1.The influence of different surgery strategies on ovarian function and the sexual function of the patients with uterine fibroid

Objective To analyze two kinds of hysteromyoma resection for patients with ovarian and sexual function.Methods 32 patients with uterine fibroids were based on different surgical procedure was divided into the observation group and control group,the control group total excision of the uterine surgery,observation group Yin type uterine fibroids rejecting operation,the postoperative patients with ovarian function and function change were observed. Results Gonadotropin in postoperative observation group was (6.52 ±1.13) mIU/L,luteinizing hormone (7.62 ± 2.01)mIU/L;Postoperative gonadotropin controls (8.65 ±2.67) mIU/L,luteinizing hormone (9.43 ±2.89) mIU/L, there was statistically significant difference between the two group(t=2.939,2.057,all P<0.05).Observation group of patients with postoperative sexual function in each dimension score:sexual desire (133.47 ±26.29) points,sexual arousal (131.03 ±22.19) points,orgasm (139.93 ±26.79),(136.27 ±31.05),psychological sex abnormalities (136.24 ±33.22)points,ejaculation time (141.29 ±33.71)points were higher than those of the control group(t=2.778,2.691,2.550,1.988,1.979,3.496,all P<0.05).Conclusion Yin uterine fibroids eliminate surgery for patients has less effects on ovarian function and sexual function,so patients with uterine fibroids should be suggested to retain the womb,in order to improve patients postoperative quality of life and the stability of the endocrine system.
2.Expression of DAB2IP and β-catenin in bladder urothelial carcinoma and their clinical significance

Purpose To investigate the clinical utility of DAB2IP (DOC-2/DAB2 interactive protein)and β-catenin expression in bladder urothelial carcinoma (BUC).Methods The expression of DAB2IP and β-catenin was detected in 104 BUC cases and 40 peritumorial tissues using EnVision two-step immunohistochemical method,and the association with BUC clinicopathological parameters was analyzed.Results The expression of DAB2IP in BUC was significantly less than that of peritumorial normal tissues,and the expression of β-catenin in BUS was significantly higher than that of peritumorial normal tissues (P < 0.05).DAB2IP expression and histologic grading,clinical pathologic staging and 5 years survival rate had statistical significance (P < 0.05).No statistical significance with gender,age,tumor diameter and in patients with incipient/recurrence.β-catenin expression and age,histologic grading,clinical pathologic staging,tumor diameter and 5 years survival rate have statistical significance (P < 0.05).No statistical significance correlated with gender and in patients with incipient/ recurrence,DAB2IP and β-catenin expression in BUC are negatively correlated (P < 0.05).Conclusion In bladder urothelial carcinoma,down-regulation of DAB2IP and up-regulation of β-catenin,are in a negative correlation.Abnormal expression of DAB2IP and β-catenin is correlated with histologic grading,clinical pathologic staging and prognosis.
3.Multiple-slice spiral CT evaluation of occipital condyle fractures

Objective To explore the MSCT findings of occipital condyle fracture (OCF) and improve its diagnostic accuracy. Methods Nineteen patients with OCF, selected from 110 patients suffering high energy injuries at the craniocervical junction, were enrolled into the study. The MSCT appearances of OCFs were retrospectively analyzed by two experienced radiologists. OCF had four types: type Ⅰ was a comminuted fracture, type Ⅱ was a extension of basilar skull fracture, type Ⅲ was an avulsion fracture at the attachment site of alar ligament on occipital condyle, type Ⅳ was a fracture of mixed pattern consisting of two or more above fracture types. Results In 19 patients, the left, right and bilateral OCFs were seen in 7,11 cases, and 1 case, respectively. Type Ⅰ was found in one case, which was a comminution of the left occipital condyle. Type Ⅱ was found in 5 cases, which involved the middle and posterior parts of occipital condyles with 2 on the left and 3 on the right, Type Ⅲ was found in 12 cases which showed various degree of fragment displacement with 3 occuring on the left, 8 on the right, 1 involving bilateral sides, 6 involving articular surfaces and 7 accompaning by enlargment of alar ligaments. Type Ⅳ was found in one case, with coexistence of Type Ⅱ and type Ⅲ. In addition, OCFs were accompanied by head and (or) cervical spine injuries in 14 cases, which included cranial fracture in 8 cases, epidural hematoma in 4 cases, subarachnoid hemorrhage in one case, cerebral contusion and laceration in one case, subfalcial hernia in one case, cervical spine fracture and dislocation in 9 cases, and so on. Conclusion OCFs can be accurately diagnosed by MSCT, which is important for selection of treatment protocols.
4.MRI Features and Dissemination Approach of Intracranial Gliomas after Operation

