (2 cases). Thirteen patients developed bilateral cranial defect. The defect region: frontal (5 slices, comprising 2 with severe orbital part defect), temporal (9 slices), frontotemporal (42 slices), temporoparietal (3 slices), parietoocipital (4 slices),frontoparietal (3 slices). The smallest single-chip defect size was 4 cm × 6 cm, and the largest one was 12 cm×18 cm. All the patients were mended by three-dimensional EH composite template using spiral CT (≤5 mm thickness) scanning, CAD three dimensional reconstruction, quickly modeling technique. Infection of incisional wound, rejection and complications were observed. RESULTS: A total of 53 patients were followed up, with an average follow-up period of 18 months, and all of them were satisfied with the shapes (100%). Mild collection of the fluid under the scalp was found in 2 cases 4 days and 1 week following surgery, and disappeared after suction and pressure dressing. No other frequent complications were observed after cranioplasty. CONCLUSION: Three-dimensional EH composite template is an ideal material for cranial bone defect, because of its good biocompatibility, easy operation, good postoperative shape and less complications.
4.Clinical and experimental pathology of Moyamoya disease.

OBJECTIVETo investigate the etiology, pathology, and mechanism of pathogenesis of Moyamoya disease.
METHODSA total of 15 human autopsies were analyzed. In addition, in order to create an animal model of the disease, 21 Japanese rabbits were divided randomly into two groups and subjected to injections of horse serum either intravenously or locally in the area of the sympathetic ganglia. Pathological and immunohistochemical characteristics were observed.
RESULTSThe pathological features of the autopsies and the animal models both involved intima hyperplasia and stenosis or even occlusion of the lumen in the terminal ends of the internal carotid artery and the anterior and middle cerebral arteries. Disconnections or even breakages of the inner layer of the lumen were also observed, without an obvious inflammatory response. Hyperplasic smooth muscle cells of the medial membrane had extended inward through broken portions of the internal elastic lamina, with intima cell hyperplasia resulting in lumen stenosis. The hyperplastic vascular walls were positive for IgG and IgM.
CONCLUSIONSThe etiology of Moyamoya disease may involve allergic angiitis. A possible mechanism is that proximal portions of the circle of Willis first develop chronic stenosis or occlusion, leading to compensatory small vessel proliferation, which perforates into the cerebral parenchyma.

Objective :
To investigate whether miR⁃19a interacts with insulin⁃like growth factor 2 receptors ( IGF⁃2R) in infantile hemangiomas (IHs) and affects the proliferation , migration , and adipogenesis of hemangioma stem
cells (HemSCs) .
Methods :
HemSCs were isolated , screened and cultured from IH specimens. IGF⁃2R expression in HemSCs was identified using immunohistochemistry. HemSCs transfected with miR⁃19a mimics and inhibitors were subjected to CCK⁃8 , wound healing , Transwell , qRT⁃PCR , and Western blot analyses.
Results :
Compared with the control , the proliferation and migration rate of HemSCs treated with miR⁃19a inhibitors were significantly increased , and overexpression of miR⁃19a significantly inhibited IGF⁃2 induced cell migration and proliferation (P < 0. 05) .
Conclusion
MiR⁃19a may inhibit HemSCs proliferation , migration , and adipogenesis by targeting IGF⁃2R.
8. Regulation of hypoxia inducible factor-1α on permeability of vascular endothelial cells and the mechanism

Objective:
To investigate the regulation of hypoxia-inducible factor-1α (HIF-1α) on permeability of rat vascular endothelial cells and the mechanism.
Methods:
Twelve male Sprague-Dawley rats aged 35 to 38 days were collected and vascular endothelial cells were separated and cultured. The morphology of cells was observed after 4 days of culture, and the following experiments were performed on the 2nd or 3rd passage of cells. (1) Rat vascular endothelial cells were collected and divided into blank control group, negative control group, HIF-1α interference sequence 1 group, HIF-1α interference sequence 2 group, and HIF-1α interference sequence 3 group according to the random number table (the same grouping method below), with 3 wells in each group. Cells in negative control group, HIF-1α interference sequence 1 group, HIF-1α interference sequence 2 group, and HIF-1α interference sequence 3 group were transfected with GV248 empty plasmid, recombinant plasmid respectively containing HIF-1α interference sequence 1, interference sequence 2, and interference sequence 3 with liposome 2000. Cells in blank control group were only transfected with liposome 2000. After transfection of 24 h, expression levels of HIF-1α mRNA and protein of cells in each group were respectively detected by reverse transcription real-time fluorescent quantitative polymerase chain reaction and Western blotting (the same detecting methods below) . The sequence with the highest interference efficiency was selected. (2) Another batch of rat vascular endothelial cells were collected and divided into blank control group, negative control group, and HIF-1α low expression group, with 3 wells in each group. Cells in blank control group were only transfected with liposome 2000, and cells in negative control group and HIF-1α low expression group were respectively transfected with GV248 empty plasmid and low expression HIF-1α recombinant plasmid selected in experiment (1) with liposome 2000. After 14 days of culture, the mRNA and protein expressions of HIF-1α in each group were detected. (3) Another batch of rat vascular endothelial cells were collected and divided into blank control group, negative control group, and HIF-1α high expression group, with 3 wells in each group. Cells in blank control group were transfected with liposome 2000, and cells in negative control group and HIF-1α high expression group were respectively transfected with GV230 empty plasmid and HIF-1α high expression recombinant plasmid with liposome 2000. After 14 days of culture, the mRNA and protein expressions of HIF-1α of cells in each group were detected. (4) After transfection of 24 h, cells of three groups in experiment (1) and three groups in experiment (2) were collected, and mRNA and protein expressions of myosin light chain kinase (MLCK), phosphorylated myosin light chain (p-MLC), and zonula occludens 1 (ZO-1) of cells were detected. Autophagy is essential for the maintenance of cellular homeostasis and its dysfunction has been linked to various diseases. Autophagy is a membrane driven process and tightly regulated by membrane-associated proteins. Here, we summarized membrane lipid composition, and membrane-associated proteins relevant to autophagy from a spatiotemporal perspective. In particular, we focused on three important membrane remodeling processes in autophagy, lipid transfer for phagophore elongation, membrane scission for phagophore closure, and autophagosome-lysosome membrane fusion. We discussed the significance of the discoveries in this field and possible avenues to follow for future studies. Finally, we summarized the membrane-associated biochemical techniques and assays used to study membrane properties, with a discussion of their applications in autophagy.
