2.Plate fixation versus intramedullary nailing for midshaft clavicular fractures: a prospective rndomized controlled trial

Objective To compare the clinical effects of closed reduction and intramedullary nailing versus open reduction and plate fixation in the treatment of displaced midshaft clavicular fractures.Methods A prospective,randomized,controlled trial was performed between July 2012 and May 2014 in 194 patients with acute displaced midshaft clavicular fracture.They were 136 males and 58 females,from 16 to 65 years of age.They were randomly divided into 2 groups to received either closed reduction and intramedullary nailing (n =98) or open reduction and plate fixation (n =96).We recorded operation time,intraoperative blood loss,hospitalization expenses,fracture healing time and complication rate.Functional assessments were conducted at 3,6 and 12 months using the Disabilities of the Arm,Shoulder and Hand (DASH) and Constant-Murley scores.The preoperative general data showed no statistical significance between the 2 groups (P > O.05).Results All the 194 patients were followed up for 12 to 27 months (average,15.6 ±3.1 months).The operation time (27.7 ± 16.3 min),intraoperative blood loss (18.6 ± 14.4 mL),hospitalization expenses (12,462.0 ±3,263.4 yuan),and fracture healing time (12.1 ± 3.0 weeks) in the intramedullary nailing group were significantly better than those(62.3 ± 19.4 min,40.3 ± 17.4 mL,24,760.0 ± 3,320.4 yuan,and 16.9 ± 2.8 weeks) in the plate fixation group (P < 0.05).At 3 months after surgery,the DASH (13.4 ± 3.5) and Constant-Murley (88.5 ±5.9) scores in the plate fixation group were significantly better than those (24.8 ± 6.7 and 69.1 ± 6.2,respectively) in the intramedullary nailing group (P < 0.05).At 6 and 12 months after surgery,there were no significant differences in the Constant-Murley scores or DASH scores between the 2 groups (P > 0.05).The complication rate in the intramedullary nailing group (25.5%,25/98) was significantly higher than in the plate fixation group (9.4%,9/96) (P < 0.05).Conclusions In the treatment of displaced midshaft clavicular fractures,compared with open reduction and plate fixation,closed reduction and intramedullary nailing has advantages of less invasion,lower cost and faster fracture healing,but a disadvantage of higher complication rate.The 2 treatments may lead to similar functional recovery of the shoulder.
3.Analysis of the Correlation between the Use Density of Antibiotic and Antifungal Drugs and the Positive Rate of Aspergillus

Objective To investigate the correlation between the use density of antibiotic and antifungal drugs and the positive rate of aspergillus in our hospital from 2009 to 2013,in order to provide a scientific basis for rational use of antibiotic and antifungal drugs. Methods The use density of carbapenems,two third-generation cephalosporins,and antifungal drugs,and the positive rate of aspergillus in our hospital from 2009 to 2013 were studied retrospectively. Their correlation was analyzed using SPSS software. Results There was significantly positive correlation between the use density of imipenem and that of fluconazole (r=0.913,P<0.05).The use density of biapenem was positively correlated with that of voriconazole (r=0.915,P<0.05).The use density of biapenem and that of voriconazole was positively correlated with positive rate of aspergillus,respectively ( r=0.918,r=0.955,both P<0.05).The other antibiotic and antifungal drugs were not significantly correlated to the positive rate of aspergillus.
4.The aberrant effective connectivity of the limbic system to happy face in patients with depression

Objective To investigate the effective connectivity of limbic circuit in patients with major depressive disorder when they recognized dynamic positive face expressions,aiming to discuss the possible mechanism of emotion processing in depressed patients.Methods Eighteen depressive patients and eighteen well-matched healthy control volunteers participated in the experiment.All subjects were asked to recognize the emotion face during the magnetoencephalograph (MEG) scanning.The regions of interested (ROI) brain areas included the orbital frontal cortex (OFC),the anterior cingulated cortex (ACC),the amygdala (AMYG),the hippocampus and the insula.The MEG data were preprocessed by the SPM8 software and further analyzed by the Granger casual model (GCM).The non-parameter permutation was used to compare the value of effective connectivity between the healthy controls and the depressed patients.Results Compared with healthy controls,the effective connectivity from the ACC to the AMYG (P=0.0052),from the OFC to the AMYG(P=0.0046),from the Hippocampus to the ACC (P=0.0016),and from the ACC to the Hippocampus (P=0.0042)was significantly reduced in depressed patients.Conclusion The depressed patients display decreased interaction of the limbic circuit during the happy facial emotion processing,indicating that the depressed patients are unable to deal with the positive stimuli,and to certain extent,explaining the abnormal neuropathophysiological mechanism of positive stimuli in MDD.
5.Investigation and analysis of scientific research multidisciplinary team collaboration intention among young researchers in a Grade-A Hospital

Objective To understand the intention and needs of young researchers in a Grade-A Hospital to carry out the Multidisciplinary Team (MDT),to provide references for hospital to condense the direction of scientific research,set up Multi-disciplinary resear Teams,as well as conduct innovative medical research.Methods A questionnaire survey was conducted among 196 young researchers under 45 years old in Grade-A Hospital,collected data were analyzed statistically.Results The total understanding and comparative understanding of the MDT model among the respondents was 52.05 %.The differences in the MDT pattern recognition were statistically significant(P<0.05) in different ages,titles and departments,and nearly 100 % of the respondents expressed their willingness to participate in the research MDT.68.37% believed that cooperation should be carried out according to the research direction.66.63% believed that the distribution of results should be based on the uniform norms issued by hospitals,and the allocation plan should be refined according to the project.61.73% believed that a consistent cooperative goal was important to set up MDT,and 46.43% emphasis on the role of academic leader in organizing and leading MDT.Conclusions The enthusiasm of young researchers in a Grade-A Hospital is very high,and there isstrong willingness to cooperate with MDT.Hospitals should bring in more effective organization mode of MDT research collaboration and incentive measures to future condense the direction of scientific research,focus on direction,solve scientific problems in depth,and ultimately promote the substantiality formation of multidisciplinary research collaboration and output.
6. Analysis on the layout of China’s emergency research projects during the public health emergency of COVID-19

