2.CLINICAL ANALYSIS OF 109 GIRLS WITH ISOSEXUAL PRECOCIOUS PUBERTY

One hundred and nine cases of female isosexual precocious puberty were reported. Among them 63 cases were true precocious, while 48 pseudoprecocious. Based on the analysis of their clinical characteristics, the etiologic classification and the differentiation between true and pseudo precocious puberty were discussed. In 43 cases of true precocious puberty, Chlormadinone was used without any influence on growth. It was effective to control the menstruation and the development of secondary sexual characteristics in all but two cases who were over 8 years old. Besides, the problem of the final height of precocious girls was also discussed.

4.Systematic Review on Efficacy and Safety of Canagliflozin in the Treatment of Type 2 Diabetes

OBJECTIVE:To make a systematic review on the efficacy and safety of canagliflozin in the treatment of type 2 dia-betes,and to provide evidence-based reference for clinical treatment. METHODS:Retrieved from Cochrane Library,PubMed,EM-Base,CBM and Wanfang database,randomized controlled trials (RCT) of canagliflozin (test group) versus placebo (control group)in the treatment of type 2 diabetes were collected,and Meta-analysis was performed by using Rev Man 5.2 statistical soft-ware after extracting data and evaluating quality. RESULTS:A total of 7 RCTs were included,involving 2 188 patients. The results of Meta analysis indicated that glycosylated hemoglobin levels [WMD=-0.82,95%CI(-0.99,-0.65),P<0.001],the proportion of HbA1c<7%[RR=2.51,95%CI(1.98,3.19),P<0.001],fasting plasma glucose levels [WMD=-32.91,95%CI(-39.65,-26.17), P<0.001] in test group were significantly better than those of control group,the incidence of genital tract infections was significant-ly higher than control group [RR=3.76,95%CI(2.23,6.35),P<0.001];however,compared with control group,there was no sig-nificant difference in the incidence of hypoglycemia [RR=1.13,95%CI(0.40,3.20),P=0.81] and urinary tract infections[RR=1.19,95%CI(0.82,1.73),P=0.36]. CONCLUSIONS:Canagliflozin is safe and effective in the treatment of type 2 diabetes,and it needs to be noticed with genital tract infections during the clinical use. Due to the limitation of methodology,large-scale and poly-centric RCT are required for further validation of the conclusions.
5.Characteristics of electrocardiographic parameters of the right heart in 75 cases of early newborns with varying gestational age

Objective To observe the characteristics of electrocardiographic parameters of the right heart in the early newborns with different gestational age.Methods Seventy-five cases of early newborns from January 2014 to January 2015 were selected.They were divided into 4 groups according to gestational age,15 cases in group A (28-30 weeks),18 cases in group B (>30-33 weeks),17 cases in group C (>33-37 weeks) and 25 cases in group D (> 37-40 weeks).All newborns were examined by the synchronous 12-lead ECG machine,and the changes in ECG time limit detection results,QRS amplitude and RV1 amplitude were compared among groups.Results (1) There was a significant difference in P-R intervals among group A [(0.090 ± 0.011) s],group B [(0.095 ± 0.012) s],group C [(0.098 ± 0.013) s] and group D [(0.105 ± 0.014) s] (F =7.458,P < 0.05).The P-R intervals of group A,group B and group C were much shorter than those of group D (P < 0.05);the comparison of QRS duration and QTc among groups had no statistical significance (F =1.626,1.569,all P > 0.05).(2) In group A:QRS amplitude was:(4.430 ± 1.380) mV,RV1 amplitude:(0.850 ± 0.420) mV;in group B:QRS amplitude was:(4.800 ± 1.350) mV,RV1 amplitude:(1.012 ± 0.425) mV;in group C:QRS amplitude was:(5.200 ± 1.600) mV,RV1 amplitude:(1.210 ± 0.520) mV;in group D:QRS amplitude was:(5.800 ± 1.800) mV,RV1 amplitude:(1.509 ± 0.525) mV.There were significant differences in QRS and RV1 amplitude among 4 groups (F =7.002,16.870,all P < 0.05).The QRS amplitude changes in group A and group C were far less than those in group D (P < 0.05),the RV1 amplitude changes in group A,group B,and group C were far less than those in group D (P < 0.05),and there was no difference between among other groups (all P > 0.05).Conclusions There are correlations between the electrocardiographic parameters of right heart and the gestational age of newborn infants.These parameters,including the P-R intervals,the amplitude of QRS and RV1,increase with the gestational age.
6.Clinical Observation of Grain-sized Moxibustion for Acute Attack of Chronic Obstructive Pulmonary Disease Due to Accumulation of Phlegm and Stasis

