MicroRNAs (miRNAs) are involved in cell growth,differentiation,apoptosis and tumor occurrence and play an important role in regulation.Many studies show that miRNAs can regulate the expression of target gene,which is associated closely with the occurrence,development,chemosensitivity and prognosis of colorectal cancer.Detection of miRNAs is expected to formulate more detailed individualized chemotherapy for colorectal cancer patients and improve their prognosis.
2.Relationship Between High-sensitivity C-reactive Protein and P Wave Dispersion in Patients With Lone Paroxysmal Atrial Fibrillation

Objective: To analyze the relationship between high-sensitivity C-reactive protein (hs-CRP) and P wave dispersion (Pd) in patients with lone atrial ifbrillation (AF), and to explore the effect of inlfammation on atrial electrophysiological remodeling.
Methods: Our research included 2 groups. AF group, containing 71 consecutive paroxysmal lone AF patients, and Control group, containing 71 paroxysmal supra ventricular tachycardia patients with the matched age and gender. The clinical characteristics, electrocardiographic Pd assessment and plasma hs-CRP levels were compared between 2 groups. The relationship between hs-CRP and Pd was studied by linear and multi linear regression analysis.
Results: Compared with Control group, AF group showed increased left atrial diameter, Pd and hs-CRP, all P<0.05. multilinear regression analysis indicated that hs-CRP was positively related to Pd in both groups, P<0.01. Multiple logistic regression analysis revealed that hs-CRP was an independent risk factor for AF occurrence (HR=15.430, 95%CI 6.031-39.476, P<0.001), with adjusted Pd, multiple logistic regression analysis presented that both Pd and hs-CRP were the independent risk factors for AF occurrence, both P<0.001, while the HR for hs-CRP predicting AF occurrence was attenuated from 15.430 to 6.246.
Conclusion: Plasma hs-CRP level and electrocardiographic Pd were the important risk factors for paroxysmal lone AF, the interaction between hs-CRP and AF occurrence could be mediated by Pd, suggesting that inlfammation might be involved in atrial electrophysiological remodeling.
4.Chondrogenesis of bone marrow mesenchymal stem cells, chondrocytes and adipose-derived adult stromal cells in three-dimensional scaffolds

BACKGROUND: The key factor for cartilage repair is an orient differentiation of seed cells in three-dimensional scaffold; however,clear cartilage is haply observed in the three-dimensional scaffold.OBJECTIVE: To investigate the chondrogenesis of bone marrow mesenchymal stem cells (BMSCs), chondrocytes (CCs) and adipose-derived adult stromal cells (ADAS) in three-dimensional scaffolds METHODS: BMSCs, CCs, and ADAS were primary-cultured from goat bone marrow following trypsin digestion and amplified in monolayer. P1 BMSCs, P3 CCs, and P3 ADAS were implanted into collagen/hyaluronic acid scaffold at different concentrations of 10%, 20%, 50%, 80%, and 100%, and the cells were then three-dimensionally cultured in serum-free culture media. Two weeks later, SO staining and immunohistochemical staining were employed to observe the synthesis between of chondroitin sulfate and type Ⅱ collagen.RESULTS AND CONCLUSION: The seeded cells had chondrogenesis in scaffolds under certain conditions. In details, BMSCs in 20% (W/W) hyaturonan/collagen Ⅰ scaffolds could perform a hyaline-cartilaginous phenotype when they were expanded with less than 3 passages and cultured in a chondrogenic medium, and CCs did better than BMSCs in chondrogenesis with high passage number; however, ADAS just showed a minor chondrogenesis even in a pellet culture. BMSCs and CCs were better than ADAS as seeded cells for cartilage tissue engineering and regenerative medicine.
5.Hematoma Aspiration With Manual Compression for Treating the Patients of Iatrogenic Femoral Pseudoaneurysm Under Ultrasound Guidance

Objective: To study the safety and efifcacy of hematoma aspiration with manual compression for treating the patients of femoral pseudoaneurysm after cardiac catheterization under ultrasound guidance. Methods: A total of 27 patients suffering from post-catheterization iatrogenic femoral pseudoaneurysm were analyzed including 14 male and 13 female at the mean age of (53.5±11.4) years. The body, neck and blood supply area of pseudoaneurysm were located by ultrasonography; 18 gauge needle was punctured into the center of pseudoaneurysm to aspirate blood, meanwhile the neck and body of pseudoaneurysm were manually compressed to block blood supply for relevant artery under ultrasound guidance. Manual compression was conducted for 15 min followed by bandage compression; the patients were lie on the back and kept lower extremity straight for 12 hours. Ultrasonography was performed at 24 hours and 1 month after the operation in all patients respectively. Results: There were 24/27 (88.9%) patients having successful aspiration with manual compression at ifrst time; 2 (7.4%) having incomplete occlusion at ifrst time and the success was obtained by second time; 1 having incomplete occlusion due to coexisted femoral arteriovenous ifstula, while the body of pseudoaneurysm was obviously decreased. The overall success rate was 96.3% (26/27), no procedural complication occurred. Conclusion Ultrasonography guided hematoma aspiration with manual compression has been safe and effective for treating the patients of iatrogenic femoral pseudoaneurysm.
6.Application of seaweed polysaccharide-agarose dressing to animal skin regeneration

