1.Clinical evaluation of xinqin granule combined with loratadine in treatment of allergic rhinitis and its effects on serum EOS, CSF and TNF-α

Objective To study clinical evaluation of xinqin granule combined with loratadine in treatment of allergic rhinitis and its effects on serum eosinophil(EOS),colony stimulating factor(CSF)and tumor necrosis factor(TNF)-α.Methods 90 patients of allergic rhinitis who received therapy from January 2014 to November 2016 in our hospital were selected.According to random number table,those patients were divided into the observation group(n=45)and the control group(n=45).The control group was treated with loratadine,while the observation group was combined with xinqin granule.Then the serum EOS,CSF and TNF-α,symptom score,clinical efficacy and adverse reactions were compared.Results After treatment,the serum levels of EOS,CSF and TNF-αin the observation group were significantly lower than that of the control group(P<0.05); the symptom score in the observation group was significantly lower than that of the control group(P<0.05); the total effective rate in the observation group was significantly higher than that of the control group[93.33%(42/45)vs.73.33%(33/45)](P<0.05); there was no significant difference in the incidence of adverse reactions between the two groups.Conclusion Xinqin granule combined with loratadine is well for allergic rhinitis,which can effectively reduce serum levels of EOS,CSF,TNF-α,relieve clinical symptoms,it's worthy of application and promotion.

Background and purpose:CpG methylation in promoter region is an essential mechanism for the dysfunction of the tumor suppression gene.Folate metabolism provides active methyl for organism methylation.Methionine synthase(MS) plays a vital role in the process of folate metabolism.This study aimed to explore the genetic polymorphism of MS, CHD5 methylation and their association with breast cancer morbidity.Methods:Fortyseven patients with primary breast cancer, 52 healthy subjects and 15 breast hyperplasia patients were enrolled in this experiment.The mRNA expression of CHD5 was determined by reverse transcription PCR(RT-PCR).Methylationspecific PCR(MSP) was used to detect the methylation status of CHD5.Polymorphisms in the MS gene were analyzed through the polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP).Results:The mRNA expression of CHD5 in breast cancer tissues(0.27?0.19) and adjacent non-cancerous tissues(0.33?0.17) were both significantly lower than that in the breast hyperplasia tissues(0.67?0.14)(P
3.Determination of anthraquinones in Niuhuang Qinghuo Pills by HPLC

Objective To establish the method of determination for rhein, emodin, chrysophanol and physcion in Niuhuang Qinghuo Pills. Methods Sample was hydrolyzed by 20% H2 SO4 , and anthraquinones were extracted by soxhlet extraction with acetone as solvent, and were determined by HPLC with Kromasil C18 as column, methanol-0.5%H3 PO4 (75:25,V/V) as mobile phase at the flow of 1.0 mL/min, 230 nm as the detection wavelength.Results The linear relationship of rhein, emodin, chrysophanol and physcion were 1.50-150, 0.950-95.0, 1.30-130 and 1.15-115 mg/L. The anthraquinones were seperated completely, the recovery of 4 anthraquinones were 98.14% -101.3%.Conclusion This method is simple, accurate, steady, and could be used for the quality control of Niuhuang Qinghuo Pills.
4.Practice of Medication Reconciliation among Chronic Renal Insufficiency Patients by Clinical Pharmacists

OBJCETIVE:To investigate the role of clinical pharmacists in medication reconciliation. METHODS:Totally 200 inpatients admitted or transferred to nephrology department of our hospital during Aug.-Oct. 2015 were selected. Within 48 h after admission,1-year medication history were collected by reviewing electronic medical records,consultation,querying self-prepared drugs and medical history;and then medication reconciliation was conducted by clinical pharmacists. RESULTS:Among drug lists collected by clinical pharmacists,there were 987 kinds of drugs,but only 9.63%(95 kinds) drugs were recorded in the medical records. There were 5 cases of ADR in total,and only 40.00% of them (2 cases) were recorded in the medical records. Among 200 patients,medication reconciliation was needed in 45 cases with reconciliation rate of 22.50%. Among 492 medical orders of 200 patients,medication errors were found in 103 medical orders;the number of medication errors per case was (2.3 ± 1.8), mainly including wrong dose,repeated medication,wrong solvent,drug interactions;the potential risk degree was mainly degree 1 (53 orders,51.46%). Among 103 medication reconciliation orders,main plans were drug withdrawal (78 cases,75.73%), followed by drug change(17 cases,16.50%)and drug supplement(8 cases,7.77%). A total of 90 reconciled medical orders were adopted by physicians,with success rate of 87.38%. CONCLUSIONS:Compared with physicians,clinical pharmacists can obtain more detailed and accurate drug list. It can reduce medication error and guarantee the safety of drug use to maximum extent that clinical pharmacists conduct medication reconciliation.
5.The expression and clinical significance of FGFR2, and c-Cbl in gastric carcinoma

