1.The prevalence and metabolic risk factors of nonalcoholic fatty liver disease among middle-aged and aged people

Objective To investigate the prevalence of nonalcoholic fatty liver disease in a population of routine check-up,and analyze the correlation between non-alcoholic fatty liver disease and metabolic risk factors.Methods Physical examination data of 2 064 routine check-up (aged 45 or more) in the Third People's Hospital of Chengdu from January 2014 to December 2015 were randomly selected.HbA1c,fasting blood glucose,blood lipids and other indicators were measured to calculate the prevalence of NAFLD,and to analyze the correlation between NAFLD and metabolic risk factors.Results Among 2 064 cases underwent physical examination,the overall prevalence of NAFLD was 16.09% (332/2 064).The male prevalence rate of NAFLD was significantly higher than that in female (18.51% vs.12.87%;x2 =11.913,P =0.001).In four age groups (< 50 years old,50 to 59 years old,60 to 69 years old,≥ 70 years old),the NAFLD prevalence rate was 16.26%,15.63%,16.97% and 14.72% respectively.No statistical significance (x2 =0.900,P =0.826) was found in NAFLD prevalence rate difference in all age groups of has.In 50 ~ 59 years old and ≥70 years old group,the NAFLD prevalence rates in male were higher than that in female,the difference was statistically significant (both P < 0.05).BMI,waist circumference,systolic blood pressure,diastolic blood pressure (DBP),HbA1c,fasting blood glucose,TC,TG,LDL-C,uric acid levels in NAFLD patients were higher than the normal population.HDL-C levels in patients with NAFLD is lower than that of the normal,and the difference is statistically significant (both P < 0.05).Logistic regression analysis showed that BMI,hypertension,diabetes,and high TG were the independent risk factors of NAFLD;HDL-C and NAFLD were the protective factors.Conclusions In the Chengdu area of Sichuan Province,the prevalence rate of NAFLD was high,the incidence rate of NAFLD in male was higher than that in female.BMI,hypertension,diabetes mellitus and high TG were the independent risk factors of NAFLD,and HDL-C was the protective factor.
3.Treatment of Tumorous Airway Obstruction by Placing Net Stents Following High-frequency Electrocautery via Fiberoptic Bronchoscope

Objective To observe the short-term efficacy of treating the tumorous airway obstruction by placing net stents following high-frequency electrocautery via fiberoptic bronchoscope.Methods 14 patients of lung cancer with airway obstruction had undergone the tumor burning excision by high-frequency electrocautery via fiberoptic bronchoscope and placed net stents , then to observe the clinical efficiency.Results 14 patients of lung cancer with airway obstruction all revealed better efficacies, obviously improving Karnofsky scores and pulmonary function, and no evident complication was found.Conclusion Placing net stents following high-frequency electrocautery via fiberoptic bronchoscope can cure efficiently the tumorous airway obstruction, and have popular value.
4.Application of reconstruction and treatment instrument of neurological function in the recovery of neurological function and hemorheology for patients with stroke

Objective:To explore the application of EMG and bio-feedback instrument in the recovery of neurological function and hemorheology for patients with stroke.Methods: 65 patients with stroke were enrolled in this study and were randomly divided into combined group (33 cases) and control group (32 cases) as random number table. The patients of combined group were treated by rehabilitation training combined with reconstruction and treatment instrument of neurological function on the basis of basic medicine therapy. The patients of control group were treated only by rehabilitation training on the basis of basic medicine therapy. The clinical effects, the recovery situation of neurological function and the change situation of hemorheology before and after treatment were compared and analyzed.Results: The effective rate of clinical treatment was 96.97% in the combined group and it was 59.38% in the control group, and the difference of effective rate between the two groups was significant (x2=12.473,P<0.05). After treatment, the differences of the daily life behavior Barthel index scale and Fug I -Meyer Assessment (FMA) of motor function score between the two groups were significant (t=7.632,t=5.693;P<0.05). Besides, the plasma viscosity, erythrocyte ratio, platelet aggregation rate, erythrocyte deformation index, erythrocyte aggregation index and fibrinogen of combined group were significantly better than control group, and the differences of them were statistically significant (t=6.859, t=10.263,t=7.626,t=6.623,t=8.257,t=6.003;P<0.05).Conclusion:The reconstruction and treatment instrument of neurological function make a synergistic effect in the treatment of stroke, and it can effectively treat patient with stroke and improve their clinical symptoms. And it also can enhance the motor function of limbs and the self -help ability, and it can significantly improve each indexes of hemorheology of patients. Therefore, it can achieve the better clinical therapeutic effect.
6.Analysis of diagnosis and treatment about tuberculosis of thyroid