Objective:
To explore the main layout and countermeasures of Scientific research projects during the public health emergency of COVID-19.
Methods:
Literature investigation method is applied to collect information of scientific research and emergency research projects of COVID-19 funded since January 20th by different entities including national, provincial and municipal administrative departments, public health agencies, research institutes, universities and industries.
Results:
Along with priorities identified for the emergency response and key research agendas, the national science and technology authorities at all levels and scientific research institutions have deployed a series of new science and technology projects, as well as a number of supporting policies and measures.
Conclusions
In the campaign of science and technology to deal with COVID-19, the leadership of Chinese government and their coordination with local agencies facilitated quick response in both R&D investment and supporting policies. Periodical achievement is recognized so far, and with the continuous development and in-depth research, the related scientific research results will be gathered into the core force of epidemic prevention. Moreover, it will improve China's capacity to deal with health emergencies and the level of medical health innovation, and better protect the health of the people.
7.The international experiences of Investigator-initiated Clinical Trials and implications

Objective:To introduce the experience of international Investigator- initiated Clinical Trials (IIT), provide suggestions for the construction of IIT management system in China. Methods:Applying the literature review research methodologies to analyze the document materials, five aspects including the funding source, technical support, personnel training, management mechanism and policy assurance were analyzed.Comparison was conducted to identify possible problems existed in Chinese IIT, and further explore possible lessons that we can learn in Chinese context.Results:Identified problems of Chinese IIT include insufficient funds, unsound scientific research design, imperfect data information system, lack of standard training for researchers, as well as the urgent need for more optimized whole-process management, quality control, and policy support.Conclusions:Suggested series of measurements that including expand the funding sources, improve the construction of scientific steering committee, improve the construction of data information platform at the national level, standardize the training of researchers, and update the operation mechanism and also the regulatory requirement are proposed to enhance the IIT research quality, as well as the IIT management.
8.The development of a framework on the need for elderly healthcare guidelines in China and related issues

Objective:Based on the serious situation of population aging and the need for geriatric healthcare guidelines in China, we constructed a framework on the need for national geriatric care guidelines and conducted relevant analysis, aiming to provide a reference for the formulation of guidelines for geriatric care.Methods:Literature review and expert consultations were used to construct a framework on the need for guidelines on geriatric care in China.Twenty-two experts were invited to form a consultation group, and two rounds of expert consultations and discussions were carried out using the Delphi method to update and adjust the framework.Hierarchical analysis was used to calculate the weight of each guideline direction.Results:A framework on the need for geriatric healthcare guidelines was developed, containing 7 first-level directions and 35 second-level directions, and the weights of the directions were derived, with the direction of guidelines on geriatric syndromes having the highest weight at 0.204, and the direction of guidelines on geriatric disease diagnosis and treatment having the second highest weight at 0.196.Conclusions:A preliminary framework on the need for geriatric healthcare guidelines has been established.It is fitting for the country's general conditions, takes into account the needs from multiple perspectives, has good coverage, and can be expected to solve a range of difficult and urgent issues in geriatric care and help promote the development of a set of high-quality guidelines in the field of geriatric healthcare.
9.Prospective multicentre study of chemotherapeutic regimen containing pirarubicin on the treatment of relapsed or refractory acute myeloid leukemia in adults.

OBJECTIVETo compare the efficacy and toxicity of the chemotherapeutic regimen containing pirarubicin and mitoxantrone on the treatment of relapsed or refractory acute myeloid leukemia (AML) in adults.
METHODSIn this open prospective multicentre study, we randomly assigned patients with relapsed or refractory AML to receive TAE regimen (pirarubicin+cytarabine+etoposide) versus MAE regimen (mitoxantrone + cytarabine + etoposide). The efficacy and toxicity were compared between the two groups.
RESULTS56 patients entered this clinical trial. The complete remission (CR) rate on TAE arm was 79.0% versus 55.6% on MAE arm with the overall response (OR) rates of 86.8% versus 88.9%, respectively. The CR was higher on TAE arm (P=0.035) but with no significant difference between the two groups regarding the overall response (OR) rate. The regimens were well tolerated in both groups. Hematologic and non-hematologic toxicity were similar except relatively lower the mean dosage of G-CSF, red blood cells and platelets transfusion on TAE arm. No significant differences were seen between the two groups regarding the overall survival and relapse free survival rates.
CONCLUSIONTAE regimen might be an effective salvage therapy in patients with relapsed or refractory AML.

10.ChatGPT and medical research ethics: Debates and norms

Objective:Analyze the ethical issues encountered or potential in the use of ChatGPT and explore its ethical norms and requirements.Methods:Based on the ethical perspective of medical scientific research, this paper analyzed the disputes existing in ChatGPT from the perspectives of morality, fairness, responsibility and supervision, and explored the reasons for the disputes from both subjective and objective aspects.Results:ChatGPT has ethical issues, fairness issues, accountability issues, and regulatory issues.Conclusions:Ethical issues in ChatGPT should be regulated from the perspectives of people-oriented, limiting monopoly, strengthening responsibility and insisting on development, to reduce potential risks and negative effects.
