1.Syndrome of extracorporeal membrane oxygenation (ECMO): a report of 7 cases

Objective To summarize and analyze the experiences on clinical application of extracorporeal membrane oxygenation (ECMO), and on the prevention of complications. Methods Seven patients, suffered from heart and respiratory failure after heart surgeries with no obvious curative effect by conventional therapy, were treated with ECMO adjunctively. Centrifugal pump assistant device was adopted for five cases, while the other two cases were treated with pulsated pump assistant device, and vasoactive agents were used in all the seven cases. Results After using of ECMO, two cases died of pulmonary infection, one died of thrombosis, one died of disseminated intravascular coagulation (DIC) induced by hemolysis, and the other three cases were successfully recovered and discharged after treatment. Other complications included hemorrhage, right heart failure, arrhythmia and hypotension, etc., and were recovered after treatment. Conclusions Clinical symptoms must be monitored after cardiac surgical operations, and ECMO should be used adjunctively once heart and/or respiratory failure appear. During the ECMO treatment, it is also import to prevent from the complications, once they occur, active therapies must be carried out to save the patients' lives.
2.Two new phragmalin-type limonoid orthesters from Chukrasia tabularis var. velutina.

As a part of our ongoing research program to isolate novel phragmalin limonoids from the stem bark of Chukrasia tabularis var. velutina, two new phragmalin limonoids orthesters, tabularin Q (1) and tabulalide O (4), along with two known compounds tabularin C (2) and tabularin H (3) were isolated. Their structures were elucidated by means of extensive 1D and 2D spectroscopic analyses which included HSQC, HMBC, and ROESY experiments and HR-ESI-MS.
3.The value of MSCT in diagnosis of gastrointestinal vascular malformation

Objective To explore the characteristics of gastrointestinal vascular malformation on contrast-enhanced multi-slice spiral computed tomography(MSCT)and assess the value of MSCT in diagnosis of gastrointestinal vascular malformation.Methods Forty-four patients with the final diagnosis of gastrointestinal vascular malformation were collected and analyzed retrospectively.We summarized the characteristics of contrast-enhanced MSCT in gastrointestinal vascular malformation and evaluated the value of MSCT in the diagnosis of gastrointestinal vascular malformation combined with image reconstructions such as MPR,MIP and VRT. Results Among 44 cases who received contrast-enhanced MSCT examination,18 cases were negative(40.9%),26 cases had positive signs(59.1%).In the 26 cases,1 case with diffuse lesions,1 case with multiple lesions,the others were local-type cases,in which 2 cases were showed contrast medium extravasations,6 cases were not only showed local mural hyper-intensification,but also multi-ple dilated small vessels around intestinal tract,10 cases were showed only local mural hyper-intensification,6 cases were showed only multiple dilated small vessels around intestinal tract.In the 24 local-type cases,lesions located at the stomach in 2 cases,the du-odenum 1 case,the jejunum 10 cases,the ileum 3 cases,the ileocecal junction 1 case,the colon 2 cases,the sigmoid colon-rectum 5 cases,respectively.Conclusion Contrast-enhanced MSCT can show the location,extent,patterns features of gastrointestinal vascu-lar malformation.It is helpful for detection and localization of the gastrointestinal vascular malformation with gastrointestinal bleed-ing.
4.Anterior debridement bone graft fusion and posterior fixation in treatment of intervertebral infection

Objective To explore the clinical outcome with anterior debridement,bone graft and posterior internal fixation with short nail of treating intervertebral infection.Methods Ninteen cases (11 male and 8 female,aged from 34 to 63,46 on average) of lumbar intervertebral space infection treated in our hospital from June 2008 to June 2013 were retrospectively analyzed.These infections occurred at L2-L3 in 2 cases,L3-L4 in 5 cases,L4-L5 in 8 cases and L5-S1 in 4 cases.All patients had history of disc surgery or puncture.The clinical outcome after operation was analyzed according to the low back paine after operation which analyzed by visual analog scale (VAS),and the bone fusion.Erythrocyte sedimentation rata and C-reactionprotein were aslo recorded before and after surgery.Results All cases were followed up,average 24 months.The lower back pain was improved obviously between preoperation and postoperation (P <0.05).Also erythrocyte sedimentation rata and C-reactionprotein returned to normal after the operation and all cases had complete bone union at the end of follow-up with no instrument failure noted.Conclusion Anterior debridement,bone graft and posterior internal fixation with short nail in of treating intervertebral infection not only can effectively reconstruct the stability of the spine,but also retain range of motion segment of spine.
5.Stereoselective metabolism of mandelic acid in rat, mouse and rabbit tissue preparations

AIM To study in vitro stereoselective metabolism of mandelic acid (MA) in 3 kinds of laboratory animal tissue fractions and observe the differences of MA biotransformation. METHODS MA enantiomers were incubated with the tissue fractions from rats, mice and rabbits. The phenylglyoxylic acid (PGA) concentrations in incubation mixture were determined by high-performance liquid chromatography. Coenzymes and inhibitors were co-incubated with MA to investigate their effects on MA metabolism. RESULTS Only S-MA showed a unique metabolism in liver and kidneys S9 fractions of rats. No inhibition of the enzyme activity was observed by addition of ethanol or 4-methyl pyrazole. NADPH caused a remarkable increase in S-MA metabolism. CONCLUSION Nonmicrosomal enzymes in kidneys and liver except alcohol dehydrogenase are responsible for the stereoselective metabolism of MA in rats. The MA biotransformation is significantly different between rats and mice or rabbits.
6.Effects of MLL-AF9 oncogene antisense oligonucleotide-mediated gene silencing on cell proliferation and apoptosis of THP-1 cells