Background and purpose: FGFR2 is a receptor tyrosine kinase and c-Cbl is a new RING finger type of ubiquitin ligase in the ubiquitin-proteasomes path. The purpose of this study was to evaluate the expression and significance of FGFR2 and c-Cbl in gastric carcinoma. Methods: The expression of FGFR2 and c-Cbl were detected by immunohistochemical method of SP. Results: The positive expression rates of FGFR2 and c-Cbl were 77.4%,71.0% in gastric carcinoma, respectively, both were higher than those normal tissue (P<0.05);The expression of FGFR2 and c-Cbl were positively correlated with depth of invasion and TNM staging, and there was a positive relationship between the expressions of FGFR2 and c-Cbl. Conclusion. The expressions of FGFR2 and c-Cbi were associated with some clinicopathologic features in gastric carcinoma, indicating that their expression may be the prognostic factors for gastric carcinoma.
6.A multi-center clinical trial of natural calcined bone repair material in repairing bone defect after tooth extraction

Objective:To study the efficacy and safety of natural calcined bone repair material(NCBM)in repairing bone defect af-ter tooth extraction.Methods:A randemized,double-blinded,parallel,positive control(Bio-Oss)and multi-center clinical trial was employed.Imaging examination was used as the main efficacy evaluation index,surgical wound healing,rejection reaction,bone me-tabolic changes,bone infection signs were the subordinate efficacy evaluation indexes,the incidence of adverse reactions was observed for safety evaluation.Results:280 cases were included,269 cases completed the trial.In NCBMand Bio-Oss group the effective rate of imaging examination was 93.08% and 93.70%(P >0.05)respectively.The wound healing time of the 2 groups was less than 7 days,no rejection reaction,bone metabolic change and bone infection sign were observed.The incidence of adverse events in NCBM and the Bio-Oss group was 0.72% and 2.14%(P >0.05)respectively.Conclusion:The efficacy and safety between natural cal-cined bone repair material is not inferior to Bio-Oss in repairing bone defect after tooth extraction.

Objective:To compare the outcomes of guide bone regeneration by calcined bovine bone and Bio-Oss graft material in alveolar ridge preservation after tooth extraction.Methods:280 patients were divided into two groups randomly.Each patient had single tooth extracted.The sockets were filled with calcined bovine bone in 140 patients and Bio-Oss graft in another 140 patients. After shaping,all the sockets were covered with Bio-Gide membrane.Buccal mucoperiosteal flap was released and sutured to close the alveolar sockets.The patients were regularly examined at the 1st,12th and 24th week after surgery.Physical examination and X-ray evaluation were applied to compare the outcomes of the two materials in alveolar ridge preservation.Results:No infection and re-jection occurred.The radiographic results showed the width and height of the alveolar bone were preserved well at the 12th and 24th weeks.No statistically difference was found in the two groups at the 1st and 24th weeks(P>0.05).Conclusion:The two graft ma-terials can effectively preserve alveolar bone after tooth extraction.

9.Study on the optimal keV and contrast agent concentration in coronary stents with a dual-layer spectral detector CT in vitro