Objective To analyse the routes and MRI characteristics of disseminated intracranial gliomas after operation. Methods 10 patients of intracranial gliomas confirmed by pathology and intracranial dissemination after operation underwent MRI examina-tions including T_1 WI, FSE T_2 WI, FLAIR and fat-suppressed T_1 WI after intravenous injection of Gd-DTPA. In addition, 4 cases were also examinated with DWI, 1 case with SWI and DTI. Results In 10 cases,there were glioblastoma in 7 cases,grade Ⅱ astro-cytoma in 2 and grade Ⅲ astrocytoma in one. The disseminated tumors were found by MRI in 4 to 56 months after operation. The le-sions in all patients were confirmed with the comparison of contrast-enhanced MRI positive signs between preoperation and post-operation. Plain MR scanning showed line-like thicking with isointensity in 1/7 case/time (C/T)and multiple noduli in 5/7 (C/T) on T_1 WI respectively;shallowed cortical sulci and cistern in 2/7(C/T) and nodular in 5/7(C/T) on T_2 WI;shaUowed cortical aulci and cistern in 2/7 (C/T) and nodular in 6/7(C/T) on FLAIR. The signal intensity of noduli of disseminated tumors in 7 cases were in complete consistency with that of primary neoplasm , however, in 3 cases, it was inconsistent. Enhanced scanning showed 7 ca-ses with the signs of line-like thicking, 7 cases with noduli , 6 cases with :cast-like shape" sign and 6 cases with different extent of hydrocephalus. Conclusion Enhanced MRI can be used as a most useful and reliable monitoring tools for detecting dissemination of brain glioma.
5.Study on rejoin detection mode for unpaid blood donors with HBsAgor anti-HCV positive reactivity

Objective To investigate the detection mode for those unpaid blood donors screening as positive HBsAg+ and/or positive anti-HCV could be permitted to recall again for re-detection under the regulation condition in order to determine whether or not regain their qualifications of donating blood and rejoin the blood donation again.Methods The unpaid blood donors preliminari-ly screening as positive HBsAg and/or positive anti-HCV in Shenzhen from Otcober 2007 to December 2013 and conforming to the rules for recalling to re-detection formulated by our center were analyzed and researched for conducting the feasibility discussion on the rejoin mode of unpaid blood donors.Results A total of 415 759 case-times of blood donation were conducted during 2007 ~2013.Among them,2 506 cases(0.60%)and 1 357 cases(0.33%)were screened as positive HBsAg or positive anti-HCV,respec-tively.The recall process of rejoin re-detection was initiated in 59 positive HBsAg donors and 16 positive anti-HCV donors with many times of blood donation.But only 31 positive HBsAg donors and 9 positive anti-HCV donors successfully completed the detec-tion items of re-detection process.Among them 29 positive HBsAg donor regained the qualifications of donating blood and 2 cases were shielded for the blood donation qualification due to unqualification in the following detection.All of the 9 recalled donators with positive anti-HCV regained the blood donation qualification.Conclusion Under present detection mode,the detection tech-nique of blood screening is hard to avoid the occurrence of false positive results caused by the reagents,instruments and personnel operating.In order to protect the donation qualifications of the unpaid blood donators,a set of scientific,reasonable and practical re-detection mode for rejoin of the blood donators should be established for protecting the limited blood donation resource.
6.Clinicopathological study of heart valve calcification in elderly patients

