1.Serotype and drug resistance of 815 Salmonella isolates in Hunan Province

Objective:
To investigate the serotype and drug resistance of 815 Salmonella isolates from Hunan Province, so as to provide insights into management of Salmonella infections.
Methods:
Salmonella isolates were collected from stool samples of foodborne diarrheal patients and food samples in Hunan Province from 2020 to 2021, and serotyped. Antimicrobial susceptibility test was performed using the broth microdilution method.
Results:
A total of 10 groups and 39 serotypes were characterized in 815 Salmonella isolates. Among the 646 Salmonella isolates of human sources, 388 isolates were identified as serogroup B (60.06%), with S. typhimurium and its variants aspredominant serotypes (364 isolates, 56.35%), and among 169 foodborne isolates, 61 isolates were characterized as serogroup B (36.09%) with S. london as the predominant serotype (26 isolates, 15.38%). There were 597 antimicrobial resistant Salmonella isolates of human sources, with a drug resistance rate of 92.41%, and the percentage of ampicillin resistance was 81.58%. There were 140 foodborne antimicrobial resistant isolates, with a drug resistance rate of 82.84%, and the proportion of tetracycline resistance was 72.78%. However, Salmonella isolates from both humans and foods were sensitive to imipenem. In addition, there were 577 multidrug resistant Salmonella isolates, including 490 multidrug resistant isolates of human sources and 87 foodborne multidrug resistant isolates.
Conclusions
S. typhimurium and its variants and S. london were predominant serotypes of Salmonella isolates from 815 foodborne diarrheal patients and food samples in Hunan Province from 2020 to 2021, and a high rate of multidrug resistance was detected.

Objective To analyze the correlationship of ACE I/D gene polymerphisms with the anti-hypertensive efficacy of irbesartan among essential hypertension (EH) patients in Yunnan han people.Methods One hundred EH patients hypertension were treated with irbesartan 150mg once daily for 4 weeks,and anti-hypertensive efficacy were monitored during the treatment.Results There was a significant difference between anti-hypertensive efficacy of irbesartan among different genotype groups,DD group >ID group >Ⅱ group (P < 0.05) Conclusions There is different therapy responsiveness in diffenent genotypes.ACE I/D polymorphism may be an important hereditary factor that impacts the efficacy of irbesartan.
3.Vascularized greater trochanter or iliac periosteal flap displacement repairs avascular necrosis of femoral head

BACKGROUND:Femoral head avascular necrosis is common in children and elderly. Though, there are many methods can be used for the treatment, it has been inconclusive in the treatment according to the age and stage of the patients. OBJECTIVE:To treat the femoral head avascular necrosis with different methods according to the age and stage, and to retrospectively analyze the fol ow-up results. METHODS:Total y 202 patients (242 hips) with femoral head avascular necrosis were included from October 1998 to October 2008. The patients were divided into groups according to age, included child group (4-14 years old, n=45, 47 hips);youth group (15-45 years old, n=100, 125 hips);and elderly group (46-81 years old, n=57, 70 hips). The patients in the child group were treated with vascularized greater trochanter or iliac periosteal flap displacement, the patients in the youth group were treated with vascularized greater trochanter or iliac periosteal flap displacement, and the patients in the elderly group were treated with hybrid or cementless total hip arthroplasty. RESULTS AND CONCLUSION:Al the 202 patients were fol owed-up for 6 months to 10 years, average 6.3 years. The excellent and good rate of the child group, youth group and elderly group were 87%, 89%and 86%respectively, and al the patients obtained the satisfactory clinical effect;the Harris score of the youth group was increased to (88.1±0.9) points, and the visual analog scale score was decreased to (0.9±0.4) points;the Harris score of the elderly group was increased to (91.5±1.0) points, and the visual analog scale score was decreased to (0.60±0.07) points. The results indicate that vascularized greater trochanter or iliac periosteal flap displacement is suitable for the children and the youths with femoral head avascular necrosis, especial y the patients with the age of 15-45 years in Ficat Ⅱ and Ⅲstage;hybrid or cementless total hip arthroplasty is suitable for the elder patients with femoral head avascular necrosis, as wel as the patients with failure femoral head retention treatment.
4.Effect of Gastrodin on Early Brain Injury and Neurological Outcome After Subarachnoid Hemorrhage in Rats.

Gastrodin is a phenolic glycoside that has been demonstrated to provide neuroprotection in preclinical models of central nervous system disease, but its effect in subarachnoid hemorrhage (SAH) remains unclear. In this study, we showed that intraperitoneal administration of gastrodin (100 mg/kg per day) significantly attenuated the SAH-induced neurological deficit, brain edema, and increased blood-brain barrier permeability in rats. Meanwhile, gastrodin treatment significantly reduced the SAH-induced elevation of glutamate concentration in the cerebrospinal fluid and the intracellular Ca overload. Moreover, gastrodin suppressed the SAH-induced microglial activation, astrocyte activation, and neuronal apoptosis. Mechanistically, gastrodin significantly reduced the oxidative stress and inflammatory response, up-regulated the expression of nuclear factor erythroid 2-related factor 2, heme oxygenase-1, phospho-Akt and B-cell lymphoma 2, and down-regulated the expression of BCL2-associated X protein and cleaved caspase-3. Our results suggested that the administration of gastrodin provides neuroprotection against early brain injury after experimental SAH.