BACKGROUND: It has vedfied that seaweed polysacchande-agarose modifiers have their medical application value. OBJECTIVE: To perform skin regeneration trials using sprayable activated agarose/gelatin solution, and to explore the possibility of egarose modifier as skin dressing for skin regeneration. METHODS: To prepare 3% sprayable mixture with the dissolved activated agarose and gelatin at a certain ratio, and then filtrated with millopore for sterilization to prepare activated agarose (egarose degradation for 8 hours) and gelatin degradation, and made into different ratios (1: 0, 1: 1, 1: 2, 1: 3). A total of four rabbits were obtained, and four sites were selected on the back of each rabbit, totally 16 experimental sites. The sprayable activated agarose/gelatin mixture (1: 1, 1: 2, 1: 3) was directly sprayed on the four lesion sites. Sprayable activated agarose for two sites and simple gelatin for two sites served as controls. The effects of the wounds sprayed with,dressing were observed at 4 weeks following surgery. RESULTS AND CONCLUSION: At 7 days following surgery, the cover film had broken in mixture of activated agarose and gelatin at 1: 3, and remaining three were intact. No infection or inflammation occurred in wound of four ratios. Following comparison, the wound was rapidly healed in 1: 2 ratio dressing. The additional gelatin showed promoting effects on wound healing significantly. Hematoxylin-eosin staining demonstrated that skin with the dressing was similar to autologous skin, which verified that sprayable activated agarose/gelatin have a premise in skin regeneration.
7.Simultaneous Determination of Sodium Valproate and Its Metabolite in Human Plasma by RP-HPLC

OBJECTIVE:To establish a method for simultaneous determination of sodium valproate(VPA)and its metabo-lite 2-propyl-2-pentenoic acid (2-ene-VPA) in human plasma. METHODS:Plasma sample was extracted with cyclohexane and experienced derivatization with 2,4′-dibromoacetophenone using n-octanoic acid as an internal standard. RP-HPLC method was adopted. The determination was performed on Zorbax SB-C18 column with mobile phase consisted of acetonitrile-water(65∶35,V/V)at the flow rate of 1 mL/min. The column temperature was set at 35 ℃,and UV dectection wavelenth was set at 258 nm. The sample size was 20 μL. RESULTS:The linear range of VPA and 2-ene-VPA were 5.0-200.0,0.5-20.0 μg/mL(r=0.999 9, n=5). The limits of quantification were 5.0,0.5 μg/mL. RSDs of inter-day and intra-day were all lower than 5%. Method recov-eries were 95.99%-98.80%and 97.40%-98.17%,and extraction recoveries were 80.46%-86.23%and 80.45%-85.61%. The plas-ma concentrations of VPA in 10 epileptic children were 27.4-93.2 μg/mL,and those of 2-ene-VPA were 0.85-3.94 μg/mL,respec-tively. CONCLUSIONS:The method is simple,specific and suitable for plasma concentration determination and pharmacokinet-ic study of VPA.
8.Ultrasonic measurement of vasodilation in cerebral infarction patients

Objective To investigate the bilateral flow-mediated dilation of brachial artery (BA-FMD) and nitroglycerin mediated dilation (BA-NMD) in patients with cerebral infarct, and to find out if the vasodilation of internal catotid artery after sublingual administration of nitroglycerin can be detected as a more direct way to reflect the vasodilation in patients with cerebral vascular diseases. Methods High resolution ultrasonic system was used to measure 13 patients with acute cerebral infarction. The diameter of brachial artery, internal carotid artery and common carotid artery was measured at relaxation, reactive congestion in the brachial artery and after sublingual administration of nitroglycerin. Meanwhile, the intima media thickness (IMT) and plaques of carotid artery was detected. Results Paralysis did not lead to differences in vasodilation of brachial artery (r=0.52, P=0.07). There was a correlation between ICA-NMD and BA-NMD (r=0.83, P=0.01). Conclusion There were no significant differences between brachial vasodilation in paretic and non-paretic limbs of patients with acute cerebral infarction. Ultrasonic measurement of ICA-NMD can be a more direct measure of the vasodilation of patients with cerebral infarction.
9.Variation of mortality and discharge against medical advice among major trauma patients admitted to ICU

