1.Recent advances of aberrant expression of nucleophosmin in acute leukemia

Nucleophosmin (NPM) is regularly identified as multifunctional nuclear protein which is widely expressed in different kinds of cells.NPM is identified not only as a potential regulator for cell proliferation,but also as an important player in tumor genesis procession.Aberrant expression of NPM,such as over-expression,rearrangement and mutation could lead to malignant transformation in solid tumor and leukemia cells.Over-expression of NPM had been detected as a poor prognostic factor and was related to drug-resistance development.It is reported that rearrangement of NPM gene played an important role in acute promyelocytic leukemia (APL,M3),myelodysplastic syndrome (MDS) and lymphoma.As the main molecular event in AML,NPM1 mutation occurred in kinds of subtypes of AML,which predicted a different prognosis.The aberrant expression of NPM in acute leukemia was reviewed.
2.Schnitzler syndrome

A 63-year-old man presented with non-pruritic urticaria for 14 months,fever of unknown origin for nearly 12 months,and arthralgias of both lower limbs for 10 months.The fever,which was unrelated to the occurrence of skin lesions,appeared once a week on average,and might be as high as 39 ℃.Physical examination revealed irregular fever and lymphadenopathy in both inguinal and axillary regions.Skin lesions,consisting of rosecolored,slightly elevated plaques and papules,affected the trunk,both upper extremities and thighs.Skin biopsy showed perivascular infiltration of neutrophils and lymphocytes in the superficial dermal layer.Laboratory examination exhibited leukocytosis,elevated erythrocyte sedimentation rate and increased C-reactive protein level.Serum protein electrophoresis revealed an IgM-κ component in the γ-globulin zone.Radiographic examination suggested slight hepatosplenomegaly and hepatic cysts.Positron emission tomography/computed tomography (PET-CT) revealed bilateral knee joint effusion,degeneration of cervical vertebrae and lumbar vertebrae,as well as lymphadenectasis in both axillary,inguinal and popliteal regions as well as around external iliac vessels.Wholebody bone scan showed abnormalities of bilateral shoulder joints,elbow joints and knee joints,which were suggestive of arthritis.A diagnosis of Schnitzler syndrome was made.The patient was given prednisone 30 mg per day,and body temperature returned to normal with the improvement of condition on the second day.Subsequently,glucoside tripterygium total 20 mg thrice daily was additionally given,and the dose of prednisone was gradually decreased to 15 mg per day,with a satisfactory control of condition,disappearance of fever,skin eruptions or other discomfort,and marked relief of ostalgia and arthralgia.
3.Discontinuation of tyrosine kinase inhibitor therapy: can chronic myeloid leukemia be cured?

It is a crucial question whether it is necessary to continue the treatment after the patients with chronic myeloid leukemia (CML) who have achieved a complete molecular response (CMR) on imatinib (IM) treatment. If tyrosine kinase inhibitor (TKI) therapy was required indefinitely to maintain CMR, the problems of financial burden and adverse effects would be increased. Approximately 40 % of patients with a stable CMR for at least 2 years could stop IM treatment and remain in molecular remission for at least 2 years without IM treatment. Therefore, the ultimate aim should be to give the patients long-term disease control without the need for ongoing treatment. Here the studies progresses of discontinuation of TKI therapy in CML reported in the 53th American Society of Hematology (ASH) annual meeting were reviewed.

5.Distribution and antibiotic resistance profile of nonfermentative gram negative bacteria

Objective To investigate the clinical distribution and antibiotic resistance of the nonfermenting bacterial strains isolated in Beijing Anzhen Hospital from 2008 to 2012.Methods The bacteria were identified by VITEK-compact system and Phoenix 100 system. Antimicrobial susceptibiity was tested by dilution method. Results A total of 2 450 strains of nonfermentative gram-negative bacteria were isolated from clinical specimens. Acinetobacter baumannii, Pseudomonas aeruginosa and Stenotrophomonas maltophilia accounted for 57.2%,25.5% and 10.0%.Specimens were mostly collected from respiratory tract (80.9%),followed by blood (8.1%)and wound exudates (3.9%).The strains were mainly collectd from ICU (50.0%),cardiac surgery ward (17.0%)and respiratory medical ward (11.5%).The prevalence of imipenem-resistant A.baumannii was 62.3%,79.2%,70.4%,76.1%,67.8% from 2008 to 2012.The prevalence of imipenem-resistant P.aeruginosa was 28.7%,25.0%,27.6%,31.1% and 32.0%.S.maltophilia strains showed the highest susceptibility to trimethoprim-sulfamethoxazole and levofloxacin.Conclusions The high prevalence of antibiotic-resistant non-fermenting bacteria poses a great challenge to clinicins. The rational treatment choice should be based on the result of suseptibility testing.