Objective To study mental status of recovered two year after in shake area crowd of gansu wen-xian. Methods Fifty former disaster crowd were assessed with SCL-90, Coping Style Questionnaire and Social Support Questionnaire two year after their discharge, 50 healthy medical staffs of gan-su CTM Affiliated Hospital of logistics ensure center were assessed with same instruments as control. Results The former disaster crowd and 5 subscores of SCL-90 with higher average scores than Chinese norm and results of control, they were somatization( F =4. 31) , phobia ( F = 5. 25) , obtrude ( F =5. 91) , P<0. 01, depression ( F = 3. 11) anxiety ( F = 3. 74) , P < 0. 05; The former disaster crowd felt more objective support, used more rationalization, but were less capable in problem solving. In former disaster crowd, problem-solving, objective social support and utility of the support were negatively correlated with subscores of SCL-90 ( r = -0. 31~-0. 40, P <0. 05) ; while immature coping, such as fantasy, withdrawal and rationalization were positively correlated with subscores of SCL-90 ( r = 0. 40 ~ 0. 60, P < 0. 05).Conclusion Former disaster crowd still have psychosomatic symptoms and need social and psychology intervene and support.
6.Progress on Immunotherapy of Gastrointestinal Cancer

Gastrointestinal cancers are the common malignant tumors of the digestive system, and their morbidity and mortality are in the forefront of malignant tumors. Currently, cancer immunotherapy is the hottest topic in cancer research field. Although cancer immunotherapy has achieved some results in the fundamental research and clinical application of gastrointestinal tumors, there are still a series of problems that need to be resolved. In this article, we review the fundamental and clinical research progress of several common methods of cancer immunotherapy in the field of gastrointestinal tumors.
7.Therapeutic effect of creatine phosphate in early diastolic dysfunction

Objective: To investigate the therapeutic effect of creatine phosphate sodium (CP, Neoton) at early stage of myocardium diastolic dysfunction. Methods: A total of 138 cases with myocardium diastolic dysfunction were divided into CP group (n=109) and routine treatment group (n=29). All the patients received routine treatment. The CP group received CP treatment based on routine treatment(CP 2g+5%glucose or 0.9% sodium chloride 100ml, iv. drip, for patients with mild diastolic dysfunction was 1time/d, patients with moderate diastolic dysfunction was 2 times/d), duration was 7~14 days. Color-coded Doppler echocardiography was used to detect transmitral peak early diastolic velocity (E-wave),transmitral peak late diastolic velocity (A-wave),peak early diastolic velocity of mitral annulus (Ea), and peak late diastolic velocity of mitral annulus(Aa), and the value of E/A ,Ea/Aa and E/Ea were calculated. Symptoms, signs and items from echocardiography were compared between the two groups before and after treatment. Results: After treatment, all symptoms and signs of patients in two groups significantly improved (P<0.05~0.01), and the marked effective rate in CP group was significantly higher than that of routine treatment group (96.2% vs. 57.1%, P<0.01). In CP group, the marked effective rate of E/A (81.65% vs. 10.34%), Ea/Aa (83.49% vs. 13.79%) and E/Ea (92.86% vs. 12.00%) were significantly higher than those in routine treatment group (P<0.01all). Conclusion: Creatine phosphate possesses marked effect for improve patient’s condition and diastolic dysfunction.
8.MiR-212 Attenuates MPP+-Induced Neuronal Damage by Targeting KLF4 in SH-SY5Y Cells.

PURPOSE: Parkinson's disease (PD) is a common age-dependent neurodegenerative disease. MiR-212 has been demonstrated to exert protective effects in several neurological disorders. The present study aimed to investigate the role and underlying molecular mechanism of miR-212 in PD. MATERIALS AND METHODS: 1-methyl-4-phenylpyridinium (MPP+)-induced SH-SY5Y cells were applied as a PD model in vitro. RTqPCR was used to measure the expression of miR-212 and Kruppel-like factor 4 (KLF4) mRNA. Western blot analysis was performed to detect the protein levels of KLF4, Notch1 and Jagged1. Cell viability and apoptosis were determined by the Cell Counting Kit-8 and flow cytometry, respectively. Quantitative analysis of caspase-3 activity, lactate dehydrogenase (LDH), reactive oxygen species (ROS), superoxide dismutase (SOD), tumor necrosis factor-α (TNF-α), and interleukin-1 beta (IL-1β) was conducted with corresponding ELISA kits. Dual-luciferase reporter assay was employed to evaluate the relationship between miR-212 and KLF4. RESULTS: MiR-212 was downregulated in MPP+-induced SH-SY5Y cells. Also, miR-212 alleviated MPP+-induced SH-SY5Y cell damage, embodied by increased cell viability, decreased caspase-3 activity, LDH release, ROS production, TNF-α, and IL-1β expression, as well as elevated SOD levels. KLF4 was a direct target of miR-212, and miR-212 repressed KLF4 expression in a post-transcriptional manner. Moreover, miR-212-mediated protection effects were abated following KLF4 expression restoration in MPP+-induced SH-SY5Y cells, represented as lowered cell viability and enhanced apoptotic rate. Furthermore, Notch signaling was involved in the regulation of miR-212/KLF4 axis in MPP+-induced SH-SY5Y cells. CONCLUSION: miR-212 might attenuate MPP+-induced neuronal damage by regulating KLF4/Notch signaling pathway in SH-SY5Y cells, a promising target for PD therapy.