Objective To observe the variation of mortality and discharge against medical advice in major trauma patients admitted to the ICU and thus to improve the level of trauma care.Methods A retrospective analysis was performed on major trauma patients who died or discharged against medical advice in the emergency 1CU from 2003 to 2011.Patients were categorized as the dying,worse,and improved according to their situation at discharge.The dying was included into the death group.Annual variation of mortality and discharge against medical advice were analyzed in these 9 years.These parameters were also compared among three diverse periods (2003-2005,2006-2008 and 2009-2011).Results Ultimately,452 patients were recruited from the 3,343 major trauma patients admitted to the emergency ICU from 2003 to 2011.There were 231 deaths occupying 6.91% (135 patients died in hospital and 96 patients were dying at discharge) and 221 discharges against medical advance occupying 6.61% (175 patients deteriorated and 46 patients improved).Within the 9 years,a significant decrease was found in the annual rate of inhospital death,dying at discharge,and total death.Meanwhile,there was an increase in the rate of deterioration at discharge (x2 =15.305,P >0.05).However,no significant difference was found in the rate of total death plus deterioration at discharge.During the three periods of 2003 to 2005,2006 to 2008,and 2009 to 2011,number of patients admitted to the emergency ICU was 687,1,143,and 1,513 respectively.Age and acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) of the dead,the patients with deterioration at discharge,and the dead plus the patients with deterioration were gradually higher through the three periods; on the contrary,GCS lowered.Total mortality decreased from 11.06% to 4.63% (x2 =31.174,P <0.01) ; rate of deterioration at discharge increased continuously from 2.77% to 6.35% (x2 =12.203,P <0.01) ; rate of death plus deterioration at discharge was declined from 13.83% to 10.97% (x2 =4.09,P > 0.05).Conclusions From 2003 to 2011,the level of severe trauma care in emergent ICU is obviously improved.Ratio of deterioration at discharge increases with more aged patients.Discharge against medical advance interferes with the accurate assessment of trauma care and should be paid more attention.
10. Construction and analysis of prognostic model for esophageal cancer specific survival rate

Objective To analyze the factors affecting the prognosis of esophageal cancer and construct a reasonable prognostic model. Methods A total of 672 patients diagnosed with esophageal cancer in 2010 were selected as the study data. The factors affecting the specific survival rate of esophageal cancer were screened and modeled by LASSO COX regression analysis. The prediction was performed by R 3.5.3 software. The model was visually constructed and the value of the predictive model was analyzed. Results The 1, 3, and 5 years tumor-specific survival rates of 672 patients with esophageal cancer were 55.32% (95% CI: 51.56% to 59.08%), 30.07% (95% CI: 26.58% to 33.56%), and 25.04% (95% CI: 21.75% to 28.33%), respectively. LASSO regression analysis screened for 11 variables most relevant to prognosis, i.e., chemotherapy, primary site, tumor grade, T stage, N stage, M stage, radiotherapy sequence, surgery, tumor size, age, and ethnicity. Univariate and multivariate COX regression analyses showed that tumor grade, N stage, M stage, surgery and tumor size were independent risk factors for the prognosis of esophageal cancer. By visualizing the prognostic analysis, a Nomogram was constructed, and the consistency index C-index was 0.726 (95% CI: 0.703-0.749), which was significantly better than that of the 7th edition of AJCC's TNM staging system (C-index=0.654, 95% CI: 0.628-0.680). There was also good agreement between the predicted survival rates of the 1, 3 and 5 year calibration curves of this prognostic model and the actual survival rate. At the same time, the ROC curve results showed that if the patient's total score was greater than 15.570, the sensitivity and specificity were the best, the patient could be judged as a high-risk group. And the Nomogram prognostic model constructed in this paper had AUC=0.831, 95% CI: 0.801-0.859, and the prediction ability was significantly better than that of the traditional AJCC 7 version TNM staging system (AUC=0.759, 95% CI: 0.725-0.791). At the same time, a formula for predicting tumor-specific survival rate was constructed: CSS=5.988×10-5×points3+(-1.740×10-3)×points2+(-2.004×10-2)×points+0.685. Conclusion This study used LASSO regression to screen the variables that affected the prognosis of esophageal cancer and visually constructed relevant independent risk factors. It has high clinical value and is of great significance for the screening of high-risk population and the formulation of subsequent personalized diagnosis and treatment plans.