Objective To investigate the prevalence rate of heart valve calcification and its relativity with pathological changes and clinical pathogenic factors in elder patients at autopsy Methods Pathology data at autopsy of 1047 patients with age from 60 to 106 years in Beijing Hospital from November 1954 to March 2016 were collected.Cases of heart valve calcification verified at autopsy were retrospectively reviewed.The prevalence of heart valve calcification and its relativity with age,clinical disease and coronary atherosclerosis was investigated.Results Among 1047 autopsies,aortic valve calcification(AVC)was found in 15.2 % (n=159),mitral valve calcification(MVC)in 9.6 % (n=101),both AVC and MVC calcification in 5.4 % (n =57)and heart valve calcification in 19.4 % (n =203).The prevalence of heart valve calcification was 6.4% (15/234)at age of 60-69,12.8%(37/289)at age of 70-79,22.5%(70/311)at age of 80-89 and 38.0% (81/213) at age of 90-106 years,respectively(tendency x2 =82.523,P<0.01).Calcification prevalence was significantly increased when complicated with coronary artery stenosis,hypertension,coronary artery disease (CAD),diabetes and chronic kidney disease (CKD).Multivariate regression analysis showed that age and CAD were independently risk factors for heart valve calcification(OR=1.066,95% CI:1.048-1.086,P< 0.01;OR =2.238,95% CI:1.396-3.589,P<0.01,respectively),while hypertension,diabetes and CKD were not independent risk factors(OR =1.223,95% CI:0.859-1.741,P> 0.05;OR =1.053,95% CI:0.700-1.586,P >0.05;OR =0.924,95% CI:0.610-1.399,P> 0.05,respectively).As compared with patients without heart valves calcification,patients with heart valve calcification had more increased risk for coronary atherosclerosis(OR =2.983,95a%CI:1.868-4.765,P<0.01).Conclusions Prevalence of heart valve calcification is increased in elder patients with increasing age.Prevalence of heart valve calcification is higher in CAD patients than in non-CAD patients.And heart valve calcification is sigmficantly associated with coronary atherosclerosis.
7.Accuracy assessment of transthoracic echocardiography in diagnosing left cardiac valve calcification in elderly patients

Objective To investigate the accuracy of transthoracic echocardiography (TTE) for diagnosing left cardiac valve calcification in elderly patients.Methods In this retrospective study,410 elderly patients died in Beijing Hospital from 1982 to 2015 were collected.They all had full pathological cardiac valve examination data in the autopsy examination and full mortem TTE data.They were classified into three groups according to the year of death:group 1982-1995,group 1996-2005 and group 2006-2015,as well as into three groups according to the age of death:group 60-75,group76-90 and group 91-106.Results The sensitivity,specificity,diagnostic coincidence rate,positive likelihood ratio (PLR),negative likelihood ratio (NLR),positive predictive value (PPV) and negative predictive value(NPV) of TTE versus autopsy pathological findings as golden standard for diagnosing left cardiac valve calcification were 88.6%,28.2%,43.7%,1.234,0.405,0.298 and 0.878.Compared with group 1982-1995,the group 1996-2005 and group 2006-2015 showed the sensitivity and PPV of TTE versus golden standard autopsy findings for diagnosing left cardiac valve calcification in the elderly were increased while the specificity,PLR,NLR and NPV were decreased.Compared with group 60-75,the group 76-90 and group 91-106 showed that the sensitivity and PPV of TTE versus golden standard autopsy findings for diagnosing left cardiac valve calcification were increased,while the specificity and diagnostic coincidence rate were decreased.The sensitivity,specificity,diagnostic coincidence rate,PLR,NLR,PPV,NPV of TTE versus golden standard autopsy findings for the diagnosis of aortic valve calcification and mitral valve calcification in the elderly were 88.2% and 44.0%,30.8% and 75.3%,42.7% and 71.5%,1.275 and 1.780,0.382 and 0.744,0.250 and 0.198,0.909 and 0.906,respectively.Conclusions TTE provides high sensitivity for diagnosing left cardiac valve calcification in the elderly,especially for diagnosing the aortic valve calcification,but the specificity is not satisfactory.
8.A preliminary study of radiomics in predicting WHO/ISUP grading of clear cell renal cell carcinoma based on unenhanced CT texture analysis

Objective:To investigate the value of radiomics based on unenhanced CT texture analysis in predicting the WHO/International Society of Urological Pathology (ISUP) grading of clear cell renal cell carcinoma (ccRCC).Methods:Postoperative pathology-confirmed ccRCC subjects ( n=90) who received CT scanning and had a definite pathological grading in Cancer Hospital of the University of Chinese Academy of Sciences were collected retrospectively from December 2016 to May 2019. The cases were randomly divided into training group ( n=63) and test group ( n=27) as a ratio of 7∶3. All cases were classified into low grade (grades Ⅰ and Ⅱ, n=57) and high grade (grades Ⅲ and Ⅳ, n=37) according to the new pathological grading (WHO/ISUP grading, version 2016) of renal carcinoma. 3D-ROI segmentation was performed on unenhanced CT images and 93 texture features were extracted. The least absolute shrinkage and selection operator (LASSO) regression was used to reduct dimension of texture parameters and then the radiomics score (Rad-score) was established. The logistic regression was used to develop the prediction model with the pathological grading as the gold standard. The ROC curve and calibration curve were used to evaluate the predictive performance of the model, and the area under the curve (AUC), accuracy, sensitivity and specificity were calculated. The Hosmer-Lemeshow test was used to evaluate calibration degree of the model. Results:The 10 non-zero coefficient texture features were screened out through dimension reduction steps. The Rad-score was formed according to the linear combination of these ten features and corresponding coefficients, and then the prediction model was developed. The AUC of the model in training group was 0.933 (95%CI 0.862-1.000), the sensitivity was 92.3%, the specificity was 89.2%, and the model accuracy was 90.5%. The calibration curve showed the good calibration ( P=0.257). The AUC value in test group was 0.875 (95%CI 0.734-1.000), the sensitivity, specificity and accuracy were 72.7%, 87.5% and 81.5%. The calibration curve showed the good calibration ( P=0.125). Conclusion:The radiomics prediction model based on unenhanced CT texture analysis have application potential for the evaluation of WHO/ISUP grading of ccRCC.
9.Changes of calcitonin gene-related peptide after rat sciatic nerve ligation

