Objective:To evaluate the effect of free interosseous posterior artery perforator flap in small hand defects. Methods:We retrospectively analyzed the case data of small hand skin and soft tissue defects treated with free interosseous posterior arterial perforator flaps from March 2019 to August 2020 in the Department of Burn Plastic Surgery, the Second Affiliated Hospital of Zunyi Medical University. According to the size of the defect during the operation, we designed free interosseous posterior artery perforator flap transplantation to repair wounds, and the perforating branch of the posterior interosseous artery is anastomosed with the proper digital artery and the common digital artery. The donor valve area is sutured directly. After the operation, the survival of the flaps was observed, and the repair effect of the flaps, hand function and complications were followed up.Results:Twenty-one cases were included, among whom there were 15 males and 6 females with a median age of 46 years (range, 23-61years). There were several causes of injury: 7 cases of heavy object injuries, 11 of machine strangulation, 3 of burns and scalds. For 8 cases, the defect sites were on the palms and 13 on the dorsal sites. The defect size ranged from 1.5 cm×2.0 cm to 3.0 cm×9.0 cm with exposed tendons and bones. The flap size ranged from 2.0 cm×2.0 cm to 3.5 cm×9.0 cm. Among them, compound tissue transplantation with tendon was used to reconstruct finger extension function in 1 case while tissue transplantation with nerve was used to repair the palmar wounds in 2 cases. The bilobed flaps were used in 4 cases, and the remaining 14 cases were single leaf free skin flaps. No arteriovenous crisis occurred in the flaps after the operation. All flaps of 21 patients survived, and there was no blood supply disorder in the flaps. After 6-12 months of follow-up, the texture and color of the flap were similar to adjacent skin, and unrestricted hand function was unrestricted. The two-point discernibility of the skin flap for patients with nerve grafts was 10-12 mm, serious complication in the donor sites, but the forearm donor area with linear scars. Conclusions:The application of interosseous posterior artery perforator flap is flexible. It can repair two adjacent small wounds at the same time with free bilobed transplantation, and can carry free cutaneous nerve transplantation to repair the palm side wound. This flap is one of the ideal flaps for repairing small defects on the hand.

Iron homeostasis plays an important role for the maintenance of human health. It is known that iron metabolism is tightly regulated by several key genes, including divalent metal transport-1(), transferrin receptor 1(), transferrin receptor 2(), ferroportin(), hepcidin(), hemojuvelin() and . Recently, it is reported that DNA methylation, histone acetylation, and microRNA (miRNA) epigenetically regulated iron homeostasis. Among these epigenetic regulators, DNA hypermethylation of the promoter region of , and bone morphogenetic protein 6 () genes result in inhibitory effect on the expression of these iron-related gene. In addition, histone deacetylase (HADC) suppresses gene expression. On the contrary, HADC inhibitor upregulates gene expression. Additional reports showed that miRNA can also modulate iron absorption, transport, storage and utilization via downregulation of and other genes. It is noteworthy that some key epigenetic regulatory enzymes, such as DNA demethylase TET2 and histone lysine demethylase JmjC KDMs, require iron for the enzymatic activities. In this review, we summarize the recent progress of DNA methylation, histone acetylation and miRNA in regulating iron metabolism and also discuss the future research directions.

To determine the etiologies and risk factors of intracerebral hemorrhage in young people.
Methods: A total of 401 young patients with intracerebral hemorrhage were enrolled, and they were assigned into a 20-29 , a 30-39, and a 40-45 age group. The differences of various etiologies and risk factors among the three groups were analyzed.
Results: There were 273 men and 128 women in the 401 young patients. The etiologies of 294 patients (73.32%) were identified while 107 patients (26.68%) were unknown. Among those with identified etiology, 226 patients (56.36%) suffered from hypertension, 41 patients (10.22%) congenital cerebrovascular malformation (including 25 patients with cerebral arteriovenous malformation, 8 intracranial cavernous hemangioma, and 8 intracranial aneurysm), and 27 other etiologies (including 9 patients with moyamoya disease, 6 cerebral venous sinus thrombosis, 4 drug abuse, 3 hemorrhagic brain tumor, 2 intracranial infection, 1 systemic lupus erythematosus, 1 drug-induced, and 1 eclampsia). Risk factors included hypertension (237 cases, 59.10%), smoking (123 cases, 30.67%), alcohol consumption (74 cases, 18.45%), and others (19 cases, 4.74%; including 8 cases of pregnancy or in the puerperium, 8 family history of intracerebral hemorrhage, and 3 taking anti-platelet aggregation/anticoagulation agents). The rate of hypertension induced hemorrhage significantly increased with age (P<0.01); the rate of vascular malformations in 20-29 age group was obviously higher than other groups (P<0.01); the rate of unknown cause in the 40-45 age group was significantly lower than other groups (P<0.01) and the rate of other etiologies showed no significant difference in the 3 groups. The rate of hypertension was significantly elevated with the age (P<0.01), while smoking, alcohol consumption, and other risk factors showed no significant difference in the 3 groups.
Conclusion: The rate of intracerebral hemorrhage in young people increases with the increasing of age and hemorrhage affects men more than women; hypertension may be the main cause and congenital cerebrovascular malformation is the second cause, which may be more common in younger patients. Hypertension, smoking, and alcohol consumption may be the major controllable risk factors in intracerebral hemorrhage in young people.