ObjectiveTo prove that grain-sized moxibustion plus Western medication is effective in preventing the attack of chronic obstructive pulmonary disease (COPD), by adopting the grain-sized moxibustion to treat patients under acute attack of COPD due to accumulation of phlegm and stasis.MethodForty patients under acute attack of COPD due to phlegm and stasis were randomized into two groups. The control group was intervened by antibiotics plus Mucosolvan and nebulization (Mucosolvan+Pulmicort+Ipratropium Bromide Solution). The treatment group was intervened by grain-sized moxibustion in addition to the above treatments.Two weeks later, the two groups were compared in the use of antibiotics, C-reactive protein, and therapeutic efficacy.ResultThe use of antibiotics and the content of C-reactive protein in the treatment group were less than that in the control group.ConclusionFor patients under the acute attack of COPD due to accumulation of phlegm and stasis, grain-sized moxibustion can shorten the time of using antibiotics and descend the content of C-reactive protein by modulating inflammation.

8.Analysis of health seeking behavior based on the planned-action theory in rural area of China

Objective:To explore the influencing factors on patients' health seeking behavior in rural China by employing the theory of planned action.Methods:Data from cross-sectional household-based health survey carried out in 2008 were used in the study.Correlations between outcomes and explanatory variables were studied by Logistic regression.Results:The studying population included 19 389 adult patients over the age of 15.Illness awareness,medical experience and structural restriction factors had strong effect on whether the patients chose medical service.Pepople in different socioeconomical status suffered different influences.Conclusion:Responding to the changing trend of health service demand among rural residents,we should recognize the differentiation and allocate the healthcare resources more rationally.
9.Cost-effectiveness Analysis of Levofloxacin and Gatifoxacin in the Treatment of Lower Respiratory Tract Infections

Objective:To evaluate the cost-effectiveness of levofloxacin and gatifoxacin in the treatment of lower respiratory tract infections.Method:The clinical data of levofloxacin and gatifoxacin in the treatment of lower respiratory tract infections were collected with the evidence-based medicine method and were evaluated with pharmacoeconomic cost-effectiveness analysis.Result:The cure rates of two drugs were 64.29%(57.70%,70.88%),and 81.43%(70.05%, 92.81%)and the cost-effectiveness ratios were 1.90 and 5.79 respectively.The?C/?E was 20.37.Conclusion:The cost-effectiveness analysis indicated that the cost-effectivenss ratio of levofloxacin is superior to that of gatifoxacin though the cure efficacy of gatifoxacin for lower respiratory tract infections is better.
10.The survival analysis of patients with Paget's disease and underlying invasive breast carcinoma

Background and purpose: Paget's disease (PD) of the breast is a rare disease. The survival of PD was reported to be related to the characteristics of the underlying carcinoma. This study aimed to investigate the char-acteristics and survival of PD patients with underlying invasive breast carcinoma (IBC). Methods: Fifty-four patients diagnosed with PD and an associated IBC in Fudan University Shanghai Cancer Center from 2002 to 2007 were re-cruited in this study as study group. Since most PD overexpress HER-2, 72 patients with HER-2-overexpressing breast cancer were randomly recruited as control group. This study compared survival between these two groups. Results:Twenty-five (46.3%) patients in study group had no clinical manifestation of PD and were diagnosed unexpectedly by a histologic examination. Thirty-five (64.8%) patients in study group had tumors larger than 2 cm, twenty-six(48.1%) patients in study group had chance of axillary lymph node involvement. Compared with control group, patients in study group tended to have more events (5-year relapse-free survival: 76.4% vs 48.5%, P<0.01). In the control group, most events occurred in the first 3 years after operation, while recurrence might happen all along in the 5 years after opera-tion in study group. Patients in study group also had a poorer overall survival than in control group (61.0% vs 80.6%, P=0.01). Conclusion: Patients with Paget's disease and underlying invasive breast cancer tend to have poorer survival than those with HER-2-overexpressing breast cancer.