9.Clinical analysis of human bocavirus infection in children with severe lower respiratory tract infection

Objective To explore the clinical characteristics of human bocavirus (HBoV) infection in the children with severe lower respiratory tract infection.Methods The clinical data of the hospitalized children with sputum HBoV-positive were analyzed retrospectively,who were hospitalized at the Pediatric Intensive Care Unit of Children's Hospital Affiliated to Capital Institute of Pediatrics from September 1,2016 to March 31,2017.Results A total of 17 children were included in the study.The ratio of male to female was 15 ∶ 2.The diagnostic age ranged from 4 months to 4 years and 10 months old.82.4% (14/17 cases) of the patients were infants less than 2 years old.Autumn and winter were high-occurrence seasons.Pediatric Critical Illness Scores(PCIS) were 68-88 scores(median 82 scores).PCIS was less than 70 scores in 1 case,and between 70 scores and 80 scores in 7 cases,and more than 80 scores in 9 cases.The main clinical manifestations were respiratory system involvement,including dyspnea in 17 cases (100%),fever in 14 cases (82.4%),cough in 16 cases (94.1%),wheezing in 13 cases (76.5%),and moist rales in 13 cases (76.5%).The main abnormal chest radiological findings showed patchy shadows in 7 cases (41.2%) and consolidation in 6 cases (35.3%).The oxygenation index was 73.9-296.0 mmHg(1 mmHg =0.133 kPa),and median was 176 mmHg.The oxygenation index was between 100-200 mmHg in 7 cases(41.2%),and less than 100 mmHg in 2 cases(11.7%).Type Ⅰ respiratory failure occurred in 11 cases (64.7%),while type Ⅱ respiratory failure occurred in 6 cases (35.3 %).All of the patients need respiratory support with mechanical ventilation.Among them,6 patients (35.3%) were treated by non-invasive ventilation and their ventilation time were 25-128 h(median 65 h),while 11 patients (64.7%) were treated by invasive ventilation and their ventilation time was 42-178 h(median 70 h).Other organ or system dysfunction is mild.The length of hospital stay ranged from 3 days to 13 days.The cure rate was 100%.Conclusions HBoV infection in the children with severe lower respiratory tract infection is common in infants under 2 years old,with acute onset and rapid recovery.The obvious clinical symptoms are fever,cough,wheezing and dyspnea.Although respiratory failure is common,other organ or system dysfunction is mild,which may bring about a good prognosis.
10.A new variant of respiratory syncytial virus glycoprotein gene and clinical features of infected children

Objective:To analyze the epidemiological features of respiratory syncytial virus (RSV) infection in Beijing, and monitor the sequence variations in RSV glycoprotein (G) gene and clinical features of infected children.Methods:Respiratory tract specimens were collected from children with acute respiratory infection in the Children′s Hospital Affiliated to Capital Institute of Pediatrics from January 1, 2023 to December 31, 2023. RSV-positive specimens screened by multiple nucleic acid testing were subjected to PCR to amplify the full-length RSV G gene. A phylogenetic tree was constructed after gene sequencing to analyze RSV subtypes and trace G gene variants. Clinical data were retrieved from the medical record system to analyze the clinical features of children with RSV infection in Beijing.Results:A total of 5 489 respiratory specimens were collected from 3 046 male patients and 2 443 female patients. The average age of the patients was 4.36 years. A total of 589 RSV-positive specimens (10.7%, 589/5 489) were detected with 349 from male patients and 240 from female patients. The average age of children with RSV infection was (2.51±2.78) years and the median age was 0.48 years. RSV had been circulating among children in Beijing since March 2023 with two epidemic peaks in May (24.6%, 122/496) and December (18.2%, 126/693). The predominant subtype of RSV in the first half of 2023 was subtype A, but it was replaced by subtype B from November 2023. Phylogenetic analysis revealed a novel G gene of RSV subtype B (RSV-B-BA9-954bp) with a length of 954 bp, which belonged to a new cluster in the phylogenetic tree. The percentage of patients admitted to the Intensive Care Unit (ICU) was higher in children with new variant of RSV subtype B infection than in those with common RSV subtype B infection [44.1% (15/34) vs 25.2% (31/123), χ 2=4.600, P=0.032], while the counts of white blood cells and the levels of C-reactive protein were lower in the children with new variant infection ( P<0.05). Conclusions:RSV has been prevalent among children in Beijing since March 2023 with two epidemic peaks. The predominant A subtype is gradually replaced by to B subtype. A new variant of RSV B G gene (RSV-B-BA9-954bp) is detected among the children.