Objective:To expore early diagnosis and treatment of tuberculosis of thyroid.Methods:One patient with tuberculosis of thyroid who was misdiagnosed for 7 months was reported,a retrospective analysis of 87 patients with tuberculosis of thyroid gland diagnosed by needle biopsy and postoperative pathologic examination.Results:15 cases were diagnosed by needle blopsy,others by postoperative pathologic examination,most of them were misdiagnosed as thyroid carcinoma,thyroid adenoma,the fewer were misdiagnosed as endemic goiter,nodular goiter,hyperthyroidism.Conclusion:Thyroid node accompanied by fever and pulmonary tuberculosis must be considered of tuberculosis of thyroid,needle biopsy is a convenient and effective method for diagnosis.

8.Relationship of fasting plasma glucose with cardiovascular events in diabetic patients treated with maintenance hemodialysis

Objective To investigate the potential contributing effects of fasting plasma glucose on cardiovascular events in diabetic patients treated with maintenance hemodialysis.Methods According to fasting plasma glucose,154 patients undergoing maintenance hemodialvsis in our department were assigned into group A (n =84) nondiabetic with normal fasting blood glucose,group B (n =41) diabetic with good control of fasting blood glucose,and group C (n =29) diabetic with poor control of fasting glucose.Anthropometric and laboratory data were collected,and the participants were followed up for 36 months.Cardiovascular events and hypoglycemia were recorded and analyzed.Results Highly-sensitive C reactive protein (hs-CRP),triglyceride,and waist circumference were significantly higher in group B and group C compared with group A.Bodv mass index in group C was the highest while high density lipoprotein-cholesterol and intact parathyroid hormone levels were the lowest among 3 groups (P<0.05).During the 36 months follow-up,47 cases of cardiovascular events occurred.Kaplan-Meier curves showed that cumulative incidence of cardiovascular events was significantly higher in group C than group A and B (both P<0.01),and no significant difference was found between group A and B.Cox regressive analysis revealed that fasting plasma glucose and hs-CRP were independant risk factors for cumulative incidence of cardiovascular events (P<0.05 or P<0.0l).Conclusions If fasting plasma glucose is contmlled within the range of 5.6 to 7.2 mmol/L,cardiovascular outcomes may be significantly ameliorated in diabetic patients treated with maintenance hemodialysis.
9.The relationship of metabolic syndrome and cardiovascular disease and mortality in maintenance hemodialysis patients

Objective To investigate the potential contributing effects of metabolic syndrome (MS) on cardiovascular events and mortality in maintenance hemodialysis (MHD) patients.Methods According to the diagnostic criteria for MS set by Chinese Diabetes Society,201 subjects undergoing MHD were assigned into MS group(n =78) or non-MS group (n =123),in August 2009.General and laboratory data were collected for cross-sectional analyses,and the participants were followed up for 36 months.Cardiovascular events,cause of death and time were recorded for longitudinal analyses.Results Prevalence of MS was 38.8%.Weight,waist circumference,triglyceride,total cholesterol,low density lipoprotein,fasting glucose level,uric acid and high-sensitivity C-reactive protein were significant higher in MS group compared to non-MS group,but duration of dialysis and high-density lipoprotein were lower (P < 0.05).During the 36 months follow-up,cumulative incidence of cardiovascular events,cardiovascular and all-cause mortality were 41.0% (32/78),19.2% (15/78),28.2% (22/78),in MS group respectively.Those of non-MS group were 22.8% (28/123),10.6% (13/123),17.1% (21/123).There was significant difference between patients with and without MS in cumulative incidence of cardiovascular events (x2 =7.601,P =0.006).There was relationship between the number of metabolic syndrome parameters and cardiovascular events (r =0.236,P =0.001).Kaplan-Meier curves showed the relationship between two groups did not remained statistically significant in cardiovascular and all-cause mortality.Impaired glycometabolism in 5 parameters of MS was a cardiovascular mortality risk factor in MHD patients.Conclusions The prevalence of MS is high in MHD patients.MS is a risk factor of cardiovascular events,but it is not associated with cardiovascular and all-cause 3-year mortality in patients with MHD.