7.Effect of cetuximab combined with FOLFOX4 on expressions of PTEN and P13 K in colon cancer tissue of elderly patients

Objective To investigate the effect of cetuximab combined with FOLFOX4 chemotherapy on expressions of cancer suppressor gene PTEN and P13K in colon cancer tissue of elderly patients.Methods 62 cases of elderly patients with colon cancer from October 2013 to July 2015 in our hospital were selected and divided into two groups according to the different therapy.The control group (n=27) received pure FOLFOX4 chemotherapy and treatment group(n=35)received cetuximab on the basis of FOLFOX4 chemotherapy, with a consecutive treatment of 4 courses.The clinical curative effect and expressions of PTEN and P13K in colon cancer tissue were compared between two groups.Results There were no significant difference in effective rate and disease control rate between treatment group and control group (51.4% vs.44.4%,χ2 =0.298,P=0.585;80.0%vs.62.9%,χ2 =2.223, P=0.136 ).After treatment, the cells positive rate of PTEN in treatment group was higher than that in control group post-treatment (82.86%vs.59.26%, P<0.05), and the cells positive rate of P13K in treatment group was lower than that in control group (37.14%vs.62.96%, P<0.05). Conclusion Cetuximab combined FOLFOX4 chemotherapy regimen could increase expression of PTEN and reduce P13K expression, which effect is remarkable in the treatment of elderly colon cancer.
8.Combined use of RT-PCR and gel electrophoresis to detect expression of transforming growth factor beta1 in mouse lung fibroblasts in vitro

BACKGROUND: As a combination of reverse transcription (RT) and polymerase chain reaction (PCR), RT-PCR has been used to detect gene expression levels in cells and tissues, RNA virus contents in cells and specific gene cloned cDNA sequences.OBJECTIVE: To detect the inhibitory effcet of Stealth siRNAs on the expression of transforming growth factor β1 (TGF-β1).METHODS: There were blank control, empty vector transfection, stealth-48, stealth-166, and stealth-594 groups. Three stealth siRNAs aimed at different sequences in TGF-β1 mRNA were made, and were then transfected into BALB/c mouse lung fibroblasts in vitro. The expressions of TGF-β1 and connective tissue growth factor were detected by RT-PCR.RESULTS AND CONCLUSION: In different time periods, the TGF-β1 expression was differentially depressed by three stealth siRNAs, especially stealth-166. The inhibitory effects varied with time, which could be detective at 48 hours,reached the peak at 72 hours and then began to attenuate at 96 hours. Our findings show that the inhibitory effect of stealth siRNAs on the TGF-β1 expression in mouse lung fibroblasts can be detected by RT-PCR.
9.Relationship of adipose chemerin and its receptor chemerinR gene expression to obesity and type 2 diabetes mellitus

Objective · To explore the relationship of adipose chemerin and its receptor chemerinR gene expression to obesity and type 2 diabetes mellitus. Methods · Twenty-four patients undergoing elective abdominal surgery were enrolled, and were divided into normal glucose regulation-normal weight group (NGR-NW), normal glucose regulation-overweight/obesity group (NGR-OW/OB), and type 2 diabetic overweight/obesity group (T2DMOW/OB) according to the body mass index (BMI). The levels of chemerin and chemerinR mRNA were detected by reverse transcription PCR (RT-PCR).Results · Compare to the NGR-NW group, the chemerin mRNA levels of abdominal subcutaneous and omental fat were significantly increased in the NGR-OW/OB and T2DM-OW/OB group (P<0.05). Correlation analysis showed that the chemerin mRNA levels of abdominal omental fat were positively correlated with BMI, fasting insulin (FINS), triglyceride and serum chemerin (r=0.577, r=0.561, r=0.472, r=0.623, P<0.05 for all). The chemerin mRNA levels of abdominal subcutaneous fat showed significant positive correlation with BMI, FINS and serum chemerin (r=0.692, r=0.513, r=0.497, P<0.05 for all). Conclusion · The chemerin mRNA levels of abdominal subcutaneous and omental fat were positively correlated with BMI, FINS and serum chemerin, suggesting that the chemerin gene may play a crucial role in the pathophysiological mechanism of obesity and type 2 diabetes.
10.Research on Auditory Steady-state Responses to Multiple Simultaneous Stimuli of Normal-hearing Adults

Objective To explore some characters of auditory steady-state responses (ASSR) to multiple simultaneous stimuli in a sample of normal-hearing adults.Methods The ASSR to multiple simultaneous stimuli were recorded in a sample of 32 normal-hearing adults (64 ears). Simultaneous carrier tones (0.5, 1, 2 and 4 kHz), modulated in amplitude at different rate of 77, 85,93, 101 Hz for left ear and 79, 87, 95, 103 Hz for right ear, were presented binaurally by the insert phones. For each subject the response thresholds (RTHs) at 0.5, 1, 2 and 4 kHz were determined automatically and compared with the corresponding behavioral thresholds (BTHs).Results ①In the normal-hearing adults, RTHs were on average between 7~19 dB above the BTHs.②2-way repeated measures ANOVA (ear?carrier frequencies) showed significant main effect between carrier frequencies on RTH ( F =31.254; ? 3,179; P =0.000). Post Hoc analysis indicated that this effect was due to the highest threshold found at 0.5 kHz. The test showed no significant difference of threshold between left ear and right ear ( F =1.942; ? 1,179; P =0.165). ③Taking 60 dB SPL as analysis level, 2-way repeated measures ANOVA (gender?carrier frequencies) revealed significant difference of amplitude between male and female( F =16.490; ? 1,175; P =0.000); and difference of amplitude between carrier frequencies ( F =2.948; ?3, 175; P =0.034).④The average testing time was 61 minutes for male and 69 minutes for female. Independent t-test showed statistical significance within sex.⑤Two- way ANOVA (state?frequency)showed that background EEG noise were significantly higher in awake than that in sleep ( F =152.511; P =0.000). There were no appreciable significance in background noise among frequencies. Conclusion The ASSR to multiple simultaneous stimuli was proven to be a frequency specific, objective and valid technique for threshold estimation at least for normal-hearing adults. Issues concern ing clinical use were discussed.
