1.Nursing care of patient with wound infection following systemic lupus erythematosus accompanied by herpes zoster

This paper summarizes nursing care of a patient with wound infection following systemic lupus erythematosus (SLE) accompanied by herpes zoster. Aimed at the patient's characteristics of long-term disease course of SLE,weak immune system caused by long-term use of glucocorticoid and immunosuppressant,big wound area with severe infection,and hard healing,moist therapy was implemented under the theoretical guidance of moist healing environment. The Purilon Gel and Biatain-Ag Foam dressing were applied in the wound combined with systemic use of antibiotics and treatment of SLE. At the same time,psychological nursing and health education were provided for the patient. These measures speeded up the wound healing.
2.The trial of penehyclidine hydrochloride combined with atropine in treatment of patients with severe acute organophosphorus poisoning

Objective To observe the clinical effect of penehyclidine hydrochloride(Changtuoning)combined with atropine in treatment of patients with severe acute organophosphorus poisoning(AOPP). Methods The clinical data of 64 patients with severe AOPP admitted into Taizhou Municipal Hospital in Zhejiang Province and Quannan County People's Hospital in Jiangxi Province from January 2004 to September 2012 were retrospectively analyzed. Patients were divided into penehyclidine hydrochloride and atropine combined group(PH-A group,32 cases)and atropine treatment group(32 cases)depending on the difference in selection and application method of anticholinergic drugs. The application of anticholinergic drugs,length of mechanical ventilation,70%cholinesterase (ChE)activity recovery time,length of stay in hospital,incidence of complications,and cure rate were compared between the two groups. Results Compared with atropine treament group,the total usage of atropine in PH-A group was significantly decreased(mg:35.39±45.76 vs. 105.46±139.87,P<0.05),the length of mechanical ventilation (days:5.3±4.2 vs. 7.8±6.8,P<0.05),70%ChE activity recovery time(days:8.2±3.8 vs. 11.0±5.4,P<0.05), and length of stay in hospital(days:12.0±3.8 vs. 14.6±5.3,P<0.05)were significantly shortened in PH-A group, and the overall incidence of complications were significantly lowered in PH-A group〔68.75 %(22/32)vs. 93.75%(30/32),P<0.05〕. The difference in cure rate between the combined group and atropine treatment group was not statistically significant〔96.87%(31/32)vs. 90.62%(29/32),P>0.05〕. Conclusion Penehyclidine hydroehloride combined with atropine in the treatment of severe AOPP can significantly improve the therapeutic efficacy,reduce the incidence of complications and length of stay in hospital.
3.Comparison of clinical efficacy of penehyclidine hydrochloride and atropine in the treatment of acute organophosphorus pesticide poisoning

Objective To compare the clinical effect of penehyclidine hydrochloride and atropine in the treatment of acute organophosphorus pesticide poisoning.Methods 86 patients with organophosphorus pesticide poisoning were selected and randomly assigned to the observation group and the control group according to the digital table,43 cases in each group.The two groups were given conventional treatment,while the control group was treated with atropine,the observation group was given penehyclidine hydrochloride.The disappearance time of main symptoms, rescue success rate and adverse reactions of the two groups were observed.Results After salvage treatment, the rescue success rate of the observation group was 97.8%,which of the control group was 88.4%,the difference was statistically significant (x2=1.433,P<0.05).The disappear time of M like symptoms,N like symptoms and central nervous system symptoms in the observation group was significantly shorter compared with the control group (all P<0.05).The number of drugs,dosage,cholinesterase recovery time and hospitalization time between the two groups had statistically significant differences (P<0.05).The incidence rates of blurred vision,restlessness,heart rate and urinary retention in the control group were significantly higher than those in the observation group (all P<0.05).Conclusion Penehyclidine hydrochloride for acute organophosphorus pesticide poisoning can significantly reduce the incidence of symptoms,shorten the disappearance time of symptoms,reduce hospitalization time,improve the efficacy of rescue,reduce the incidence of adverse reactions, it is safe,effective and has great clinical significance.
4.Training of ultrasound-guided modified Seldinger technique for PICC cannulation

Objective To investigate training of the ultrasound-guided modified Seldinger technique(MST)for cannulation of PICC.Method Twenty-nine nurses trained with the traditional training on ultrasound-guided MST during December 2009 to December 2011 were set as control group and another 29 nurses trained with the systematic training during December 2011 to December 2013 were as experiment group.The training effect were compared between the two groups after 2 months training.Results The theoretical knowledge in the experiment group was improved significantly compared with the control group. Moreover, the success rate of catheterization in the experiment group was 100.0%,significantly higher than that in the control group(both P<0.01).Conclusion Systematic training system is more effective than the traditional training.It can improve the nurses’technical and theoretical knowledge on ultrasound-guided MST for PICC.
5.Effect of famitinib malate on adverse reactions by radiotherapy and chemotherapy in treatment of advanced nasopharyngeal carcinoma

Objective To investigate effect of famitinib malate on adverse reactions by radiotherapy and chemotherapy in treatment of advanced nasopharyngeal carcinoma and the nursing strategies.Methods From November 2011 to December 2013, 20 cases of advanced nasopharyngeal carcinoma in our hospital were treated with famitinib malate combined with radiotherapy and chemotherapy.During the treatment,we observed the adverse reactions and gave the symptomatic treatment.Results The rates of adverse reactions such as hypertension,renal toxicity,oral mucositis,myelosuppression,gastrointestinal reactions,nasal bleeding, abnormal liver function,and hand-foot skin reaction were 60.0%,70.0%,90.0%,100.0%,85.0%,10.0%,45.0%and 25.0%respectively.After symptomatic treatment,all completed the treatment.Conclusions The incidence rate of adverse reactions of famitinib malate combined with radiotherapy and chemotherapy for treating advanced nasopharyngeal carcinoma is high.Close monitoring and observation during treatment courses can ensure the successful completion of treatment.
6.Nursing of patients with Grave's opththalmopathy undergoing endoscopic trans - ethmoid orbital decompression

Objective To introduce an effective nursing practice for patients with Grave's ophthalmopathy(GO) undergoing endoscopic trans-ethmoid orbital decompression(ETOD). Methods The treatment and nursing experience of 31 cases (57 eyes) undergoing orbital decompression through endoscopic trans-ethmoid approach were reviewed and analyzed. Results Three months after operation, exophthalmos was corrected in all 31 cases (57 eyes) with satisfying result, and the visual acuity was improved in all 9 cases (16 eyes),who complained of visual loss preoperatively. At the 3-months' review, 2 patients presented diplopia on the primary eye position due to newly on-set global displacement. Conclusions Advanced treatment and careful nursing are very important to improve the surgical safety and decrease the complications.