Objective:To investigate the optimal monoenergetic level of spectral reconstructions and contrast agent concentration for the structure and lumen of isolated coronary stents on a dual-layer spectral detector CT (DLCT).Methods:Eighteen scaffolds of different materials were implanted into the artificial simulated coronary artery, and were filled with conventional and low concentration contrast agents (1/2 of the conventional dose). Spectral CT scan was performed, and 50-100 keV (interval 10 keV) and 120 keV single-level reconstruction were performed at the conventional concentration and low concentration, which were recorded as A 50-100, 120 keV group and B 50-100, 120 keV group respectively; Another group of conventional concentration data was reconstructed with conventional iDose 4, which was recorded as conv group. The image was evaluated subjectively, including stent structure score (Likert 5-score) and stent lumen score (Likert 4-score); objective evaluation, including background noise, contrast noise ratio (CNR) and stent inner diameter difference (AlN). The differences of subjective and objective evaluation indexes were analyzed. Results:The stent structure scores of A 80, 90 keV group [(3.56±0.64), (4.11±0.58) points] and B 90 keV group [(3.61±0.50) points] were significantly better than those of other groups ( P<0.01), and the A 90 keV group had the highest score. The scores of stent lumen in A 70 keV group [(3.61±0.50) points] and B 70 keV group [(3.47±0.63) points] were significantly better than other groups ( P<0.01), and the score of A 70 keV group was the highest. There was no significant difference in noise, CNR and AlN ( F=1.67, 0.15, 1.16, P>0.05). The main effect analysis was energy level, there were significant differences in noise, CNR and AlN between groups a, B and conv ( F=83.74, 23.95, 24.63, P<0.01). A 70, 80, 90 keV group [(11.43±0.48), (11.81±0.34), (12.01±0.33) HU] and B 70, 80, 90 keV group [(11.50±0.31), (11.58±0.18), (12.13±0.30) HU], image background noise was significantly lower than other groups ( P<0.01). There was no significant difference between the six groups ( P>0.05). The noise of A 70 keV group was the least. The CNR in stent lumen of A 50, 60, 70 keV groups (26.85±5.19, 26.03±4.29, 24.46±5.40) and B 50, 60, 70 keV groups (25.88±5.67, 24.18±5.46, 24.91±7.67) was significantly higher than that of other groups ( P<0.01). The highest CNR value was found in A 50 keV group. The AlN value of A 90, 100, 120 keVgroup [(32.22±6.16) %, (30.79±5.55) %, (27.05±4.47)%] was higher than that of conv group [(41.76±6.00)%], the difference was statistically significant ( P<0.01). The AlN values of A 90, 100, 120 keV group and B 90, 100, 120 keV group [(34.34±5.73)%, (33.95±5.46)%, (33.92±5.45)%] were lower than those of other subgroups, and the differences were statistically significant ( P<0.01). There was no statistically significant difference between the six groups ( P>0.05). The AlN of A 120 keV group was the lowest. Conclusions:Regarding stent phantom imaging, DLCT monoenergetic imaging was better than conventional mixed energy imaging. When DLCT scanning was performed, 90 keV (structure) and 70 keV (lumen) combined reconstruction could not reduce the imaging quality when the concentration was halved.
10.Analysis of 43 cases of anti-NMDA receptor encephalitis misdiagnosed as mental disorder

Objective:To investigate the clinical features of patients with anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis misdiagnosed as mental disorder, improve the early diagnosis rate and reduce misdiagnosis.Methods:The clinical data of patients with anti-NMDA receptor encephalitis diagnosed at the First Affiliated Hospital of Zhengzhou University from 2012 to 2018 were collected. Patients misdiagnosed as mental disorders were screened out. Their psychiatric symptom characteristics, disease course characteristics, imaging and laboratory findings, treatment and prognosis were retrospectively analyzed.Results:A total of 121 cases of anti-NMDA receptor encephalitis were collected, and 43 cases of mental disorders were screened out. Sixteen of the 43 patients (37.2%) had prodromal symptoms, and all the patients had psychiatric behavioral abnormalities (100%), including 32 cases (74.4%) of seizures, 13 cases (30.2%) of decreased level of consciousness, 21 cases (48.8%) of involuntary movements, 15 cases (34.9%) of decreased memory, 8 cases (18.6%) of speech dysfunction, and 8 cases (18.6%) of other neurological symptoms (central hyperventilation, autonomic dysfunction). Memory loss was observed in 15 cases (34.9%), speech dysfunction in 8 cases (18.6%), other neurological symptoms (central hypoventilation, autonomic dysfunction) in 8 cases (18.6%), and various symptoms may appear simultaneously or successively in the same patient. Thirty-eight cases had complete resolution of symptoms or only minor physical impairment, and 5 cases had recurrent admissions with mental abnormalities and seizures. The recurrence rate accounted for 11.6% (5/43).Conclusions:The clinical manifestations of anti-NMDA receptor encephalitis are complex and varied. Most of them have mental behavior abnormalities as the first symptom, which is easily misdiagnosed as mental disorder and delayed treatment will lead to prolonged disease course and poor prognosis.
