1.Relationship between Autophagy and Repair of Spinal Cord Injury Based on Pathogenesis of Stasis Obstructing Meridian (review)
Chinese Journal of Rehabilitation Theory and Practice 2017;23(6):654-656
Spinal cord injury is a kind of central nervous system injury disease caused by trauma. It was found that the up-regulation of autophagy after spinal cord injury could promote the repair of spinal cord injury. Under the condition of stress, autophagy can reduce the number of misfolded proteins and damaged organelles in cells to maintain the stability of intracellular environment. Chinese medicine with Huoxue Quyu Tongluo (against stasis obstructing meridian) efficacy has attracted much attention in the treatment of spinal cord injury, which may associate with the role of regulation of autophagy.

Objective To evaluate the perioperative management and therapeutic efficacy of procedure for prolapse and hemorrhoids (PPH) in elderly patients with serious hemorrhoids. Methods The clinical data of 52 elderly patients (aged 60 years or over) with serious hemorrhoids treated by PPH from March 2005 to November 2007 were analyzed restrospectively. Results All the operations were successfully completed. The average operation time was (15.3±3.2) min, the average blood loss was (15±1.7) ml and the average hospitalization time was (4.5±2.6) days.Patients were followed for 1-20 months with (14.2±2.4) months for average. The follow-up rate was 92.3%(48/52). Complications included urinary retention in 18 cases(37.5%), anastomotic bleedings in 2 cases (4.2%), postoperative pain of anus in 4 cases (8.4%), recurrence in 1 case (2.1%),hypoglycemia in 1 case(2.1%), and 1 case (2.1%)was found stricture in rectum at 2 months after the procedure and no case developed rectovaginal fistula. Conclusions Old age is not the contraindication for PPH treatment of the serious hemorrhoids. Proper perioperative management may decrease the postoperative complications and improve the life quality of the patients.

Objective:To prepare urokinase targeted thrombus microbubbles combined with low frequency ultrasound to dissolve thrombus in rabbit femoral artery, and to explore the mechanism of thrombolysis through the change of urokinase concentration.Methods:Twenty-four rabbits with thrombosed femoral artery were randomly divided into four treatment groups: urokinase alone (UK) group, ultrasound with non-targeted microbubble and urokinase (US+ M+ UK) group, platelet-targeted microbubble with urokinase (R+ UK) group, ultrasound with platelet-targeted microbubble and urokinase (US+ R+ UK) group. Thrombus-targeted microbubbles were injected through the ear vein and irradiated by ultrasound for 30 minutes, and the pulsed Doppler blood flow meter was used to continuously monitor the blood flow at 0 min, 10 min, 20 min, 30 min, 60 min, 90 min and 120 min after injection. At the same time, the changes in urokinase concentration were monitored, and the characteristics of blood flow and urokinase concentration changes were analyzed. Results:UK and US+ M+ UK groups failed to show recanalization at 120 minutes after treatment. In contrast, the US+ R+ UK and R+ UK groups both achieved persistent recanalization( P<0.001). Compared with the basal state. Both the R+ UK group and R+ UK+ US group showed a decrease at 60 minutes, and the difference was statistically significant( P<0.05). Conclusions:In the thrombolysis process of low-frequency ultrasound combined with a targeted contrast agent carrying urokinase, the targeting can make the local urokinase concentration of thrombus reach the highest level. At the same time, ultrasound and targeted microbubbles promote urokinase to enter the thrombus, and finally achieve the strongest thrombolytic effect.

AIM: To study the effect of safflower injection on expression of COX-2 mRNA in chronic hypoxic hypercapnic rat pulmonary arterioles.METHODS: Sprague-Dawley rats were randomly divided into normal control group, hypoxic hypercapnic group(B),hypoxic hypercapnia+ safflower injection group(C).The concentration of TXB2 and 6-Keto-PGF1? in plasma and in lung were detected by the technique of radioimmunoassay.COX-2 mRNA was observed in arterioles from rats by the technique of in situ hybridization.RESULTS: ① Mean pulmonary arterial pressure(mPAP),weight ratio of right ventricle(RV) to left ventricle plus septum(LV+S) were much higher in B group than those in control group. No significant difference of mean carotid arterial pressure(mCAP) was observed in three groups.② The concentration of TXB2 and the ratio of TXB2/6-keto-PGF1? were significantly higher in B group than those in control group.③ Light microscopy showed that vessel wall area/total area, the density of medial smooth muscle cells and the thickness of medial smooth cell layer were significantly higher in B group than those in control group.Electron microscopy showed proliferation of medial smooth muscle cells and collagenous fibers in pulmonary arterioles in B group.Safflower injection reversed the changes mentioned above.④ Expression of COX-2 mRNA in pulmonary arterioles was much higher in C group than those in B group.Differences of COX-1 mRNA in pulmonary arterioles were not significant between these two groups.CONCLUSION: Safflower injection increases the expression of COX-2 mRNA in chronic hypoxic hypercapnic rat pulmonary arterioles, indicating an important mechanism that safflower injection inhibits the formation of hypoxic hypercapnia pulmonary hypertension and pulmonary vessel remodeling.