Objective To study the effects of antisense oligonucleofide(ASODN)fusion gene MLL-AF9 on cell proliferation and apoptosis of human acute monocytic leukemia cell line THP-1 expressing MLL-AF9.Methods ASODN and sense oligonucleotide(SODN)targeting MLL-AF9 were designed,constructed and transfected into THP-1 by lipofectmine.The level of MLL-AF9 mRNA expression was examined by reverse transcription polymerage chain reaction(RT-PCR)and the expression of MLL-AF9 protein was detected by Western blotting. Cell proliferation rate was analyzed by modified-MTT assay.Hoechst33258 staining Wag used to detect the apoptotic bodies in the cells.The change of apoptosis rate was detected by flow cytometry.Results The level of MLL-AF9 mRNA and protein expression after transfection was significantly decreased in ASODN-treated group in comparison with that in the control group,liposome group and SODN group(P<0.01).Proliferation of the ASODN-treated cells was inhibited and apoptosis was evidently increased by(10.4±3.0)%,(22.8±2.5)%and(24.7±3.1)% for 0,24 and 48 h,respectively in a time-dependent manner (P<0.01).Staining of the ASODN-treated cells with Hoechst 33258 showed the morphology characteristic of apoptosis.Conclusion Special ASODN targeting MLL-AF9 effectively inhibited MLL-AF9 expression in THP-1 cell,reduced cell proliferation and induced apoptosis.
7.Design and application of cross contamination experiment for Beckman AU5800 automatic biochemical analyzer

Objective To design and verify an analytical method for the cross contamination of Beckman AU5800 automatic bio‐chemical analyzer′s reagent needle and stir bar .Methods The outer ring items (AST ,ALP ,TP ,LDH ,CK‐MB ,HBDH ,Urea ,UA , TG ,LDL‐C ,APOB ,Mg ,P ,Zn ,AMY ,CHE ,LPS ,ADA) were tested as the experiment objects .The cross contamination experiment was proceeded according to the test order .The impact rate range of 95% -105% was the criteria for cross contamination of items . Results Among all of the outer ring items ,TP ,HBDH ,APOB and TG were found to have cross contamination effect on CK‐MB , Mg ,Mg and LPS respectively .Conclusion The method established in this experiment can effectively analyze the cross contamina‐tion of all the items in Beckman AU5800 automatic biochemical analyzer .In addition ,it can help to adopt corresponding measures to reduce cross contamination and assure the accuracy of test results .
8.Therapeutic effect of polymethyl methacrylate augmented pedicle screw instrumentation and fusion for old osteoporotic vertebral compression fractures complicated with spinal instability

Objective To explore the clinical efficacy of polymethyl methacrylate (PMMA) augmented pedicle screw instrumentation and fusion for old osteoporotic vertebral compression fractures complicated with spinal instability. Methods The clinic data of 26 old patients with osteoporotic vertebral compression fractures complicated with spinal instability treated from June 2011 to June 2015 were retrospectively analyzed. Among them, there were 5 male patients aged from 61 to 72 years and 21 female patients aged from 56 to 75 years. All patients had history of chronic low back pain. The clinical outcome after operation was analyzed according to visual analog scale (VAS) and Oswestry dysfunction index (ODI). The status of bone fusion were aslo recorded before and after surgery. Results There were no pulmonary embolism, infection, toxic reaction of PMMA, and no nerve root compression and damage occurred in patients. The operation time was (105.0 ± 20.5) min, and bleeding volume was (200 ± 55) ml. The operative incision was healed in Ⅰ stage. All patients were followed up for 12-27 months, and average was 14 months. The levels of VAS scores and ODI scores after operation and 1, 3, 6, and 12 months after operation were significantly increased compared with those before operation (P<0.05). Conclusions PMMA augmented pedicle screw instrumentation and fusion is an effective method for old osteoporotic vertebral compression fractures complicated with spinal instability.
9.Empirical study of titration of IgG antibody against blood group in pregnant women by using domestic microcolumn gel Coombs'test

Objective:To compare microcolumn gel Coombs test and test tube Coombs test for IgG anti-A and anti-B titre in serum of pregnant women with blood type O.The critical titre for IgG anti-A and anti-B should be established in domestic microcolumn gel Coombs' test.Methods:524 blood samples of pregnant women with blood type O,whose husbands were of blood type A or B,were detected simultaneously by domestic microcolumn gel Coombs' test and test tube Coombs test.The results were analyzed using paired "t " test,"?2 " test and regression analysis.Results:IgG anti-A mean titres determined by the two methods separately were 249.98 and 120.85,and IgG anti-B mean titre were 156.98 and 76.38.IgG anti-A and anti-B titration in domestic microcolumn gel Coombs'test showed significantly higher titres(mean 2.07 fold and mean 2.06 fold) than in test tube Coombs test in all samples studied(t=19.64,P

Object To study on the structure of sapogenins from pod of Gleditsia sinensis Lam Methods Pod of G. sinensis was extracted with solvents and the constituents in the extraction isolated, after hydrolised completely, with the aid of flash chromatography to obtain 2 gledgenins Results The structures of the 2 gledgenins were proved to be 3 hydroxy 12 oleanen 28 oic acid and 3, 16 dihydroxy 12 oleanen 28 oic acid respectively as shown by IR, NMR, MS and crystallographic data The structures were further verified by acylation, methylation and followed by reaction with 3, 5 dinitro benzoyl chloride Conclusion New crystalline derivatives were obtained to provide further evidence of their structures