Objective To study the temporo-spatial changing laws of calcitonin gene-related peptide(CGRP)in the rat model of sciatic nerve ligation.Methods Totally 48 Sprague-Dawley rats were randomly divided into sham-operation group(n=6)and experimental groups which survived for 1,3,5,7,14,21,28 d respectively after sciatic nerve ligation(n=6 at each time point).The distribution and quantity of CGRP in sciatic nerve,dorsal root ganglion(DRG)and spinal cord were detected by immunohistochemistry and image analysis methods.Results A large amount of CGRP piled up in the sciatic nerve at 1 d after the ligation,significantly higher in proximal segment than that in distal segment,gradually dropping till disappear basically at 14 d in proximal segment and at 7 d in distal segment.The expression of CGRP in DRG at 7 d began to drop after the ligation,at 21 d to the minimal value and at 28 d lower than normal.The CGRP-immunoreactivety in ipsilateral spinal dorsal horn at 14 d decreased after operation,but the immuno-positive areas of CGRP were of no changes.There are no changes in the CGRP-positive spinal ventral motoneurons of all groups.Conclusion The changes of CGRP presented a temporo-spatial pattern following peripheral nerve legation,maybe resulting from the deficiency of neurothrophins from target organs.
10.MRI study on the relationship between the invasion patterns and staging of nasopharyngeal carcinoma

Objective To investigate the peri-nasopharyngeal invasion patterns of nasopharyngeal carcinoma (NPC) on MRI and its relationship with tumor staging. Methods One thousand five hundred and seventy-three patients with newly diagnosed NPC which were histo-pathologically proved were retrospectively studied. The MRI manifestations and invasion patterns of the NPCs were elevated according to the 2008 Tstaging system of NPC. Z test was used to analyze the rate of adjacent structures invasion in NPCs. Results The structures invaded by NPCs included pharyngobasilar fascia in 1299 cases (82. 58% ); parapharyngeal space, 1090 ( 69. 29% ); nasal cavities, 304 ( 19. 33% ); oropharynx, 49 ( 3. 12% ); carotid space,514(32. 68% ); medial pterygoid muscle, 661 (42. 02% ); lateral pterygoid muscle, 210( 13. 35% ); skull base bones, 943(59. 95% ); cranial nerves, 630(40. 05% ) and paranasal sinuses, 242 ( 15.38% ). The T-stage distribution was T1, 242 cases ( 15.38% ); T2, 288 ( 18. 31% ); T3, 410 (26. 06% ) and T4,633 (40. 24% ). Among the cases with nasal cavities invasion, 90. 46% (275/304)showed the involvement of the structures seen in T3 or T4 stage, which was found in all cases with oropharynx invasion. In addition,69. 14% (457/661) of cases with medial pterygoid muscle invasion and 92. 15% (223/242) of cases with paranasal sinuses invasion showed the involvement of structures seen in T4 stage. As for the invasion patterns of NPC, the lateral invasion of pharyngobasilar fascia was more frequent than upward invasion of skull base (Z = 14. 025, P < 0. 01 ) and downward invasion of oropharynx ( Z = 45.032, P < 0. 01 ), and the downward invasion of oropharynx was less frequent than upward invasion of skull base ( Z = 34. 301, P < 0. 01 ) and forward invasion of nasal cavities ( Z = 14. 404, P < 0. 01 ). Conclusion NPC has a predilection of lateral invasion rather than upward and downward invasion, and its upward and forward invasion are more common than downward invasion.
